Abstract:Objective This study aimed to evaluate the accuracy of the automated Health & Life oscillometric upper arm blood pressure monitor HL868BA for home blood pressure monitoring according to the European Society of Hypertension International Protocol for blood pressure measuring devices in adults. Method Systolic and diastolic blood pressures were sequentially measured in 33 adult Chinese using a mercury sphygmomanometer (two observers) and the HL868BA device (one supervisor). Ninety-nine pairs of comparisons were obtained from 15 participants in phase 1 and further 18 participants in phase 2 of the validation study. Data analysis was performed using the ESHIP Analyzer. Results The HL868BA device successfully passed phase 1 of the validation study with a number of absolute differences between device and observers within 5, 10, and 15 mmHg for at least 31/45, 42/45, and 44/45 measurements (required 25, 35, and 40, respectively), respectively. The device also achieved the targets for phase 2.1, with 68/99, 91/99, and 95/99 differences within 5, 10, and 15 mmHg, respectively, for systolic blood pressure, and with 71/99, 88/99, and 94/99 within 5, 10, and 15 mmHg, respectively, for diastolic blood pressure. In phase 2.2, 22 and 28 participants had at least two of the three device-observers differences within 5 mmHg (required >= 22) for systolic and diastolic blood pressure, respectively. Conclusion The Health & Life upper arm blood pressure monitor HL868BA can be recommended for home use in adults.

Ninety-nine pairs of comparisons were obtained from 15 participants in phase 1 and a further 18 participants in phase 2 of the validation study. Data analysis was performed using the ESHIP Analyzer. Results The LD-578 device successfully passed phase 1 of the validation study with a number of absolute differences between device and observers within 5, 10, and 15 mmHg for at least 32 of 45, 41 of 45, and 45 of 45 measurements (required 25, 35, and 40), respectively. The device also achieved the targets for phase 2.1, with 67 of 99, 90 of 99, and 98 of 99 differences within 5, 10, and 15 mmHg, respectively, for systolic blood pressure, and with 69 of 99, 95 of 99, and 98 of 99 within 5, 10, and 15 mmHg, respectively, for diastolic blood pressure. In phase 2.2, 24 participants had at least two of the three device-observers differences within 5 mmHg (required >= 22) for systolic and diastolic blood pressure. Conclusion The HONSUN upper arm blood pressure monitor LD-578 can be recommended for home use in adults. Abstract:Objective: The present study aimed to evaluate the accuracy of the Health & Life automated oscillometric upper-arm blood pressure monitor HL868ED for blood pressure monitoring according to the British Hypertension Society Protocol. Method: We recruited individuals until there were 85 eligible participants and their blood pressure could meet the blood pressure distribution requirements specified by the British Hypertension Society Protocol. For each individual, we sequentially measured systolic and diastolic blood pressures using a mercury sphygmomanometer (two observers) and the HL868ED device (one supervisor). Data analysis was carried out according to the British Hypertension Society Protocol. Results: The device achieved grade A. The percentage of blood pressure differences within 5, 10 and 15 mmHg was 68%, 91% and 98%, respectively, for systolic blood pressure, and 79, 96 and 99%, respectively, for diastolic blood pressure. The average (+/- SD) of the device-observer differences was -1.8 +/- 6.0 mmHg for systolic and 0.1 +/- 4.7 mmHg for diastolic blood pressures, respectively. Conclusion: The Health & Life automated upper-arm blood pressure monitor HL868ED has passed the requirements of the British Hypertension Society Protocol, and hence can be recommended for blood pressure measurement in adults. Abstract:Objective To assess the accuracy of wearable electrocardiogram(ECG)monitoring device for detecting arrhythmias(sinus rhythm,premature ventricular contraction,premature atrial contraction,and atrial fibrillation),and heart rate in a clinical setting.Methods A total of 321 volunteers(875 ECG records)participated in this study,including 146 males(531 ECG records)and 175 females(344 ECG records).A wearable ECG monitoring device was employed to record ECGs for detecting arrhythmias and heart rate.The detection results were compared with physicians'clinical diagnostic results based on 12-lead ECG machine(regraded as the gold standard for diagnosing arrhythmias and detecting heart rate).Accuracy and consistency evaluation of the two detection methods was conducted in all the subjects,including accuracy,sensitivity,specificity,F1 score,area under curve(AUC)of the receiver operating characteristic curve,Bland-Altman consistency analysis,mean absolute error,and correlogram.Results Regarding arrhythmia identification,wearable ECG monitoring device exhibits impressive performance metrics,with an AUC of 0.98,accuracy of 0.98,sensitivity of 0.96,specificity of 0.99,and an F1 score of 0.96.In terms of heart rate monitoring,a robust correlation between the wearable ECG monitoring equipment and the ECG machine is evident,with a substantial total correlation coefficient of 0.84(P＜0.01).Bland-Altman consistency analysis reveals that the two methods display a mean difference of-0.97 times per minute,a mean standard deviation of 8.02 times per minute,and a 95%confidence interval ranging from-16.69 to 14.75 within an acceptable level.Conclusion Wearable ECG monitoring device can accurately detect arrhythmias and heart rate.

Abstract:Epidemiological investigation and large scale prospective study shows that hypertension is an independent risk factor for cardiovascular disease. The daily blood pressure self-monitoring by using electronic BP monitor is an important way to prevent serious cardiovascular events. A new method for designing electronic BP monitor is proposed; firstly, determine the single-wave's peak points of Sasser; secondly, determine the best fitting curve and fit the contour line of Sasser; then, find maximum of the contour line, select integration interval and determine the location of DBP and SBP by the proposed integral formula and the location of the maximum, and finally determine DBP and SBP. Compared BP measured by the proposed method and auscultation, results show that BP measured by the proposed method has an accuracy consistent with AAMI standard and BHS(B) standard. So the proposed method is applicable, electronic BP monitor designed by using the proposed method can be used to monitor BP daily.

Abstract:ObjectiveThe present study aimed to evaluate the accuracy of the automated oscillometric wrist blood pressure monitor AVITA BPM15S for home blood pressure monitoring according to the International Protocol revision 2010 of the European Society of Hypertension.MethodSystolic and diastolic blood pressures were sequentially measured in 33 Chinese adults (15 women, mean age 51 years) using a mercury sphygmomanometer (two observers) and the AVITA BPM15S device (one supervisor). Ninety-nine pairs of comparisons were obtained from 33 participants for judgments in two parts with three grading phases.ResultsThe AVITA BPM15S device achieved the targets in part 1 of the validation study. The number of absolute differences between the device and observers within 5, 10, and 15 mmHg were 85/99, 94/99, and 98/99, respectively, for systolic blood pressure, and 82/99, 96/99, and 98/99, respectively, for diastolic blood pressure. The device also achieved the criteria in part 2 of the validation study. Thirty-two and 28 participants for systolic and diastolic blood pressure, respectively, had at least two of the three device-observer differences within 5 mmHg (required24). No participant had all of the three device-observer comparisons greater than 5 mmHg for systolic or diastolic blood pressure.ConclusionThe AVITA wrist blood pressure monitor BPM15S fulfilled the requirements of the International Protocol revision 2010 and hence can be recommended for home use in an adult population.

Abstract:Objective The present study aimed to evaluate the accuracy of the fully automated oscillometric upper-arm blood pressure monitor TM-2656 according to the British Hypertension Society (BHS) Protocol 1993.Methods We recruited individuals until there were 85 eligible participants and their blood pressure could meet the blood pressure distribution requirements specified by the BHS Protocol. For each individual, we sequentially measured the systolic and diastolic blood pressures using a mercury sphygmomanometer (two observers) and the TM-2656 device (one supervisor). Data analysis was carried out according to the BHS Protocol.Results The device achieved grade A. The percentage of blood pressure differences within 5, 10, and 15 mmHg was 62, 85, and 96%, respectively, for systolic blood pressure, and 71, 93, and 99%, respectively, for diastolic blood pressure. The average (+/- SD) of the device-observer differences was -2.1 +/- 7.8 mmHg (P<0.0001) and -1.1 +/- 5.8 mmHg (P<0.0001) for systolic and diastolic blood pressures, respectively.Conclusion The A&D upper-arm blood pressure monitor TM-2656 has passed the requirements of the BHS Protocol, and can thus be recommended for blood pressure measurement. Abstract:Atrial fibrillation (AF) has been considered as a growing epidemiological problem in the world, with a substantial impact on morbidity and mortality. Ambulatory electrocardiography (e.g., Holter) monitoring is commonly used for AF diagnosis and therapy and the automated detection of AF is of great significance due to the vast amount of information provided. This study presents a combined method to achieve high accuracy in AF detection. Firstly, we detected the suspected transitions between AF and sinus rhythm using the delta RR interval distribution difference curve, which were then classified by a combination analysis of P wave and RR interval. The MIT-BIH AF database was used for algorithm validation and a high sensitivity and a high specificity (98.2% and 97.5%, respectively) were achieved. Further, we developed a dataset of 24-h paroxysmal AF Holter recordings (n=45) to evaluate the performance in clinical practice, which yielded satisfactory accuracy (sensitivity=96.3%, specificity=96.8%).

The device also fulfilled the criteria in part 2 of the validation study. Twenty-four and 30 participants for systolic and diastolic blood pressure, respectively, had at least two of the three device-observer differences within 5 mmHg (required ≥24). Three participants for systolic and one participant for diastolic blood pressure had all the three device-observer comparisons greater than 5 mmHg. Conclusion The Health & Life blood pressure monitor HL868ED has passed the requirements of the International Protocol revision 2010 and hence can be recommended for blood pressure measurements in adults.
Accuracy Study of Digital Blood Pressure Monitor in Clinic Measurement

Abstract:To achieve the standardization, informatization and automation management of the hospital, the certified digital blood pressure (BP) monitors were used to measure BP instead of traditional auscultatory method. However, since the BP measurement was easily effected by several uncertain factors, the results may be unreliable and due to the measurement could not be retrospected effectively, some hypertension patients may be missed or misdiagnosed. To study the accuracy of the clinic measurement by digital BP monitor, this paper randomly selected 640 patients from the hospital to compare with the other ambulatory BP monitor, which could be re-diagnosed after BP measurement. Using the evaluation method proposed by the British Hypertension Society (BHS), the percentage of the systolic and diastolic BP in the ranges of ≤5 mmHg, ≤10 mmHg, ≤15 mmHg, was 41%, 69%, 86% and 49%, 81%, 93%,respectively. The result shows that the accuracy could not satisfy the recommendation of the BHS standard in clinic application. Therefore, to achieve the accurate and effective management of BP database, we suggest that besides the standardization of the BP measurement procedure, it is recommended to establish a system to retrospect the BP results effectively.

Abstract:It is controversial to use oscillometric blood pressure (BP) monitors for BP measurement in atrial fibrillation (AF) patients due to the variation in stroke volume. Several recent studies revealed no significant difference in oscillometric and intra-arterial BP bias between AF and sinus rhythm (SR) patients, but the population sizes were limited. We performed a cross-sectional study based on an extensive real-world database to further demonstrate the impact of AF on the accuracy of oscillometric blood pressure monitoring in the intensive care unit setting. Adult patients with AF or SR records were enrolled from Medical Information Mart for Intensive Care-III (MIMIC-III) database. Concurrently-recorded non-invasive oscillometric BPs (NIBPs) and invasive intra-artery BPs (IBPs) were extracted and classified as AF or SR group according to the heart rhythm. Pairwise comparison was performed on NIBP/IBP bias between AF and SR circumstances. Bland-Altmann plots were used to assess bias and limits of agreement. A multivariable linear mixed-effect model was used to adjust confounders. Subgroup and interaction analyses were performed. 2335 patients (71.95 ± 11.23 years old, 60.90% of male) were included. The systolic (SBP), diastolic (DBP), and mean (MBP) NIBP/IBP biases were not clinically different between AF and SR circumstances (SBP: AF vs. SR, 0.66 ± 19.64 mmHg vs. 1.21 ± 21.96 mmHg, P = 0.002; DBP: -5.29 ± 12.03 vs. -5.17 ± 11.54, P = 0.124; MBP: -4.45 ± 12.31 vs. -4.19 ± 12.61, P = 0.014). After adjusting age, gender, heart rate, arterial BP, and vasopressor usage, the effect of heart rhythm on NIBP/IBP bias was still within ± 5 mmHg for SBP and DBP [adjusted effect on SBP bias: 3.32 mmHg (95% CI 2.89 to 3.74), P < 0.001; DBP: -0.89 (-1.17 to -0.60), P < 0.001], while the effect on MBP bias was not significant [adjusted effect 0.18 mmHg (-0.10 to 0.46), P = 0.200]. Atrial fibrillation would not significantly influence the accuracy of oscillometric monitoring in critically ill patients compared with sinus rhythm circumstance. Data analysis was done using the ESHIP Analyzer.RESULTS:The BP101H device successfully passed phase 1 of the validation study with a number of absolute differences between device and observers within 5, 10, and 15 mmHg for at least 34/45, 42/45, and 43/45 measurements, respectively. The device also achieved the targets for phase 2.1, with 79/99, 94/99, and 96/99 differences within 5, 10, and 15 mmHg, respectively, for systolic blood pressure, and with 81/99, 94/99, and 98/99 within 5, 10, and 15 mmHg, respectively, for diastolic blood pressure. Diagnostic accuracy for detecting atrial fibrillation using a novel machine learning algorithm in a blood pressure monitor

Abstract:Background: Early detection of atrial fibrillation (AF) is key for preventing strokes. Blood pressure monitors (BPMs) with built-in AF screening features have the potential for early detection at home. Recently, 2 BPMs (HEM-7371T1-AZ and HEM-7372T1-AZAZ, Omron Healthcare Co., Ltd.) that share a novel AF screening feature have been developed. Their AF screening feature utilizes an algorithm that incorporates machine learning, with the potential to improve diagnostic accuracy. Objective: The purpose of this study was to evaluate the performance of this AF screening feature in a multicenter, prospective clinical study at 5 sites in the United States. Methods: A total of 559 subjects were enrolled for this study: 267 in AF cohort and 292 in the non-AF cohort. AF screening was performed in all subjects by the 2 Omron BPMs and by 1 Microlife BPM (BP 3MX1-3, WatchBP Home A, Microlife Corp.), and a simultaneous 12-lead electrocardiogram (ECG) was recorded for comparison. All 12-lead ECGs were interpreted by a board-certified cardiologist who was blinded to the BPM results. Sensitivity, specificity, and accuracy for the diagnosis of AF were calculated. Results: Omron HEM-7371T1-AZ BPM had sensitivity of 95.1% (95% confidence interval [CI] 91.8%-97.4%), specificity 98.6% (95% CI 96.6%-99.7%), and accuracy of 97.0% (95% CI 95.2%-98.2%). Equivalent results were obtained with the Omron HEM-7371T1-AZAZ BPM. This compared favorably to the Microlife BPM (sensitivity 78.5%, 95% CI 73.1%-83.3%; specificity 97.6%, 95% CI 95.1%-99.0%; accuracy 88.4%, 95% CI 85.5%-91.0%). Conclusion: These data support both home and professional use of these novel Omron BPMs for the detection of AF.

Abstract:It is recommended that oscillometric devices be calibrated by auscultation when first used, but this is difficult in practice. Here, we introduce a smartphone-based technique to verify the accuracy of blood pressure monitors (BPMs). We enrolled 99 consecutive subjects and tested 6 brands of BPMs in this study. During measurements of electronic oscillometric BPMs, Korotkoff sounds were simultaneously collected using a stethoscope head beneath a cuff connected to a smartphone, and an app named Accutension Stetho could then yield an auscultatory BP reading as a reference. Next, differences in BP between the different BPMs and Accutension Stetho were determined. The percentage of BP differences falling within 5, 10, and 15 mm Hg; the mean (MD) value; and the standard deviation (SD) of BP differences and deflation errors were analyzed among all the BPMs. We found that the percentages of SBP differences falling within 5 mm Hg of the 6 BPMs were 80%, 79%, 77%, 72%, 68%, and 63%, in turn. The deflation rates among the 6 BPMs were 2.23, 3.48, 6.10, 2.44, 3.66, and 4.85 mm Hg/beat, respectively. Deflation errors, which were defined as deflation prior to the end of the Korotkoff sounds, existed in 4 BPMs. In conclusion, Accutension Stetho could detect BP differences between oscillometric BPM readings and simultaneous auscultatory readings. Diastolic BP was overestimated when the device deflated prior to the end of the Korotkoff sounds. Using the app, it is possible to evaluate the accuracy of BPMs among the same subjects.

Abstract:ObjectiveThe present study aimed to evaluate the accuracy of the AVITA BPM63S upper arm blood pressure monitor for home blood pressure monitoring according to the International Protocol of the European Society of Hypertension revision 2010.MethodsSystolic and diastolic blood pressures were sequentially measured in 33 adult Chinese (14 women, mean age of 47 years) using a mercury sphygmomanometer (two observers) and the AVITA BPM63S device (one supervisor). Ninety-nine pairs of comparisons were obtained from 33 participants for judgments in two parts with three grading phases.ResultsAll the blood pressure requirements were fulfilled. The AVITA BPM63S device achieved the targets in part 1 of the validation study. The number of absolute differences between device and observers within 5, 10, and 15 mmHg was 68/99, 89/99, and 96/99, respectively, for systolic blood pressure, and 75/99, 95/99, and 97/99, respectively, for diastolic blood pressure. The device also achieved the criteria in part 2 of the validation study. Twenty-four and 25 participants for systolic and diastolic blood pressure, respectively, had at least two of the three device-observers differences within 5 mmHg (required 24). One and two participants for systolic and diastolic blood pressure, respectively, had all three device-observers differences greater than 5 mmHg.ConclusionThe AVITA BPM63S automated oscillometric upper arm blood pressure monitor has passed the requirements of the International Protocol revision 2010, and hence can be recommended for blood pressure measurement at home in adults.
Accuracy of oscillometric blood pressure measurement using a Cardell Touch multiparameter monitor in anesthetized pigs

Abstract:Objective: To assess the accuracy of an oscillometric monitor in anesthetized pigs and its ability to track changes in mean arterial pressure (MAP) and to detect arterial hypotension and hypertension in reference to direct measurements. Methods: Mean arterial pressure was measured simultaneously from a catheter in a femoral artery and with an oscillometric cuff placed over the metatarsus in 9 anesthetized pigs (∼6 months old, 35 to 55 kg). Pigs were subjected to maneuvers to alter MAP. Paired values for invasive and noninvasive MAP (iMAP and NiMAP) were recorded every 2 minutes. Bland-Altman plots with bias, limits of agreement, and percentage error were constructed using each pair and the average of 5 consecutive values. Concordance was calculated using changes in MAP at 20-minute intervals. Receiver operating characteristics (ROC) curves were constructed to test NiMAP for detection of hypotension (< 70 mm Hg) and hypertension (≥ 120 mm Hg). Results: Bias of NiMAP was -8.59 mm Hg for consecutive pairs and -8.85 mm Hg for averaged pairs, relative to iMAP. Limits of agreement and percentage error were reduced for averages (19%) over individual pairs (26%). Concordance was 82%. The area under the ROC curve for detecting hypotension with NiMAP was 0.936, with a best cutoff value of 63 mm Hg NiMAP. The area under the ROC curve for hypertension was 0.940, with a best cutoff value of 101 mm Hg NiMAP. Conclusions: Averaging several consecutive values improves the accuracy of NiMAP measurements. Clinical relevance: This device correctly tracked changes in MAP approximately 80% of the time and appears reliable for diagnosing arterial hypotension.
