Abstract:Posterior medial meniscus root tears (PMMRTs) make up a relatively notable proportion of all meniscus pathology and have been definitively linked to the progression of osteoarthritis (OA). While known risk factors for development of OA in the knee include abnormal tibial coronal alignment, obesity and female gender, PMMRTs have emerged in recent years as another significant driver of degenerative disease. These injuries lead to an increase in average contact pressure in the medial compartment, along with increases in peak contact pressure and a decrease in contact area relative to the intact state. Loss of the root attachment impairs the function of the entire meniscus and leads to meniscal extrusion, thus impairing the force-dissipating role of the meniscus. Anatomic meniscus root repairs with a transtibial pullout technique have been shown biomechanically to restore mean and peak contact pressures in the medial compartment. However, nonanatomic root repairs have been reported to be ineffective at restoring joint pressures back to normal. Meniscal extrusion is often a consequence of nonanatomic repair and is correlated with progression of OA. In this study, the authors will describe the biomechanical basis of the natural history of medial meniscal root tears and will support the biomechanical studies with a case series including patients that either underwent non-operative treatment (5 patients) or non-anatomic repair of their medial meniscal root tears (6 patients). Using measurements derived from axial MRI, the authors will detail the distance from native root attachment center of the non-anatomic tunnels and discuss the ongoing symptoms of those patients. Imaging and OA progression among patients who were treated nonoperatively before presentation to the authors will be discussed as well. The case series thus presented will illustrate the natural history of meniscal root tears, the consequences of non-anatomic repair, and the findings of symptomatic meniscal extrusion associated with a non-anatomic repair position of the meniscus.

Abstract:A foundational knowledge of the anatomy and biomechanics of meniscal root tears is warranted for proper repair of meniscal root tears and for preventing some of their commonly described iatrogenic causes. Meniscal root tears are defined as either a radial tear occurring within one cm of the root attachment site of the meniscus or a complete bony or soft tissue avulsion of the root attachment altogether. Meniscal root tears disrupt the protective biomechanical function of the native meniscus. Biomechanical analyses of the current techniques for meniscal root repair highlight the importance of restoring menisci to their correct anatomic orientation, thereby restoring their biomechanical function. A comprehensive understanding of the clinical and radiographic presentations of these injuries is critical to preventing their underdiagnosis. The poor long-term outcomes associated with conservative treatment measures, namely, ipsilateral compartment osteoarthritis, warrants the surgical repair of meniscal root tears whenever possible. While excellent patient-reported outcomes exist for the various surgical repair techniques, adherence to stringent post-operative rehabilitation protocols is critical for patients to avoid damaging the integrity of a repaired root. This review will focus on current concepts pertaining to the anatomy, biomechanics, diagnosis, treatment, and postoperative rehabilitation for meniscal root tears.
Global research status and hotspot analysis of meniscal root tears based on the WOS database

Abstract:Background Meniscal root tears are one of the common diseases in the field of orthopedics and sports medicine today and are the subject of many current investigative efforts. Purpose This study aims to identify and evaluate the global trends, hotspots and frontiers in meniscal root tear research using bibliometric analysis. Methods A bibliometric analysis of research findings related to meniscal root tears over the past three decades was performed. CiteSpace was used to conduct document co-citation and cluster analyses on the collected data. The research was conducted based on the following factors: country and institution distribution, chronological distribution, source journal analysis, keyword co-occurrence analysis, and reference co-citation analysis. Results A total of 626 research articles on meniscal root tears in English published from 1989 to 2021 were obtained. There was a significant upward trend in the total number of scientific publications over the past decades, especially in 2015–2020. The most productive countries, institutions, journals and authors are the USA, STEADMAN PHILIPPON, KNEE SURGERY SPORTS TRAUMATOLOGY ARTHROSCOPY , and LAPRADE RF . North America and East Asia made outstanding contributions to the research on meniscal root tears, but cooperation and exchanges between countries and institutions were not close enough. A total of 9 clusters were obtained from the citation analysis, and 8 clusters were obtained from the keyword analysis. The main keywords that ranked first were posterior root tear, medial meniscus, menisci tibial, and ACL reconstruction, and these clusters combined with the corresponding emergence reflected the current status of research at different times. Conclusion Research in this field over the past 32 years has gone through a phase of exploration in the understanding of the anatomy of the meniscal root and the diagnosis of this disease and a phase of development with in-depth biomechanical studies and improved and innovative surgical techniques. The current research focuses on the innovation of meniscal root tear repair techniques, the long-term efficacy of surgery, the variability in the efficacy of different surgical techniques, and surgical strategies for combined injuries. There will be more breakthroughs in surgical techniques, surgical equipment and surgical materials to resolve meniscal root tears.

Abstract:Menisci play an essential role in maintaining normal pain-free function of the knee. While there are decades of MRI literature on the tears involving the meniscus body and horns, there is now a surge in knowledge regarding injuries at the meniscus roots and periphery. The authors briefly highlight new insights into meniscus anatomy and then summarize recent developments in the understanding of meniscus injuries that matter, emphasizing meniscus injuries at the root and peripheral (eg, ramp) regions that may be missed easily at MRI and arthroscopy. Root and ramp tears are important to diagnose because they may be amenable to repair. However, if these tears are left untreated, ongoing pain and accelerated cartilage degeneration may ensue. The posterior roots of the medial and lateral menisci are most commonly affected by injury, and each of these injuries is associated with distinctive clinical profiles, MRI findings, and tear patterns. Specific diagnostic pitfalls can make the roots challenging to evaluate, including MRI artifacts and anatomic variations. As with root tears, MRI interpretation and orthopedic treatment have important differences for injuries at the medial versus lateral meniscus (LM) periphery (located at or near the meniscocapsular junction). Medially, ramp lesions typically occur in the setting of an anterior cruciate ligament rupture and are generally classified into five patterns. Laterally, the meniscocapsular junction may be injured in association with tibial plateau fractures, but disruption of the popliteomeniscal fascicles may also result in a hypermobile LM. Updated knowledge of the meniscus root and ramp tears is crucial in optimizing diagnostic imaging before repair and understanding the clinical repercussions. ©RSNA, 2023 Online supplemental material is available for this article. Abstract:OBJECTIVE:To analyze the causes of meniscal root tears (MRTs), and to review the progress in its diagnosis and therapy.METHODS:The published literature about MRTs was extensively reviewed and summarized.RESULTS:MRTs can be divided into acute traumatic injuries and chronic degenerative injuries; MRI examination is an effective way to diagnose according to meniscal extrusion, ghost sign, and meniscal root linear defects. Conservative treatments are mainly performed for chronic MRTs; partial meniscectomy can obtain predictable improvement in symptoms. Acute MRTs can be treated by arthroscopic meniscus repair and reconstruction which can effectively restore meniscal root anatomy and function, and knee joint degeneration can be postponed.CONCLUSION:MRTs diagnosis relies on MRI, appropriate treatment plan should be made based on the specific circumstances of the patient after accurate diagnosis. Usually the satisfied short-term clinical results can be obtained, while the long-term results need more clinical evaluation.

Introduction:Medial Meniscus Posterior root tears (MMPRT) are defined as complete radial tear within 10 mm from the meniscus attachment or bony root avulsion. Currently, repair of meniscal root injuries is the treatment of choice with the aim of restoring joint kinematics, contact pressures, and delaying the development of OA. Although encouraging results have been reported, the repair techniques do not provide any biological substance to stimulate biological healing, remembering that the attachment of medial meniscus root to the tibial plateu contain a ligament-like structures, the usage of graft may recreate that ligamentous structure. Therefore the purpose of this case series is to describe the clinical outcome of medial meniscus root reconstruction in 3 cases of degenerative MMPRT.Method:Three patients was diagnosed with MMPRT by a history taking, physical examination and the use of non-contrast knee MRI. All of the patients underwent arthroscopic surgical reconstruction with 2 tunnel transtibial pullout repair (TPR) technique and the use of gracillis tendon autograft (3 mm in diameter, 6 cm in length) that is passed through the meniscus. The surgery was performed by one orthopaedic surgeon. Follow up was done at 3 month after the surgery. KOOS and VAS are used to assess both patient subjective outcome and pain score.Result and Discussion:Patients underwent reconstruction shows an improvement in KOOS and VAS score. The average improvement in total KOOS is 43% and VAS score is 6,3 points. From the study by Wendel et al. meniscus root reconstruction can provide reattachment to the anatomic footprint and cartilaginous graft to facilitate healing. Also in agreement with the study by Li et al arthroscopically assisted meniscus root reconstruction with gracillis autograft showed superior clinical outcome and meniscus root healing rate.Conclusion:Meniscus root reconstruction showed a promising result in managing patients with MMPRT. No serious complications are reported.

Abstract:Objectives: Meniscal root disruption results in a loss of hoop strain resistance, increased articular cartilage contact pressure, and acceleration of degenerative changes. There is limited data comparing the outcomes of similarly matched patients with a medial meniscus posterior root tear treated with non-operative management, partial meniscectomy, or repair. The purpose of this study is to compare treatment failure, clinical outcome scores, and radiographic findings for a matched cohort of patients who underwent either non-operative management, partial meniscectomy, or transtibial pull-through repair for a medial meniscus posterior root tear (MMPRT). Our hypothesis is that patients who underwent meniscus root repair will have lower rates of progression to arthroplasty than patients who were treated with non-operative management or partial meniscectomy. Methods: Patients who underwent transtibial medial meniscus posterior horn root repair were matched by meniscus laterality, age, sex and K-L grades to patients treated non-operatively or with a partial meniscectomy. Progression to arthroplasty rates, IKDC and Tegner scores, and radiographic outcomes were analyzed between groups. Results: Forty-five patients were included in this study (15 non-operative, 15 partial meniscectomy, 15 root repair). Progression to arthroplasty demonstrated significant differences among treatment groups at a mean 74 months (non-operative 4/15, partial meniscectomy 9/15, meniscus repair 0/15, p=.0003). The meniscus root repair group had significantly less arthritic progression, as measured by change in K-L grade from pre-op to post-op (non-operative 1.0, partial mensicectomy 1.1, and meniscus repair 0.1, p=.001). Conclusion: Meniscus root repair leads to significantly less arthritis progression and subsequent knee arthroplasty compared to non-operative management and partial meniscectomy in a demographically matched cohort.

Medial meniscus posterior root tear (MMPRT) refers to the avulsion or complete radial tear occurring within a distance of 1 cm from the posterior tibial attachment of the medial meniscus. Root tears result in the extrusion of the meniscus, which subsequently leads to elevated joint contact pressures. These condition increased pressures contribute to the rapid progression of joint space narrowing and the occurrence of subchondral bone edema that were indistinguishable from total meniscectomy.Studies have reported that MMPRT represents A substantial percentage of meniscus tears, accounting for approximately 10 to 20% of all meniscus tears diagnosed. MMPRT frequency was reportedly 21.5% in all arthroscopic surgeries for meniscus medial tear. Nevertheless, despite the significant role that these injuries play in the development of degenerative joint disease, they frequently remain undetected during the initial evaluation. MMPRT are relatively common among middle-aged adults, female sex, and higher BMI with clear onset of significant injury. This might be attributed to aging associated degenerative alteration that makes the meniscus more susceptible to root detachment.The primary objective of managing MMPRT is to mitigate the progression of osteoarthritis by reinstating optimal joint contact pressures and kinematics. Given the unfavourable results observed with conservative treatment, surgical repair is currently regarded as the preferred choice. This approach has demonstrated encouraging clinical outcomes specifically in patients who meet the appropriate indications. Patients undergoing medial meniscus root repair have better functional scores, less osteoarthritis progression, and less conversion to total knee replacement compared to meniscectomy. The most common repair method for MMPRT is using transtibial pull-out repair at anatomic footprint of the meniscus root. Nevertheless, the pull-out suture method exhibits a vulnerability at the location of meniscus suturing, where cuts frequently occur during cyclical loading. Additionally, this technique cannot restore the meniscus extrusion. Furthermore, directly repair of the meniscus to bone may lead to a biologically weak structure at the footprint of the meniscus root. Nonanatomical repair itself shows less tibiofemoral contact area and increased mean contact pressure compared to anatomic repair.Several additional procedures have been proposed to maximize the result of meniscus root repair, including peripheral meniscocapsular release, centralization suture, and root reconstruction to minimize meniscus extrusion. In case of a significant tear gap found, root reconstruction is recommended to address the gap.

Abstract:The management of meniscal root injuries has changed as biomechanical studies have demonstrated the importance of meniscal integrity in load distribution across the knee joint. Meniscal injury causes altered joint mechanics, which is postulated to be related to the onset of arthrosis. Arthroscopic meniscal root repair has been shown to restore more normal joint mechanics and is considered a treatment option in the appropriately indicated patient. Short- and midterm clinical results of meniscal root repair are promising, but long-term results are yet to be established. Herein, we review meniscal root injuries and repairs with respect to their anatomy, biomechanics, clinical diagnosis, treatment indications, operative techniques, potential complications, postoperative management, and clinical outcomes.

Abstract:Orthopaedic Journal of Sports Medicine, Volume 12, Issue 11, November 2024. Background:Meniscus root tears comprise 10% to 21% of all meniscal tears. These tears alter knee biomechanics, elevating contact pressure, akin to a meniscectomy. Consequently, they are linked to advanced joint degeneration and cartilage damage in the affected compartment.Purpose:To systematically evaluate and relate the current literature describing the diagnosis and treatment strategies for meniscus root tears.Study Design:Scoping review; Level of evidence, 4.Methods:This review was conducted following the guidelines of the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) extension for scoping reviews. Inclusion criteria encompassed English-language clinical and preclinical research, technical notes, and narrative reviews on meniscus root tears. Exclusion criteria were studies on patellar tendon rupture, studies on medial patellofemoral ligament rupture with additional knee joint ligament injuries, studies of patients <16 years old, and studies involving open fractures. The data were summarized using a descriptive analysis and a thematic analysis.Results:After 1425 articles were identified, 461 studies were included; 17% (n = 78) were case reports or case series, 15% (n = 71) were technical notes, 9% (n = 41) addressed aspects of diagnosis, 7% (n = 32 ) were narrative reviews, and 5% (n = 21) were systematic reviews or meta-analyses. Studies presenting original data comprised 57% (n = 262) of all included studies, and 97% of the studies were of evidence levels 3 to 5. Contributions were mainly from the United States (n = 123; 27%), Republic of Korea (n = 102; 22%), and Japan (n = 99; 21%). Many studies (n = 216; 47%) focused on the treatment and outcomes of meniscus root tears. A significant chronological surge in the quantity of studies addressing the diagnosis and treatment of meniscus root tears was evident, particularly over the past 3 years. Consensus was found regarding the definition of meniscus root tears, the advantages of early repair, and postoperative rehabilitation protocols. The variations in surgical techniques and operative strategies created the greatest amount of contention, along with clinical assessment and imaging modalities.Conclusion:High-level evidence studies for diagnosing and managing meniscus root tears were scarce. A consensus has yet to be reached regarding the role of concomitant osteotomy, comparison of repair techniques, the use of a centralization stitch, patient factors affecting outcomes, and long-term outcomes of nonoperative management.

Abstract:Objectives: To clarify the morphological features of normal meniscal roots on magnetic resonance (MR) imaging with an isotropic 3D proton density-weighted (PDW) sequence. Materials: 3D PDW MR was performed in sixty-two patients at 3 T before knee arthroscopy. MR images of 34 normal medial menisci and 33 intact lateral menisci confirmed by arthroscopy were retrospectively evaluated. MR signals, insertion sites, dimensions and courses of four meniscal roots were recorded. Results: The anterior root of medial meniscus (ARMM) was typically hypointense, while the posterior root of medial meniscus (PRMM) and the anterior root of lateral meniscus (ARLM) were prone to be hyperintense or showing a comblike signal, and the posterior root of lateral meniscus (PRLM) was usually hypointense or comblike on PDW MR images. ARMM and PRLM had more complex and diverse insertion patterns than ARLM and PRMM. There were significant statistical differences of the lengths, widths, heights and course angles among four meniscal roots (all P < 0.001). Conclusions: The signal intensity of each meniscal root can be hypointense, hyperintense, or comblike on 3D PDW MR images. ARMM and PRLM have more complex and diverse insertion patterns than ARLM and PRMM. The dimensions and courses of four meniscal roots all differ. © 2012 Elsevier Ireland Ltd. All rights reserved.
Root Repair Has Superior Radiological and Clinical Outcomes Than Partial Meniscectomy and Nonoperative Treatment in the Management of Meniscus Root Tears: A Systematic Review

Abstract:Purpose: To compile and analyze structural and clinical outcomes after meniscus root tear treatment as currently described in the literature. Methods: A review was conducted to identify studies published since 2011 on efficacy of repair, meniscectomy, and nonoperative management in the treatment of meniscus root tears. Patient cohorts were grouped into treatment categories, with medial and lateral root tears analyzed separately; data were collected on patient demographics, structural outcomes including joint space width, degree of medial meniscal extrusion, progression to total knee arthroplasty, and patient-reported outcome measures. Risk of bias was assessed using the MINORS (methodological index for non-randomized studies) criteria. Heterogeneity was measured using the I-statistic, and outcomes were summarized using forest plots without pooled means. Results: The 56 included studies comprised a total of 3,191 patients. Mean age among the included studies ranged from 24.6 to 65.6 years, whereas mean follow-up ranged from 12 to 125.9 months. Heterogeneity analysis identified significant differences between studies. Change in joint space width ranged from -2.4 to -0.6 mm (i.e., decreased space) after meniscectomy (n = 186) and -0.9 to -0.1 mm after root repair (n = 209); change in medial meniscal extrusion ranged from -0.6 to 6.5 mm after root repair (n = 521) and 0.2 to 4.2 mm after meniscectomy (n = 66); and event rate for total knee arthroplasty ranged from 0.00 to 0.22 after root repair (n = 205), 0.35 to 0.60 after meniscectomy (n = 53), and 0.27 to 0.35 after nonoperative treatment (n = 93). Root repair produced the greatest numerical increase in International Knee Documentation Committee and Lysholm scores of the 3 treatment arms. In addition, root repair improvements in Knee Injury and Osteoarthritis Outcome Score Pain (range: 22-32), Sports and Recreational Activities (range: 23-36), Quality of Life (range: 22-42), and Symptoms subscales (range: 10-19), in studies with low risk of bias. Conclusions: The literature reporting on the treatment of meniscus root tears is heterogenous and largely limited to Level III and IV studies. Current evidence suggests root repair may be the most effective treatment strategy in lessening joint space narrowing of the knee and producing improvements in patient-reported outcomes. Abstract:Meniscus tears are a common orthopedic pathology and planning a single, effective treatment is challenging. The diagnosis of meniscal tears requires detailed history-taking, physical examinations, special diagnostic tests, and most likely magnetic resonance imaging (MRI) to confirm the lesion. A good understanding of the meniscal structure including vascularity, zones, function, and affected movements with associated symptoms plays a crucial role in establishing an optimal management plan. A careful assessment of the patient's characteristics, comorbidities, post-repair rehabilitation, and patient's overall function and satisfaction are also important for ideal management. While conservative management is commonly implemented and the only option for certain patients, partial meniscectomy remains to be the most performed treatment procedure. However, partial meniscectomy is no longer the first-line therapy due to the limitation of certain patient characteristics and side effects in the long run. Instead, meniscal repair has been shown to have better long-term outcomes and is therefore recommended for all tears, especially for young patients with acute traumatic lesions. Tissue engineering has been of high interest in the current research with promising therapeutic results. This review critically evaluates and compares the management of meniscal tears with surgical versus comprehensive management using the current literature.
Increased Posterior Tibial Slope Is Associated With Increased Risk of Meniscal Root Tears: A Systematic Review

Abstract:The American Journal of Sports Medicine, Ahead of Print. Background:While increased posterior tibial slope (PTS) is an established risk factor for anterior cruciate ligament tears, the association between tibial slope and meniscal posterior root tears is not well-defined.Purpose:To summarize the available literature evaluating the association between PTS and meniscus root injuries compared with patients without root tears.Study Design:Systematic review; Level of evidence, 4.Methods:A literature search was performed using the Scopus, PubMed, and Embase databases. Human clinical studies evaluating the associations between the medial tibial slope (MTS), lateral tibial slope (LTS), lateral-to-medial (L-to-M) slope asymmetry, and the risk of meniscus root tears were included. Patients with medial meniscus posterior root tears (MMPRTs) and lateral meniscus posterior root tears (LMPRTs) were compared with a control group without root injury. Study quality was assessed using the methodological index for non-randomized studies criteria.Results:Ten studies with 1313 patients were included (884 patients with root tears; 429 controls). The LMPRT subgroup (n = 284) had a significantly greater LTS (mean ± SD, 7.3°± 1.5° vs 5.7°± 3.91°; P < .001), MTS (5.26°± 1.2° vs 4.8°± 1.25°; P < .001), and increased L-to-M asymmetry (2.3°± 1.3° vs 0.65°± 0.5°; P < .001) compared with controls. The MMPRT group (n = 600) had significantly increased MTS relative to controls (8.1°± 2.5° vs 4.3°± 0.7°; P < .001). Furthermore, there was a higher incidence of noncontact injuries (79.3%) and concomitant ramp lesions (56%) reported in patients with LMPRT.Conclusion:Increased MTS, LTS, and L-to-M slope asymmetry are associated with an increased risk of LMPRTs, while increased MTS is associated with MMPRTs. Surgeons should consider how proximal tibial anatomy increases the risk of meniscus root injury.

Abstract:Objectives: To determine the relationship between the severity of the tear gap resulting from medial meniscus posterior root (MMPR) tear and medial meniscal extrusion, cartilage, bone, and ligament lesions on MRI. Methods: A total of 133 patients with MMPR tear were retrospectively evaluated. The patients were divided into two groups according to the tear gap width as minor (≤ 4 mm) and widely (4 mm<) displaced. Medial meniscal extrusion and medial compartmental chondromalacia, bone and ligament lesions were analyzed. Results: There were 61 patients (56 women and 5 men) in the minor displaced group (mean age: 56.3 years, range: 29-82 years) and 72 patients (59 women, 13 men) in the widely displaced group (mean age: 53.2 years, range: 20-86 years). There was no significant difference in terms of age and sex (p = 0.31 and p = 0.09, respectively). The mean absolute extrusion was 3.51 mm (range: 1.5-5 mm) in the minor displaced group and 4.52 mm (range: 2.4-7.2 mm) in the widely displaced group (p < 0.001). High-grade medial femoral condylar chondromalacia was more common in the widely displaced group (p = 0.002). Osteophyte, bone marrow edema, and subchondral cyst in the medial compartment, and ligament injury were higher in the widely displaced group, but were not statistically significant (p > 0.05). Conclusion: The amount of medial meniscal extrusion and the prevalence of high-grade medial femoral condylar chondromalacia were found to be significantly more in patients with wider tear gap. Determining the amount of tear gap in the evaluation of root ligament tears on MRI is important to predict knee joint internal derangement.
Bony morphology of the knee and non-traumatic meniscal tears: Is there a role for meniscal impingement?

Abstract:ObjectiveMeniscal tears are an important cause of morbidity. The aim of this study was to examine the relationship between non-traumatic meniscal tears and the intrinsic bony morphology of the knee.MethodsA retrospective analysis of 160 knee MRI scans in 150 patients was carried out who met the following criteria: (a) age between 20 and 45 years, (b) no history of knee trauma, surgery, infection, metabolic bone disease, and (c) no collateral or cruciate ligamentous injury. The medial tibial slope (MTS), lateral tibial slope (LTS), medial tibial plateau depth (MTPD), and medial and lateral femoral condylar offset ratios were calculated. The anterior horn, body, and posterior horn of the menisci were graded as 0 (no tear), 1 and 2 (degenerative changes), or 3 (definitive tear). One-way ANOVA and linear regression was used for statistical analysis.ResultsIn patients with grade 3 tears of the posterior horn of the medial meniscus, there was a significant association with shallower MTS (p < 0.05), smaller medial femoral offset ratio (p < 0.05) and smaller lateral femoral offset ratio (p < 0.05). Patients with grade 3 tears of anterior horn of the lateral meniscus had a significant association with shallower LTS (p < 0.05). No significant association was seen between MTPD and meniscal tears.ConclusionsOur results suggest an association between bony morphology of the knee and non-traumatic meniscal tears. Shallower MTS and LTS may result in impingement of posterior horn of medial meniscus and anterior horn of lateral meniscus, respectively. Future kinematic studies will be needed to help confirm our findings.

Abstract:Purpose: To analyze the presenting symptoms and clinical examination findings of patients undergoing meniscal root repairs to aid physicians in diagnosing this injury. Methods: All patients undergoing isolated arthroscopic meniscal root repair from January 1, 2016, to September 1, 2021, were identified. Patients younger than the age of 40 years were excluded. Clinical notes were reviewed for presenting symptoms and physical examination findings. Preoperative radiographs were graded using the Kellgren-Lawrence (KL) scale for osteoarthritis. Root tears were graded using the radiographic criteria of Chung et al. and articular cartilage injury was graded using a validated system, AMADEUS (mean total Area Measurement And Depth & Underlying Structures). Results: In total, 221 patients met inclusion criteria; 65.6% of patients reported that their pain began after an acute injury, with 39.4% of patients reporting a "pop." On examination, an effusion was present in 71% of knees. McMurray test was reported positive in 85.5% and a positive hyperflexion test in 53.8% of knees. In total, 49.5% of knees were graded KL 1. 154 had medial root tears, 10 had lateral root tears, and 24 suffered both root tears. In total, 44.1% of tears occurred at the midsubstance of the root, with 28.0% occurring at the enthesis and 28.0% occurring at the root-posterior horn junction. The mean AMADEUS score was 94.4 ± 11.4. Conclusions: Although most patients reported pain began after acute injury, less than one-half reported hearing a "pop." When patients were evaluated, an effusion, positive McMurray test, and positive hyperflexion test were present in most meniscal root tears. Level of evidence: Level IV, diagnostic case series.

Abstract:The knee meniscus plays a key role in shock absorption, joint stability, and congruity at the knee joint. However, due to several reasons, meniscal tears are increasing that in turn causes osteoarthritis. The objective of the current study was to evaluate the stresses on the meniscus during kinematic activities and correlate them with the potential tears observed clinically. From the MRI data of a healthy volunteer, 3D models of the menisci were segmented in Mimics®, post-processed in 3-matics®, meshed in HyperMesh® and were analyzed in Abaqus® for stresses arising due to compressive loads from both femoral and tibial ends; under various angles of flexion. The critical regions with higher stress distribution were correlated with the potential meniscal tears. It was observed that the stresses were higher when loads were applied from the tibial end than femoral end. Though the loads were applied proportional to the area, higher stresses were observed in the lateral meniscus. Results of the study show that at higher knee flexion angles, loading from tibial end leads to longitudinal tear in region E1–E2 (as per Cooper's Classification) on the lateral meniscus and horizontal tear in A-1 region of the medial meniscus. Longitudinal, horizontal, and root detachment tears reported in the literature, coincide with our predictions. The novelty of the study is the estimation of stresses due to forces from the tibial end and the correlation of the stresses to the type of meniscal tears observed clinically.

Abstract:Orthopaedic Journal of Sports Medicine, Volume 12, Issue 11_suppl4, November 2024. ObjectivesTo investigate changes in external moments in medial meniscus posterior root tear (MMPRT) patients, and the effect of medial meniscus posterior root repair (MMPRR) in restoring normal kinetics.MethodsWe present preliminary findings from six subjects with MMPRT, four with MMPRR, and three healthy age-matched control participants (CON). Participants walked on level ground at a self-selected gait speed. Data were collected using high-speed cameras and ground-embedded force plates. Peak knee moments for flexion-extension (KFM and KEM), abduction-adduction (KAbM, KAdM), and internal-external rotation (KERM, KIRM) were calculated using inverse dynamics during the stance phase of gait, and normalized to body mass and height. Group comparisons were conducted using the Kruskal-Wallis and Dunn test with Bonferroni correction. Results were considered statistically significant if p-values were less than 0.05.ResultsBoth MMPRT and MMPRR groups exhibited significantly lower KFM relative to the CON group, suggesting a strategy to reduce quadriceps activation in both patient groups, which may persist after surgery. MMPRT patients showed significantly diminished KEM relative to both MMR and CON groups, indicating a strategy to reduce external knee forces following injury. MMPRR patients presented KEM that resembled the healthy control group, showing that repair surgery could potentially restore this gait variable. KAbM was statistically higher in the MMPRR and CON groups relative to MMPRT patients, which presented lower KAM than the other groups, suggesting a protective strategy to offload the medial compartment. KERM were significantly higher in the CON group compared to the MMT group, suggesting a protective strategy to avoid excessive external rotation by the MMT group. No significant differences were observed in KIRM across the three groups.ConclusionMMPRT leads to alterations in gait kinetics, compatible with a protective strategy to unload the knee and compensate for increased medial compartment contact pressures, which are partially restored following root repair.
