Abstract:For patients with central bronchopulmonary malignancies, sleeve lobectomy serves as an alternative to pneumonectomy. In recent years, robotic-assisted technique has been innovatively developed and applied in sleeve lobectomy. Here, we introduce a patient treated with chemotherapy who received robotic bronchial sleeve lobectomy without pulmonary vessel anastomosis in our department. The patient was a 53-year-old male smoker, who had suffered irritable cough for 6 months. During surgery we found that the tumor was tightly adhered to the pulmonary artery. Robotic scissors were used to separate the tumor from the surface of the pulmonary artery. Finally, the tumor was removed along with the relevant bronchus, and the pulmonary artery remained intact. The tumor margins were verified as negative by fast frozen pathology during the procedure. This experience in our department demonstrates the advantages of robotassisted bronchial lobectomy in preserving pulmonary vessels in patients with pulmonary arterial adventitia involvement.

Abstract:The robotic surgical system has been increasingly utilized for thoracic surgeries in recent years. For the treatment of lung cancer, the safety and feasibility of robotic lobectomy, segmentectomy, and even sleeve resection have been demonstrated by a series studies (1-3). Robotic-assisted thoracic surgery (RATS) was initiated in May 2015 in our department, and, as of June 2019, more than 1,000 cases have been performed, including lung surgery, esophagectomy for esophageal tumors, and mediastinal tumor resection. For robotic-assisted lung surgery, 530 patients underwent lobectomy, 204 patients underwent segmentectomy, and 8 patients underwent sleeve resection. We started from robotic lobectomy and segmentectomy, and sleeve resection was carried out when we acquired greater proficiency of the robotic surgical technique. To summarize our experience and improve the efficacy of robotic lung cancer surgery, several retrospective studies and prospective clinical trials have been conducted in our department.

Comparison of Sleeve Lobectomy for Lung Cancer Using Mini-Thoracotomy and an Optimized Robot-Assisted Technique

Abstract:Objective: To evaluate the clinical significance of an optimized approach to improve surgical field visualization and simplify anastomosis techniques using robotic-assisted sleeve lobectomy for lung or bronchial carcinoma. Method: A total of 26 consecutive patients who underwent sleeve lobectomy between January 2017 and April 2020 were enrolled in the study. The cohort included 11 cases of robotic-assisted surgery (RAS group) and 15 cases of mini-thoracotomy (MT group). RAS was performed via an exclusive optimized approach utilizing the "3 to 4-6 to 8/9" four-port technique. Retrieved demographical and clinical data included operation time, anastomosis time, blood loss, chest drainage time and volume, postoperative pain scores, complications, white blood cell (WBC) levels, and duration of hospital stay and follow-up. Results: No cases of perioperative death were recorded. Compared to MT group, the RAS group had a similar anastomosis time (30.82 ± 6.08 vs 33.20 ± 7.73 min, respectively, p > 0.05) and shorter operation time (189.73 ± 36.41 vs 225.33 ± 38.19 min, respectively, p < 0.05). The RAS group had lower pain scores (4.23 ± 0.26 vs 4.91 ± 0.51, p < 0.05), lower levels of WBC (p < 0.05), and no anastomotic complications postoperatively. The RAS and MT groups demonstrated a successful bronchus reconstruction with low risk of angulation (1/11 vs 1/15, p > 0.05) and satisfactory disease-free survival (eight cases, 72.73% and 12 cases, 80%, respectively). Conclusion: The optimized approach to RA sleeve lobectomy is convenient and efficient and provides satisfactory clinical outcomes. Further study with a large sample size and evaluation of long-term survival are warranted. Key points: (i) we present a novel, convenient, and efficient approach for robotic-assisted sleeve lobectomy, ie, "3 to 4-6 to 8/9" four-port technique. The optimized approach for RA sleeve lobectomy is convenient and efficient and provides satisfactory clinical outcomes; (ii) details for the "3 to 4-6 to 8/9" four-port method: the assistant port was located at the fourth intercostal space. The 1-cm camera port was inserted at the sixth intercostal space in the posterior axillary line. The 0.5-cm da Vinci ports of the instrument arms were placed at the third intercostal space in the anterior axillary line and the eighth or ninth intercostal space in the posterior axillary line. The patient cart was inserted from the back of the patient's head and shoulders at 75° to the longitudinal line.

Abstract:OBJECTIVES:We summarize our experiences of video-assisted thoracoscopic surgery (VATS) sleeve lobectomy with bronchoplasty for non-small-cell lung cancer and discuss the indications and technical details of the operation.METHODS:From September 2011 to December 2012, 15 patients underwent VATS sleeve lobectomy with bronchoplasty at our institution (right upper lobe 10, right middle and lower lobes 1, left lower lobe 2 and left upper lobe 2), with mediastinal lymphadenectomy. Three incisions were utilized. The utility incision was made at the fourth intercostal space, anterior axillary line. Simple continuous and simple interrupted suturing of the membranous and cartilaginous portions of the bronchus were performed for the anastomosis.RESULTS:All procedures were uneventful, with a median operative time of 165 min, a median bronchial anastomosis time of 44 min and a median blood loss of 150 ml. There were no conversions to thoracotomy. There were 14 squamous carcinomas and 1 adenocarcinoma. All patients recovered well, and 1 experienced a minor complication. The median duration of chest tube drainage was 5.4 days, and the median length of hospital stay was 7 days. All patients were followed postoperatively for a range of 1-16 months without tumour recurrence.CONCLUSIONS:VATS sleeve lobectomy with bronchoplasty is safe and effective. The utility incision placed at the fourth intercostal space, anterior axillary line, is convenient for the anastomosis, and the suturing technique is expeditious and secure. Preserving the azygos vein does not compromise exposure for the anastomosis. This technique is very suitable for centrally located lung cancers <3 cm in diameter, particularly when the cancers are located within the brachial lumen.

Abstract:Background. The purpose of this study was to identify the technical aspects and short-term results of robotic sleeve resection for lung cancer patients.Methods. Twenty-one consecutive cases of robotic sleeve resection from September 2014 to September 2015 were reviewed.Results. There were 17 single sleeve resection (bronchial) and4 double sleeve resection(bronchial andvascular) cases. Nineteen of 21 cases (90.5%) achieved R0 resection. The mean console time was 120.4 +/- 37.3 minutes. Themean operation time was 158.4 +/- 42.0 minutes. There was no massive bleeding (800 mL or more) during operation. The mean intraoperative blood loss was 157.1 +/- 97.8 mL. One case (4.8%) was converted to thoracotomy owing to severe calcification of lymph node. There was no intraoperative death. The overall complication rate was 19.0%. The major complications were subcutaneous emphysema (14.4%), cardiac arrhythmia (9.6%), pneumonia (9.6%), pyothorax (9.6%), bronchial anastomosis bleeding (4.8%), bronchial anastomosis leakage (4.8%), and multiple organ failure (4.8%). The 30-day mortality rate was 4.8%. The mean postoperative length of stay was 10.7 +/- 7.6 days.Conclusions. Abstract:Single-port video-assisted thoracoscopic surgery (VATS) has recently been proposed as an innovative minimally invasive alternative to the standard three-port VATS for lobectomies, most of which are performed using a conventional rigid thoracoscope. Here, we report a single-port VATS approach for right upper lobectomy and systematic lymph node dissection using a flexible endoscope. A 61-year-old male smoker presented with a pulmonary nodule. A single-port VATS procedure was performed through a 4-cm intercostal incision using a flexible laparo-thoraco videoscope. Right upper lobectomy and systematic lymph node dissection were performed. The total operating time was 106 in. The procedure was successful and the recovery uneventful. The patient's chest tube was removed on the third day, and he was discharged home on the fourth. The use of a flexible videoscope facilitated the single-port VATS procedure by avoiding interference between the videoscope and other operating instruments and providing ample space for the surgeon.

Abstract:Minimally invasive techniques have been widely applied in general thoracic surgery. Compared with video-assisted thoracoscopic surgery (VATS), due to its theoretic superiority, robotic surgery is challenging the traditional position of VATS. With its unique advantages, including 3D vision and a high-freedom endowrist, it leads to easier lymph node dissection, more convenient blood vessel dissection, a shorter learning curve and competence for the completion of complex surgery. However, as a new surgical technology, the safety and efficacy of robotic-assisted thoracoscopic surgery (RATS) still need to be further verified. Thus, in this article, we review and summarize the application of RATS versus VATS in general thoracic surgery.

Abstract:This report focuses on the technique of two‐port robotic bronchial sleeve lobectomy and bronchial anastomosis, and postoperative outcomes. The bronchial anastomosis between the proximal main bronchus and the distal lobar bronchus was performed by using a half‐continuous suture technique with two tensile strength size 3‐0 v‐loc sutures.ObjectivesTo explore the feasibility of two‐port robotic sleeve lobectomy using Stratafix sutures for central lung tumors, and to summarize the surgical techniques and clinical outcomes.MethodsWe retrospectively evaluated 15 consecutive patients who underwent robotic bronchial sleeve lobectomy, performed by a single surgeon between March 2021 and September 2021. A half‐continuous suture technique with two Stratafix sutures was used for bronchial anastomosis. The operative techniques and outcomes were analyzed.ResultsComplete resection was achieved in all patients undergoing different types of robotic bronchial sleeve lobectomy. There were no conversions to thoracotomy. The mean duration of surgery was 102.35 ± 46.31 min, mean time for bronchial anastomosis was 25.8 ± 15.2 min, mean blood loss was 64.71 ± 38.59 ml, and mean postoperative hospital stay was 4.76 ± 2.54 days. There was no death on follow‐up within 90 days after surgery.ConclusionsTwo‐port robotic bronchial sleeve lobectomy and the novel anastomotic technique are both feasible and safe for selected patients.

Effect of da Vinci robot-assisted versus traditional thoracoscopic bronchial sleeve lobectomy

Abstract:Objective: To analyze the short-term effect of Da Vinci robot-assisted thoracoscopic (RATS) bronchial sleeve lobectomy, so as to summarize its safety and effectiveness. Methods: It was a retrospective single-center study with the inclusion of 22 cases receiving RATS lobectomy and 49 cases of traditional thoracoscopic surgery. Further comparison was performed focusing on the baseline characteristics and perioperative performance of the two groups. Results: Compared with the traditional thoracoscopic surgery group, RATS group had more advantages in the number of lymph nodes dissected (P = 0.003), shorter postoperative length of stay in the hospital (P = 0.040), shorter drainage time (P = 0.022), reduced drainage volume (P = 0.001). Moreover, this study found for the first time that there was a shortening in the operation of sleeve lobectomy by using Da Vinci robot-assisted surgical system (P = 0.001). The operation cost of RATS group is more expensive (96000 ± 9100.782 vs 63000 ± 5102.563 yuan; P＜0.001). Conclusion: Compared with the traditional thoracoscopic bronchial sleeve lobectomy, RATS lobectomy shows advantages of higher operating sensitivity, shorter operation time, faster postoperative recovery, and more lymph nodes dissected. Collectively, RATS bronchial sleeve lobectomy is safe and effective in operation.
Subxiphoid Uniportal Robotic Assisted Right Upper Lobectomy in Swine: A Novel Technology

Abstract:Background: Uniportal video assisted thoracoscopic surgery has become widely accepted with increasing subxiphoid approach for major pulmonary resections. The purpose of this study was to verify the safety and feasibility of a SHURUI single port (SP) Robotic Surgical System in subxiphoid right upper lobectomy. Methods: Five swine models were used to perform subxiphoid right upper lobectomy with SHURUI SP Robotic Surgical System. Operation details were recorded to assess the safety and feasibility of the SHURUI system. Results: Results showed that the operation success was 100%. The time from anaesthesia to complete docking of the robotic system was 30.0 (20.0, 36.0) minutes and the total operation time was 125.0 (62.0, 142.5) minutes. Blood loss during operation was 30.0 (25.0, 125.0) mL. No death or complications occurred during the operation. Conclusions: Uniportal robotics assisted right upper lobectomy via subxiphoid approach with SHURUI system performed in this study preliminary showed its safety and feasibility.

Abstract:Minimally invasive thoracic surgery, including video-assisted thoracoscopic surgery and robot-assisted thoracoscopic surgery, has been proven to have an advantage over open thoracotomy with less pain, fewer postoperative complications, faster discharge, and better tolerance among elderly patients. We introduce a uniportal robot-assisted thoracoscopic double-sleeve lobectomy performed on a patient following neoadjuvant immunotherapy. Specialized instruments like customized trocars with a reduced diameter, bulldog clamps, and double-needle sutures were utilized to facilitate the maneuverability through the single incision. This technique integrates the merits of multiport robot-assisted thoracic surgery with uniportal video-assisted thoracoscopic surgery.

Abstract:Minimally invasive techniques, typified by video-assisted thoracoscopic surgery, are widely practiced in the treatment of thoracic diseases all around the world, and video-assisted thoracoscopic surgery has been recognized as a standard treatment method for early staged lung cancer. Among them, robotic-assisted thoracoscopic surgery, which has the advantages of providing a three-dimensional view and better maneuverability, has emerged as a next-generation technique in the field of minimally invasive surgery and is also gaining its popularity with the idea of Enhanced Recovery After Surgery deeply rooted in patients' minds. Up to now, robotic-assisted thoracoscopic surgery usually requires 3 or 4 ports with 1 or 2 additional access incisions. Meanwhile, traditional video-assisted thoracoscopic surgery can now be completed with uniportal method, with less postoperative pain and higher patient satisfaction with respect to the number of incisions in comparison with the multi-port technique. To inform the integration of these new minimally invasive techniques, here, we present a case in which uniportal right upper lobectomy was performed using the 4<sup>th</sup> generation <i>da Vinci</i> Robotic Surgical System (Xi). With continuous innovation in robotic minimally invasive techniques and improvements in surgical skills, we believe more patients will benefit from robotic-assisted thoracoscopic surgery with single port in the near future.

Abstract:Background: Robotic-assisted thoracic surgery (RATS) has been widely used in the treatment of lung cancer. The perioperative outcomes of right upper lobectomy (RUL) using RATS and video-assisted thoracic surgery (VATS) were retrospectively investigated and compared. We aimed to summarize a single-center experience of RATS and 4-port unidirectional VATS in RUL, and to discuss the safety and the essentials of the surgery. Methods: We retrospectively analyzed the 685 with non-small cell lung cancer (NSCLC) patients who underwent minimally invasive RUL in our center by the same surgical group from January 2015 to December 2019. Both RATS and VATS were performed with three ports with utility incision. The 685 participants were divided into RATS (335 cases) and VATS (350 cases) groups according to surgical method. Baseline characteristics and perioperative outcomes including dissected lymph nodes, postoperative duration of drainage, postoperative hospital stay, and incidence of postoperative complications were compared between the groups. Results: In the 685 patients enrolled, the baseline characteristics were comparable, and no postoperative 30-day mortality or intraoperative blood transfusion were observed. Compared with VATS, RATS had less surgical duration (90.22 +/- 12.16 vs. 92.68 +/- 12.26 min, P<0.001), less length of stay (4.71 +/- 1.37 vs. 5.26 +/- 1.56 days, P<0.001), and decreased postoperative duration of drainage (3.49 +/- 1.15 vs. 4.09 +/- 1.57 days, P<0.001). No significant difference was observed in the lymph nodes dissection, blood loss, conversion rate and morbidities. The cost of RATS was much higher than VATS (85,329.41 +/- 12,893.44 vs. 68,733.43 +/- 14,781.32 Conclusions: Robot assisted RUL had similar perioperative outcomes compared to VATS RUL lobectomy using similar three port with utility incision technique. The advantages of RATS included finer dissection of lymph node, relatively less operation time, earlier chest tube removal and discharge.
Subcostal uniportal robotic anatomic lung resection: A pilot trial

Abstract:Objective: Robot-assisted thoracoscopic surgery typically necessitates the use of multiple ports. The new single-port robotic system (da Vinci SP system) platform is designed to perform uniportal surgery. The purpose of this clinical trial is to evaluate the feasibility, efficacy, and safety of the da Vinci SP system when used for anatomical lung resection. Methods: Patients diagnosed with clinical stage I lung cancer requiring anatomical lung resections were considered eligible for this trial. The primary outcome measure was the rate of conversion, whereas the secondary objective focused on assessing the incidence of perioperative complications. Results: The study included 35 patients with a median age of 63 years (range, 48-74 years). Of these, 30 underwent lobectomy and 5 received segmentectomy. All surgeries were successfully performed using a subcostal approach, except for 1 patient, who required a thoracotomy conversion due to bleeding (conversion rate: 2.9%). The median docking time was 2 minutes (range, 1-8 minutes). For the 34 patients who completed uniportal surgery, the median total operating time was 194 minutes (range, 63-405 minutes), whereas the console time was 153 minutes (range, 93-267 minutes). The median number of harvested nodes was 13 (range, 5-37), while the median number of nodal stations was 6 (rang, 4-8). There were no in-hospital fatalities, and the median postoperative stay was 3 days (range, 2-12 days). Conclusions: This study demonstrates the feasibility and safety of using the da Vinci SP system for anatomical lung resection through a subcostal approach. Clinicaltrialsgov identifier: NCT05535712.
