Ascending Aortic Pseudoaneurysm As an Unusual Cause of Superior Vena Cava Syndrome

Abstract:A 41-year-old man presented with a 2-month history of dyspnea, chest pain, progressive facial and upper arm edema, and head fullness. His medical history was significant for poorly controlled Behçet’s syndrome. He underwent a Bentall procedure with a mechanical valved conduit at our center 7 years ago because of dilated aortic root and ascending aorta, severe aortic regurgitation, and bicuspid aortic valve. However, this patient was lost to follow-up after discharge. Physical examination revealed facial edema and plethora, bilateral upper arm swelling, and jugular venous distention. A painless and throbbing mass was palpated at the right sternal border. Computed tomography angiography confirmed a pseudoaneurysm located in the distal ascending aorta (Fig 1, white arrow: pseudoaneurysm). The superior vena cava was severely compressed by the pseudoaneurysm (Figs 2 and 3, white arrow: pseudoaneurysm; yellow arrow: superior vena cava). We performed an emergency reoperative aortic pseudoaneurysm resection and repair. Timely diagnosis with computed tomography angiography and early surgical intervention are critical to prevent catastrophic consequences.
