Abstract:Objective. To present our experience of using transperitoneal subcostal access, Palmer's point (3 cm below the left costal margin in the midclavicular line), and its right corresponding site, in urologic laparoscopy. Methods. We used Palmer's point and the right corresponding site for initial access in 302 urologic surgeries (62 cases with prior surgeries). The record of these cases was reviewed. Results. Success rate of initial access is 99.4%, and complication rate of puncturing is only 3.4% with no serious complication. In the caseswith prior surgeries, there were only two caseswith access complication on the right side (minor laceration of liver). For people with BMI more than 30 kg/m(2) (12, 3.9%), the success rate was also 100 percent. Conclusions. Palmer's point and the corresponding right location are feasible, effective, and safe for initial access in urologic laparoscopic surgeries. This entry technique should be used routinely in urologic laparoscopic surgeries.
Clinical Experience of Laparoscopic Pancreatoduodenectomy Via Orthotopic Resection

Abstract:Objective: To examine the surgical approach, practical cognition as well as clinical effect of the orthotopic resection for laparoscopic pancreatoduodenectomy(OLPD). Methods: From March 2019 to December 2019, 32 cases were treated with laparoscopic pancreatoduodenectomy (LPD) in a novel approach without mobilization of pancreatoduodenum in Pancreas Center of the Second Affiliated Hospital of Guangzhou University of Chinese Medicine.There were 16 male patients and 16 female patients.The mean age was (64.8±9.5) years old.Body mass index was 14.9 to 31.0 kg/m(2).All patients were diagnosed as ampullary or pancreatic head tumors and were not unresectable cases.In the surgical strategy, Kocher's dissociation, turning and pulling of the pancreaticoduodenal region, was not performed first.Anatomy in situ, separation of vessels which enter and exit from pancreas, separation of lymphatics and isolation of tumors were carried out in priority through the combined middle and left posterior approaches.Finally, the pancreatic head and duodenum region was mobilized and the entire resection of pancreas in situ was carried out.Digestive tract reconstruction was performed through Child method. Results: Postoperative pathology showed that 27 cases were pancreatic or ampullary malignant tumors and five cases were benign tumors among 32 patients.The operative time was (357.3±64.3) minutes.The diameter of pancreatic ducts was (3.0±1.0) mm. The pancreas of 20 cases (62.5%) were soft. Five patients suffered from pancreatic fistula (Grade B) and one patient suffered from intra-abdominal hemorrhage postoperatively.No other complications like pancreatic fistula (Grade C) or biliary fistula delayed gastric emptying or mortality were encountered.The postoperative hospital day was (13.7±3.6) days. Conclusions: Combining the multi-angle of the laparoscopic approaches and excising the pancreaticoduodenal specimen in situ, OLPD is a kind of surgical method which can realize the concept of no touch tumor surgery.Patients who undergo the OLPD can receive better treatments and results.
Total Laparoscopic Right Hemihepatectomy Through Anterior Approach for Larger Tumors in the Right Lobe of the Liver

Abstract:Objective To explore the feasibility and safety of laparoscopic right hemihepatectomy (LRH) via anterior approach for larger tumors in the right lobe of the liver. Methods A retrospective study was conducted based on the clinical data of ten consecutive patients with large right liver cancer undergoing LRH through anterior approach and thirty-seven patients undergoing open hemihepatectomy by anterior approach in recent 6 years. Results Between the two groups there were no significant difference in gender, average age, the mean tumor size, preoperative liver reserve function, cut margin and intraoperative blood transfusion. The LRH group had less average intraoperative blood loss [(408±158)ml vs.(520±153)ml, t=2.047, P=0.046] and shorter postoperative hospital stay [(11.5±2.8)d vs.(16.2±4.6)d, t=3.091, P=0.003], longer operation time [(302±38)min vs.(251±55)min, t=2.732, P=0.009]. There was no perioperative death and no significant difference in complications (20.0% vs.35.1%, χ2=0.812, P=0.367) and similar median survival time (36 mon vs.29 mon, χ2=1.266, P=0.261). Conclusions LRH via anterior approach for larger tumors in the right lobe of the liver is safe and feasible. Key words: Liver neoplasms; Laparoscopes; Hepatectomy
Experience of Ipsilateral Laparoseopic Reoperations in Urology

Abstract:Objective To evaluate the feasibility and clinical results of laparoseopic reoperation for patients with history of previous ipsilateral urology laparoscopic surgeries. Methods Thirteen patients that underwent second ipsilateral urology laparoscopic surgeries were retrospectively ana-lysed. The reasons for a second operation included nonfunctional kidney after pyeloplasty, ure-terolithotomy or pyelolithotomy in 4 cases, recurrence of urinary calculi in 3 cases, pelviureteric june-tional stenosis after pyeloplasty in 1 case, recurrence of renal cyst in 1 case, recurrence of adrenal tumor in 1 case, residual adrenal tumor in 1 case, progression of polycystic kidney in 1 case and renal carcinoma after laparoscopic surgery for renal cyst in 1 case. Transperitoneal laparoscopie surgeries were performed in all cases and the first trocar was placed with open incision to avoid puncture injury. The adhesion between intestines and retroperitoneal space was dissected to expose the operative field. The lateral peritoneum and perirenal fascia were sutured after surgery in all cases except nephrectomy cases. Results For the first operation, the mean operative time was 93 min, the mean estimated blood loss was 70 ml and the average postoperative hospital stay was 4.8 d. The second operations on the 13 cases were successfully performed with mean operative time of 97 rain, mean estimated blood loss of 62 ml and average postoperative hospital stay of 5.0 d which were not significantly different from the first operation parameters(P＞0.05). During the secondary operations, adhesions and abnor-mal anatomic structure observed increased the difficulty of surgery. All patients after secondary opera-tions were followed up for 2--24 months and no major complication was observed. Conclusion La-paroscopic reoperation on patients with history of ipsilateral urology laparoscopic surgery is feasible in skilled and experienced hands and in properly selected cases.

Abstract:Objective To summary the initial experience of laparoendoscopic single-site sur-gery in our department of urology. Methods There were ninety-five patients underwent laparoen-doscopic single-site surgery using the single-site and common surgical instruments of laparoen-doseopie from October 201 1 to October 2013,consisted of forty patients of renal cyst decortication, twelve patients of high ligation of spermatic vein,ten patients of radical nephrectomy,fifteen pa-tients of adrenalectomy,two patients of nephrectomy,fourteen patients of ureterolithotomy,one pa-tient of bladder diverticulum resection,one patient of resection of bladder tumor.Record the opera-tion time,intraoperative blood loss,postoperative hospital stay,complications or conversion to open surgery,postoperative pain in patients with index. Results All the operations were successfully completed.No significant intraoperative complications and postoperative complications occurred. Conclusions Laparoendoscopic single-site surgery represents a safe and feasible option for urologic patients.With more clinical practices,laparoendoscopic single-site surgery in urology will be applied generally.
Experience of the Robot-Assisted Laparoscopic Radical Prostatectomy via Completely Posterior and Intrafascial Approach: A Report of 5 Cases

Abstract:Objective To briefly introduce the initial experience of the robot-assisted laparoscopic radical prostatectomy (RALP) by completely posterior and intrafascial approach.Methods By retrospectively reviewing the clinical data of the 5 cases who received RALP by completely posterior and intrafascial approach between August 2016 and October 2016.Results All these 5 RALP by completely posterior and intrafascial approach were successfully completed.The operation time was between 124 and 155 minutes [(139.8±13.9) minutes on average].The estimated intraoperative blood loss was between 30 to 80 ml [(52.0±19.2) ml on average].No case with positive surgical margin was found.The serum PSA levels decreased to the criteria of radical prostatectomy 6 weeks postoperatively.Three patients were able to fully control their urination, with no need for diapers, and the other two regained full urinary continence within 1 month after surgery.Two patients recovered their erectile function within 1 month after surgery, while the other 3 patients didn't report erection function recover.Conclusions The tumor control, the postoperative urinary continence, and the erectile function recovery of the RALP by completely posterior and intrafascial approach are relatively satisfactory.It is feasible to perform this approach for selected suitable patients.
Transumbilical Laparoscopic Single-site Surgery for Upper Urologic Diseases: Single-center Experience

Abstract:Objective To evaluate the feasibility of a novel homemade single-port device for transumbilical laparoendoscopic single-site surgery(LESS) on the upper urinary system.Methods The clinical data of 30 cases of LESS that were performed(24 cases on the left side,and 6 cases on the right side) from January 2010 to May 2011 in our hospital were analyzed retrospectively.The cases included 14 cases of radical nephrectomy,12 cases of renal cyst resection,3 cases of non-functional kidney resection and 1 case of adrenal cystectomy.With side-lying position at the healthy side(45°-60°),the patients received LESS by a homemade single-port device through a 4-to 6-cm umbilical incision.Laparoscope(30°),ultrasonic scalpel,and flexible or non-flexible instruments were employed for the operation.Results The procedure was completed in all the 30 cases within 28 to 185 min,without conversion to open surgery.Intraoperative blood loss ranged from 10 to 200 ml,and the hospital stay ranged from 4 to 8 days.Postoperative pathological examination confirmed preoperative diagnosis in all the patients.No hematoma,infection,intestinal obstruction,or incisional hernia occurred in any of the patients.A follow-up for a mean of 21.4 months was achieved in all the patients(ranged from 12 to 29 months),no recurrence was detected during the period.Conclusions Because of its cosmetic outcomes,LESS especially suits female patients.We recommend our homemade single-port device to be widely used for its convenience,flexibility,and low cost.

Abstract:Objective: To evaluate the feasibility and safety of the operation of transumbilical single-port laparoscopic cholecystectomy (TSPLC) by traditional laparoscopic instruments and summarize the initial experience. Methods: Sixty subjects with cholelithiasis were divided into two groups. One group (36 cases) underwent TSPLC and the control group (24 cases) underwent traditional three-port laparoscopic cholecystectomy (LC). Postoperative complications were observed and operation time, hospital days, visual analogue scale (VAS) after 6 and 24 h of operation, and subject satisfaction score were measured. Results: TSPLC and traditional LC were performed successfully in the two groups. The operation time in the TSPLC group was significantly longer than that in the control group. There was no statistically significant difference in hospital stay and VAS between the TSPLC and control groups. The subject satisfaction score in the TSPLC group was 91.2, significantly higher than that in the control group (P<0.01). All subjects recovered from the operation and no postoperative complication occurred during the period of two weeks after operation. Conclusions: TSPLC is a feasible and safe method for cholecystectomy, although it may be more time-consuming. However, it is welcomed by patients who are more concerned with cosmetic outcomes. Future studies are needed to confirm its disadvantages and contraindications.

Abstract:Objective To investigate the feasibility and safety of transumbilical laparoendoscopic single-site surgery(LESS) for acute abdomen.Methods We reviewed 100 cases of acute abdomen who were admitted to our hospital between January and September 2011.With a silicon incisional protector,LESS for exploration and further procedures were carried out via a 20-mm transumbilical vertical incision.All the procedures were completed with a 5-mm laparoscope through the transumbilical port.ResultsOf the 100 patients,85 cases were acute appendicitis,of which,76 cases underwent transumbilical single-site laparoscopic-assisted extracorporeal appendectomy,4 cases underwent single-site intraabdominal appendectomy,and the other 5 needed a 5-mm suprapubic port to remove the appendix.In the other 15 patients,gastric perforation was detected in 8,6 of them received transumbilical single-site laparoendoscopic repair,and the other 2 needed a 5-mm suprapubic port to complete the procedure;hemorrhagic enteritis was found in one patient,who received transumbilical resection of the effected ileum,and then jejunostomy;pelvic infection was detected in two female patients,who then underwent abdominal and pelvic irrigation;torsion of ovarian cyst and lower abdominal intestinal adhesion were found in 2 patients,respectively,who then received resection of the ovarian cyst or lysis by transumbilical LESS;intraabdominal chyle effusion was explored intraoperatively in the other 2 patients,one of them was confirmed with spontaneous chyle peritonitis,and was cured by closing the leaking lymphatic with titanium clips and intraabdominal irrigation,and the other one was diagnosed with lymphangioma of the jejunum mesentery,and thus was converted to an open surgery for resection of the tumor.After the operations,3 patients developed umbilical secretion,and two showed enteroparalysis.All the patients were followed up for 6 to 9 months,during which the umibical incision was healed well in all the cases without any incisional hernia occurred.ConclusionTransumbilical LESS with a silicon incisional protector is feasible and safe for acute abdomen with good cosmetic results.
Laparoscopic Distal Pancreatectomy: the Experience of 68 Cases in a Single Centre

Abstract:OBJECTIVE:To evaluate the feasibility and efficacy of laparoscopic distal pancreatectomy.METHODS:Totally 68 patients (male 23, female 45) aged 17 to 77 years, with distal pancreatic lesions, underwent laparoscopic distal pancreatectomy from November 2003 to December 2010. The clinical data were collected. Safety, feasibility and crucial technique manipulation were analyzed retrospectively.RESULTS:All 68 operations were successful with two cases conversion to open, including 48 cases combined with splenectomy, and 18 cases with preservation of spleen. Fourteen cases received with combination resection of multi-organs, including 4 cases with cholecystectomy, 1 case resection of right adrenal adenoma and cholecystectomy, 1 case with myomectomy and left ovarian teratomectomy; 1 case with right ovarian teratomectomy, 1 case with resection of left adrenal adenoma, 1 case with resection of both adrenal adenoma, 1 case with resection of liver metastasis, 1 case with cholecystectomy and resection of liver metastasis, 1 case with resection of left adrenal adenoma and liver metastasis, 1 case with resection of left adrenal adenoma and colon and spleen, 1 case with biopsy of liver nodule. The mean operative time was (209 ± 58) minutes, the mean intraoperative blood loss was (191 ± 123) ml, and the mean postoperative hospital stay was (8 ± 4) days. The rate of overall postoperative complications was 18.1%, including an 12.1% rate of clinical pancreatic fistula. Only one case needed a reoperation, and there was no postoperative mortality.CONCLUSION:Laparoscopic distal pancreatectomy with or without splenectomy is safe and feasible in the treatment of most distal pancreatic tumors.

Abstract:Introduction: The introduction of Single-Port (SP) platform opened the field to new surgical options, allowing to perform major urological robot-assisted procedures extraperitoneally and with a supine patient positioning (1-3). Nevertheless, a comprehensive description of different supine access options is still lacking (4-6). In this light, we provided a step-by-step guide of SP extraperitoneal supine access options also exploring preliminary surgical outcomes. Materials and methods: Transvesical access was performed by a transversal incision 3cm above the pubic bone, after the anterior abdominal sheet incision, the bladder was insufflated with a flexible cystoscope and the detrusor muscle was incised at the level of the bladder dome. Similarly, the extraperitoneal access was carried out with a 4cm incision above the pubic bone, once visualized the preperitoneal space the prevesical fat was gently spread. The Low Anterior Access was performed with a 3cm incision at the McBurney point, the abdominal muscles were then spread. A gentle dissection was used laterally to develop the retroperitoneal space. Results: Overall, sixteen different procedures were performed with supine extraperitoneal access on 623 consecutive patients. No intraoperative conversions occurred. The median access time was 16 (IQR 12-21), 11 (IQR 7-14) and 14 (IQR 10-18) minutes in case of transvesical, extraperitoneal and low anterior access, respectively. Notably, 81.5 % of patients were discharged on the same day with a postoperative opioid free rate of 73%. Conclusion: The Atlas provides a comprehensive step-by-step guide to successfully perform all major urological SP procedures extraperitoneally and with supine patient positioning.

Abstract:Objective To report out initial experience of retroperitoneal laparoendoscopic single-site surgery(LESS).Methods Between June 2010 and December 2010,15 patients(male=8,female=7) underwent retroperitoneal LESS for various indications.Mean age of the patients was 46.3 years(range,15-72 years).Patient demographics and operative data was noted.All procedures were performed using a homemade multichannel single-site device and a varying combination of standard and specialized laparsocopic instruments.Result All retroperitoneal LESS procedures were successfully completed without any complications occurred,no procedure was converted into standard laparoscopic or open surgery.Conclusion Retroperitoneal LESS for the treatment of urological disease is safe and effective.

Abstract:Background Laparoendoscopic single-site surgery (LESS) approaches have been reported for treating various kidney and pelvic procedures, and are feasible and effective in selected patients. In this study, we aimed to present the initial experience and evaluate the efficacy of laparoscopic radical prostatectomy performed through a single incision using a multichannel port.Methods Between July 2010 and April 2011, six patients diagnosed with early stage prostate cancer underwent LESS radical prostatectomy (RP) in our institute. A multichannel port was inserted transperitoneally through a 2-cm umbilical incision. Specially articulating and flexible laparoscopic were used. Some technical tricks and points were applied during the operation to overcome the drawbacks and reduce the difficulties of this approach. Two continuous urethrovesical sutures in both sides were performed to complete both lateral aspects of anastomosis. The two ends of the suture threads were fixed by double Lapro-Clips, instead of the difficult knot-tying.Results Total operative time was (265 +/- 43) minutes. Mean blood loss was (230 +/- 65) ml. All cases were completed successfully, without conversion to open surgery or adding additional abdomen ports. No patient required a blood transfusion and no intraoperative complications occurred. The Foley catheter was removed at the 14th day (range 12th-16th) after surgery. At the 12th week of follow-up, all patients had an undetectable prostate-specific antigen level. Two patients used 2 or 1 pad for continence daily; other patients had achieved good continence.Conclusion In selected cases, LESS-RP is feasible and effective; these technic points and the flexible-articulating instruments are helpful to reduce the operation difficulties. [Application of Transperineal Single-Port Access Assisted Extralevator Abdominoperineal Excision in Low Rectal Cancer].

Abstract:OBJECTIVE:To explore the feasibility and safety of transperineal single-port laparoscopy assisted exralevator abdominoperineal excision(TPSP-ELAPE).METHODS:Three cases (2 males and 1 female) who underwent TPSP-ELAPE at lithotomy position in the Peking University People's Hospital from January to February 2016 were analyzed retrospectively. The mean age was(55.3±7.2) years. Mean body mass index (BMI) was (23.0±1.3) kg/m(2). Mean distance between tumor and anal verge was (1.8±0.3) cm. Abdominal procedure was done firstly and then perineal procedure under lithotomy position. Abdominal procedure was performed by routine laparoscopy with 5 trocars. TPSP-ELAPE procedure was performed in perineal procedure. The perineal defect was closed by purse string, followed by spindle incision around the anus. Perianal skin and subcutaneous tissue was dissected until the junction between extra anal sphincter and levator ani muscle under direct vision. Perineal skin was sutured with double purse string by 2-0 prolene, and single port canal was placed inside in order to close perineal defect. The pneumoperitoneal pressure was set to 10 mmHg and laparoscopy and instrument was placed through the single-port canal. Space between isochioanal fossa and levator ani muscle was exposed by the contraction of laparoscopic clam. Lateral and posterior side was mobilized firstly. The anterior was dissected under the help of vaginal examination and contraction by abdominal group. The levator ani muscle was divided near the initial site, and negative resection margin was confirmed in the operation. Specimen was exteriorized through the perineum.RESULTS:The mean intraoperative blood loss of three cases was (123.3±25.2) ml. The total operative time was (296.7±25.1) minutes. The time used by the perineal group was (196.7±20.8) minutes. The volume of drainage was (39.0±10.1) ml at the third day after operation. The time to first stoma output was (2.3±0.6) days. The perineal drainage removal time was (7.7±0.6) days. The circumferential resection margins were negative in all specimens, and there were no perioperative complications.CONCLUSION:TPSP-ELAPE is feasible and safe, which could avoid turning over the patient and facilitate anesthesia. It follows the anatomic plane of open ELAPE procedure with minimally invasive advantage.
Analysis of 17 cases underwent laparos copic rectal cancer surgery with transanal natural orifice specimen extraction and resection

Abstract:Objective To study the feasibility, safety and short-term efficacy of total laparoscopic rectal cancer surgery with transanal natural orifice specimen extraction and resection.Methods From May 2014 to March 2016, 17 patients with rectal carcinoma were treated by total laparoscopic rectal cancer surgery with transanal natural orifice specimen extraction and resection.The clinical data of these patients was collected and retrospectively analyzed to assess the impact of the operation on postoperative recovery time and the incidence of complications.Results All operations had been successfully accomplished without conversion to open surgery or conversional laparoscopic-assisted surgery.The median operative time was 105 minutes.The median blood loss was 35 ml.The median proximal and distal margin of tumor is 16 cm and 3.5 cm.The median number of lymph nodes harvest is 21,and the median first bowl movement is 43 hours.The hospitalization after operation is 8 days.No patient underwent abdomen hemorrhage or anastomotic leakage. Conclusion Laparoscopic rectal cancer surgery with transanal natural orifice specimen extraction and resection appears to be feasible,safe and with promising efficacy for selected patients.

Abstract:Objective To study the feasibility and safety of transumbilical single-port laparoscopic cholecystectomy(TSLC).Methods The clinical data of 35 patients,who underwent TSLC in our hospital from March 2010 to August 2011,were retrospectively analyzed.A 3-cm curve incision was made on the left edge of the umbilicus,so that to insert one 2.5-cm 3-hole adaptor,through which we placed a 5-mm pliable grasper in the middle hole,10-mm bending forceps in the right hole,and 5-mm bending forceps in the left.The cystic artery and duct were separated,and then blocked with titanium or absorbable clips,so that to remove the gallbladder through a 3-cm incision on the abdomen.Results Two of the patients were converted to open surgery because of incarcerated calculi or difficult exposure of the gallbladder.LC was completed in the other 33 patients successfully.The mean operation time was 65 min(ranged from 45 to 125 min),and intraoperative blood loss was 20 ml in average(5-200 ml).The patients were followed up for 3 to 20 months with a mean of 10 months,none of them had massive hemorrhage or biliary leakage after the procedure.ConclusionTSLC is safe and feasible with good cosmetic outcomes.

Abstract:Objective To evaluate the laparoscopy combined with transperineal extralevator abdominoperineal excision (TP-ELAPE) for locally advanced low rectal caner. Methods Clinical data of 12 patients with locally advanced low rectal cancer undergoing laparoscopy combined with TP-ELAPE in our department from May 2013 to March 2015 were retrospectively analyzed. There were 8 male and 4 female patients with median aged of 63 (46 to 72) years. The median distance from tumor lower margin to anal verge was 3.5 (2.0 to 4.0) cm. A self-made transanal suit for minimally invasive operation was used to make a sealed lacuna outside the sphincter , thus laparoscope can be applied to perform transperineal operation. Results All the patients underwent operations successfully without conversion to open abdominal operation. The median operating time was 206 (180 to 280) minutes with perineal operating time 95 (80 to 120) minutes. The median intraoperative blood loss was 120 (50 to 200) ml. The median postoperative hospital stay was 12 (9 to 18 ) days. Postoperative pathology revealed that all circumferential margins (CRM) were negative. The area of sample horizontal section was (2 824 ± 463) mm2, and of outer muscularis propria was (2 190 ± 476) mm2. Postoperative complications included chronic sacrococcygeal region pain in 2 cases, urinary retention in 3 cases, perineal wound infection in 1 case. No perineal seroma, perineal hernia, wound dehiscence and sinus tract formation were observed. Among 8 patients with preoperative normal sexual function , sexual dysfunction occurred in 2 patients. There was no local recurrence and metastasis during a median follow-up of 21 (12 to 34) months. Conclusion Laparoscopy combined with TP-ELAPE has the potential to simplify the operation procedure for low rectal cancer , can ensure the radical treatment and safety of operation, and may be carried out in experienced centers.
