Abstract:Purpose: Health-system pharmacists play a crucial role in monitoring the pharmaceutical pipeline to manage formularies, allocate resources, and optimize clinical programs for new therapies. This article aims to support pharmacists by sharing new and anticipated novel drug approvals. Summary: Selected drug approvals anticipated in the 12-month period covering the third quarter of 2024 through the second quarter of 2025 are reviewed. The analysis emphasizes drugs expected to have significant clinical and financial impact in hospitals and clinics selected from 54 novel drugs awaiting US Food and Drug Administration approval. New cell therapies for treating cancers continue to enter the drug pipeline, while novel targeted therapies for biliary tract, gastric, pancreatic, and breast cancers, as well as 3 subcutaneous versions of already approved drugs given intravenously, are awaiting approval. Additionally, many novel drugs are being developed for treatment of rare and ultra-rare diseases such as hereditary angioedema, macular telangiectasia, congenital adrenal hyperplasia, and Barth syndrome. Two new subcutaneous drugs for hemophilia, a new oral medication for hereditary angioedema, a novel monoclonal antibody for atopic dermatitis, and the first oral penem antibiotic are also in the pipeline. Conclusion: New drugs with various indications for cancers and rare diseases continue to strengthen the drug pipeline.

Sales Revenues for New Therapeutic Agents Approved by the United States Food and Drug Administration From 1995 to 2014

Abstract:Objectives: This study aimed to analyze worldwide sales of new therapeutic agents and to estimate the time it takes for product sales to exceed industry-wide average drug development costs. Methods: Data obtained from company reports were analyzed to track worldwide sales of new medicines approved by the US Food and Drug Administration from 1995 to 2014. All sales figures were reported in 2019 US dollars. Kaplan-Meier curves were used to evaluate the time it took for discounted product sales to exceed the average costs associated with developing 1 new drug (accounting for the costs of failed trials), using published estimates of these costs. Results: Based on data for 361 of 558 new therapeutic agents approved over the study period (median follow-up 13.2 years), mean sales revenue per product was $15.2 billion through the end of 2019; the median was $6.7 billion. These products jointly generated global sales of $5.5 trillion since approval. Revenues were highly skewed, with the 25 best selling products (7%, 25 of 361) accounting for 38% of this amount ($2.1 trillion of $5.5 trillion). Approximately 47% of products had discounted sales that exceeded the estimated industry-wide average costs of development within 5 years of approval, and 75% within 10 years. After attributing potential production, marketing, and other costs, these numbers dropped to 21% of products within 5 years of approval, and 46% within 10 years. Conclusions: Sales of new medicines approved from 1995 to 2014 were highly skewed, but many products had net discounted sales that exceeded the industry-wide average costs of development within 10 years of approval. An understanding of how sales revenues accrue in the years after initial approval, alongside data on business costs, can inform discussions about how to incentivize private investment in innovation while ensuring affordable prices for patients and the healthcare system.

Abstract:Context: Recent public outcry has highlighted the rising cost of prescription drugs worldwide, which in several disease areas outpaces other health care expenditures and results in a suboptimal global availability of essential medicines. Method: A systematic review of Pubmed, the Financial Times, the New York Times, the Wall Street Journal and the Guardian was performed to identify articles related to the pricing of medicines. Findings: Changes in drug life cycles have dramatically affected patent medicine markets, which have long been considered a self-evident and self-sustainable source of income for highly profitable drug companies. Market failure in combination with high merger and acquisition activity in the sector have allowed price increases for even off-patent drugs. With market interventions and the introduction of QALY measures in health care, governments have tried to influence drug prices, but often encounter unintended consequences. Patent reform legislation, reference pricing, outcome-based pricing and incentivizing physicians and pharmacists to prescribe low-cost drugs are among the most promising short-term policy options. Due to the lack of systematic research on the effectiveness of policy measures, an increasing number of ad hoc decisions have been made with counterproductive effects on the availability of essential drugs. Future challenges demand new policies, for which recommendations are offered. Conclusion: A fertile ground for high-priced drugs has been created by changes in drug life-cycle dynamics, the unintended effects of patent legislation, government policy measures and orphan drug programs. There is an urgent need for regulatory reform to curtail prices and safeguard equitable access to innovative medicines.

Abstract:Abstract With 54 new drugs and seven cellular and gene therapy products, the approvals by the US Food and Drug Administration (FDA) recovered 2023 from the 2022 dent back to the levels of 2020–2021. As in previous years of this annual review, we assign these new drugs to one of three levels of innovation: first drug against a condition (“first-in-indication”), first drug using a novel molecular mechanism (“first-in-class”), and “next-in-class,” i.e., a drug using an already exploited molecular mechanism. We identify four (7%) “first-in-indication,” 22 (36%) “first-in-class,” and 35 (57%) “next-in-class” drugs. By treatment area, rare diseases (54%) and cancer drugs (23%) were once again the most prevalent (and partly overlapping) therapeutic areas. Other continuing trends were the use of accelerated regulatory approval pathways and the reliance on biopharmaceuticals (biologics). 2023 marks the approval of a first therapy based on CRISPR/Cas9 gene editing.

Drug discovery in the next decade: innovation needed ASAP

Abstract:Pharmaceutical companies must find a better way to increase their output of truly new drugs for the benefit of patients and for their business survival. Here, I highlight a general perspective from within pharmaceutical research as it pertains to research advances in chemistry, biology, pharmacology, pharmacokinetics and toxicology that, if well integrated, stands to put the industry on a productive path. In addition, I provide a complementary perspective on the corporate culture aspect of innovation. I also introduce a new concept, termed 'innovation ASAP' (iASAP; asking powerful questions, seeking the outliers, accepting defeat and populating astutely) and provide support for it using examples of several successful drugs.

Abstract:This paper examines how pharmaceutical Artificial Intelligence advancements may affect the development of new drugs in the coming years. The question was answered by reviewing a rich body of source material, including industry literature, research journals, AI studies, market reports, market projections, discussion papers, press releases, and organizations' websites. The paper argues that continued innovation in pharmaceutical AI will enable rapid development of safe and effective therapies for previously untreatable diseases. A series of major points support this conclusion: The pharmaceutical industry is in a significant productivity crisis today, and AI-enabled research methods can be directly applied to reduce the time and cost of drug discovery projects. The industry already reported results such as a 10-fold reduction in drug molecule discovery times. Numerous AI alliances between industry, governments, and academia enabled utilizing proprietary data and led to outcomes such as the largest molecule toxicity database to date or more than 200 drug safety predictive models. The momentum was recently increased by the involvement of tech giants combined with record rounds of funding. The long-term effects will range from safer and more effective therapies, through the diminished role of pharmaceutical patents, to large-scale collaboration and new business strategies oriented around currently untreatable diseases. The paper notes that while many reviewed resources seem to have overly optimistic future expectations, even a fraction of these developments would alleviate the productivity crisis. Finally, the paper concludes that the focus on pharmaceutical AI put the industry on a trajectory towards another significant disruption: open data sharing and collaboration.

Abstract:Significant advances in the understanding of areas such as neuroscience, genetics, pharmacology, psychology, and social policy have taken place in recent years. This new knowledge has moved one to a new level in the understanding of how the brain work, how chemicals affect the brain's performance and, in turn, how this affects human behavior. At the same time, more is expected from the human brain. The knowledge of the way the human brain and behavior work is growing at pace. This knowledge has brought significant gains through better treatments for many mental health disorders and the diagnosis of new conditions such as attention deficit hyperactivity disorder (ADHD). There is a significant investment in the research on drugs and the brain and it is starting to bear fruit. Many of the advances are unexpected and are still not understood. In particular, new approaches are being developed for medicines for mental health. Preventative treatments are being trialed for Alzheimer's disease and one may be able to develop them for schizophrenia. At the same time, pharmacogenetics is allowing one to identify people at higher risk of adverse or in some cases even beneficial drug side effects. This chapter enlists the support of academic experts ranging from neuroscientists to economists and presents some reviews of the science that are the most thorough analyses ever written of the issues faced by the United Kingdom and other countries in the sphere of legal and illegal drug use and their interaction with brain science.

Abstract:Generic drugs are an important component of public health that ensures patient access to affordable medications. Recently, the US Food and Drug Administration (FDA)-approved generic drugs account for 90% of prescriptions filled in the United States, but only represent about 23% of the spending.1 In the next 10 years, synergistic amendment, innovation, and advancements in regulatory, scientific, and global public health areas (Figure 1) could further enable "genericizing" medicinal products, including more complex ones to advance public health.

A Decade of FDA-Approved Drugs (2010–2019): Trends and Future Directions

Abstract:A total of 378 novel drugs and 27 biosimilars approved by the U.S. Food and Drug Administration (FDA) between 2010 and 2019 were evaluated according to approval numbers by year, therapeutic areas, modalities, route of administration, first-in-class designation, approval times, and expedited review categories. From this review, oncology remains the top therapy area (25%), followed by infection (15%) and central nervous system disorders (11%). Regulatory incentives have been effective as evidenced by an increase in orphan drugs as well as antibacterial drugs approved under the GAIN act. Clinical development times may be increasing, perhaps as a result of the increase in orphan drug indications. Small molecules continue to mostly adhere to "Rule of 5" (Ro5) parameters, but innovation in new modalities is rapidly progressing with approvals for antisense oligonucleotides (ASO), small-interfering RNA (siRNAs), and antibody-directed conjugates (ADCs). The Pharmaceutical Industry in 2023: An Analysis of FDA Drug Approvals from the Perspective of Molecules

Abstract:With the COVID-19 pandemic behind us, the U.S. Food and Drug Administration (FDA) has approved 55 new drugs in 2023, a figure consistent with the number authorized in the last five years (53 per year on average). Thus, 2023 marks the second-best yearly FDA harvest after 2018 (59 approvals) in all the series. Monoclonal antibodies (mAbs) continue to be the class of drugs with the most approvals, with an exceptional 12, a number that makes it the most outstanding year for this class. As in 2022, five proteins/enzymes have been approved in 2023. However, no antibody–drug conjugates (ADCs) have been released onto the market. With respect to TIDES (peptides and oligonucleotides), 2023 has proved a spectacular year, with a total of nine approvals, corresponding to five peptides and four oligonucleotides. Natural products continue to be the best source of inspiration for drug development, with 10 new products on the market. Three drugs in this year's harvest are pegylated, which may indicate the return of pegylation as a method to increase the half-lives of drugs after the withdrawal of peginesatide from the market in 2013. Following the trends in recent years, two bispecific drugs have been authorized in 2023. As in the preceding years, fluorine and/or N-aromatic heterocycles are present in most of the drugs. Herein, the 55 new drugs approved by the FDA in 2023 are analyzed exclusively on the basis of their chemical structure. They are classified as the following: biologics (antibodies, proteins/enzymes); TIDES (peptide and oligonucleotides); combined drugs; pegylated drugs; natural products; nitrogen aromatic heterocycles; fluorine-containing molecules; and other small molecules.

Merger mania: mergers and acquisitions in the generic drug sector from 1995 to 2016

Abstract:Background: Drug shortages and increasing generic drug prices are associated with low levels of competition. Mergers and acquisitions impact the level of competition. Record merger and acquisition activity was reported for the pharmaceutical sector in 2014/15, yet information on mergers and acquisitions in the generic drug sector are absent from the literature. This information is necessary to understand if and how such mergers and acquisitions can be a factor in drug shortages and increasing prices. Methods: Data on completed merger and acquisition deals that had a generic drug company being taken over (i.e. 'target') were extracted from Bloomberg Finance L.P. The number and announced value of deals are presented globally, for the United States, and globally excluding the United States annually from 1995 to 2016 in United States dollars. Results: Generic drug companies comprised 9.3% of the value of all deals with pharmaceutical targets occurring from 1995 to 2016. Globally, in 1995 there were no deals, in 2014 there were 22 deals worth $1.86 billion, in 2015 there were 34 deals totalling $33.56 billion, and in 2016 there were 42 deals worth in excess of $44 billion. This substantial increase was partially attributed to Teva's 2016 acquisition of Allergan's generic drug business. The surge in mergers and acquisitions for 2015/16 was driven by deals in the United States, where they represented 89.7% of the dollar value of deals in those years. Conclusions: The recent blitz in mergers and acquisitions signals that the generic drug industry is undergoing a transformation, especially in the United States. This restructuring can negatively affect the level of competition that might impact prices and shortages for some products, emphasizing the importance of updating regulations and procurement policies.

Abstract:At the intersection of artificial intelligence and the pharmaceutical R&D sector, a new era in drug development has started. In this piece, we provide a thorough analysis of AI-driven drug development and discuss how it might revolutionize the R&D sector in the pharmaceutical business. This research approach, which includes data collection, model construction, and ethical considerations, has shown promising results. Artificial intelligence models have shown very high prediction accuracy, reducing costs and enhancing equity while speeding up the creation of new medications. These findings may improve patient outcomes and increase therapy accessibility. Future priorities will include therapeutic subspecialization, worldwide access initiatives, interpretability, regulatory compliance, data exchange, and multi-omics integration. AI and medications working together might result in a healthcare system that is more accessible to everyone and more effective.

Abstract:KEYWORDS: High drug pricing is an important issue at the intersection of healthcare, economics, and ethics. The increasing cost of pharmaceuticals, advanced therapy medicinal products (ATMPs), and more personalized treatment options jeopardizes the accessibility and affordability of interventions that are sometimes lifesaving globally. Increased drug pricing is increasingly leading to unaffordable treatments even in affluent societies. The prospect for the near future, with treatment prices per patient soaring and negative reimbursement decisions solely based on failed pricing negotiations is negative. This polarizes the discussion between different stakeholders including patients, health care providers, pharmaceutical companies, regulatory agencies, Health Technology Assessment (HTA) agencies, patient advocacy groups, payers (insurers and governments), (bio)ethicists, shareholders, academia, and Non-Governmental Organizations (NGOs) and international bodies. Unfortunately, these discussions are often emotional. We discuss a method to rationalize the debate by explicitly incorporating uncertainties in value estimations throughout the drugs lifecycle.
