Tubulointerstitial Changes Are Less Important in Membranoproliferative Glomerulonephritis Than in IgA Nephropathy

Abstract:Accessible online at: www.karger.com/journals/nef Dear Sir, Tubulointerstitial changes (TICs) have been reported to correlate well with the decline in renal function in various renal diseases, especially in IgA nephropathy [1]. To our knowledge, however, there has been no study showing the significance of TICs in membranoproliferative glomerulonephritis (MPGN). To this end, we compared the significance of TICs in patients with IgA nephropathy and MPGN who underwent repeated biopsies. Subjects were 23 patients with IgA nephropathy and 26 with MPGN who were all treated with prednisolone and antiplatelet agents after the first biopsy. Index of glomerular lesion (IGL) [2], percent interstitial volume (%IV) [1], serum creatinine (SCr), and 24-hour urinary protein excretion were recorded at the time of renal biopsy. These patients were divided into two groups based on whether they responded to therapy or not. Responders in IgA nephropathy (IgA-R group) were defined as patients who showed, at the time of second biopsy, both proteinuria !1 g/day and elevation of SCr !50% of baseline (n = 14), and the non-responder group (IgA-N group) included the remaining patients (n = 9). The responders in MPGN (MPGN-R group) were defined as patients who showed, at the time of second biopsy, both normocomplementemia and proteinuria !1 g/day (n = 14), and the non-responder group (MPGN-N group) consisted of all the remaining patients (n = 12). At the first biopsy, there were no significant differences in age, urinary protein excretion, and SCr between IgA-R and IgA-N groups and between MPGN-R and MPGNN groups, although the mean age of the IgAN group tended to be higher than that of the IgA-R group. The intervals between the first and second biopsies in IgA-N and MPGN-N groups tended to be longer than those of IgAR and MPGN-R groups, but the differences were not significant (table 1). At the first biopsy, the mean IGL was comparable between groups R and N in both diseases. The mean %IV was significantly higher in the IgA-N group than in the IgA-R group. However, a difference in %IV was not observed between groups R and N in MPGN. At the second biopsy, the mean values of both IGL and %IV in the IgA-N group were significantly increased when compared with the values at the first biopsy. By contrast, in the MPGN-N group, the mean %IV remained unchanged, even though the mean IGL values were significantly elevated (table 2). Table 1.
