Abstract:Objective:To evaluate the application value of fetal intelligent navigation echocardiography (FINE, 5D Heart) combined with virtual intelligent sonographer assistance (VIS-Assistance?) in the diagnosis of fetal tetralogy of Fallot (TOF).Methods:From February to January 2020, 57 pregnant women who received echocardiography examination for fetuses at Sir Run Run Shaw Hospital were selected as subjects in this study. Doctor A with rich fetal echocardiography experience collected two to five spatio-temporal image correlation (STIC) volume datasets for each fetal heart. The inexperienced doctor B and the more experienced doctor C screened out the best volume dataset in each fetus, and applied 5D Heart and VIS-Assistance? for post-processing. According to different spine positions, 57 TOF fetuses were divided into three subgroups: A (spine 2-4 o'clock), B (spine 5-7 o'clock), and C (spine 8-10 o'clock). Five diagnostic views (three-vessel tracheal view, four-chamber view, left ventricular outflow view, right ventricular outflow view, and abdomen/stomach view) and 17 main diagnostic elements of TOF fetuses were scored, and in particular the following parameters were investigated: the display rates of five diagnostic views and 17 diagnostic elements in the three subgroups and the differences between subgroups; comparison of the display rates of 17 diagnostic elements twice by two doctors with different experience; and repeatability and consistency test of two different senior doctors' scoring results in five diagnostic views.Results:With the application of 5D Heart and VIS-Assistance?, there was no significant differences in the display rates of five diagnostic views and 17 diagnostic elements between the three subgroups (P＞0.05). The display rates of the diagnostic elements of two different senior doctors are as follows: in the three-vessel tracheal view, the display rates of diagnostic elements (dilated aorta, stenosis of pulmonary artery, trachea, and superior vena cava) were 94.7%-96.5%, 93.0%-98.2%, 70.2%-84.2%, and 86.0%-91.2%, respectively. In the four-chamber view, the display rates of the diagnostic elements (four chambers, atrioventricular valves, mitral valve-tricuspid valve distance, ovale foramen, cardiac cross, ventricular septum, and moderator band) were 100.0%, 100.0%, 93.0%-96.5%, 78.9%-82.5%, 94.7%-96.5%, 100.0%, and 100.0%, respectively. In the left ventricular outflow view, the display rates of diagnostic elements (ventricular septal defect, dilated aorta, and the aorta overriding ventricular septal defect) were 96.5%-100.0%, 100.0%, and 96.5%-98.2%, respectively. In the right ventricular outflow view, the display rates of stenosis of the pulmonary artery was 80.7%-91.2%. In the abdomen/stomach view, the display rate of the diagnostic elements (stomach and descending aorta) was 100.0%. There was no statistical differences for the results between two doctors with different experience, and for scores of the five diagnostic views by the same doctor 1 month before and after (P＞0.05).Conclusion:5D Heart combined with VIS-Assistance? has high display rates in various diagnostic views and diagnostic elements in fetal TOF, and has good repeatability and consistency. It has relatively less dependence on the experience of the operating physician, which may has potentials in the prenatal diagnosis of TOF. Using 5D Heart, the fetal spine position within a certain range seems not to affect the TOF image display rate.
Study on Diagnostic Performance of Fetal Intelligent Navigation Echocardiography for Congenital Heart Defect

Abstract:Objective:To explore the sensitivity and specificity of fetal intelligent navigation echocardiography(FINE, 5D Heart) in the prenatal detection of congenital heart defect(CHD), and to compare its diagnostic performance with spatiotemporal image correlation(STIC) loop (STICloop).Methods:A total of 250 pregnant women having a singleton pregnancy in the second and third trimesters from April 2018 to May 2019 in Sir Run Run Shaw Hospital, Zhejiang University College of Medicine were enrolled in this prospective study. 2D ultrasound diagnosis has been made before acquiring STIC volume data set. After the appropriateness of each volume data set was preliminarily determined, all identifying informations were concealed and the appropriate volumes were randomly distributed to less experienced sonographer for analysis using 5D Heart software. The sensitivity and specificity, positive and negative likelihood ratios, as well as area under the receiver operating characteristic curve (AUC) of the two methods were determined respectively, and the differences between the two methods were statistically analyzed.Results:The diagnostic performance of 5D Heart for the prenatal detection of CHD was: sensitivity of 95.45% (105/110), specificity of 94.29% (132/140), positive likelihood ratio of 16.72, negative likelihood ratio of 0.05, and AUC of 0.95, respectively. Among cases with confirmed CHD, the 5D Heart diagnosis completely matched the final diagnosis in 77.27% (85/110). The diagnostic performance of STICloop for the prenatal detection of CHD was: sensitivity of 90.91% (100/110), specificity of 92.86% (130/140), positive likelihood ratio of 12.73, negative likelihood ratio of 0.10, and AUC of 0.92. The sensitivity and secificity of 5D Heart in diagnosing CHD were both higher than 90%, and it could successfully detect a broad spectrum of severe fetal CHD. Among cases with confirmed CHD, even the STICloop diagnosis completely matched the final diagnosis in 61.81%(68/110), its diagnostic accuracy was relatively lower than 5D Heart ( P=0.013). However, there were no statistical differences between the two methods in sensitivity and specificity(all P>0.05). Conclusions:The sesitivity and specifity of 5D Heart in diagnosis CHD were both higher than 90%, and it could successfully detect a broad spectrum of severe fetal CHD. STICloop could be used for preliminary screening of CHD, as it has the same high sensitivity and specificity as 5D Heart. Further examination may be recommended when fetal heart abnormalities are suspected.
Application of Fetal Echocardiography in Detection of Fetal Arrhythmia and Its Clinical Significance

Abstract:OBJECTIVE To investigate application of fetal echocardiography in diagnosis of fetal arrhythmia and its clinical significance. METHODS Fetal echocardiography was performed on 725 fetuses for evaluation of fetal heart structures and arrhythmias. Two-dimensional, M-mode, Color M-mode and pulsed Doppler echocardiography were used. RESULTS Ninety fetuses were documented with fetal arrhythmia, the commonest fetal arrhythmia encountered in 72 cases extrasystole was (65 atrial extrasystoles, 7 ventricular extrasystoles), followed by bradycardia in 9 cases, tachycardia in 6 cases, 2:1 atrioventricular block in 2 cases, atrial flutter in 1 case. There were 4 fetuses with arrhythmias and structural heart diseases: 2 fetuses were found died in uterus within two-week follow-up (1 with single ventricle and pulmonary stenosis, 1 with cardiac rhabdomyoma, fetal echocardiographic findings were confirmed at autopsy), another 2 cases (1 with mitral atresia and 1 with atrioventricular canal defect, autopsy confirmed the fetal echocardiographic diagnoses) received termination of pregnancy later. One 38-week fetus with atrial flutter underwent cesarean section, neonatal ECG confirmed the arrhythmia, and Cedilanid D induced successful conversion. All the other 85 fetuses were with intermittent arrhythmia, normal heart structures, and had no fetal hydrops. After receiving routine treatment, all of them had term deliveries and follow-up monitoring showed normal neonatal heart rhythms (auscultation). CONCLUSIONS Fetal echocardiography is the main diagnostic tool for prenatal evaluation of fetal arrhythmias. The outcomes of the vast majority of fetal arrhythmias are benign, especially for fetuses with extrasystoles. Their arrhythmias are always well tolerated and disappear during the perinatal period. Fetuses with intermittent arrhythmias without structural heart malformations, fetal hydrops or heart failure, can be followed up with routine prenatal care without the need for special intervention.
A Preliminary Study of Fetal Intelligent Navigation Echocardiography in the Display of 9 Views in the Normal Fetuses by a New Sonographer

Abstract:Objective To explore the applicability of fetal intelligent navigation echocardiography (FINE, 5D Heart) in displaying the normal fetal heart structure by a new sonographer and assess the value of this technique in fetal cardiac ultrasonography. Methods Eighty cases of normal fetal heart at the second trimester were scanned by an echocardiographic expert. STIC volume datasets with satisfactory from apical four-chamber and common 2D modality with 9 standard views of fetal echocardiogram were achieved for each case. The STIC volume images were post-processed with 5D heart software by two physicians with different levels of echocardiographic examination. The quality of the section images of 9 standard views from 2D 5D heart and were analyzed. The intraobserver and interobserver with common 2D modality repeatability were analyzed for the qualification of section images. The time spent in post-processing the images was record and analyzed. Results The qualified 9 section images with 2D were 75~80 cases (degree of qualification 93.8%~100.0%). The data were assessed twice by two observers and qualified 9 section images were got in 73~80 cases (degree of qualification 91.3%~100.0%). The intra-observer agreement by two observers was absolute concord in four chamber view and abdomen/stomach view (Kappa=1); excellent in three vessel trachea view (3VT, Kappa=0.851~0.882); good to excellent in five chamber view (Kappa=0.787~0.882); good in aortic arch view (Kappa=0.738~0.787), left ventricular outflow tract view (Kappa=0.709~0.787), right ventricular outflow tract view (Kappa=0.655~0.794), superior and inferior vena cava view (Kappa=0.647~0.649) and ductal arch view (Kappa=0.640~0.707). The inter-observer agreement by each two observers and 2D was absolute concord in four chamber view and abdomen/stomach view (Kappa=1); excellent in five chamber view (Kappa=0.851~0.902); good to excellent in ductal arch view (Kappa=0.749~0.820), aortic arch view (Kappa=0.707~0.882), right ventricular outflow tract view (Kappa=0.661~0.851) and 3VT (Kappa=0.655~0.851); good in left ventricular outflow tract view (Kappa=0.737~0.749), superior and inferior vena cava view (Kappa=0.655~0.794). The average time spent in post-processing the images for first time by three observers was (2.64±1.35) min, and there was no significant difference during the each observers (P>0.05). Conclusions 5D Heart represents a single method in obtaining 9 fetal diagnosis planes, which can be used easily by a sonographer lack of skills, and has high reliability and repeatability. Abstract:Objective To investigate the application of fetal intelligent navigation echocardiography ( FINE ,5D Heart) in the diagnosis of conotruncal anomalies ( CTD ) . Methods For prospective study , volume transducer was used to collect the three-dimensional data of fetal heart at 20th - 30th week of gestation . The STIC volume images were processed by three independent physicians of different hierarchies via 5D Heart software afterwards ,and the quality of the STIC diagnosis as well as the consistence among the physicians were analyzed according to the gold standard of postpartum ultrasonic follow-up or surgery results . Results Twelve out of 16 CTD cases were finally included in the study ,and the volume data were analyzed . Senior physicians A could apply 5D Heart diagnosis to achieve 2D-US diagnosis efficiency and gold standard consistently( P = 0 .157 ,0 .083) ,medium and low seniority physician B and C got roughly equal score using 5D Heart diagnosis( P = 0 .705) ,but the diagnostic performance could not reach 2D-US ( P =0 .000 ,0 .000) and gold standard( P = 0 .000 ,0 .000) . Conclusions Fetal cardiac anomalies of CTD can be diagnosed correctly by an expert using 5D Heart ,which has important application prospects in remote consultation , training and scientific research and teaching of congenital heart disease of fetus .
Is Fetal Intelligent Navigation Echocardiography Helpful in Screening for D-Transposition of the Great Arteries?

Abstract:Objectives To evaluate the performance of fetal intelligent navigation echocardiography (FINE) applied to spatiotemporal image correlation (STIC) volumes in generating 3 specific abnormal cardiac views (left ventricular outflow tract, right ventricular outflow tract, and 3-vessel and trachea) used to screen for d-transposition of the great arteries (d-TGA). Methods In this prospective study, 1 or more STIC volumes were obtained from the 4-chamber view in 34 second- and third-trimester fetuses with d-TGA. Each appropriate STIC volume was evaluated by STICLoop (Samsung Medison, Seoul, Korea) before applying the FINE method. One optimal volume per fetus was selected by observers. The visualization rates of the 3 specific abnormal cardiac views of d-TGA and their diagnostic elements were calculated, and the reliability between 2 observers was verified by the intraclass correlation coefficient. Results Fetal intelligent navigation echocardiography applied to STIC volume data sets of fetuses with d-TGA successfully generated the 3 specific abnormal cardiac views in the following manner for 2 observers: 75.0% (n = 21) for the left ventricular outflow tract, 89.2% (n = 25) for the right ventricular outflow tract, and 85.7% (n = 24) for the 3-vessel and trachea view. Twenty-four (85.7%) of the STIC volume data sets showed 2 or 3 of the abnormal cardiac views. The interobserver intraclass correlation coefficients between the 2 observers ranged from 0.842 to 1.000 (95% confidence interval), indicating almost perfect reliability for the 2 observers. Conclusions In cases of d-TGA, the FINE method has a high success rate in generating 3 specific abnormal cardiac views and therefore can be performed to screen for this congenital defect.

Abstract:Objective To investigate the value of fetal intelligent navigation echocardiography (FINE,5D Heart)in displaying the normal fetal heart structure during 20 to 38 weeks'gestation.Methods Three-dimensional data of fetal heart during 20 to 38 weeks'gestation were acquired using volume transducer prospectively.The STIC volume images were post-processed with 5D heart software by three physicians with different levels of echocardiographic examination and the quality of the section images were analyzed.The intra-and interobserver repeatability was analyzed for the qualification of section images.The correlation between the quality of image and gestational age,body mass index,the placenta position,fetal initial plane position were evaluated using univariate and multivariable logistic regression analysis.Results Two hundreds and five cases of pregnant women were included,and 9 cases were excluded.Volume data acquisition was completed in 180 pregnant women,and the image acquisition rate was 91.8%.The data were assessed twice by three observers and qualified 9 section images were gotten in 156-180 cases(degree of qualification 86.7%-100%).The screening images were satisfied in 152 cases(84.4%),dissatisfied with 28 cases(15.6%).The intra-observer agreement by three observers was absolute concord in four chamber view and abdomen、stomach view;excellent in three vessel trachea view(3VT),five chamber view, left ventricular outflow tract view and superior and inferior vena cava view;good in right ventricular outflow tract view,ductal arch view and aortic arch view.The inter-observer agreement by each of three observers was absolute concord in four chamber view and abdomen、stomach view;excellent in superior and inferior vena cava view;excellent to good in five chamber view,left ventricular outflow tract view,right ventricular outflow tract view;good in 3VT,ductal arch view and aortic arch view.In a univariate analysis,the quality of the image was associated with obesity(χ2=15.338,P =0.000),gestational age(χ2=23.356,P =0.000).Multivariable analysis showed that the obesity(OR=5.587,P =0.001)and gestational age (OR=8.843,P=0.000)were factors affecting the quality of the image.The average time spent in post-processing the images for first time by three observers was(2.77±1.39)min,and there was no significant difference among observers(P > 0.05).Conclusions Fetal intelligent navigation echocardiography (FINE,5D Heart)based on STIC volumes can successfully generate 9 standard fetal echocardiography views in 86.7%-100% of fetuses in the 2nd and 3rd trimesters.This method can simplify examination of the fetal heart and reduce operator dependency.
Clinical Value of Fetal Intelligent Navigation Echocardiography (5D Heart) in the Display of Key Diagnostic Elements in Basic Fetal Echocardiographic Views

Abstract:Objective To evaluate the clinical value of fetal intelligent navigation echocardiography (5D Heart) in the display of key diagnostic elements in basic fetal echocardiographic views.Methods Using volume probe,3D volume datasets of the 209 normal singleton fetuses in the second and third trimesters were acquired from a four-chamber view.After processing the datasets by using 5D Heart,9 cardiac diagnostic planes were acquired and then the image qualities of key diagnosis elements of the heart and great vessels were graded by 3 doctors with different experiences of performing fetal echocardiography.Results Two hundred and sixty-two volume datasets were acquired from 209 normal fetuses,including 53 volume datasets collected when cardiac apex was in different directions,satisfactory datasets failed to be acquired in 27 fetuses because of the maternal obesity,the artifacts of fetal movement,or other influencing factors.Two hundred and thirty-one volume datasets of 182 fetuses could be used for 5D Heart analyzing and displaying,and the displaying rate of 9 standard diagnostic views was up to 88.2％.No significant difference was found when comparing the grading results of one observer in different periods of time,the grading results between two experienced doctors and the grading results among the experienced doctor and one relatively inexperienced doctor (P ＞ 0.05).Conclusions 5D Heart has high reliability and repeatability in obtaining fetal echocardiographic diagnosis views and displaying fetal elemental diagnostic information.
Clinical Application of Echocardiography in Detection of Foetal Arrhythmia:A Retrospective Study of 451 Foetuses

Abstract:Foetal arrhythmia is defined as any irregular foetal cardiac rhythm or regular rhythm at a rate outside the reference range of 120 to 160 beats/minute. Foetal echocardiography permits accurate determination of the structural characteristics and the nature of the arrhythmia in foetuses. It is particularly indicated for mothers from high-risk groups. Management of diagnosed heart disease, including foetal arrhythmia, leads either to the termination of pregnancy or to optimal postnatal care of baby and mother. This retrospective study examined the application of foetal echocardiography in the diagnosis of foetal arrhythmia and its clinical significance.
A Preliminary Study on the Prenatal Diagnosis of Fetal Conotruncal Defects Using Intelligent Navigation Echocardiography

Abstract:ObjectiveTo compare the accuracy, efficiency, and consistency between experienced and less-experienced professionals using intelligent navigation echocardiography. MethodsIn this prospective study, we enrolled 93 second- and third-trimester fetuses with conotruncal defects (CTD) from July 2017 to February 2018. One or more spatiotemporal image correlation volume data sets were collected per case. The fetuses with CTD were diagnosed by the following two groups of professionals (n = 20 in each) with different experience levels using intelligent navigation echocardiography and two-dimensional ultrasound: group A with 15 years of experience and group B with 1 year of experience. The diagnostic consistency and accuracy of the technologies between the two groups were analyzed. ResultsSatisfactory consistency was noted in the two groups (group A, tau = 0.855, P < 0.05, and group B, = 0.821, P < 0.05), and no significant difference in accuracy ((2 )= 3.218, P > 0.05) in using intelligent navigation echocardiography was reported between the two groups. However, there a significant difference in accuracy (chi (2 )= 0.021, P < 0.05) when using two-dimensional ultrasound was observed between the two groups. ConclusionIntelligent navigation echocardiography was found to be efficient and accurate for the diagnosis of CTD and good consistency existed in the experienced and less-experienced professionals. Intelligent navigation echocardiography was feasible and accurate for the diagnosis of fetal conotruncal defects.
Preliminary study of cardiac morphology and function in fetuses with tetralogy of Fallot using fetal heart quantification

Abstract:Objective:To evaluate the fetal heart shape and function in tetralogy of Fallot (TOF) by fetal heart quantitative analysis (fetal HQ).Methods:A total of 52 fetuses with TOF diagnosed by fetal echocardiography and 200 normal fetuses matched with their gestational weeks from March 2020 to March 2022 at Sir Run Run Shaw Hospital, Zhejiang University were retrospectively evaluated. The basic parameters of fetal cardiac blood vessels in the two groups were measured by fetal HQ technology and conventional M-mode ultrasound technology: aortic valve diameter(AV), pulmonary artery valve diameter(PV), main pulmonary artery diameter (MPA) and Z-score. The overall morphometric measurements including end-diastolic length diameter, transverse diameter, area, and global spherical index (GSI) of the fetal heart in the 4-chamber view(4CV), area and length of the right and left ventricles and their ratios. Measurements of left and right ventricular function included ejection fraction (EF), fraction area change rate (FAC), tricuspid annular plane systolic excursion (TAPSE), left and right ventricular global longitudinal strain (GLS), and left and right ventricular end-diastolic diameter (ED), spherical index (SI), and fractional shortening rate (FS) of 24-segments. The differences of above parameters between TOF group and control group were compared. In addition, the relationships between the absolute value of left and right ventricular GLS of TOF fetus and PV/AV, PV Z-score and MPA Z-score were analyzed. The optimal critical values of GSI, left ventricular EF and left ventricular FAC of TOF fetus were determined by ROC curve, and their corresponding sensitivity and specificity were obtained.Results:Compared with control group, there were significant differences in 4CV end-diastolic length, area, GSI, left ventricular area, left ventricular length, left ventricular EF, left ventricular FAC and left ventricular GLS in TOF group (all P<0.05). There were significant differences in ED between left ventricular 15-24 segments and right ventricular 1-21 segments (all P<0.05). There were significant differences in SI between left ventricular 1-16 segments, 21-24 segments and all segments of right ventricle (all P<0.05). The differences in FS were statistically significant (all P<0.05) when comparing all segments of the left ventricle and 1-2 segments of the right ventricular, and the remaining parameters were not statistically significant (all P>0.05). The left ventricular GLS absolute value of TOF fetuses was positively correlated with PV/AV, PV Z-score and MPA Z-score( rs=0.338, 0.441 and 0.458, all P<0.05), the right ventricular GLS absolute value of TOF fetuses was positively correlated with PV AV, PV Z-score and MPA Z-score( rs=0.418, 0.368 and 0.338, all P<0.05). The optimal critical values of GSI, left ventricular EF, and left ventricular FAC in the diagnosis of fetal TOF were 1.19, 59.05%, and 44.4%, respectively. At this time, the sensitivities of diagnosis of TOF were 0.78, 0.75, and 0.80, respectively. The specificities were 0.88, 0.88 and 0.83, respectively. The areas under ROC curve were 0.89, 0.88 and 0.89, respectively. Conclusions:Fetal HQ technology can provide a simple and reliable quantitative evaluation of fetal heart shape and function, and provide certain theoretical parameters for the study of fetal heart shape and function.
Clinical value of Fetal Intelligent Navigation Echocardiography and Virtual Intelligent Sonographer Assistance in screening of fetal double-outlet right ventricle

Abstract:Objective:To evaluate the value of Fetal Intelligent Navigation Echocardiography (FINE) and Virtual Intelligent Sonographer Assistance (VIS-Assistance?) in the screening of fetal double-outlet right ventricle (DORV).Methods:This study included 30 pregnant women diagnosed with fetal DORV who underwent fetal echocardiography at the Sir Run Run Shaw Hospital from September 2018 to December 2019. Three to five sets of three-dimensional volume data of the heart were collected and the best image quality was selected using the FINE technique for off-line analysis. The diagnostic plane image was optimized by using VIS-Assistance? function, and nine standard diagnostic planes were obtained. The essential information of four diagnostic sections (three-vessel tracheal view, four chamber view, left ventricular outflow view , and right ventricle outflow view) was scored, the display rates of each diagnostic plane and its diagnostic factors were analyzed, and the scoring results obtained by the same observer in different periods and those obtained by different observers in the same period were compared.Results:FINE and VIS-Assistance? were applied to the fetal volume data in 30 cases, of which 25 were analyzed, with a success rate of 83.3%. The display rates of the four diagnostic sections were 84.0%, 88.0%, 72.0%,and 84.0%, respectively. There was no significant difference in the scores obtained by the same observer at different time points, the scores obtained by two relatively inexperienced physicians, and the scores obtained by two relatively inexperienced physicians and one experienced physician (P＞0.05).Conclusion:FINE combined with VIS-Assistance? demonstrates a high success rate in producing four specific cardiac diagnostic sections in fetuses with DORV, has good consistency and repeatability, and therefore can be used for prenatal screening of fetal DORV.
Application Value of Fetal Heart Ultrasound Intelligent Navigation Technique in Display of Key Diagnostic Elements in Rapid Screening Views of Fetal Echocardiography

Abstract:Objective To evaluate the application value of fetal heart echocardiography intelligent navigation (5D Heart) in the display of key diagnostic elements in five rapid screening fetal echocardiographic views.Methods Three hundred and eighty-four 3D volume datasets of 220 normal singleton fetuses in the second and third trimesters were acquired.After processing the datasets using 5D Heart,five rapid screening views were acquired,and 21 key diagnostic elements of the five cardiac diagnostic sections were graded.The 2 test was used to compare the difference in the key diagnostics elements of the five rapid screening views between 5D heart and the traditional technique.Pearson correlation was used to compare the detection rate of key diagnostic elements between 5D Heart technique and traditional two-dimensional echocardiography in normal fetuses and fetuses with congenital heart disease.The rank sum test was used to detect the consistency and reliability of the results.Results A total of 384 fetal volume datasets were obtained from 220 fetuses,of whom 317 heart volume datasets were available for analysis,with a success rate of 82.6％.There was no difference in the key diagnostic elements of the five rapid screening views between 5D heart and the traditional technique (x2=5.17,P=0.27).Compared with the abnormal fetal heart group (89.6％),the detection rate of diagnostic elements in the normal fetal heart group was 85.2％ (r=0.94,P=0.02).No significant differences were found when comparing the grading results of one observer in different periods of time,the grading results between two experienced doctors,or the grading results among the experienced doctor and one relatively inexperienced doctor (P ＞ 0.05).Conclusions Fetal cardiac ultrasound intelligent navigation technique has high reliability and repeatability in obtaining fetal rapid screening views and displaying fetal elemental diagnostic information.It has tremendous potential in standardized data collection and analysis of cardiac diagnostic sections in fetuses with congenial heart disease,as well as in training and teaching,remote medical consultation,and screening in primary hospitals.

Abstract:Objective To assess the feasibility of Doppler tissue imaging(TDI) evaluation of arrhythmias in the fetuses. Methods Thirty-six normal fetuses between 16 to 41 weeks of gestation were studied to identify color- TDI patterns consistent with the various phases of the cardiac cycle. The characteristics of TDI of mitral (or tricuspid) annulus spectrum of 29 fetuses with arrhythmias were compared with the results obtained with M-mode and/or diastolic mitral color Doppler spectrum in evaluating fetal arrhythmias. The ability and diagnose accuracy of TDI were further evaluated independently by two observers with experience in fetal echocardiography. The agreement between them was assessed with the Kappa statistics. Results TDI examinations including 2D-TDI, M-mode TDI and PW-TDI were acceptably reproducible in all 65 fetuses. The TDI spectrum of mitral(or tricuspid) annulus were readily analyzed and yielding diagnostic results in all 29 fetuses with arrhythmias.The ability and diagnose accuracy of DTI were evaluated independently by 2 observers indicated satisfactory agreements.Conclusions TDI evaluation of the fetal heart is feasible and reproducible, this technique may aid in the diagnosis and understanding of fetal arrhythmias.
Novel Foetal Echocardiographic Image Processing Software (5D Heart) Improves the Display of Key Diagnostic Elements in Foetal Echocardiography

Abstract:Background To evaluate the clinical value of foetal intelligent navigation echocardiography (5D Heart) for the display of key diagnostic elements in basic sections. Methods 3D volume datasets of 182 normal singleton foetuses were acquired with a four chamber view by using a volume probe. After processing the datasets by using 5D Heart, eight cardiac diagnostic planes were demonstrated, and the image qualities of the key diagnostic elements were graded by 3 doctors with different experiences in performing foetal echocardiography. Results A total of 231 volume datasets acquired from the 182 normal foetuses were used for 5D Heart analysis and display. The success rate of 8 standard diagnostic views was 88.2%, and the success rate of each diagnostic view was 55.8–99.2% and 70.7–99.0% for the random four chamber view as the initial section and for the apical four chamber view as the initial section, respectively. The success rate of each diagnostic element in the 8 diagnostic sections obtained by 5D Heart was 58.9%~ 100%. Excellent agreement was found between experienced sonographers and less-experienced sonographers (kappa> 0.769). Inter- and intra-observer agreement were substantial to near-perfect, kappa values ranging from 0.612 to 1.000 (Cohen’s kappa). Conclusions 5D Heart can significantly improve the image quality of key diagnostic elements in foetal echocardiography with low operator dependency and good reproducibility.
