P2‐030: 3‐D Mapping of Verbal Fluency Performance in Amnestic Mild Cognitive Impairment and Alzheimer's Disease

Abstract:Executive dysfunction is an important characteristic of Alzheimer's disease (AD). Verbal fluency is a component of executive function. This study aimed to investigate the structural neuroimaging correlates of verbal fluency in amnestic mild cognitive impairment (aMCI) and Alzheimer's disease (AD), using optimized voxel-based morphometry (VBM). Fifty-six patients with AD (NINCDS-ADRDA criteria, AD group), 14 subjects with aMCI meeting Petersen criteria (aMCI group), and 16 healthy elderly (control group) were enrolled in this study. All subjects completed the comprehensive neuropsychological battery tests and the performance on verbal fluency test (animal naming in one minute) was used in this study. High-resolution 3D T1 structural images were acquired with 3D-MPRAGE sequence (TR=2530ms, TE=3.44ms, TI=1100ms, slice thickness=1 mm with no gap, FOV=256×256, voxel size=1 mm × 1 mm × 1 mm). All 3D T1 images were analyzed with SPM5 by following the procedures of co-registration, modulated, segmentation, and smoothing. The scores of verbal fluency test were entered as covariates in a general linear model that predict the gray matter density at each voxel. The 3D statistical maps showed strong left-sided lateralization and more cortical areas are linked to verbal fluency test scores in left hemisphere (Figure 1). Among all subjects, verbal fluency test scores correlated strongly with gray matter density in left superior frontal gyri, insula, precuneus, inferior parietal lobule, right superior frontal gyri and paracentral lobule (PFWE-corrected<0.01). Strong correlation was also seen with left middle frontal gyri (PFWE-corrected=0.018) in AD group. Verbal fluency test, reflecting executive function and assessing language function simultaneously, manifests strong correlation with frontal lobe structural changes across the spectrum of neurodegenerative disorders, from normal aging to amnestic mild cognitive impairment, and eventually to Alzheimer's disease. These findings further corroborate the role of left frontal lobe in the executive dysfunction and language impairment in Alzheimer's disease.
