Abstract:Objectives The purpose of this study was to demonstrate real status of clinical management for patients with ACS (acute coronary syndromes) in China. Background The extent of use of established practice guidelines for ACS that have been implemented in contemporary clinical practice in China is unknown. Methods This study is a retrospective observational survey, 64 hospitals across China, involving 3,139 in-patients with ACS were included and standard questionnaires were used to get baseline information on patients' demographic, treatment practice and in-hospital and postdischarge outcomes. Data were analyzed to present the full spectrum of ACS in specific geographic populations. Results 1. The proportion of patients involved presented the spectrum as follows: ST elevation MI (myocardial infarction) 39.2% (1,304), non-ST MI 10.5% (348) and unstable angina 44.8% (1,487), no significant difference was observed between tertiary hospitals and secondary hospitals. 2. In ST elevation MI, reperfusion was applied more often in tertiary hospitals (57.9%) than in secondary hospitals (42.3%). However, the rate of lytic treatment was higher in secondary hospitals than that in tertiary hospitals (37.4% vs 14.5%). 3. Statins and glycoprotein IIb/IIIa antagonists were applied more frequently in tertiary hospitals for all classes of patients with ACS (p<0.001). 4. Major in-hospital events and death rates were higher in secondary hospitals than tertiary hospitals. Conclusions In contrast with European and some western countries, current management of ACS in China showed that it less closely follows the recommendations of guidelines. Differences in practice can be observed between different level hospitals.
