Abstract:Unobserved individual heterogeneity is a common challenge in population cancer survival studies. This heterogeneity is usually associated with the combination of model misspecification and the failure to record truly relevant variables. We investigate the effects of unobserved individual heterogeneity in the context of excess hazard models, one of the main tools in cancer epidemiology. We propose an individual excess hazard frailty model to account for individual heterogeneity. This represents an extension of frailty modelling to the relative survival framework. In order to facilitate the inference on the parameters of the proposed model, we select frailty distributions which produce closed-form expressions of the marginal hazard and survival functions. The resulting model allows for an intuitive interpretation, in which the frailties induce a selection of the healthier individuals among survivors. We model the excess hazard using a flexible parametric model with a general hazard structure which facilitates the inclusion of time-dependent effects. We illustrate the performance of the proposed methodology through a simulation study. We present a real-data example using data from lung cancer patients diagnosed in England, and discuss the impact of not accounting for unobserved heterogeneity on the estimation of net survival. The methodology is implemented in the R package IFNS.

Abstract:Historically, the primary goal of conventional survival study methods has been to reduce the frequency of failures over time. If the associated observed and unobserved variables are not known when studying such events, this can have detrimental effects. Frailty models offer a tempting solution for investigating the impact of unknown variables in such a case. In this article, we assume that frailty affects the hazard rate. We find that the weighted Lindley frailty models, which use general versions of the Weibull and log-logistic type II distributions as the baseline distributions, are a reliable method for ensuring the influence of endogenous variability. The parameters involved are estimated according to different loss functions using the Bayesian structure as the basis of Markov Chain Monte Carlo. Bayesian evaluation strategies are then implemented to evaluate the models. The results are demonstrated on known data of kidney infections. It is shown that the novel models outperform those based on the inverse Gaussian and gamma frailty distributions.

Abstract:✓ The authors report a further 3-year follow-up review of a series of 59 patients suffering with intractable pain due to lumbar spondylosis and treated by multiple-level bilateral laminectomies and facetectomies. Routine postoperative x-ray examination revealed that six of 59 patients had developed spondylolisthesis. Two of the six were symptomatic and required a secondary fusion procedure. Analysis of the patients who had developed vertebral slippage revealed that 6% of the patients with only two levels (L-4 and L-5) removed developed spondylolisthesis, but in those with three or more levels removed 15% showed slipped vertebrae. After an average follow-up period of 6 years, 87% of patients still had a worthwhile result as contrasted with 91% of the same series who had had a good result after a follow-up period averaging 3 years.
Impact of model misspecification in shared frailty survival models

Abstract:Survival models incorporating random effects to account for unmeasured heterogeneity are being increasingly used in biostatistical and applied research. Specifically, unmeasured covariates whose lack of inclusion in the model would lead to biased, inefficient results are commonly modelled by including a subject-specific (or cluster-specific) frailty term that follows a given distribution (e.g. Gamma or log-Normal). Despite that, in the context of parametric frailty models little is known about the impact of misspecifying the baseline hazard, the frailty distribution, or both. Therefore, our aim is to quantify the impact of such misspecification in a wide variety of clinically plausible scenarios via Monte Carlo simulation, using open source software readily available to applied researchers. We generate clustered survival data assuming various baseline hazard functions, including mixture distributions with turning points, and assess the impact of sample size, variance of the frailty, baseline hazard function, and frailty distribution. Models compared include standard parametric distributions and more flexible spline-based approaches; we also included semiparametric Cox models. The resulting bias can be clinically relevant. In conclusion, we highlight the importance of fitting models that are flexible enough and the importance of assessing model fit. We illustrate our conclusions with two applications using data on diabetic retinopathy and bladder cancer. Our results show the importance of assessing model fit with respect to the baseline hazard function and the distribution of the frailty: misspecifying the former leads to biased relative and absolute risk estimates while misspecifying the latter affects absolute risk estimates and measures of heterogeneity.

Abstract:In statistical models for the analysis of time-to-event data, individual heterogeneity is usually accounted for by means of one or more random effects, also known as frailties. In the vast majority of the literature, the random effect is assumed to follow a continuous probability distribution. However, in some areas of application, a discrete frailty distribution may be more appropriate. We investigate and compare various existing families of discrete univariate and shared frailty models by taking as our focus the variance of the relative frailty distribution in survivors. The relative frailty variance (RFV) among survivors provides a readily interpretable measure of how the heterogeneity of a population, as represented by a frailty model, evolves over time. We explore the shape of the RFV for the purpose of model selection and review available discrete random effect distributions in this context. We find non-monotone trajectories of the RFV for discrete univariate and shared frailty models, which is a rare property. Furthermore, we proof that for discrete time-invariant univariate and shared frailty models with (without) an atom at zero, the limit of the RFV approaches infinity (zero), if the support of the discrete distribution can be arranged in ascending order. Through the one-to-one relationship of the RFV with the cross-ratio function in shared frailty models, which we generalize to the higher-variate case, our results also apply to patterns of association within a cluster. Extensions and contrasts to discrete time-varying frailty models and contrasts to correlated discrete frailty models are discussed.

Abstract:Frailty models are effective in broadening the class of survival models and inducing dependence in multivariate survival distributions. In proportional hazards, the random effect multiplies the hazard function. The scale-change model incorporates unobserved heterogeneity through a random effect that enters the baseline hazard function to change the time scale. We interpret this random effect as frailty, or other unobserved risks that create heterogeneity in the population. This model produces a wide range of shapes for univariate survival and hazard functions. We extend this model to multivariate survival data by assuming that members of a group share a common random effect. This structure induces association among the survival times in a group and provides alternative association structures to the proportional hazards frailty model. We present parametric and semiparametric estimation techniques and illustrate these methods with an example.

Abstract:Dependent survival data arise in many contexts. One context is clustered survival data, where survival data are collected on clusters such as families or medical centers. Dependent survival data also arise when multiple survival times are recorded for each individual. Frailty models is one common approach to handle such data. In frailty models, the dependence is expressed in terms of a random effect, called the frailty. Frailty models have been used with both Cox proportional hazards model and the accelerated failure time model. This paper reviews recent developments in the area of frailty models in a variety of settings. In each setting we provide a detailed model description, assumptions, available estimation methods, and R packages.

Abstract:Frailty models are survival analysis models which account for heterogeneity and random effects in the data. In these models, the random effect (the frailty) is assumed to have a multiplicative effect on the hazard. In this paper, we present frailty models using phase-type distributions as the frailties. We explore the properties of the proposed frailty models and derive expectation-maximization algorithms for maximum-likelihood estimation. The algorithms' performance is illustrated in several numerical examples of practical significance.

The Geneva Papers on Risk and Insurance Issues and Practice

Abstract:For the life insurance industry and pension schemes, mortality projections are critical for accurately managing exposure to longevity risk in terms of both premium setting and reserving. Frailty has been identified as an important latent factor underpinning the evolution of mortality rates. It represents the comorbidities that drive the deterioration of the human body's physiological capacity. In this paper, we propose a stochastic mortality model that incorporates the trend in frailty, and we analyse the gap between the actuarial evaluations of premiums and technical provisions calculated under frailty-based and traditional stochastic mortality models. We observe that the frailty-based model leads to higher levels of uncertainty in estimates and projections (compared to a traditional stochastic mortality model), which is attributed to the explicit modelling of the comorbidities. This leads to proposing a potentially important policy-oriented recommendation: the incorporation of frailty in mortality modelling would allow for the profiling of mortality according to the portfolio in force for the insurer (or pension scheme), thereby mitigating the problem of adverse selection.

Abstract:Background Mortality prediction models are useful to guide clinical decision-making based on prognosis. The frailty index, which allows prognostication and personalized care planning, has not been directly compared with validated prognostic models. Objective To compare the discrimination of mortality, disability, falls, and hospitalization between a frailty index and validated prognostic indices. Design Secondary Analysis of the National Health and Aging Trends Study. Participants Seven thousand thirty-three Medicare beneficiaries 65 years or older. Measurements We measured a deficit-accumulation frailty index, Schonberg index, and Lee index at the 2011 baseline assessment. Primary outcome was mortality at 5 years. Secondary outcomes were decline in activities of daily living (ADL), decline in instrumental activities of daily living (IADL), fall, and hospitalization at 1 year. We used C-statistics to compare discrimination between indices, adjusting for age and sex. Results The study population included 4146 (44.8%) with age ≥ 75 years, with a median frailty index of 0.15 (interquartile range 0.09–0.25). A total of 1385 participants died (14.7%) and 2386 (35.2%) were lost to follow-up. Frailty, Schonberg, and Lee indices predicted mortality similarly: C-statistics (95% confidence interval) were 0.78 (0.77–0.80) for frailty index; 0.79 (0.78–0.81) for Schonberg index; and 0.78 (0.77–0.80) for Lee index. The frailty index had higher C-statistics for decline in ADL function (frailty index, 0.80 [0.78–0.83]; Schonberg, 0.74 [0.72–0.76]; Lee, 0.74 [0.71–0.77]) and falls (frailty index, 0.66 [0.65–0.68]; Schonberg, 0.61 [0.58–0.63]; Lee, 0.61 [0.59–0.63]). C-statistics were similar for decline in IADL function (frailty index, 0.61 [0.59–0.63]; Schonberg, 0.60 [0.59–0.62]; Lee, 0.60 [0.58–0.62]) and hospitalizations (frailty index, 0.68 [0.66–0.70]; Schonberg, 0.68 [0.66–0.69]; Lee, 0.65 [0.63–0.67]). Conclusions A deficit-accumulation frailty index performs as well as prognostic indices for mortality prediction, and better predicts ADL disability and falls in community-dwelling older adults. Frailty assessment offers a unifying approach to risk stratification for key health outcomes relevant to older adults.

Abstract:Background: Colorectal cancer (CRC) is the 3 rd most common cancer in men and 2 nd most common malignancy in females across the globe leading to high mortality rates. Frailty is an age-related syndrome that has been associated with high morbidity and mortality. This systematic review aimed to examine if frailty can predict long-term (>1 year) outcomes of patients with CRC. Methods: This PROSPERO registered review examined the databases of PubMed, Embase, and Web of Science till 4 th September 2023 for cohort studies assessing the association between frailty and long-term outcomes of CRC. Results: 15 studies with 45288 patients were included. 6573 patients (14.5%) were frail. Meta-analysis demonstrated that frailty was associated with statistically significant poor overall survival (OS) (HR: 2.11 95% CI: 1.44, 3.08 I 2 = 94%) (14 studies), cancer-specific survival (CSS) (HR: 4.59 95% CI: 2.75, 7.67 I 2 = 38%) (2 studies), and disease-free survival (DFS) (HR: 1.46 95% CI: 1.28, 1.66 I 2 = 0%) (5 studies) after CRC. Subgroup analysis for OS based on study type, location, sample size, stage of cancer, percentage with frailty, treatment, adjustment for CRC stage and comorbidities, and follow-up did not change the results. These results were not altered in significance on sensitivity analysis. Conclusion: Our results show that frail CRC patients have poor OS and DFS as compared to non-frail patients. Variations in frailty measurement tools and high inter-study heterogeneity are major limitations of the review. Abstract:Frailty models are multiplicative hazard models for studying association between survival time and important clinical covariates. When some values of a clinical covariate are unobserved but known to be below a threshold called the limit of detection (LOD), naive approaches ignoring this problem, such as replacing the undetected value by the LOD or half of the LOD, often produce biased parameter estimate with larger mean squared error of the estimate. To address the LOD problem in a frailty model, we propose a flexible smooth nonparametric density estimator along with Simpson's numerical integration technique. This is an extension of an existing method in the likelihood framework for the estimation and inference of the model parameters. The proposed new method shows the estimators are asymptotically unbiased and gives smaller mean squared error of the estimates. Compared with the existing method, the proposed new method does not require distributional assumptions for the underlying covariates. Simulation studies were conducted to evaluate the performance of the new method in realistic scenarios. We illustrate the use of the proposed method with a data set from Genetic and Inflammatory Markers of Sepsis study in which interlekuin‐10 was subject to LOD. Copyright © 2015 John Wiley & Sons, Ltd.

Abstract:Frailty and resilience models provide a way to introduce random effects in hazard and reversed hazard rate modeling by random variables, called frailty and resilience random variables, respectively, to account for unobserved or unexplained heterogeneity among experimental units. This paper investigates the effects of frailty and resilience random variables on the baseline random variables using some shifted stochastic orders based on some ageing properties of the baseline random variables. Relevant examples are provided to illustrate the results. Some results are illustrated with real-world data.

Abstract:Abstract While frailty is associated with risk of numerous adverse health outcomes including mortality, little is known about the most common specific causes of death among frail older adults or how these causes might differ by gender. This information may be important to understanding the frailty syndrome and to informing screening and treatment. We used linked data from the Health and Retirement Study (2004 – 2012) and the National Death Index (NDI). We analyzed data from HRS participants age 65 and older who completed a general health interview and physiological measures (n=10,490). Frailty was operationalized using the phenotype criteria – low weight, low energy expenditure, exhaustion, slow gait, and weakness. Causes of death were determined using International Classification of Diseases (v10) codes from death certificates. We used Cox proportional hazards to compare incidence of cause-specific mortality by frailty status and gender. The attributable risk of mortality due to frailty in the sample was 16.6% among women and 17.3% among men. Overall, frail older adults had greater risk of death from heart disease (hazard ratio (HR): 2.97; 95% CI: 2.18, 4.04), cancer (HR: 2.81; 95% CI: 2.01, 3.93), and dementia 2.86 (95% CI: 1.46, 5.58) but not cerebrovascular disease or accidents. Frail women were more approximately 29% more likely to die from heart disease than frail men. Findings suggest that frailty is a significant risk factor for mortality from several different causes, especially among women. Findings may help inform screening and treatment decisions for older adults at risk for frailty.

Abstract:In many countries life expectancy gains have been substantially higher than predicted by even recent forecasts. This is primarily due to increasing rates of improvement in old-age mortality not captured by existing models. In this paper we show how the concept of frailty can be used to model both changing rates of improvement and the deceleration of mortality at old ages, also seen in data. We present a "fragilization" method by which frailty can be added to standard mortality models. The aim is to improve the modeling and forecasting of old-age mortality while preserving the structure of the original model and the underlying stochastic processes. Estimation is based on a general pseudo-likelihood approach which allows the use of essentially any frailty distribution and mortality model. We also consider a class of generalized stochastic frailty models with both frailty and non-frailty terms, and we describe how these models can be estimated by the EM-algorithm. The method is applied to the Lee-Carter model and a parametric time-series model. For both applications the effect of adding frailty is illustrated with mortality data for US males.

Abstract:Frailty is a state of increased risk for adverse health outcomes. Their degree of frailty makes frail individuals more susceptible to cardiovascular disease and mortality than are their fitter counterparts. Frailty can be quantified clinically in various ways. Most common are the frailty phenotype and frailty index. These have been reverse-translated for use in aging mice. This review describes these instruments and explains how they can quantify the degree of frailty in mouse models. Understanding the degree of frailty is critical, as frailty predicts cardiac morbidity and mortality better than do traditional risk factors for cardiovascular diseases.

Abstract:BACKGROUND Epidemiologic evidence reporting the role of frailty in survival among older adults with a prior cancer diagnosis is limited. METHODS A total of 2050 older adults (>= 60 years old) surviving for at least 1 year after a cancer diagnosis and 9474 older adults without a cancer history from the National Health and Nutrition Examination Survey (1999-2014) were included for analysis. The exposure variable, a 45-item frailty index (FI), was categorized on the basis of validated cutoffs (FI <= 0.10 [fit], 0.10 < FI <= 0.21 [prefrail], and FI > 0.21 [frail]). All-cause mortality was ascertained via the National Death Index. Multivariable Cox proportional hazards models were used to estimate adjusted hazard ratios (aHRs) and 95% confidence interval (CIs) for the FI, and this was followed by restricted cubic splines depicting dose-response curves. RESULTS For older cancer survivors, the mean age at the baseline was 72.6 years (SD, 7.1 years); 5.9% were fit, 38.2% were prefrail, and 55.9% were frail. Older adults without a cancer history were slightly younger (mean age, 70.0 years) and less frail (47.9% were frail). At each level of the FI, cancer survivors (1.9 per 100 person-years for FI <= 0.10, 3.4 per 100 person-years for 0.10 < FI <= 0.21, and 7.5 per 100 person-years for FI > 0.21) had higher mortality than their cancer-free counterparts (1.4 per 100 person-years for FI <= 0.10, 2.4 per 100 person-years for 0.10 < FI <= 0.21, and 5.4 per 100 person-years for FI > 0.21). The multivariable model suggested a positive association between the FI and all-cause mortality for survivors (aHR for FI > 0.21 vs FI <= 0.10, 2.80; 95% CI, 1.73-4.53) and participants without a cancer history (aHR for FI > 0.21 vs FI <= 0.10, 2.75; 95% CI, 2.29-3.32). Restricted cubic splines indicated that all-cause mortality risk increased with the FI in a monotonic pattern. CONCLUSIONS Frailty is associated with a higher risk of death in older cancer survivors and the elderly without a cancer history.;

Abstract:Incidence and survival of breast cancer, the most common cancer among women, have been increasing, leaving survivors at risk of aging-related health conditions. In this matched cohort study, we examined frailty risk with the Hospital Frailty Risk Score among breast cancer survivors (n = 34,900) and age-matched comparison subjects (n = 290,063). Women born in 1935-1975, registered in the Swedish Total Population Register (1991-2015), were eligible for inclusion. Survivors had a first breast cancer diagnosis in 1991-2005 and survived ≥5 years after initial diagnosis. Death date was determined by linkage to the National Cause of Death Registry (through 2015). Cancer survivorship was weakly associated with frailty (subdistribution hazard ratio (SHR) = 1.04, 95% confidence interval (CI): 1.00, 1.07). In age-stratified models, those diagnosed at younger ages (<50 years) had higher risk of frailty (SHR = 1.12, 95% CI: 1.00, 1.24) than those diagnosed at ages 50-65 (SHR = 1.03, 95% CI: 0.98, 1.07) or >65 (SHR = 1.09, 95% CI: 1.02, 1.17) years. Additionally, there was increased risk of frailty for diagnoses in 2000 or later (SHR = 1.15, 95% CI: 1.09, 1.21) compared with before 2000 (SHR = 0.97, 95% CI: 0.93, 1.17). This supports work from smaller samples showing that breast cancer survivors have increased frailty risk, particularly when diagnosed at younger ages.

Abstract:This paper studies Cox's regression hazard model with an unobservable random frailty where no specific distribution is postulated for the frailty variable, and the marginal lifetime distribution allows both parametric and non-parametric models. Laplace's approximation method and gradient search on smooth manifolds embedded in Euclidean space are applied, and a non-iterative profile likelihood optimization method is proposed for estimating the regression coefficients. The proposed method is compared with the Expected-Maximization method developed based on a gamma frailty assumption, and also in the case when the frailty model is misspecified.
