Abstract:We congratulate the authors Love Cyon et al for their published article 'Long-term prognosis of low high-sensitivity cardiac troponin T in the emergency department compared with the general population'.1 We want to highlight some important points that should be addressed. First, the authors excluded the population with high levels of high-sensitive cardiac troponin T (hs-cTnT (≥14 ng/L), which has been shown to have a greater risk of adverse cardiac outcomes.2 Another study by Kvisvik et al shows that elevated levels of hs-cTnT are associated with a worse long-term outcome, including all-cause mortality, composite cardiovascular events and heart failure hospitalisation in patients with non ST segment elevation-acute coronary syndrome (NSTE-ACS), compared with individuals with low levels of 14 ng/L.3 Including the cohort with elevated values would help clinicians better categorise risk assessment.
