Nuclear Medicine and Wall Street: An Evolving Relationship

Abstract:Until recently, it has been challenging to engage Wall Street and large pharmaceutical companies in radiopharmaceutical opportunities. The modest economic prospects of most diagnostic radiopharmaceuticals have not attracted keen interest from the broader business community, despite the rapid advancement of diagnostic imaging capabilities and their increasingly crucial role in the therapeutic process. Similarly, compelling science supporting select radiopharmaceutical therapies in oncology has been overshadowed by the unique challenges posed by this class of drugs and historical commercial failures that serve as sobering reminders of risk. Fortunately, a few notable successes in the targeted radioligand therapeutic space are changing this dynamic, fueling a new flow of investor capital into these technologies and inciting increased merger and acquisition activity that has yielded significant value creation for investors. If the nuclear medicine industry is able to continue to effectively manage historical challenges, then there is significant opportunity for a new and promising wave of radioligand therapies to significantly change the oncology treatment paradigm and elevate the profile of the entire nuclear medicine sector.

A significant focus is placed on the crucial role of radiopharmaceuticals in medical imaging and theranostics, highlighting their contribution to diagnostic accuracy and image-guided curative interventions. Abstract:Radiotracers, or radiopharmaceuticals, are bioactive molecules tagged with a radionuclide used for diagnostic imaging or radiotherapy and, when a positron-emitting radionuclide is chosen, the radiotracers are used for PET imaging. The development of novel PET radiotracers in many ways parallels the development of new pharmaceuticals, and small molecules dominate research and development pipelines in both disciplines. The 4 decades since the introduction of [<sup>18</sup>F]FDG have seen the development of many small molecule PET radiotracers. Ten have been approved by the US Food and Drug Administration as of 2016, whereas hundreds more are being evaluated clinically. These radiotracers are being used in personalized medicine and to support drug discovery programs where they are greatly improving our understanding of and ability to treat diseases across many areas of medicine including neuroscience, cardiovascular medicine, and oncology.

Abstract:Radiation oncologists, radiopharmacists, nuclear medicine physicians, and medical oncologists have seen a renewed clinical interest in radiopharmaceuticals for the curative or the palliative treatment of cancer. To allow for the discovery and the clinical advancement of targeted radiopharmaceuticals, these stakeholders have reformed their trial efforts and remodeled their facilities to accommodate the obligations of a program centered upon radioactive investigational drug products. Now considered informally as drugs and not beam radiotherapy, radiopharmaceuticals can be more easily studied in the traditional clinical trial enterprise ranging from phase 0–I to phase III studies. Resources and physical facilities allocated to radiopharmaceuticals have brought forth new logistics and patient experience for safe and satisfactory drug delivery. The clinical use of theranostic agents—that is, diagnostic and therapeutic radionuclide pairs—has accelerated radiopharmaceutical development.

Abstract:Abstract This review provides an essential overview on the progress of rapidly-developing China’s radiopharmaceuticals in recent years (2014–2021). Our discussion reflects on efforts to develop potential, preclinical, and in-clinical radiopharmaceuticals including the following areas: (1) brain imaging agents, (2) cardiovascular imaging agents, (3) infection and inflammation imaging agents, (4) tumor radiopharmaceuticals, and (5) boron delivery agents (a class of radiopharmaceutical prodrug) for neutron capture therapy. Especially, the progress in basic research, including new radiolabeling methodology, is highlighted from a standpoint of radiopharmaceutical chemistry. Meanwhile, we briefly reflect on the recent major events related to radiopharmaceuticals along with the distribution of major R&D forces (universities, institutions, facilities, and companies), clinical study status, and national regulatory supports. We conclude with a brief commentary on remaining limitations and emerging opportunities for China’s radiopharmaceuticals.

Abstract:Purpose: Health-system pharmacists play a crucial role in monitoring the pharmaceutical pipeline to manage formularies, allocate resources, and optimize clinical programs for new therapies. This article aims to support pharmacists by sharing new and anticipated novel drug approvals. Summary: Selected drug approvals anticipated in the 12-month period covering the third quarter of 2024 through the second quarter of 2025 are reviewed. The analysis emphasizes drugs expected to have significant clinical and financial impact in hospitals and clinics selected from 54 novel drugs awaiting US Food and Drug Administration approval. New cell therapies for treating cancers continue to enter the drug pipeline, while novel targeted therapies for biliary tract, gastric, pancreatic, and breast cancers, as well as 3 subcutaneous versions of already approved drugs given intravenously, are awaiting approval. Additionally, many novel drugs are being developed for treatment of rare and ultra-rare diseases such as hereditary angioedema, macular telangiectasia, congenital adrenal hyperplasia, and Barth syndrome. Two new subcutaneous drugs for hemophilia, a new oral medication for hereditary angioedema, a novel monoclonal antibody for atopic dermatitis, and the first oral penem antibiotic are also in the pipeline. Conclusion: New drugs with various indications for cancers and rare diseases continue to strengthen the drug pipeline.

Abstract:Radiopharmaceuticals are of growing interests in the development of theranostic agents and personalized medicine because of their unique advantages. Diagnosis (via molecular imaging) and treatment (via targeted radionuclide therapy) can be achieved simultaneously by altering the radionuclides with minimum effect on the pharmacokinetics of the agent, or by administration at different doses. Theranostic radiopharmaceuticals have advanced both preclinically and clinically, but methods for the production, purification, and analyses of these nuclides remain to be developed and optimized to support growing clinical trials. Herein, we summarized the production, preclinical and clinical development of several theranostic radionuclides and α-particle-emitting radionuclides. We focused on the progress and unmet need in the radiochemistry and analytical chemistry development during the clinical translation of radiopharmaceuticals, as efforts are required to provide guidance in dosimetry and quality control of radiopharmaceuticals in clinical applications.

Abstract:Molecular Imaging is entering the most fruitful, exciting period in its history with many new agents under development, and several reaching the clinic in recent years. While it is unusual for just one laboratory to take an agent from initial discovery through to full clinical approval the steps along the way are important to understand for all interested participants even if one is not involved in the entire process. Here, we provide an overview of these processes beginning at discovery and preclinical validation of a new molecular imaging agent and using as an exemplar a low molecular weight disease-specific targeted positron emission tomography (PET) agent. Compared to standard drug development requirements, molecular imaging agents may benefit from a regulatory standpoint from their low mass administered doses, they nonetheless still need to go through a series of well-defined steps before they can be considered for Phase 1 human testing. After outlining the discovery and preclinical validation approaches, we will also discuss the nuances of Phase 1, Phase 2 and Phase 3 studies that may culminate in an FDA general use approval. Finally, some post-approval aspects of novel molecular imaging agents are considered.

Abstract:Key PointsDrug development is a costly, time-intensive and high-risk endeavour, with only two to three approvals for novel therapeutics in new drug classes per year eventually making it to market. New strategies are required to identify promising new drug candidates early on and, likewise, to terminate those candidates that are unlikely to be successful, thus allowing a more rapid and efficient move to pivotal trials.Molecular imaging attempts to characterize and quantify biological processes at the cellular and subcellular level in intact living subjects. It exploits specific molecular probes and intrinsic tissue characteristics as the source of image contrast, providing the potential for understanding of integrative biology, earlier detection and characterization of disease, and evaluation of treatment.As most molecular imaging techniques are routine in clinical radiology departments and have counterparts in the experimental research setting, it is possible to design preclinical experiments that not only predict clinical imaging observations but also provide a mechanistic understanding of the observed biological response.Molecular imaging has the potential to have a significant impact on different phases of drug development, including target expression, compound screening and optimization, as well as Phases I to III clinical studies. Examples are detailed in the text of the article.The option of an exploratory IND (eIND) initiated by the US Food and Drug Administration enables first-in-human molecular imaging studies to be performed with reduced preclinical support as compared to that required for a full IND.Consortia such as the Alzheimer's Disease Neuroimaging Initiative, the American College of Radiology Imaging Network and the High-Risk Plaque Initiative seek to correlate a number of imaging biomarkers with the clinical manifestations of diseases and, if successful, will greatly increase the inclusion of these imaging techniques in exploratory clinical development of novel therapeutics.

Abstract:The pharmaceutical industry plays a crucial role in China's national economy. China's domestic pharmaceutical market is huge and has great development potential. However, at present, the scale and development level of Chinese pharmaceutical enterprises are uneven, and their international competitiveness is weak. In this paper, China Resources Medicine is selected as the research sample, with a specific focus on analyzing continuous mergers and acquisitions initiated by China Resources Medicine during 2016-2021. It is concluded that pharmaceutical enterprises need to allocate resources reasonably and utilize synergistic effect to give full play to their advantages. The identification of risks and deep exploration of potential should be carried out simultaneously, and the blind integration of the industrial chain should be avoided, so as to provide effective reference value for China Resources Pharmaceutical and other related pharmaceutical enterprises in the future development and merger and acquisition activities.

Abstract:Single-domain antibody (sdAb) is among the most promising vectors for developing molecular imaging tracers. Several sdAb tracers tar-geting human epidermal growth factor receptor 2 or programmed death ligand 1 have entered clinical practice. However, radiolabeled single-valent sdAbs generally have high kidney retention, limiting their therapeutic applications. Therefore, engineering strategies such as PEGylation or incorporation of renal cleavable linkers can be adapted to improve pharmacokinetics and reduce kidney retention. In this Focus on Molecular Imaging review, we try to summarize the latest developments in sdAb-derived agents and propose potential strate-gies that can be used to improve the theranostic value of radiolabeled sdAbs.

Abstract:Nuclear medicine plays an irreplaceable role in the diagnosis and treatment of tumors. Radiopharmaceuticals are important components of nuclear medicine. Among the radiopharmaceuticals approved by the Food and Drug Administration (FDA), radio-tracers targeting prostate-specific membrane antigen (PSMA) and somatostatin receptor (SSTR) have held essential positions in the diagnosis and treatment of prostate cancers and neuroendocrine neoplasms, respectively. In recent years, FDA-approved serials of immune-therapy and targeted therapy drugs targeting programmed death 1 (PD-1)/programmed death ligand 1 (PD-L1), human epidermal growth factor receptor 2 (HER2), and nectin cell adhesion molecule 4 (Nectin 4). How to screen patients suitable for these treatments and monitor the therapy? Nuclear medicine with specific radiopharmaceuticals can visualize the expression level of those targets in systemic lesions and evaluate the efficacy of treatment. In addition to radiopharmaceuticals, imaging equipment is also a key step for nuclear medicine. Advanced equipment including total-body positron emission tomography/computed tomography (PET/CT) and positron emission tomography/magnetic resonance imaging (PET/MRI) has been developed, which contribute to the diagnosis and treatment of tumors, as well as the development of new radiopharmaceuticals. Here, we conclude most recently advances of radiopharmaceuticals in nuclear medicine, and they substantially increase the "arsenal" of clinicians for tumor therapy.

Abstract:Zirconium-89 (89Zr) is a cyclotron-produced positron-emitting radioisotope with a half-life of 3.27 days, which makes delayed or longitudinal imaging possible. It is a superior isotope for tracking particles over several days at a high sensitivity, resolution, and specificity. 89Zr-monoclonal antibodies (89Zr-mAb) have gained significant attention in the field of molecular imaging. However, the past decade has shown an avid increase in research concerning 89Zr-radiopharmaceuticals apart from 89Zr-mAb. In this article we highlight and discuss the status and challenges attributed to current preclinical and clinical investigations of 89Zr-radiopharmaceuticals developed beyond 89Zr-mAb, e.g., mAb-derived variants and macro-biomolecules, proteins, peptides, nanoparticles, and living cells.
