We want to establish a standardized FISH method for detecting bladder cancer in Chinese population. METHODS 52 first-level hospitals were involved in a multi-center clinical study for massive screening of bladder cancer with FISH in China. In all, 988 healthy volunteers were enrolled in the study to set up a normal FISH population threshold and 3984 hematuria patients suspected to be urothelial carcinoma, but without the history of bladder cancer were randomly selected for simultaneous analysis of urine cytology and FISH testing. Then all patients were determined whether they suffered from bladder cancer or not by pathological diagnosis through cystoscopic biopsy and finally staged pathologically after operation. RESULTS In Chinese population, the overall sensitivity of FISH was 82.6% in detecting bladder cancer to compare with that of urine cytology 32.0%, which was significantly different in statistics. There were no significant difference for their specificities between FISH and urine cytology (94.2% and 93.8%, P>0.05). Its separate sensitivities on stage Ta, T1, T2, T3 and T4 tumors were 81.5%, 81.1%, 91.2%, 87.7% and 94.4% respectively to compare with those of urine cytology 23.8%, 30.4%, 44.5%, 50.8% and 66.7% (P<0.001). For FISH, there were no statistical differences between its sensitivities for different pathological stages (P>0.05). FISH sensitivities for low-grade and high-grade tumors were 79.0% and 91.1% respectively, which was statistically different (P<0.001), to compare with those of urine cytology 21.6% and 46.5%. FISH detection rates were 72.9%, 71.3%, 72.2% and 67.4% for chromosome abnormalities at p16, 3, 7, and 17 respectively. CONCLUSIONS In Chinese population, massive screening of bladder cancer with FISH showed that the genetic change of chromosome abnormalities at p16, 3, 7, 17 loci were consistent with bladder cancer. For detection sensitivities, FISH was significantly higher than urine cytology at any pathological stages and low and high grade tumors, whereas there was no significant difference between their detection specificities.
