Many challenging liver can be performed laparoscopically to decrease the surgical trauma. Abstract:Background Laparoscopic surgery is advantageous for minimal invasiveness and rapid postoperative recovery. Since the use of laparoscopic hepatectomy in liver resection in the 1990s, it has been performed in a few institutions worldwide. Lack of efficient and safe techniques for liver transaction is the major obstacle preventing from its further development. We developed a new technique for laparoscopic hepatectomy by curettage and aspiration in 1998. In this paper we analyze the clinical outcomes of this technique after 7 years of practice. Methods Altogether 59 consecutive patients underwent laparoscopic hepatectomy by curettage and aspiration from August 1998 to January 2005 at our institution. These patients included 33 males and 26 females, with a mean age of 47 years. For liver transecsion laparoscopic Peng's multifunctional operative dissector (LPMOD) was used. Lesions included malignant liver tumors in 19 patients, benign liver tumors in 17, intrahepatic calculus in 18, and other liver lesions in 5. Procedures included local resections in 30 patients, left lateral segmentectomy in 28, and right hemihepatectomy in 1. Results Laparoscopic operation was completed in 57 patients. Two patients (3.4%) had the operation converted to laparotomy. The mean operating time was 143 minutes and the mean intraoperative blood loss was 456 ml. The mean length of postoperative hospital stay was 7 days. Complications occurred in 2 patients (3.4%), and there was no perioperative death. Conclusion Laparoscopic hepatectomy by curettage and aspiration is efficient and safe for liver resection.

Abstract:Laparoscopic pancreaticoduodenectomy is eventually gaining momentum after 10 years’development of laparoscopic surgical skills and endoscopic surgical devices. Some large volume minimal invasive pancreatic surgery centers have proved that LPD had same short and long-term outcomes compared with OPD,and shown some minimal invasive benefits than traditional OPD. In our country,less than 10 surgical centers have experience of more than 100 cases of LPD,most of the traditional large volume pancreatic centers are not interested in performing LPD. However, most of the surgeons who perform LPD are in small volume pancreatic centers with potential high risks of postoperative mortality and morbidity. To reduce the risks of LPD during the learning curve period,it is important to standardize the surgical procedure and perioperative treatment of these patients.

Abstract:AIM: To review current applications of the laparoscopic surgery while highlighting the standard procedures across different fields.METHODS: A comprehensive search was undertaken using the PubMed Advanced Search Builder. A total of 321 articles were found in this search. The following criteria had to be met for the publication to be selected: Review article, randomized controlled trials, or meta-analyses discussing the subject of laparoscopic surgery. In addition, publications were hand-searched in the Cochrane database and the high-impact journals. A total of 82 of the findings were included according to matching the inclusion criteria. Overall, 403 full-text articles were reviewed. Of these, 218 were excluded due to not matching the inclusion criteria.RESULTS: A total of 185 relevant articles were identified matching the search criteria for an overview of the current literature on the laparoscopic surgery. Articles covered the period from the first laparoscopic application through its tremendous advancement over the last several years. Overall, the biggest advantage of the procedure has been minimizing trauma to the abdominal wall compared with open surgery. In the case of cholecystectomy, fundoplication, and adrenalectomy, the procedure has become the gold standard without being proven as a superior technique over the open surgery in randomized controlled trials. Faster recovery, reduced hospital stay, and a quicker return to normal activities are the most evident advantages of the laparoscopic surgery. Positive outcomes, efficiency, a lower rate of wound infections, and reduction in the perioperative morbidity of minimally invasive procedures have been shown in most indications.CONCLUSION: Improvements in surgical training and developments in instruments, imaging, and surgical techniques have greatly increased safety and feasibility of the laparoscopic surgical procedures.

Abstract:Surgical treatment is the classic treatment modality for focal liver diseases. In 140 years, liver surgery is constantly evolving, along with advances in understanding anatomy, physiology, and emergence of technologies. During the last 30 years, many surgical techniques used in the modern ages became available since the 1990s, and liver surgery is becoming a routine procedure in many hospitals in China because of the significant decrease in surgical mortality. Furthermore, liver surgery is playing an increasingly important role in multimodality treatment for liver cancer while the treatment pattern is also changing because of progresses in systemic treatment. The progresses in liver surgery are summarized in the article.

Abstract:This review focused on the progress in laparoscopic pancreaticoduodenectomy(LPD) in the past six years.With the appropriate approaches under laparoscopy, including the resection and reconstruction, LPD has been proved to be safe and feasible. In some centers, LPD has been routine with rapid growth of numbers, it not only benefit the patients with fast recovery, but also benefit the trainees with similar sights as the primary surgeon and good videos of the procedures. However, LPD is still controversial as the more complications in some centers and inconclusive oncologic outcomes. Thus, in the further, a long-time outcome monitoring of LPD is essential. A registry of a prospectively maintained database may be a need for LPD to evaluate its outcomes by multicenter randomized control trials, and real world research may be of value. Structured LPD training programs are valuable for the new surgeons.

Abstract:After a brief account of the origins of laparoscopy and of its development into an interventional technique thanks to technical improvement, the author gives a dynamic state of the art in laparoscopic surgery, beginning with a description of his original classification method for laparoscopic procedures according to their level of use. Before tackling the highly controversial issue of laparoscopic surgery for cancer, he reviews in detail the various operations successfully performed by laparoscopy for the treatment of nonmalignant abdominal disorders and acute syndromes, all the while checking their results against those obtained with their open counterparts. As the various procedures are described, the interest of the laparoscopic approach becomes clearly visible, based on the excellent view of the operative field that allows diagnostic accuracy, thus avoiding unnecessary operations, and precise dissection with minimal damage. However, the laparoscopic approach is highly dependent on the surgeon's proficiency, itself depending on experience and therefore on the frequency of occurrence of the disorder, and on the progress in and availability of adequate equipments. The last chapter looks into the future of this ever-expanding approach and defines two great trends in its evolution, one accessible to the individual surgeon, the gradual adaptation of open procedures into laparoscopic ones, the other at the institutional level, in view of the huge financial support involved if high technologies are to be systematically integrated in this new surgery. What will the future be like?

Abstract:The development of science and technology dramatically promotes the application of minimally invasive technique in the field of gastrointestinal surgery.Today,almost all kinds of gastrointestinal operations can be accomplished through or with the assistance of a laparoscope.Compared to traditional open surgery,lapm'oscopic surgery,which is accomplished by experienced surgeons.can leads to a similar or even better surgical outcome.Although the endoscopic technique was firstly invented as a diagnostic method,its application has expanded to many different medical areas.Now,endoscopic gastrointestinal polypectomy is extensively applied in primary hospitals.Moreover,endoscopic mucosal resection and endoscopic submucosal dissection are also ca~ied out in many large medical centers in China.More and more patients are benefiting a lot from the minimally invasive technique.As the concept of minimally invasive technique is widely accepted,minimally invasive surgery will be a major aspect in the development of gastrointestinal surgery.As a contemporary gastrointestinal surgeon,it is imperative to handle the minimally invasive technique proficiently. Abstract:Since its introduction in China in 2000, laparoscopic gastrectomy has shown classical advantages of minimally invasive surgery over open counterpart. Like all the pioneers of the technique, Chinese gastrointestinal surgeons claim that laparoscopic gastrectomy led to faster recovery, shorter hospital stay and more rapid return to daily activities respect to open gastrectomy while offering the same functional and oncological results. There has been booming interest in laparoscopic gastrectomy since 2006 in China. The last decade has witnessed national growth in the application of laparoscopic gastrectomy and yielded a significant amount of scientific data to support its clinical merits and advantages. However, few prospective randomized controlled trials have investigated the benefits of laparoscopic gastrectomy in China. In this article, we make an overview of the current data and state of the art of laparoscopic gastrectomy for gastric cancer in China.

Abstract:With the advent of the era of minimally invasive surgery, hernia and abdominal wall surgery as a microcosm of the development history of surgical have also been affected. Laparoscopic hernia surgery has the advantages of less trauma and fast recovery. Once it came out, it was quickly promoted worldwide. Compared with foreign countries, China’s laparoscopic hernia surgery started relatively late. However, due to the large number of patients and the good professional quality of Chinese hernia surgeons, after 20 years of development, China’s laparoscopic hernia surgery has reached the international advanced level in terms of quantity and quality. With the advent of the 5G era, the advantages of high-speed communication technology should be given full pay to benefit more hernia patients with minimally invasive technology.

Abstract:With progress of technique and accumulation of experience, laparoscopic liver resection (LLR) has become a standard operation. China is a country with high incidence of hepatocellular carcinoma (HCC). In the past 20 years, LLR has overcome "double barriers of concept and technique" . After exploration, development and optimization, it evolves from initial wedge resection to hemihepatectomy, extended hemihepatectomy, mesohepatectomy, followed by precise anatomical hepatectomy. With the continuous improvement of technical approaches, the standardized operation system of LLR has preliminarily taken shape. From the perspectives of surgical and oncological view, it has been proven to bring "dual advantages of minimal invasion and survival" to patients. At present, surgical techniques develop with each passing day, and systemic therapeutic drugs emerge in an endless stream. Along with new pattern and new challenge, China’s laparoscopic liver resection for HCC is striding into a new era with a brilliant future.

Abstract:Laparoscopic pancreaticoduodenectomy (LPD) is an extremely challenging surgery. First described in 1994, LPD has been gaining a favorable position in the majority of pancreatic surgery. Now, LPD is worldwide accepted. A literature search was conducted in PubMed, and only papers written in English containing more than 26 publications of LPD were selected. Papers in distal and robotic pancreatic procedure were not included in the review of a total of 222 LPD publications. The total number of patients analyzed was 1,082 from 25 articles and the largest series. Six of these studies came from the United States, 1 from France, 5 from South Korea, and 1 from India, 2 from Japan, 5 from China, 1 from Italy, 1 Germany, 2 from UK. The overall pancreatic fistula rate was 20.5%. The overall conversion rate was 10.4%. LPD seems to be a valid alternative to the standard open approach with similar technical and oncological results. LPD is a safe procedure, providing many of the advantages typically associated with laparoscopic procedures. We expect this operation to continue to gain in popularity as well as be offered in increasingly more complex cases. In future studies, it will be beneficial to look further at the oncologic outcome data of LPD including survival.

Abstract:In the past 30 years, laparoscopic hepatectomy has developed rapidly, which multi-dimensionally promoted the innovation of hepatectomy technology and strategy. This is mainly attributed to the key factors such as the improvement of domestic and foreign scholars′ under-standing of liver anatomy, the conversion and application of laparotomy technology, the expansion of laparoscopic vision and the feedback in surgery related fields.
