Abstract:Coronary artery fistula (CAF) is a rare entity. Sometimes it may associate with mild diffuse or segmental coronary ectasia. CAF with giant coronary artery is exceptionally rare. We present a unique case of a 49-year-old female patient with a giant right coronary artery of diffuse ectasia coexisting with a fistula draining into the right ventricle. To our best knowledge, CAF with diffuse coronary ectasia of such giant size has never been reported. The patient was treated successfully by resection of the dilated right coronary artery, fistula closure, and coronary artery bypass grafting.

Abstract:Background Coronary artery aneurysm is a rare condition which constitutes a small proportion of coronary artery disease. Such condition mimicking an intra-cardiac mass is extremely rare and poorly understood. Case presentation We present an unusual case of a 53-year-old female with decreased exercise tolerance and lower extremity edema for 3 months. The echocardiography showed moderate tricuspid regurgitation and a right intra-ventricular mass below the tricuspid valve. No ventricular wall akinesia or ST segment change was found on echocardiography or electrocardiogram. Coronary computed tomographic angiography confirmed the diagnosis of intra-ventricular mass with feeding vessel originated from the right coronary artery. The patient was scheduled for tumor resection, and the mass turned out to be a thrombosed giant right coronary artery aneurysm. The patient received successful aneurysm resection and had an uneventful postoperative recovery. Unfortunately, a fistula between right coronary artery and right ventricle was detected on follow-up three months later by echocardiography. Conclusions Coronary artery aneurysms presenting as intra-cardiac masses are extremely rare. Comprehensive preoperative evaluation is highly recommended because the surgical strategies for tumor and aneurysm are completely different. Aneurysm resection with bypass surgery is preferred rather than aneurysm repair. To our best knowledge, coronary artery aneurysms presenting as intra-ventricular masses have seldom been reported.

Abstract:Coronary sinus aneurysm (CSA) is an extremely rare entity. Herein, we present an unusual case of an 18-year-old symptomatic female patient with a giant CSA. Secondary vena cava aneurysms were also manifested. The final diagnosis was confirmed by enhanced computed tomography (CT) and cardiac catheterization. As far as we know, it is the first case that such a giant CSA coexists with secondary vena cava aneurysms. Considering the complexity of postoperative reconstruction, we believe that heart transplantation may be the optimal way for treatment. The patient received anticoagulant due to the superior vena cava (SVC) thrombosis while waiting for a donor.

Abstract:We report a case of a 10-month-old Chinese girl with single left coronary artery, left coronary artery-to-right ventricle fistula, pulmonary stenosis, and right aorta arch with bicuspid aortic valve. The diagnosis was made by transthoracic echocardiography and confirmed by angiography. A surgical repair was performed with closure of the fistula at the point where it drained into the right ventricle plus a pulmonary valvulotomy.

Abstract:The giant coronary artery aneurysm combined with coronary artery fistula is extremely uncommon. In our case, there was a giant aneurysm of the left main coronary artery with fistulous communication to the right atrium, combined with moderate aortic valve regurgitation, which was initially found by transthoracic echocardiogram and subsequently confirmed by the 256-slice multidetector computer tomography (MDCT) coronary angiography. After consultation, the patient received surgical treatment, including the closure of the drainage and origin sites of the aneurysm and the aortic valve replacement. The patient recovered uneventfully.

Abstract:Background: Coronary artery ectasia (CAE) complicated with concomitant congenital coronary artery fistula (CCAF) is rare. This study characterizes the clinical characteristics of CAE combining CCAF, and reports a single-institution experience with surgical correction of CAE combining CCAF. Methods: A total of 24 symptomatic patients (8 males, median 52.5 years old) who underwent surgical correction of CAE combining CCAF in this center were reviewed. Based on the size of ectatic segment, the CAE were classified as a giant CAE (>20 mm, n = 14) and a non-giant CAE (<= 20 mm, n = 10). Individualized surgical approaches were chosen. The patients were followed up for a median of 3.8 years. Results: The overwhelming majority of CAEs were solitary, and only 4.2% of CAEs were associated with multiple lesions. CAEs were predominantly located in the right coronary artery with predilection to women more than to men (2:1). 95.8% of patients with the CCAF had single fistula defect. The right atrium was the most frequent drainage site (33.3%) followed by the left ventricle (25.0%). Surgical mortality was 4.2%. All 22 follow-up patients survived with recovery from symptoms and New York Heart Association (NYHA) functional class I-II. In 10 patients with non-giant CAEs undergoing closure of fistula alone, favorable in-hospital outcomes were recorded, but residual fistula (one patient) and acute inferior wall myocardial infarction related to intracoronary thrombosis (one patient) were observed at follow-up. In 11 patients with giant CAEs undergoing aneurysm resection plus distal bypass grafting at the time of closure of fistula, favorable in-hospital outcomes and encouraging midterm results were recorded. Additionally, in 3 patients with giant CAEs undergoing closure of fistula plus aneurysmal plication, adverse events occurred, including surgical death related to rupture of the ectatic segment (one patient), perioperative myocardial infarction caused by acute thromboembolism (one patient), nonfatal inferior wall myocardial infarction related to intracoronary thrombosis (one patient) at follow-up. Conclusion: Individualized surgical approaches based on the size and the location of ectatic coronary artery as well as fistula should be offered to symptomatic patients with CAE combining CCAF.

Abstract:A coronary aneurysm is a rare type of cardiovascular disease. We report a case of a 53-year-old male patient who presented to our hospital with a giant left circumflex coronary fistula aneurysm (LCCA) (75 mm × 70 mm). Since coronary angiography and coronary computed tomography angiography failed to detect the fistula of the coronary aneurysm, interventional occlusion surgery could not be performed. We discovered the fistula in the right atrium by anterograde perfusion with blood-containing myocardial protective fluid after switching to intraoperative exploration during cardiac surgery. The coronary aneurysm's fistula and inlet were then sutured, and the aneurysm was resected. The patient recovered successfully after the operation. This case was instructive in managing LCCA, especially with an unidentified fistula.

Abstract:Congenital coronary artery fistula (CAF) represents a remarkable rarity within the realm of cardiovascular anomalies, characterized by an aberrant connection between coronary arteries and either cardiac chambers or major vessels. Clinical manifestations of CAFs often remain unspecified or may even be entirely absent, posing diagnostic challenges. Notably, patients harboring substantial CAFs may exhibit symptoms such as palpitations, chest tightness, and dyspnea. Although right-sided congenital CAFs are relatively prevalent, the occurrence of a CAF accompanied by a colossal pseudoaneurysm imposing compression upon the pulmonary vein is an exceedingly rare phenomenon. This exceptional case report delineates a singular fistula originating from the right coronary artery, extending its course to the right atrium, and remarkably featuring a substantial pseudoaneurysm exerting compression upon the right superior pulmonary vein. Therapeutic intervention encompassed surgical closure of the proximal artery and excision of the pseudoaneurysm, underscoring the complexity and criticality of managing such intricate cardiac anomalies to ensure optimal patient outcomes.

Abstract:Prevalence of primary coronary cameral fistula (CCF) is extremely rare, especially for CCF with the drainage site into the left ventricle. We describe a 45-year-old patient with chest tightness in whom a giant aneurysm associated with proximal right coronary artery (RCA), and the distal end of RCA terminated into the left ventricle through a fistula was discovered by echocardiography. Dual-source computer tomography revealed the CCF-related giant RCA aneurysm. The drainage site and coexistent abnormality could not be visualized well by coronary artery angiography because of the severely diluted contrast medium into the aneurysm. Finally, the patient received surgical treatment and is asymptomatic now.

Abstract:Giant coronary artery aneurysm with a fistula is a rare condition. The presence of a giant aneurysm imposes considerable health risks. We report a case of a 67-year-old woman who presented with frequent ventricular premature contractions caused by a giant coronary aneurysm arising from a branch of the left anterior descending coronary artery that had a fistulous connection to the pulmonary artery. The patient was referred for cardiac surgery. The giant aneurysm was resected, and the proximal and distal openings were closed directly. The main pulmonary artery was opened longitudinally and the fistula was also closed directly. The patient's symptoms of frequent ventricular premature contractions disappeared postoperatively as confirmed by electrocardiography. Although the standard therapeutic strategies of the disease are not well established because of the rarity of this condition, our clinical results indicate that the surgical treatment is an effective choice.

Abstract:Congenital fistula between the first diagonal branch of the coronary artery and left ventricle with a giant coronary artery aneurysm is extremely rare. We present the case of a 50-year-old asymptomatic male patient with such a condition that was diagnosed by transthoracic echocardiography, coronary computed tomography angiography, and coronary angiography. The patient was treated by surgical fistula closure under cardiopulmonary bypass. The postoperative coronary computed tomography angiography showed the patient got a complete cure, and the patient remains asymptomatic after 5-year follow up.

Abstract:A previously healthy 9‐year‐old girl was referred to us for the evaluation of a murmur on a routine clinical examination. Routine electrocardiogram and chest x‐ray were normal. The cardiac enzymes were normal. Combining ultrasound and CCTA, it was confirmed that the hemodynamics of the heart was a left‐to‐right shunt and that RVOT stole blood from the left ventricle through the single coronary artery (SCA).

Clinical diagnosis of CAFs is difficult due to symptomless or lack of specific symptoms. Dual-source computed tomography (DSCT) might be a useful diagnostic tool for CAFs. The study aimed to retrospectively summarize the imaging features of CAFs delineated at DSCT in 48 CAF patients detected from consecutive 6624 patients with suspected or confirmed coronary artery disease in our institution. METHODS Forty-eight patients underwent DSCT angiography by using retrospective electrocardiographic (ECG) gating after infusion of 70 ml of intravenous contrast material during breath hold. Maximum intensity projection (MIP), curved planar reconstruction (CPR), and volume rendering technique (VR) were obtained. Anomalous termination of coronary artery in each subject was evaluated by two radiologists (with more than 10 years experience with cardiovascular imaging), and disagreement between diagnosis readers was settled by a consensus reading. Ten of 48 patients also underwent traditional coronary angiography (CAG) simultaneously. RESULTS In each CAF case, DSCT angiography clearly demonstrated the origin, the termination, the size of abnormal vessel and its course in relation to surrounding great vessels. CAF arising from right coronary artery was the most common type, left circumflex was the least one involved among three coronaries, and pulmonary artery was the most common drainage site. Aneurismal fistulous tract, coronary atherosclerosis, myocardial bridging and anomalous origin of coronary artery were also detected in this group. The demonstration of drainage sites in CAG was consistent with DSCT angiography in 9 patients, and judgment on one anomalous connection in CAG was inconsistent with that in DSCT angiography. CONCLUSIONS DSCT angiography could provide accurate delineation of anomalous communications, size and numbers of fistulas in patients with CAFs. It suggested that DSCT is a useful tool for the assessment of CAFs and may be considered as the first-choice imaging modality, especially for patients with coronary artery disease.

Abstract:BACKGROUND: We describe a giant right coronary artery (RCA) to coronary sinus (CS) fistula in a 59-year-old woman who presented to our institution with right heart enlargement.METHODS: Investigation revealed an ectatic 22mm RCA draining into an enlarged CS, and a Qp:Qs of 2.0. We proceeded with surgery. After initiating cardiopulmonary bypass and electrical arrest, the distal RCA was opened. An opening into the CS was confirmed and closed with a bovine pericardium patch. The RCA origin was triply ligated. Anastomoses of a saphenous vein graft to the posterior descending artery and the right ventricular marginal artery were performed, followed by anastomosis to the ascending aorta.RESULTS: Post-operatively, mixed venous oxygen saturation was 76%. Six months later the patient was doing well.CONCLUSION: Coronary artery fistulas are rare congenital anomalies for which the ideal management strategies remain under study, with surgical repair being the mainstay of treatment in complex, aneurysmal fistulas.

Abstract:Abstract. Rationale:. A coronary artery fistula (CAF) is an anomalous communication between a coronary artery and a cardiac chamber or great vessel. It is a rare congenital anomaly that is often small and asymptomatic, occurring in only 0.002% of the general population. Most CAFs originate from the right coronary artery and flow into the right cardiac system. Although extremely rare, some cases may originate from the bilateral coronary arteries and flow into the left ventricle. Patient concerns:. Herein, we report a rare case of a 55-year-old male smoker with no history of heart disease or cardiac surgery, who presented with a 5-year history of recurrent chest congestion, palpitations, and shortness of breath. On physical examination, his heart and lungs revealed normal findings without cardiac murmurs and no systemic or pulmonary edema. Moreover, 24-hour ambulatory electrocardiography showed no signs of ischemia but exhibited a short array of ventricular tachycardia and short atrial tachycardia. Chest computed tomography showed left apical emphysema without cardiomegaly and pulmonary congestion. Furthermore, coronary angiography revealed dual congenital coronary cameral fistula, a complex CAF with a left circumflex artery–left ventricle fistula and a right coronary artery–left ventricle fistula, complicated with a myocardial bridge. Diagnosis and interventions:. A diagnosis of left circumflex artery–left ventricle fistula complicated with a right coronary artery–left ventricle fistula and myocardial bridge was made. Since the patient refused surgery, medical management with enteric-coated aspirin, sustained-release metoprolol, and atorvastatin calcium was initiated. Outcomes and lesson:. Currently, the patient is now asymptomatic and in good condition since 6 months after undergoing conservative treatment with β-blockers.
