Abstract:Endometrial carcinoma is a common malignancy among peri-menopausal and menopausal females, even among some women of reproductive age. The treatment approach to endometrial cancer is a platinum-based regimen combined with paclitaxel, which may be unsatisfactory. A copper-mediated binding of lipoylated constituents of tricarboxylic acid cycle has been found recently, which brings about lethal protein stress and cell death, a phenomenon termed cuproptosis. As an innovative method of cell death, cuproptosis could be designed for cancer treatment and many aspects remain unaddressed. In our study, clinical, genomic, and mutational profiles of uterine corpus endometrial carcinoma (UCEC) patients were obtained from The Cancer Genome Atlas and cuproptosis-related genes and long non-coding RNAs (lncRNAs) were acquired thereafter. Co-expression and Cox regression analyses led to the development of a prognostic signature. Patients were separated into two groups (high- and low-risk groups) and survival analysis, risk score calculation, multivariate Cox analysis, and subgroup validation were implemented to determine the utility of the signature. Differentially expressed genes (DEGs) between the two groups were subjected to Gene Ontology and Kyoto Encyclopedia of Genes and Genomes enrichment analyses. Immune-related functional analysis and tumor mutation burden (TMB) analysis were performed. Three independent high-risk cuproptosis-related lncRNAs were finally confirmed and incorporated into the prognostic model, including BX322234.1, LINC01545 and LINC01224. Areas under the curve for 1-, 3- and 5-year survival were 0.717, 0.688 and 0.714, respectively. The risk model served as an independent factor to predict prognosis. Patients with high-risk and low TMB tended to have poor prognoses. Enrichment analysis demonstrated that DEGs were mostly associated with immune responses. In conclusion, the three high-risk cuproptosis-related lncRNAs could predict the prognosis of UCEC patients with higher power, where patients with high-risk and low TMB are prone to have the worst prognosis, which broadens the pattern of clinical treatment and applications.

Abstract:Abstract Background Currently, there is no reliable blood-based marker to track tumor recurrence in endometrial cancer (EC) patients. Liquid biopsies, specifically, circulating tumor DNA (ctDNA) analysis emerged as a way to monitor tumor metastasis. The objective of this study was to examine the feasibility of ctDNA in recurrence surveillance and prognostic evaluation of high-risk EC. Methods Tumor tissues from nine high-risk EC patients were collected during primary surgery and tumor DNA was subjected to next generation sequencing to obtain the initial mutation spectrum using a 78 cancer-associated gene panel. Baseline and serial post-operative plasma samples were collected and droplet digital PCR (ddPCR) assays for patient-specific mutations were developed to track the mutations in the ctDNA in serial plasma samples. Log-rank test was used to assess the association between detection of ctDNA before or after surgery and disease-free survival. Results Somatic mutations were identified in all of the cases. The most frequent mutated genes were PTEN , FAT4 , ARID1A , TP53 , ZFHX3 , ATM , and FBXW7 . For each patient, personalized ddPCR assays were designed for one-to-three high-frequent mutations. DdPCR analysis and tumor panel sequencing had a high level of agreement in the assessment of the mutant allele fractions in baseline tumor tissue DNA. CtDNA was detected in 67% (6 of 9) of baseline plasma samples, which was not predictive of disease-free survival (DFS). CtDNA was detected in serial post-operative plasma samples (ctDNA tracking) of 44% (4 of 9) of the patients, which predicted tumor relapse. The DFS was a median of 9 months (ctDNA detected) versus median DFS undefined (ctDNA not detected), with a hazard ratio of 17.43 (95% CI, 1.616–188.3). The sensitivity of post-operative ctDNA detection in estimating tumor relapse was 100% and specificity was 83.3%, which was superior to CA125 or HE4. Conclusions Personalized ctDNA detection was effective and stable for high-risk EC. CtDNA tracking in post-operative plasma is valuable for predicting tumor recurrence.

Abstract:Objective: To evaluate the role of circulating tumour cells (CTCs) in patients with endometrial cancer (EC). Study design: This study included 40 patients with a pre-operative diagnosis of high-risk EC between April 2015 and May 2016. Patients were further divided into high-risk (grade 3, non-endometrioid, myometrial invasion >= 1/2 and stage III-IV) and high-intermediate-risk (grade 2-3, endometrioid, myometrial invasion <1/2 and stage I-II) groups according to postoperative pathological results. CTCs were detected using the CellSearch system, and CTC results were correlated with standard clinicopathological characteristics and serum tumour marker CA125/HE4 status using Chi-squared test, continuity correction or Fisher's exact test. The pharmacodynamic effect was detected after the first cycle of adjuvant therapy. Patients were followed up for 13 months to assess outcomes.Results: Fifteen percent of patients had one or more CTCs. The presence of CTCs was found to be significantly associated with cervical involvement (83.33% vs 11.76%, p = 0.00). No significant difference in CTC-positive rates was detected between the high-risk and high-intermediate-risk groups, and no significant correlation was found between CTCs and serum CA125/HE4, either by positive rates or exact serum levels of the conventional tumour markers. No more CTCs were detected after the first cycle of standard chemotherapy in this study, and no distant metastases or recurrence were found in the CTC-positive patients during the follow-up period.Conclusion: The presence of CTCs was correlated with cervical involvement. Early-stage EC patients with CTCs may benefit from additional adjuvant therapies. Assessment of CTCs may be useful in the management of high-risk EC patients. (C) 2016 Elsevier Ireland Ltd. Abstract:Abstract Endometrial cancer (EC) is the sixth most common cancer in women globally and is one of the few cancers for which mortality is increasing; to address the need to develop new methods for the diagnosis and treatment of this disease, we performed a comprehensive proteogenomic characterization of 95 endometrial carcinomas. We prospectively collected 83 endometrioid and 12 serous tumors, along with 49 normal tissue samples, for multi-omic characterization. Each sample underwent whole exome sequencing, whole genome sequencing, total RNA and miRNA sequencing, and DNA methylation array analyses. In addition, we evaluated all samples for protein, phosphorylation, and acetylation levels using an integrated, isobaric tandem mass tag labeling–based quantitative proteomics workflow. This dataset, publicly available through the Genomic Data Commons, CPTAC data portal, and other sources, provides a valuable resource for researchers and clinicians. Our analyses of these tumors revealed, among other things, a potential role for circRNAs in the epithelial-mesenchymal transition and new information about proteomic markers of clinical and genomic tumor subgroups, including relationships to known druggable pathways. We found possible new consequences of perturbations to the Wnt/β-catenin pathway, particularly CTNNB1 hotspot mutations; moreover, from the genome-wide acetylation survey, we found potential regulatory mechanisms linking Wnt signaling and histone acetylation. Phosphorylation data analysis suggests an elevated DNA damage response in serous tumors, which we hypothesize may be targetable by certain FDA-approved drugs. We found that TP53 hotspot mutations play unique trans-acting roles as compared to the same mutations in colon and ovarian cancers. We also characterized aspects of the tumor immune landscape, including immunogenic alterations, neoantigens, common cancer/testis antigens, and the immune microenvironment, all of which can inform immunotherapy decisions; of particular translational interest, we identified a subset of patients who, despite having high amounts of microsatellite instability, exhibit biomarkers which suggest that immunotherapy would be an ineffective treatment for their disease. By integrating global measurements of protein abundance, phosphorylation, and acetylation with next-generation genomics and transcriptomics, we have provided novel insights into biological processes underlying the development of endometrial cancers and generated strong hypotheses for new approaches to personalized treatment of individuals with endometrial cancer. Abstract:Introduction/Background: Endometrial cancer (EC) is the most common gynecological cancer in the developing world. While significant progress has been made through molecular sub-classification, the treatment of EC for many women remains contentious and better biomarkers are required to guide therapy selection. Circulating tumor DNA (ctDNA) a 'liquid biopsy', involves collection of tumor specific DNA via a blood test and can quantify cancer burden and detect tumor specific genomic information. The CODEC study is a prospective pilot study to assess the utility of ctDNA as a prognostic biomarker in EC with the hope that it may help predict relapse and guide selection of adjuvant therapy. Methodology: Women with Type 2 EC planned for surgery and consenting to the collection of biospecimens were enrolled prior to surgery and followed prospectively. Blood was collected perioperatively and then every three months for up to two years and at relapse. Broad sequencing of participants' tumors was performed, the results of which guided the design of personalized droplet digital PCR (ddPCR) assays for ctDNA analysis. ctDNA was extracted from 5ml of plasma and tested in quadruplicate. PET defined metabolic tumor volume was compared to ctDNA and analysis of ctDNA and relapse free survival (RFS) performed using a Cox proportional hazards regression model. Results: Somatic mutations to guide ddPCR analysis were detected in 45 enrolled women stages 1- 4 including pathogenic variants in POLE. ctDNA results are currently available for 30 participants (remaining results awaited). 12/30 participants had detectable ctDNA preoperatively and 5/25 at Time Point 3 (2-6 weeks post-surgery). ctDNA levels varied significantly across the cohort but results to date indicate that perioperative ctDNA positivity is associated with relapse. Interestingly in those that had detectable ctDNA preoperatively, the majority had a rise in ctDNA intraoperatively. Conclusion: This proof-of-concept analysis showed that ctDNA can successfully be detected in the perioperative setting and is the first study in EC to compare ctDNA with PET findings. Although testing is still underway, the early results indicate that ctDNA maybe a useful biomarker to predict relapse and guide the selection of adjuvant therapy. Abstract:Our results suggest that LNM plays an important role in the prognosis, and HMGB3 was potential as a biomarker for EC patients. By detecting the mutation of the risk signature, clinicians can accurately treat patients with targeted therapy, thereby improving their survival rate. Background Endometrial cancer (EC) is one of the most common malignant tumors in female reproductive system. The incidence of lymph node metastasis (LNM) is only about 10% in clinically suspected early‐stage EC patients. Discovering prognostic models and effective biomarkers for early diagnosis is important to reduce the mortality rate. Methods A least absolute shrinkage and selection operator (LASSO) regression was conducted to identify the characteristic dimension decrease and distinguish porgnostic LNM related genes signature. Subsequently, a novel prognosis‐related nomogram was constructed to predict overall survival (OS). Survival analysis was carried out to explore the individual prognostic significance of the risk model and key gene was validated in vitro. Results In total, 89 lymph node related genes (LRGs) were identified. Based on the LASSO Cox regression, 11 genes were selected for the development of a risk evaluation model. The Kaplan–Meier curve indicated that patients in the low‐risk group had considerably better OS (p = 3.583e−08). The area under the ROC curve (AUC) of this model was 0.718 at 5 years of OS. Then, we developed an OS‐associated nomogram that included the risk score and clinicopathological features. The concordance index of the nomogram was 0.769. The survival verification performed in three subgroups from the nomogram demonstrated the validity of the model. The AUC of the nomogram was 0.787 at 5 years OS. Proliferation and metastasis of HMGB3 were explored in EC cell line. External validation with 30 patients in our hospital showed that patients with low‐risk scores had a longer OS (p‐value = 0.03). Finally, we revealed that the most frequently mutated genes in the low‐risk and high‐risk groups are PTEN and TP53, respectively. Conclusions Our results suggest that LNM plays an important role in the prognosis, and HMGB3 was potential as a biomarker for EC patients.

Abstract:Objective: The aim of this study was to identify biomarkers with prognostic value in the setting of surgically treated endometrial cancer. Methods: Medical data for 282 patients with surgically treated endometrial cancer were reviewed retrospectively. Preoperative concentrations of six serum biomarkers (CA125, CA15-3, C-reactive protein [CRP], D-dimer [D-D], platelet-to-lymphocyte ratio [PLR], and neutrophil-to-lymphocyte ratio [NLR]) were analysed to determine potential associations with clinicopathologic characteristics and to assess prognostic values separately via Kaplan-Meier method and multivariate Cox regression. Results: In univariate analyses, the 5-year overall survival (OS) rate was 86.5% for a maximum follow-up period of 75 months. High concentrations of CA125, CA15-3, CRP, D-D, PLR, and NLR each proved significantly predictive of poor survival (log-rank test, P<0.01). CRP and D-D were identified as independent prognosticators, using a Cox regression model. Study patients were then stratified (based on combined independent risk factors) into three tiers (P<0.001), marked by 5-year OS rates of 92.1%, 78.4%, and 33.3%. Conclusions: All serum biomarkers assessed (CA125, CA15-3, CRP, D-D, PLR, and NLR) proved to be valid prognostic indices of surgically treated endometrial cancer. A novel prognostic grouping system, incorporating independent risk factors (CRP and D-D Concentrations), may have merit in assessing these patients preoperatively, providing a biologic basis for improved clinical staging.

Abstract:Background: Emerging evidence shows that genome instability-related long non-coding RNAs (lncRNAs) contribute to tumor–cell proliferation, differentiation, and metastasis. However, the biological functions and molecular mechanisms of genome instability-related lncRNAs in endometrial cancer (EC) are underexplored. Methods: EC RNA sequencing and corresponding clinical data obtained from The Cancer Genome Atlas (TCGA) database were used to screen prognostic lncRNAs associated with genomic instability via univariate and multivariate Cox regression analysis. The genomic instability-related lncRNA signature (GILncSig) was developed to assess the prognostic risk of high- and low-risk groups. The prediction performance was analyzed using receiver operating characteristic (ROC) curves. The immune status and mutational loading of different risk groups were compared. The Genomics of Drug Sensitivity in Cancer (GDSC) and the CellMiner database were used to elucidate the relationship between the correlation of prognostic lncRNAs and drug sensitivity. Finally, we used quantitative real-time PCR (qRT-PCR) to detect the expression levels of genomic instability-related lncRNAs in clinical samples. Results: GILncSig was built using five lncRNAs (AC007389.3, PIK3CD-AS2, LINC01224, AC129507.4, and GLIS3-AS1) associated with genomic instability, and their expression levels were verified using qRT-PCR. Further analysis revealed that risk score was negatively correlated with prognosis, and the ROC curve demonstrated the higher accuracy of GILncSig. Patients with a lower risk score had higher immune cell infiltration, a higher immune score, lower tumor purity, higher immunophenoscores (IPSs), lower mismatch repair protein expression, higher microsatellite instability (MSI), and a higher tumor mutation burden (TMB). Furthermore, the level of expression of prognostic lncRNAs was significantly related to the sensitivity of cancer cells to anti-tumor drugs. Conclusion: A novel signature composed of five prognostic lncRNAs associated with genome instability can be used to predict prognosis, influence immune status, and chemotherapeutic drug sensitivity in EC.
