Abstract:Objective To research the connection between the sagittal orientation of facet joints and disc degeneration in lumbar spine and to investigate the contribution of which to development the degenerative spondylolisthesis. Methods A new effective CAD method was used to accurately establish 9 finite element models(FEM)which were assembled respectively with facet-joint angle 65°, 45°, 25°, with normal disc, light degenerative disc and severe degenerative disc. The biomechanical parameters of 9 finite element models were measured under axial compressive load within physiological range. Results Compared with FEMs with facet-joint angle 45° and 25°, anterior displacement of L4 vertebra in FEMs with facet-joint angle 65° was increased, where the maximum von Mises stresses on facet surface, isthmus and the contact force on facet surface in horizontal orientation were obviously increased. Meanwhile, FEMs with facet-joint angle 65° showed a decrease in end-plate bulge and an increase in stress of annular matrix. The stiffness in light degenerative disc FEMs was reduced and the von Mises stresses on facet surface and isthmus was slightly increased compared with the normal disc FEMs. Among all, the FEM with facet-joint angle 65° combined with light degenerative disc showed the poorest resisting anterior shear force. Conclusion Sagittal orientation of facet joints is not only the primary inducement of the degenerative spondylolisthesis but the secondary pathological change of remodeling of the facet-joints induced by the regional stress change. The inherent instability of lumbar motion segment of sagittal orientation of facet joints is influenced by the lumbar disc degeneration. The lumbar disc degeneration has no manifested contribution to the aggravation of the sagittal orientation of facet joints.

Abstract:Background Facet tropism is the angular asymmetry between the left and right facet joint orientation. Although debatable, facet tropism was suggested to be associated with disc degeneration, facet degeneration and degenerative spondylolisthesis in the lumbar spine. The purpose of this study was to explore the relationship between facet tropism and facet degeneration in the sub-axial cervical spine. Methods A total of 200 patients with cervical spondylosis were retrospectively analyzed. Facet degeneration was categorized into 4 grade: grade I, normal; grade II, degenerative changes including joint space narrowing, cyst formation, small osteophytes (<3 mm) without joint hypertrophy; grade III, joint hypertrophy secondary to large osteophytes (>3 mm) without fusion of the joint; grade IV, bony fusion of the facet joints. Facet orientations and facet tropisms with respect to the transverse, sagittal and coronal plane were calculated from the reconstructed cervical spine, which was based on the axial CT scan images. The paired facet joints were then categorized into three types: symmetric, moderated tropism and severe tropism. Univariate and multivariate analysis were performed to evaluate the relationship between any demographic and anatomical factor and facet degeneration. Results The mean age of enrolled patients was 46.23 years old (ranging from 30 to 64 years old). There were 114 males and 86 females. The degrees of facet degeneration varied according to cervical levels and ages. Degenerated facet joints were most common at C2-C3 level and more common in patients above 50 years old. The facet orientations were also different from level to level. By univariate analysis, genders, ages, cervical levels, facet orientations and facet tropisms were all significantly different between the normal facets and degenerated facets. However, results from multivariate logistic regression suggested only age and facet tropism with respect to the sagittal plane were related to facet degeneration. Conclusion Facet degeneration were more common at C2-C3 level. Older age and facet tropism with respect to the sagittal plane were associated with the facet degeneration.

Abstract:Objective To identify the potential relation of geometric characteristics of the lumbar spine to the isthmic spondylolysis and degenerative joints under loading.Methods A series of new effective computer aided design(CAD) methods such as modified "no-seed region segmentation" and non-parallel "best cross-section planes" were used to accurately establish finite element(FE) models of the lumbar L4～L5 segment,representing the variation of disc height,facet-joint angle and lordosis angle of the intervertebral disc.The isthmic stress,facet contact force and load transmission through disc were investigated under axial compression 2700N.Results The isthmic stress and facet contact force of the FE models increased with the increasing of the disc height and facetjoint angle,while decreased with the increase of the lordosis angle of the intervertebral disc.Conclusion The variation of the disc height,facet-joint angle and the lordosis angle of the intervertebral disc had a manifest influence on the isthmic stress and facet contact force.It can be presumed that the occurrence of the lumbar spondylolysis and degenerative joints in athletes was closely correlated to the geometric characteristics of the lumbar spine.
Determination of Contact Force at Facet Joint with Different Sagittal Orientation under Shearing Loads:A Finite Element Analysis

Abstract:[Objective] The efleet of sagittal orientations of articular facet on the contact force and contact regions of a lumbar spine segment was evaluated.[Methods] [Results]Three-dimensional geometrical and mechanical accurate finite element models of the lumbar L4、5 motion segment representing three sagittal-orientations articular facet were generated and invalidated by an effective CAD method.Contact force and contact regions of the articular facets of three finite element models were predicted under the pure shearing loads.[Result] Under anterior shear condition,the contact force on the facet decreased with the increase of sagittal 0rientation of articular facet.however.in posterior shear load,the contact force on the facet increased with the increase of sagittal orientation.Meanwhile,under both pure shear loading,the percentages of the sagittal and the transversal components are increased with the dectease of the sagittal angle.However,under the salne load condition,there are no difference between the contact regions of the three models with different facet joint angle.[Conclusion]The spatial orientation and geometric forms of the coronal facet articular surfaces are more effective in restricting motion in transversal and sagittal planes while assuming a minor role in resisting axial force or motion than sagittal facet articular surface.It Was presumed that anterior shear force play a more prominent contribution on the degeneration of the facet joint with coronal articular surface compared with posterior shear force.

Abstract:BACKGROUND: Histomorphological change of endplate may affect the nntritional transmission of intervertebral disk, eventually leading to intervertebral disc degeneration. OBJECTIVE: To observe the effects of endplate concavity variation on mechanical response of lumbar motion segment. DESIGN, TIME AND SETTING: Repeated measurement design, biomechanical analysis of finite element models, performed at the Laboratory of Biomechanics, Department of Orthopedics, the Second Affiliated Hospital of Medical College of Zhejiang University between January 2005 and January 2007. MATERIALS: SOMATOM SENSATION 16 spiral CT machine (Siemens, German) and ANSYS (Inc. Pennsylvania, USA).METHODS: A three-dimensional nonlinear geometrical and mechanical accurate finite element model of lumbar L<,4-5> segment was developed. Parametric studies were undertaken by studying endplate of three different concave angles, whereas disc angle, the gap of facet joint, finite element mesh density, and all other parameters were kept constant. Biomechanical parameters of three kinds of finite element models were tested under 5 different loading conditions, including vertical compression, flexion, extension, and anterior and posterior shearing loading. MAIN OUTCOME MEASURES: Endplate-intervertebral disc interface strain, intervertebral disc stiffness, nucleus pulposus pressure, annular fiber stress, radial disc bulge, stress in the annulus ground substance, stresses in posterior structure and facet contact force. RESULTS: The decrease in the endplate concavity stimulated by an increasing endplate concave angle would result in decreased strains of endplate-intervertebral disc interface, increased disc stiffness and nucleus pulposus pressure, decreased annular fiber stress, radial disc bulge and stress in the annulus ground substance, and simultaneously produce decreased facet contact force and stresses in posterior structure. CONCLUSION: The decrease of endplate concavity enhances the protective effects of the disc on vertebral body breakage. Small endplate deformation results from decreased endplate concavity would contribute to the reduction of nutritional diffusion to the intervertebral disc.
Correlation Study Between Facet Angle and Facet Violation in Percutaneous Pedicle Screw Placement in Lumbar Vertebrae

Abstract:Objective To investigate the relationship between the facet angle (FA) and facet violation in percutaneous pedicle screw placement in lumbar vertebrae.Methods From December 2013 to November 2016,atotal of 115 lumbar fracture or degenerative disease patients who had undertaken percutaneous pedicle screw operation was retrospectively analyzed.There were 56 males and 59 females,with an average age of 53.71±12.19 years (ranged from 15 to 77 years).Measure the FA at the level of pedicle through CT scan,diagnosis and evaluate the grade of facet joint violation after the operation.Analyzed the effect of variant FA and lumbar segment (L-L5) on the facet violation (FV) with two-way analysis of variance,and evaluate the correlation between the FA and FV in percutaneous pedicle screw placement.Results There was no significant difference between the two groups on age,gender,and body mass index.476 percutaneous pedicle screws were operated in this study:L1 144 screws,L2 136 screws,L3 64 screws,L,72 screws and L5 60 screws.The total FV rate was 30.46％ (145/476).344 screws in the upper lumbar group,and the FV rate was 28.78％ (99/344);132 screws in lower lumbar group,and the FV rate was 34.85％ (46/132).There was no significant difference of FV rate between the two groups (x2=1.66,P=0.20).The result of two-way analysis of variance indicated that the FV rate increased dramatically when FA ＞ 35° (F=20.12,P ＜ 0.001),but FV rate was not related to the lumbar segment statistically (F=0.93,P=0.45).Spearman rank correlation analysis was performed between FA and FV rate,FV grade.The result was both positive (r=0.25,P ＜ 0.001 and r=0.27,P ＜ 0.001).Conclusion The traditional C-arm fluoroscopy percutaneous pedicle screw placement technique has a high rate of FV,and the size of FA significantly affects the incidence and severity of FV.
The Effect of Disc Height and Degeneration on the Mechanical Behavior of Lumbar Spine:A Finite Element Analysis

Abstract:Objective: To evaluate the effects of disc height and degeneration on mechanical behavior of human lumbar spine segments. Methods: A new effective CAD method was used to accurately establish three finite element models, including normal height disc (NHD) model, pure height decreased disc (PHDD) model and degeneration combined height decrease disc (DHDD) model. The biomechanical parameters of three finite element models were measured under axial compressive load. Results: The height and degeneration of disc had obviously effects on the mechanical behavior of lumbar spine segment in terms of axial displacement, posterolateral disc bulge, the maximum stress in the peripheral anulus fibers. The stress distribution differed between DHDD and non-DHDD model. Conclusions: The stiffness of disc obviously increases after height decreasing. The happening of disruption of inner anulus fibers is the lowest in DHDD model. The ratio between the gap of facet articulation and axial displacement are crucial to determine the load strength of disc. The degenerated disc plays an effective role for stress distribution.

Abstract:BACKGROUND:Many studies have explored the relationship between facet tropism and facet joint osteoarthritis, disc degeneration and degenerative spondylolisthesis. However, the associations between facet orientation and tropism, and paraspinal muscles have not been studied.OBJECTIVE:To analyze the associations between facet orientation and tropism, and parameters of paraspinal muscles in patients with chronic low back pain.METHODS:Ninety-five patients with chronic low back pain were consecutively enrolled. Their facet joint angles were measured on computed tomography (CT) while gross cross-sectional area (GCSA), functional cross-sectional area (FCSA) and T2 signal intensity of lumbar paraspinal and psoas muscle were evaluated on magnetic resonance imaging (MRI).RESULTS:The GCSA and FCSA were significantly smaller for multifidus muscle (P< 0.001), but significantly larger for erector spinae and psoas muscles (P< 0.001), in coronally-orientated group than those in sagittally-orientated group. The differences of bilateral GCSA and FCSA of multifidus muscle were significantly larger in facet tropism group than those in no facet tropism group (P= 0.009 and P= 0.019).CONCLUSIONS:Muscular asymmetries may develop in the lumbar region of the spine, which are associated with facet asymmetry in patients with chronic low back pain. Longitudinal studies are needed to understand the causal relationship between facet orientation and tropism and muscular asymmetry in future.
The relationship between degeneration and instability of lumbar spine: study of imaging

Abstract:Objective: To discuss the relationship along disc degeneration, fecet joint osteoarthritis and the instability of the degenerative lumbar spine. Methods: 120 patients suffering from lower back pain were analyzed adopting both MRI and X-rays of flexion and extension radiographs for the lumbar spine. The instability of lumbar spine was classified into abnormal tilting on flexion, rotatory instability in the sagittal plane, and translatory instability. Disc degeneration was classified into five grades according to T2-weighted sagittal images. Facet joint osteoarthritis as seen on axial T1-weighted images was divided into four grades. Results: On three intervertebral level, the grade of disc degeneration increased significantly (P<0.01) with age. Lumbar instability could be identified within 93 segments(26.3%) and correlated with grade 1 and grade 2 degeneration of the intervertebral discs(P<0.01). The kinematics of the lumbar motion segment were correlated with facet joint osteoarthritis(r=0.937,P<0.05). Anterior translatory instability was positively associated with disc degeneration and facet joint osteoarthritis(P<0.01). Conclusions: There is a positive relationship along the type of instability, disc degeneration and facet joint osteoarthritis.

The biomechanical influence of facet joint parameters on corresponding segment in the lumbar spine: a new visualization method

Abstract:Background context: Facet joints have been discussed as influential factors in the development of lumbar degeneration, which includes disc herniation and degenerative lumbar spondylolisthesis. Facet orientation (FO) and facet tropism (FT) are two important structural parameters of the lumbar facet joints. Many previous studies have focused on single parameter analysis of the lumbar spine. Owing to the correlation between independent variables, single-factor analysis cannot reflect the interaction between variables; however, there has been no corresponding biomechanical method developed to address this problem. Purpose: To investigate the complex biomechanical influences on the lumbar spine when vertebral FO and FT are varied using finite element analysis (FEA) and contour maps visualization, and analyze the biomechanical role of facet joint structural parameters in the process of lumbar degenerative diseases. Study design: A biomechanical modelling, analysis, and verification study was performed. Methods: A three-dimensional non-linear FEA model of 3 denucleated intervertebral discs (L2-3, L3-4, L4-5) with adjacent vertebral bodies (L2-L5) was created. Previously performed in vitro experiments provided experimental data for the range of motion in each load direction that was used for calibration. For 12 lumbar models, different facet joint angles relative to the sagittal plane at both L3-4 facet joints were simulated for 35°≤FO≤50° and 0°≤FT≤15°. By modifying different values of FO and FT, FEA simulation of different lumbar spine models was performed. Contour maps were used to visualize the FO- and FT-relevant data. Results: Under flexion, extension, and torsion moments, facet joint contact force and intradiscal stress increased with increasing FT. In the condition where FT remained 0° and increasing FO values, facet joint contact force and intradiscal stress remained low with no apparent increasing or decreasing trend when the model was under flexion, extension, and torsion moments. In the condition where FO and the FT values were varied at the same time, the highest force and stress regions in the contour maps were observed when all three types of moments were applied. Stress distributions of the L3-4 disc with different FT and FO values showed disc stress increased significantly with increases of FT and was concentrated on the ipsilateral region of the facet joint with the more sagittal orientation. Conclusions: The combination of FO and FT has an important impact on the corresponding disc and facet joints, but FT played a more significant role. Moreover, disc stress was concentrated on the ipsilateral region of facet joint with greater sagittal orientation when FT existed. FT with high sagittal orientation may increase risk of recurrent LDH due to increase ipsilateral disc pressure. Clinical significance: These biomechanical findings may help clinicians to understand the prognosis of some lumbar degenerative conditions.

Abstract:Study Design. Computed tomography study. Objective. To obtain detailed information on the facet orientation in the subaxial cervical spine and explore the correlation to the cervical movements and relevant clinical conditions. Summary of Background Data. Although facet orientation was well studied in the lumbar spine, the literatures on the cervical facet orientation were limited and the descriptions were nonspecific. Methods. The computed tomography scans of 100 individuals were reconstructed. For each level from C2/C3 to C6/C7, the horizontal plane, the mid-sagittal plane, the coronal plane, and the two facet planes were established. The normal vectors of the five planes were used for the calculation of the facet orientation and the facet tropism. Results. The angle of the facet plane with respect to the horizontal plane at the C6/7 level was the largest (left side: 64.34 degrees +/- 6.60 degrees, right side: 63.37 degrees +/- 6.81 degrees, P > 0.05). The angle of the facet plane with respect to the coronal plane decreased from C2/C3 level to C6/C7 level. Regarding the angle of the facet plane with respect to the sagittal plane, for the paired facet joints, three types were found: posteromedially oriented, posterolaterally oriented, and ipsilaterally oriented. All (100%) of the facet joints at the C2/C3 level and 65% at the C3/C4 level were posteromedially oriented. In the lower levels of the cervical spine, the posteromedially oriented facet joints were less common. The facet tropism was common phenomenon in the subaxial cervical spine. Conclusion. This study provided detailed information on the facet orientation in the subaxial cervical spine. The cervical facet orientation correlated well with the spinal movements and related clinical conditions.
Associations between facet tropism and vertebral rotation in patients with degenerative lumbar disease

Abstract:Abstract Background Vertebral rotation and facet tropism are very common in various lumbar degenerative diseases. Facet tropism means the presence of asymmetric angles on both sides of the facet joints. Studies have shown that facet tropism contributes to lumbar degenerative disease, and also inevitably leads to the asymmetry of movement and the imbalance of force, which may be possible to rotate the vertebral body. The aim of this study was to explore the correlation between lumbar vertebral rotation and facet tropism in patients with lumbar degenerative diseases. Methods A total of 198 patients with lumbar degenerative diseases from 2018 to 2019 were enrolled. Five hundred and seventy vertebral rotation angles and 1140 facet angles were measured. The vertebral bodies are divided into non-rotation group (Group A) and rotation group (Group B) with the vertebral rotation angle of 3° as the boundary. The information including gender, age, BMI (body mass index), bone mineral density, history of smoking, drinking, hypertension, diabetes, diagnosis, segment distribution, and degree of facet degeneration were also counted. Using inter-class correlation coefficients (ICC) to test the reliability of measurement results. Univariate and multivariate logistic regression analysis were used to analyze the relationship between vertebral rotation and facet tropism. Results The consistency of the ICC within the groups of the observers is above 0.8, with good agreement. The results of univariate analysis showed that facet tropism was significantly different between group A and group B (OR (odds ratio) = 3.30, 95% CI = 2.03–5.35, P < 0.0001). Other significant factors were included as adjustment variables into the multivariate regression model. Three models were analyzed separately (Model 1: non-adjusted. Model 2: adjust for age; facet degeneration; Model 3: adjust for age; disease distribution; segment distribution; facet degeneration). The results showed that after adjusting the confounders, the correlation between facet tropism and vertebral rotation did not change (Model 1: OR = 3.30, 95% CI = 2.03–5.35, P < 0.0001; Model 2: adjusted OR = 2.87, 95% CI = 1.66–4.97, P = 0.0002, Model 3: adjusted OR = 2.84, 95% CI = 1.56–5.17, P = 0.0006). Conclusion Current research demonstrates that there is an association between vertebral rotation and facet tropism, suggesting that vertebral rotation may also have a certain degree of correlation with lumbar degenerative diseases.

Abstract:The facet joint contributes to lumbar spine stability as it supports the weight of body along with the intervertebral discs. However, most studies on the causes of degenerative lumbar diseases focus on the intervertebral discs and often overlook the facet joints. This study aimed to investigate the impact of facet joint degeneration on the degenerative changes and diseases of the lumbar spine. A finite element model of the lumbar spine (L1–S1) was fabricated and validated to study the biomechanical characteristics of the facet joints. To simulate degeneration of the facet joint, the model was divided into four grades based on the number of degenerative segments (L4–L5 or L4–S1) and the contact condition between the facet joint surfaces. Finite element analysis was performed on four spine motions: flexion, extension, lateral bending, and axial torsion, by applying a pure moment to the upper surface of L1. Important parameters that could be used to confirm the effect of facet joint degeneration on the lumbar spine were calculated, including the range of motion (ROM) of the lumbar segments, maximum von Mises stress on the intervertebral discs, and reaction force at the facet joint. Facet joint degeneration affected the biomechanical characteristics of the lumbar spine depending on the movements of the spine. When analyzed by dividing it into degenerative onset and onset-adjacent segments, lumbar ROM and the maximum von Mises stress of the intervertebral discs decreased as the degree of degeneration increased in the degenerative onset segments. The reaction force at the facet joint decreased with flexion and increased with lateral bending and axial torsion. In contrast, lumbar ROM of the onset-adjacent segments remained almost unchanged despite severe degeneration of the facet joint, and the maximum von Mises stress of the intervertebral discs increased with flexion and extension but decreased with lateral bending and axial torsion. Additionally, the facet joint reaction force increased with extension, lateral bending, and axial rotation. This analysis, which combined the ROM of the lumbar segment, maximum von Mises stress on the intervertebral disc, and facet joint reaction force, confirmed the biomechanical changes in the lumbar spine due to the degeneration of isolated facet joints under the load of spinal motion. In the degenerative onset segment, spinal instability decreased, whereas in the onset-adjacent segment, a greater load was applied than in the intact state. When conducting biomechanical studies on the lumbar spine, considering facet joint degeneration is important since it can lead to degenerative spinal diseases, including adjacent segment diseases.

Abstract:Artificial cervical disc replacement (ACDR) is a widely accepted surgical procedure in the treatment of cervical radiculopathy and myelopathy. However, some research suggests that ACDR may redistribute more load onto the facet joints, potentially leading to postoperative axial pain in certain patients. Earlier studies have indicated that facet tropism is prevalent in the lower cervical spine and can significantly increase facet joint pressure. The present study aims to investigate the changes in the biomechanical environment of the cervical spine after ACDR using different prosthese when facet tropism is present.

Abstract:Aims: The presence of facet tropism has been correlated with an elevated susceptibility to lumbar disc pathology. Our objective was to evaluate the impact of facet tropism on chronic lumbosacral discogenic pain through the analysis of clinical data and finite element modelling (FEM). Methods: Retrospective analysis was conducted on clinical data, with a specific focus on the spinal units displaying facet tropism, utilizing FEM analysis for motion simulation. We studied 318 intervertebral levels in 156 patients who had undergone provocation discography. Significant predictors of clinical findings were identified by univariate and multivariate analyses. Loading conditions were applied in FEM simulations to mimic biomechanical effects on intervertebral discs, focusing on maximal displacement and intradiscal pressures, gauged through alterations in disc morphology and physical stress. Results: A total of 144 discs were categorized as 'positive' and 174 discs as 'negative' by the results of provocation discography. The presence of defined facet tropism (OR 3.451, 95% CI 1.944 to 6.126) and higher Adams classification (OR 2.172, 95% CI 1.523 to 3.097) were important predictive parameters for discography-'positive' discs. FEM simulations showcased uneven stress distribution and significant disc displacement in tropism-affected discs, where loading exacerbated stress on facets with greater angles. During varied positions, notably increased stress and displacement were observed in discs with tropism compared to those with normal facet structure. Conclusion: Our findings indicate that facet tropism can contribute to disc herniation and changes in intradiscal pressure, potentially exacerbating disc degeneration due to altered force distribution and increased mechanical stress.

Abstract:Although degenerative spondylolisthesis (DS) and lumber disc herniation (LDH) are common causes of back pain, the associations between the lumbar facet joint angle orientation and tropism in these conditions are still contentious. The objective of the meta-analysis was to identify the role of facet joint orientation and tropism on the development of lumbar degenerative disease, including DS and LDH. Electronic searches of PubMed and Embase were conducted up to April 2017. P value<0.05 was regarded as significant. Seventeen studies were included in this meta-analysis. Depending on the analysis, patients with DS have more sagittal orientated facet joints compared with control subjects (weighted mean difference [WMD] -10.5, 95% confidence interval [CI] -11.71 to -8.60; P<0.00001). We also found that DS patients have more remarkable facet joint tropism (WMD 1.84, 95% CI 0.77-2.91; P=0.0007). No significant correlation was observed between facet joint angle with LDH. There is a possibility that morphological deformity of facet joints could be associated with accelerated degeneration of stabilizing structures and affect the progress of DS. Our conclusion did not support the correlation between LDH and abnormalities of the facet joint. We tend to assume that the mechanics related to biomechanical changes in LDH disease progress might be more sophisticated.
Novel Risk Factors for Cervical Facet Joint Degeneration in the Subaxial Cervical Spine: Correlation with Cervical Sagittal Alignment and Bone Mineral Density

Abstract:Objective: The impact of cervical sagittal alignment on cervical facet joint degeneration (CFD) and the risk factors for CFD in patients with degenerative cervical myelopathy (DCM) were investigated in the current study. Methods: A total of 250 surgical patients with DCM were recruited. The clinical data and radiographical characteristics, including CFD, cervical sagittal balance parameters, Hounsfield unit (HU) values, disc degeneration (DD), and modic change, were collected. The detailed correlation between these characteristics and CFD was analyzed. Characteristics, including CFD, were compared among the various cervical alignment types and different CFD groups. Finally, the risk factors for CFD were revealed via logistic regression. Results: CFD was prevalent in DCM patients. Age, cervical sagittal vertical axis (cSVA), range of motion, T1 slope, thoracic inlet angle, DD, HU value, and modic change correlated with CFD segmentally and globally (P < 0.05). The lordosis and sigmoid types had a significantly higher CFD prevalence (P < 0.05). Furthermore, the average CFD threshold for the severe CFD group was 1.625 (area under the curve, 0.958). Additionally, 167 patients with average CFD <1.625 and 83 patients with CFD of ≥1.625 were classified into the mild CFD group and severe CFD group, respectively. Finally, multivariate analysis was performed, and age, cSVA, HU value, modic change, and DD were determined to be independent risk factors for CFD. Conclusions: The load distribution tends to shift to a more shear-like pattern in the sigmoid and kyphosis types and in those with a higher cSVA, thereby promoting CFD. Aging, cervical malalignment, low bone mineral density, DD, and modic change were revealed to result in high risks of CFD.
