We aim to establish common and extrapolatable intratumor bacteria (ITB)-associated biomarkers in ccRCC based on rigorous decontamination process and integration of multiple sequencing platforms across cohorts that vary in region, race, batch, sequencing tech etc. METHODS: This retrospective study included 2 original Chinese cohorts (Huashan and Renji) and 7 publicly or upon-request accessible transcriptome datasets, all comprised of ccRCC cases. ITB presence, association with clinicopathological parameters, and with host genetics and immunity were evaluated using a rigorous in-house tailored decontamination algorithm.Huashan cohort encompassed paraffin-embedded samples that underwent 16S rRNA sequencing and Renji cohort encompassed fresh samples that underwent RNA-seq. All data underwent a rigorous 3-pass decontamination process developed in-house. Presence of ITB was demonstrated by 16S FISH staining, electron microscopy and single-cell sequencing. Composition was presented by relative abundance. Overall and progression-free survival were primary outcomes profiled by ITB risk score using LASSO. Potential host interaction was exploratory analyzed using Sparse CCA. RESULTS: Nine cohorts encompassing a total of 1157 ccRCC cases and 141 paired normal tissues were initially analyzed and underwent decontamination. Two phyla were present in all cohorts and 11 genera were present in at least 5 cohorts. Surprisingly, neither diversity nor composition was differentially distributed between normal and cancer tissue. Notably, a 6-genera dichotomized ITB risk score was generated and validated in all cohorts. In particular, High abundance of g_Exiguobacterium was associated with both worsened overall and progression-free survival. Though g_Corynebacterium was associated with immunity and response to Nivolumab, ITB showed robust association with host ribosomal signaling while solely modest association with immunity. CONCLUSIONS: ITB exists in ccRCC. We developed a robust ITB score predicative of prognosis regardless of sequencing tech, sample processing or racial disparity. Those panels serve as novel biomarkers for ccRCC whereas association with host ribosome warrants mechanistic analyses on bacterial metabolites.
