Abstract:Purpose Adenoid hypertrophy is a pathological hyperplasia of adenoids and may cause snoring, apnea, and impede breathing during sleep. In clinical practice, radiologists diagnose the severity of adenoid hypertrophy by measuring the ratio of adenoid width (A) to nasopharyngeal width (N) according to the lateral cephalogram, which indicates the locations of four keypoints. The entire diagnostic process is tedious and time-consuming due to the acquisition of A and N. Thus, there is an urgent need to develop computer-aided diagnostic tools for adenoid hypertrophy. Methods In this paper, we first propose the use of deep learning to solve the problem of adenoid hypertrophy classification. Deep learning driven by big data has developed greatly in the image processing field. However, obtaining a large amount of training data is hard, making the application of deep learning to medical images more difficult. This paper proposes a keypoint localization method to incorporate more prior information to improve the performance of the model under limited data. Furthermore, we design a novel regularized term called VerticalLoss to capture the vertical relationship between keypoints to provide prior information to strengthen the network performance. Results To evaluate the performance of our proposed method, we conducted experiments with a clinical dataset from the First Affiliated Hospital of Anhui Medical University consisting of a total of 688 patients. As our results show, we obtained a classification accuracy of 95.6%, a macro F1-score of 0.957, and an average AN ratio error of 0.026. Furthermore, we obtained a macro F1-score of 0.89, a classification accuracy of 94%, and an average AN ratio error of 0.027 while using only half of the data for training. Conclusions The study shows that our proposed method can achieve satisfactory results in the task of adenoid hypertrophy classification. Our approach incorporates more prior information, which is especially important in the field of medical imaging, where it is difficult to obtain large amounts of training data.

Abstract:Adenoid hypertrophy can lead to adenoidal mouth breathing, which can result in "adenoid face" and, in severe cases, can even lead to respiratory tract obstruction. The Fujioka ratio method, which calculates the ratio of adenoid (A) to nasopharyngeal (N) space in an adenoidal-cephalogram (A/N), is a well-recognized and effective technique for detecting adenoid hypertrophy. However, this process is time-consuming and relies on personal experience, so a fully automated and standardized method needs to be designed. Most of the current deep learning-based methods for automatic diagnosis of adenoids are CNN-based methods, which are more sensitive to features similar to adenoids in lateral views and can affect the final localization results. In this study, we designed a local attention-based method for automatic diagnosis of adenoids, which takes AdeBlock as the basic module, fuses the spatial and channel information of adenoids through two-branch local attention computation, and combines the downsampling method without losing spatial information. Our method achieved mean squared error (MSE) 0.0023, mean radial error (MRE) 1.91, and SD (standard deviation) 7.64 on the three hospital datasets, outperforming other comparative methods.
Automated Adenoid Hypertrophy Assessment with Lateral Cephalometry in Children Based on Artificial Intelligence

Abstract:Adenoid hypertrophy may lead to pediatric obstructive sleep apnea and mouth breathing. The routine screening of adenoid hypertrophy in dental practice is helpful for preventing relevant craniofacial and systemic consequences. The purpose of this study was to develop an automated assessment tool for adenoid hypertrophy based on artificial intelligence. A clinical dataset containing 581 lateral cephalograms was used to train the convolutional neural network (CNN). According to Fujioka’s method for adenoid hypertrophy assessment, the regions of interest were defined with four keypoint landmarks. The adenoid ratio based on the four landmarks was used for adenoid hypertrophy assessment. Another dataset consisting of 160 patients’ lateral cephalograms were used for evaluating the performance of the network. Diagnostic performance was evaluated with statistical analysis. The developed system exhibited high sensitivity (0.906, 95% confidence interval [CI]: 0.750–0.980), specificity (0.938, 95% CI: 0.881–0.973) and accuracy (0.919, 95% CI: 0.877–0.961) for adenoid hypertrophy assessment. The area under the receiver operating characteristic curve was 0.987 (95% CI: 0.974–1.000). These results indicated the proposed assessment system is able to assess AH accurately. The CNN-incorporated system showed high accuracy and stability in the detection of adenoid hypertrophy from children’ lateral cephalograms, implying the feasibility of automated adenoid hypertrophy screening utilizing a deep neural network model.

Abstract:Adenoid hypertrophy is a pathological condition characterized by the enlargement of the adenoids in children, which may lead to various problems. The conventional manual measurement is time-consuming and subject to subjective errors. Previous automated methods based on landmark detection in X-ray images have shown reliability. However, these methods neglect global features, making it difficult to locate landmarks accurately and thus affecting adenoid-to-nasopharyngeal (AN) ratio estimation when migrating to MRI images. In this paper, we first apply a deep-learning method to automatically assess adenoid hypertrophy in MRI images. We propose an adenoid network (ADNet) to capture local and global features near landmarks to achieve accurate landmark localization. Specifically, ADNet uses an encoder-decoder architecture where we employ a depthwise separable convolution-based encoder to extract local features and then employ an adaptive convolution-based decoder to capture global features. We collected a dataset of 500 cephalometric MRI images to train and evaluate the performance of the proposed model. Our experimental results demonstrate that our network achieves state-of-the-art performance with an average radial error of 4.85 pixels for landmark detection, an average successful detection rate of 96% within 15 pixels, and an error of 0.026 for the calculated AN ratio.

Abstract:Abstract Purpose Adenotonsillar hypertrophy is considered as one of the primary causes of pediatric obstructive sleep apnea. In clinical practice, pediatric patients with potential obstructive apnea should undergo assessment of upper airway obstruction and adenotonsillar size. As lack of well-trained physicians in China, large numbers of such patients could not be accurately evaluated in time. We attempted to find a better way to assess upper airway obstruction. Methods We developed a computational method which may allow clinicians to trace upper airway obstruction. We utilized a Detectron2 Mask R-CNN architecture that was pretrained on ImageNet-1k dataset. We then trained it on our COCO dataset using transfer learning for segmentations of upper airway and related key structures. With the instance segmentations, we utilized a tracing algorithm developed by our own to sketch the contours of key segments. At the end, our system would use the traced landmarks to calculate the targeted clinical data. Results We validated the effective-ness of the algorithm in two steps. First, we used a validation set (COCO dataset) to evaluate the segmentation performance of our system. Our system achieved a mean segmentation AP of 61.30, with airway segmentation AP of 55.64 and cranial base segmentation AP of 66.96. In addition, the AP 50 was 99.49 and AP 75 was 73.62. Second,we analyzed the data from our system and experts using a single rater, absolute-agreement, 2-way mixed-effects model, and got ICC value of 0.73 with 95% confident interval = 0.63-0.81. Conclusions In this study, we created a deep-learning based system to help clinicians evaluate upper airway in lateral cephalometric radiographs which we believe could improve clinical practice. With the evolving of technology, our system would become more integrated into medical care of OSA, freeing the clinical practitioners from repetitive tasks and enabling them to concentrate on improved patient care.

Abstract:Adenoid hypertrophy (AH) is a common cause of airway obstruction in children, which is the etiology of changes in facial growth, obstructive sleep apnea syndrome, and/or more serious problems. Most sequelae can be avoided or reversed by timely diagnosis and treatment. It is difficult to frequently assess the changes in adenoids among children using the commonly available diagnostic tools owing to invasiveness and radiation exposure. A new method called respirdynamicsgram (RDG) is proposed for the assessment and diagnosis of AH. Specifically, RDG is generated by visualizing the respiratory dynamics extracted from nasal airflow through dynamic learning. The morphology of RDG remarkably differs between children with adenoidal hypertrophy (irregular shapes) and healthy children (regular butterfly-like shapes). Three features are utilized to represent the morphology of RDG, including temporal heterogeneity, spatial heterogeneity, and wing shape. These morphology features of RDG are fed into the linear support vector machine for the AH classification task. As a result, the proposed method achieves a mean accuracy of 86.6% over 100 resample folds on a total of 74 subjects. It is shown that RDG has the capability of distinguishing children with AH from healthy children. As it is simple, non-invasive, and secure, it is hopeful that RDG may become an effective method for timely and frequent assessment and diagnosis of AH.

Abstract:Physiognomy has long been recorded in ancient Greece and ancient China. It predicts a person's character and health through facial features because some traits of diseases may illustrate in face. Based on this, we apply a multidisciplinary method to investigate face appearance in photograph, identify adenoidal face, and early treatment in nasal respiratory obstruction. By using computer vision in feature selection, we identified most salient feature points of adenoid face including lip thickness, inner and outer eye distances. Through machine learning techniques, predictive models are constructed to discriminate adenoid face and non-adenoid face. The model-based analytical methods this article employed included decision tree, support vector machines, KNN and XGBoost. The reliability of forecasts was assessed by 5-fold cross validation. Two specific challenges were addressed in the study: Challenge 1, solve the problem of head orientation and different illumination direction; Challenge 2, identify relevant facial prediction features which could be convert into regression problem; Our research suggests that, compared to other approaches, computer vision feature selection provides a more reliable outcome forecasting of adenoids face, for example with a best specificity 89.19%, and sensitivity 88.24%.
MIB-ANet: A novel multi-scale deep network for nasal endoscopy-based adenoid hypertrophy grading

Abstract:Introduction To develop a novel deep learning model to automatically grade adenoid hypertrophy, based on nasal endoscopy, and asses its performance with that of E.N.T. clinicians. Methods A total of 3,179 nasoendoscopic images, including 4-grade adenoid hypertrophy (Parikh grading standard, 2006), were collected to develop and test deep neural networks. MIB-ANet, a novel multi-scale grading network, was created for adenoid hypertrophy grading. A comparison between MIB-ANet and E.N.T. clinicians was conducted. Results In the SYSU-SZU-EA Dataset, the MIB-ANet achieved 0.76251 F1 score and 0.76807 accuracy, and showed the best classification performance among all of the networks. The visualized heatmaps show that MIB-ANet can detect whether adenoid contact with adjacent tissues, which was interpretable for clinical decision. MIB-ANet achieved at least 6.38% higher F1 score and 4.31% higher accuracy than the junior E.N.T. clinician, with much higher (80× faster) diagnosing speed. Discussion The novel multi-scale grading network MIB-ANet, designed for adenoid hypertrophy, achieved better classification performance than four classical CNNs and the junior E.N.T. clinician. Nonetheless, further studies are required to improve the accuracy of MIB-ANet.

Abstract:BACKGROUND: Training deep learning (DL) models to automatically recognize diseases in nasopharyngeal MRI is a challenging task, and optimizing the performance of DL models is difficult.PURPOSE: To develop a method of training anatomical partition-based DL model which integrates knowledge of clinical anatomical regions in otorhinolaryngology to automatically recognize diseases in nasopharyngeal MRI.STUDY TYPE: Single-center retrospective study.POPULATION: A total of 2485 patients with nasopharyngeal diseases (age range 14-82 years, female, 779[31.3%]) and 600 people with normal nasopharynx (age range 18-78 years, female, 281[46.8%]) were included.SEQUENCE: 3.0 T; T2WI fast spin-echo sequence.ASSESSMENT: Full images (512 × 512) of 3085 patients constituted 100% of the dataset, 50% and 25% of which were randomly retained as two new datasets. Two new series of images (seg112 image [112 × 112] and seg224 image [224 × 224]) were automatically generated by a segmentation model. Four pretrained neural networks for nasopharyngeal diseases classification were trained under the nine datasets (full image, seg112 image, and seg224 image, each with 100% dataset, 50% dataset, and 25% dataset).STATISTICAL TESTS: The receiver operating characteristic curve was used to evaluate the performance of the models. Analysis of variance was used to compare the performance of the models built with different datasets. Statistical significance was set at P < 0.05.RESULTS: When the 100% dataset was used for training, the performances of the models trained with the seg112 images (average area under the curve [aAUC] 0.949 ± 0.052), seg224 images (aAUC 0.948 ± 0.053), and full images (aAUC 0.935 ± 0.053) were similar (P = 0.611). When the 25% dataset was used for training, the mean aAUC of the models that were trained with seg112 images (0.823 ± 0.116) and seg224 images (0.765 ± 0.155) was significantly higher than the models that were trained with full images (0.640 ± 0.154).DATA CONCLUSION: The proposed method can potentially improve the performance of the DL model for automatic recognition of diseases in nasopharyngeal MRI.LEVEL OF EVIDENCE: 4 TECHNICAL EFFICACY STAGE: 1.

Abstract:Objective:The purpose of this study was to evaluate the methodology of 2D and 3D image measurement of adenoid and upper airway.Methods:The subjects were patients who had previously taken lateral cephalograms and cone-beam computed tomography (CBCT) images due to impacted teeth or temporomandibular joint disease before orthodontic treatment. Thirty patients met the inclusion criteria, of which 18 were girls, 12 were boys, and the age range was 7 to 14 years. Measurements of adenoid (A), nasopharynx (N), R-PNS and A/N were evaluated on cephalometrics; and A, N and A/N were measured on mid-sagittal, cross-sectional and coronal section on CBCT. The accuracy of measuring adenoid and upper airway on lateral cephalogram was evaluated.Results:The values of A, N, A/N measured in lateral cephalogram(13.01±3.04 mm、26.50±3.05 mm、0.49±0.10) were not significantly different from those measured on mid-sagittal plane of CBCT(13.29±3.42 mm, 26.62±2.19 mm, 0.50±0.11, P>0.05), but were different on axial(15.36±4.87 mm、28.54±4.65 mm、0.54±0.10, P<0.01) and coronal plane(A: 16.55±4.31 mm, P<0.01), nonetheless there remained a good consistency( P<0.05.)Measurements of nasopharynx, velopharynx, and lower pharynx on cephalometric distance were relative with volume and cross-sectional area on CBCT( P<0.05). Conclusions:To a certain extent, lateral cephalometric can reflect the size of adenoid and upper airway.
Automatic detection of temporomandibular joint osteoarthritis radiographic features using deep learning artificial intelligence. A Diagnostic accuracy study

Abstract:Objective: The purpose of this study was to investigate the diagnostic performance of a neural network Artificial Intelligence model for the radiographic confirmation of Temporomandibular Joint Osteoarthritis in reference to an experienced radiologist. Materials and methods: The diagnostic performance of an AI model in identifying radiographic features in patients with TMJ-OA was evaluated in a diagnostic accuracy cohort study. Adult patients elected for radiographic examination by the Diagnostic Criteria for Temporomandibular Disorders decision tree were included. Cone-beam computed Tomography images were evaluated by object detection YOLO deep learning model. The diagnostic performance was verified against examiner radiographic evaluation. Results: The differences between the AI model and examiner were non-significant statistically, except in the subcortical cyst (P=0.049*). AI model showed substantial to near-perfect levels of agreement when compared to those of the examiner data. Regarding each radiographic phenotype, the AI model reported favorable sensitivity, specificity, accuracy, and highly statistically significant Receiver Operating Characteristic (ROC) analysis (p<0.001). Area Under Curve ranged from 0.872, for surface erosion, to 0.911 for subcortical cyst. Conclusion: AI object detection model could open the horizon for a valid, automated, and convenient modality for TMJ-OA radiographic confirmation and radiomic features identification with a significant diagnostic power.

Abstract:Background. The differential diagnosis of subcentimetre lung nodules with a diameter of less than 1 cm has always been one of the problems of imaging doctors and thoracic surgeons. We plan to create a deep learning model for the diagnosis of pulmonary nodules in a simple method. Methods. Image data and pathological diagnosis of patients come from the First Affiliated Hospital of Zhejiang University School of Medicine from October 1, 2016, to October 1, 2019. After data preprocessing and data augmentation, the training set is used to train the model. The test set is used to evaluate the trained model. At the same time, the clinician will also diagnose the test set. Results. A total of 2,295 images of 496 lung nodules and their corresponding pathological diagnosis were selected as a training set and test set. After data augmentation, the number of training set images reached 12,510 images, including 6,648 malignant nodular images and 5,862 benign nodular images. The area under the P-R curve of the trained model is 0.836 in the classification of malignant and benign nodules. The area under the ROC curve of the trained model is 0.896 (95% CI: 78.96%~100.18%), which is higher than that of three doctors. However, the P value is not less than 0.05. Conclusion. With the help of an automatic machine learning system, clinicians can create a deep learning pulmonary nodule pathology classification model without the help of deep learning experts. The diagnostic efficiency of this model is not inferior to that of the clinician.
Graphical user interface-based convolutional neural network models for detecting nasopalatine duct cysts using panoramic radiography

Abstract:Nasopalatine duct cysts are difficult to detect on panoramic radiographs due to obstructive shadows and are often overlooked. Therefore, sensitive detection using panoramic radiography is clinically important. This study aimed to create a trained model to detect nasopalatine duct cysts from panoramic radiographs in a graphical user interface-based environment. This study was conducted on panoramic radiographs and CT images of 115 patients with nasopalatine duct cysts. As controls, 230 age- and sex-matched patients without cysts were selected from the same database. The 345 pre-processed panoramic radiographs were divided into 216 training data sets, 54 validation data sets, and 75 test data sets. Deep learning was performed for 400 epochs using pretrained-LeNet and pretrained-VGG16 as the convolutional neural networks to classify the cysts. The deep learning system's accuracy, sensitivity, and specificity using LeNet and VGG16 were calculated. LeNet and VGG16 showed an accuracy rate of 85.3% and 88.0%, respectively. A simple deep learning method using a graphical user interface-based Windows machine was able to create a trained model to detect nasopalatine duct cysts from panoramic radiographs, and may be used to prevent such cysts being overlooked during imaging.

Influences of Adenoid Hypertrophy on Children’s Maxillofacial Development

Abstract:This study aims to investigate the association between adenoid hypertrophy and facial development. A total of 388 children aged 1–13 years old who had undergone head MRI in Foshan Maternal and Child Health Hospital were collected, including 196 hypertrophic cases and 192 normal cases. The maxillofacial soft tissue indicators were measured and compared. The A/N ratio and adenoid thickness consistently increased with age in the hypertrophic group and the A/N ratio reached a maximum value three years earlier than the normal group. The pharyngeal airway space, vallecula of epiglottis to anterior plane distance of the third/fourth cervical vertebrae, angle of convexity, total angle of convexity, and the nasolabial angle in the hypertrophy group were smaller than those in the control group (p < 0.05). The thickness of adenoids, palate height, palate length, and tongue length in the hypertrophy group exceeded that of the control group (p < 0.05). To conclude, adenoid hypertrophy was associated with craniofacial features such as a convex facial profile, a narrowed nasopharyngeal airway, an elongated and heightened palate, a lengthened tongue or a lower tongue position. Abstract:Objective: The main objectives of the study were to investigate the reliability and accuracy of cone-beam computed tomography (CBCT) in the diagnosis of adenoid hypertrophy in Chinese children and to evaluate its value in clinical diagnosis.Methods: From January 2019 to January 2020, 300 children with sleep snoring in Xinhua Hospital Affiliated to Medical College of Shanghai Jiaotong University were retrospectively studied. All patients underwent nasopharyngoscopy (NE) and CBCT scanning. The sensitivity, specificity, positive predictive value, negative predictive value, positive likelihood ratio, and negative likelihood ratio of CBCT were determined according to the diagnostic criteria of NE, and the consistency between CBCT and NE was evaluated.Results: The clinical study of 300 children patients found that compared with NE, CBCT had a sensitivity of 87.3%, specificity of 89.2%, the positive predictive value of 93.20%, the negative predictive value of 80.5%, the positive likelihood ratio of 8.08, the negative likelihood ratio of 0.14, and Kappa value of .748.Conclusion: CBCT is a reliable and accurate tool for the diagnosis of adenoid hypertrophy and can be used as an alternative examination method for children with contraindications or intolerance during NE.Level of Evidence: 4.

Abstract:Pituitary microadenoma (PM) is often difficult to detect by MR imaging alone. We employed a computer-aided PM diagnosis (PM-CAD) system based on deep learning to assist radiologists in clinical workflow. We enrolled 1,228 participants and stratified into 3 non-overlapping cohorts for training, validation and testing purposes. Our PM-CAD system outperformed 6 existing established convolutional neural network models for detection of PM. In test dataset, diagnostic accuracy of PM-CAD system was comparable to radiologists with > 10 years of professional expertise (94% versus 95%). The diagnostic accuracy in internal and external dataset was 94% and 90%, respectively. Importantly, PM-CAD system detected the presence of PM that had been previously misdiagnosed by radiologists. This is the first report showing that PM-CAD system is a viable tool for detecting PM. Our results suggest that PM-CAD system is applicable to radiology departments, especially in primary health care institutions.

Abstract:To improve the diagnostic accuracy of adenoid hypertrophy (AH) in children and prevent further complications in time, it is important to study and quantify the effects of different degrees of AH on pediatric upper airway (UA) aerodynamics. In this study, based on computed tomography (CT) scans of a child with AH, UA models with different degrees of obstruction (adenoidal-nasopharyngeal (AN) ratio of 0.9, 0.8, 0.7, and 0.6) and no obstruction (AN ratio of 0.5) were constructed through virtual surgery to quantitatively analyze the aerodynamic characteristics of UA with different degrees of obstruction in terms of the peak velocity, pressure drop (△P), and maximum wall shear stress (WSS). We found that two obvious whirlpools are formed in the anterior upper part of the pediatric nasal cavity and in the oropharynx, which is caused by the sudden increase in the nasal cross-section area, resulting in local flow separation and counterflow. In addition, when the AN ratio was ≥ 0.7, the airflow velocity peaked at the protruding area in the nasopharynx, with an increase 1.1–2.7 times greater than that in the nasal valve area; the △P in the nasopharynx was significantly increased, with an increase 1.1–6.8 times greater than that in the nasal cavity; and the maximum WSS of the posterior wall of the nasopharynx was 1.1–4.4 times larger than that of the nasal cavity. The results showed that the size of the adenoid plays an important role in the patency of the pediatric UA.

A Deep Learning Based Smartphone Application for Early Detection of Nasopharyngeal Carcinoma Using Endoscopic Images

Abstract:Nasal endoscopy is crucial for the early detection of nasopharyngeal carcinoma (NPC), but its accuracy relies significantly on the physician's expertise, posing challenges for primary healthcare providers. Here, we retrospectively analysed 39,340 nasal endoscopic white-light images from three high-incidence NPC centres, utilising eight advanced deep learning models to develop an Internet-enabled smartphone application, "Nose-Keeper", that can be used for early detection of NPC and five prevalent nasal conditions and assessment of healthy individuals. Our App demonstrated a remarkable overall accuracy of 92.27% (95% Confidence Interval (CI): 90.66%-93.61%). Notably, its sensitivity and specificity in NPC detection achieved 96.39% and 99.91%, respectively, outperforming nine experienced otolaryngologists. Explainable artificial intelligence was employed to highlight key lesion areas, improving Nose-Keeper's decision-making accuracy and safety. Nose-Keeper can assist primary healthcare providers in diagnosing NPC and related diseases efficiently, offering a valuable resource for people in high-incidence NPC regions to manage nasal cavity health effectively.
