Study on the Rehabilitation Effect of Different Chest Drainage Methods Based on the Concept of Accelerated Rehabilitation Surgery on Patients after VATS Lobectomy

Abstract:Objective: To explore the effect of two kinds of closed thoracic drainage under the guidance of rapid rehabilitation surgery (ERAS) on the rapid rehabilitation of patients after video-assisted thoracoscopic (VATS) lobectomy.Methods: From July 2018 to October 2020, 148 patients who underwent VATS right upper lobectomy were divided into a control group (n=70) and an observation group (n=78) by different thoracic closed drainage methods.The control group used closed thoracic drainage with a thick tube.The observation group used a thin tube(Central Venous Catheter)assisted thoracic drainage with the thick tube.The hospitalization time, the postoperative activity time, the indwelling time of chest tube, the postoperative drainage volume, the postoperative NRS(Numerical Rating Scale), postoperative complications, patient comfort rate, and satisfaction rate were compared.Results: The NRS of the observation group were lower than those of the control group on the second day, the third day after operation, and the day of discharge (P<0.05).The hospitalization time, the postoperative activity time, the indwelling time of the chest tube, the postoperative complications rate of pneumothorax, pleural effusion, and the incidence of re-catheterization were all lower than those of the control group (P<0.05),patient comfortable rate and satisfaction rate were higher than those of the control group (P<0.05).Conclusions: Based on ERAS, the application of closed thoracic drainage assisted with thin tubes in patients with thoracoscopic lobectomy can reduce the incidence of postoperative pain and complications, shorten the length of hospital stay, and improve the patient's comfort and satisfaction.
