Abstract:Objective To observe the tolerability and effectiveness of hypofraction irradiation for patients with non small cell lung cancer(NSCLC). Methods 35 cases which confirmed by CT or MRI scan were treated with hypofraction irradiation. Three schedules for brain radiotherapy were carried out, the first was whole brain irradiation with dose of 29.5～30.5 Gy/10 fx 12～14 days for 13 patients with active extracranialfoci. The second was also whole brain irradiation with dosagce of 38.4～39.5 Gy/13 fx for 17～19 days for 16 patients without active extracranial tumors but more than one lesion in the brain. And the third was whole brain irradiation with dosagce of 29.5～30.5 Gy/10 fx for 12～14 days then boosted to 44.9～45.2 Gy/15 fx, for 19～23 days in 6 patients with no active extracranial tumors but only one lesion.Results 34 cases with brain metastasis of NSCLC enrolled in this study(one patient was withdrawn due to seizure). All patients had no marked side effect. Symptoms and signs were improved after irradiation. The median survival period was much prolonged in the first and the second groups( P 0.05).Conclusion The schedules for brain radiotherapy could be tolerated for patients with brain metastasis of NSCLC. These data suggest that the survival period can be improved by given higher dosages for patients without active extracranial tumors.(Shanghai Med J, 1999,22∶606 608)

Abstract:Background: The prognostic factors for patients with brain metastases from non-small cell lung cancer treated with whole brain radiotherapy (WBRT)-based combined modality therapy were investigated. Materials and Methods: Out of 135 patients treated with WBRT, 47 (34.8%) received a radiation boost, 84 (62.2%) underwent systemic chemotherapy, and 39 (28.9%) were given epidermal growth factor receptor tyrosine kinase inhibitors (EGFR-TKIs). Results: The mean survival time was 9.3 months, and the 1-year and 2-year survival rates were 46.3 and 16.1%, respectively. In univariate analysis, improved survival was associated with age < 60 years, no extracranial metastasis, Karnofsky performance score ≥ 70, ≥ 3 cycles of chemotherapy after diagnosis of brain metastases, combined treatment with EGFR-TKIs, and no metastases in the cerebellum. In multivariate analysis, the above prognostic factors maintained significance with the exception of age. In an additional analysis of the 58 patients with 1-3 brain metastases, combination of WBRT with radiation boost was associated with better survival. Conclusion: We confirm previously described prognostic factors. Moreover, we found the absence of cerebellar metastases to be an independent prognostic factor for favorable outcome.

Abstract:Abstract Background About half of lung cancer patients are found to have brain metastasis during the course of the disease. Fractionated stereotactic radiotherapy (FSRT) is a reliable treatment for patients with localized brain metastases. However, there is currently controversy over the prescription doses of FSRT. The aim of this study is to determine maximum tolerated dose of FSRT for brain metastasis from non-small cell lung cancer. Methods This study is a prospective mono-center phase I dose escalation trial which will recruit patients with brain metastases from non-small cell lung cancer receiving FSRT in 5 fractions. A standard 3 + 3 statistical design was employed. The dose escalation consists of 7 cohorts. FSRT will starting at 6 Gy × 5 fractions (dose level 1). The highest dose level will be 10 Gy × 5 fractions. The primary objective of this study is to determine the maximum tolerated dose. Secondary outcomes include acute and late toxicities following FSRT, intracranial progression-free survival (iPFS) and overall survival (OS). Discussion Since the prescription dose of FSRT treatment for patients with localized brain metastases are still unknow, we conducted a Phase I study to determine the maximum tolerated dose of FSRT for limited brain metastases. This trial protocol has been approved by the Ethics committee of Hangzhou cancer hospital. The ethics number is HZCH-2022 #124. This study was registered at ClinicalTrials.gov on 10/20/2022 (NCT05588206).
Hypofractionated Stereotactic Radiation Therapy for Intact Brain Metastases in 5 Daily Fractions: Effect of Dose on Treatment Response

Abstract:Purpose: Multileaf collimator (MLC) linear accelerator (Linac)-based hypofractionated stereotactic radiation therapy (HSRT) is increasingly used not only for large brain metastases or those adjacent to critical structures but also for those metastases that would otherwise be considered for single-fraction radiosurgery (SRS). However, data on outcomes in general are limited, and there is a lack of understanding regarding optimal dosing. Our aim was to report mature image-based outcomes for MLC-Linac HSRT with a focus on clinical and dosimetric factors associated with local failure (LF). Methods and materials: A total of 220 patients with 334 brain metastases treated with HSRT were identified. All patients were treated using a 5-fraction daily regimen and were followed with clinical evaluation and volumetric magnetic resonance imaging every 2 to 3 months. Overall survival and progression-free survival were calculated using the Kaplan-Meier method, with LF determined using Fine and Gray's competing risk method. Predictive factors were identified using Cox regression multivariate analysis. Results: Median follow-up was 10.8 months. Median size of treated metastasis was 1.9 cm; 60% of metastases were <2 cm in size. The median total dose was 30 Gy in 5 fractions; 36% of the cohort received <30 Gy. The median time to LF and 12-month cumulative incidence of LF was 8.5 months and 23.8%, respectively. Median time to death and 12-month overall survival rates were 11.8 months and 48.2%, respectively. Fifty-two metastases (15.6%) had an adverse radiation effect, of which 32 (9.5%) were symptomatic necrosis. Multivariable analysis identified worse LF in patients who received a total dose of <30 Gy (hazard ratio, 1.62; P = .03), with LF at 6 and 12 months of 13% and 33% for patients treated with <30 Gy versus 5% and 19% for patients treated with >30 Gy. Exploratory analysis demonstrated a dose-response effect observed in all histologic types, including among breast cancer subtypes. Conclusion: Optimal local control is achieved with HSRT of ≥30 Gy in 5 daily fractions, independent of tumor volume and histology, with an acceptable risk of radiation necrosis.

Abstract:BACKGROUND AND OBJECTIVE:It has been known that stereotactic radiotherapy has been widely used in clinical practice. The aim of this study is to evaluate the feasibility to treat small lung neoplasms with hypofraction stereotactic radiotherapy.METHODS:From September 2005 to November 2009, 15 patients with non-small cell lung cancer and solitary metastatic lung cancer were treated with hypofraction stereotactic radiotherapy. Dose volume histogram was used to evaluated treatment plans. Lesion diameter 1.5 cm-4.0 cm. Toxicity was evaluated with the NCI-CTCAE 3.0 version. A dose 48 Gy-50 Gy was given in 4-10 fractions.RESULTS:The short-term results showed that the complete response (CR) rate, the partial response (PR) rate, the overall response (CR+PR) rate, the one-year local control rate, the one-year survival rate was 60%, 20%, 80%, 100% and 86.67% respectively. Two patients had radiation pneumonitis of 0 grade. Acute radiation pneumonitis of grade I occurred in 9 patients, 4 patients had radiation pneumonitis of grade II and no patient developed serious (grade III) radiation pneumonitis.CONCLUSIONS:The hypofraction stereotactic radiotherapy is safe for small lung neoplasms.

Abstract:Stereotactic irradiation of extracranial targets is an emerging treatment approach in clinical radiotherapy. Unlike the situation for intracranial stereotactic irradiation or radiosurgery, hypofractionated stereotactic radiotherapy of extracranial targets is complicated by internal organ motion and difficulty regarding patient rigid fixation. Therefore, for clinical implementation of extracranial stereotactic radiotherapy, treatment planning strategies to reduce normal lung toxicity have to be analyzed and properly addressed.

Abstract:ObjectiveTo assess the efficacy and safety of linear accelerator-based fractionated stereotactic radiotherapy (LINAC-FSRT) in patients with brain metastases (BM).MethodsWe retrospectively analyzed 214 patients treated with LINAC-FSRT, categorized based on biologically effective dose (BED10, α/β = 10) into two groups (≤55 Gy, >55 Gy). Stratified analyses were conducted based on targeted therapy to compare survival outcomes. To examine brain tissue dose-tolerance volume, patients were divided into two groups: the standard Hypofractionated Treatment Effects in the Clinic (HyTEC) protocol group and an adjusted HyTEC protocol group where dose-volume restrictions exclude the planning target volume (PTV).ResultsResults as of December 2023 showed median intracranial progression-free survival (iPFS) at 12.4 months, with median overall survival (OS) not reached and a one-year local control (LC) rate of 68.7%. Mild to moderate toxicity affected 17.3% of patients, while severe toxicity occurred in 2.8%. Multivariate Cox analysis indicated that uncontrolled extracranial disease significantly reduced iPFS (HR = 2.692, 95%CI:1.880–3.853, P < 0.001) and OS (HR = 3.063, 95%CI:1.987–4.722, P < 0.001). BED10 >55 Gy (HR = 0.656, 95%CI:0.431–0.998, P = 0.049) improved OS, showing statistical significance (P = 0.037) without affecting iPFS or CNS toxicity (P = 0.127, P = 0.091). Stratified analysis highlighted nearly significant OS improvements with high-dose FSRT and targeted therapy (P = 0.054), while concurrent therapy markedly enhanced iPFS (P = 0.027). No significant differences were observed in intracranial local failure (ILF—which represents progression in previously treated areas during follow-up), one-year LC rates, iPFS, or OS between dose-volume groups. Adjusting HyTEC volume restrictions did not significantly increase CNS adverse reactions (P = 0.889).ConclusionsLINAC-FSRT is safe and effective in BM. BED10>55 Gy notably enhances OS post-LINAC-FSRT and may benefit LC. High BED10 FSRT with targeted therapy likely boosts synergy, and concurrent targeted therapy significantly improves iPFS. Diminishing dose volume constraints at different fractions based on the HyTEC guidelines is feasible.

Abstract:Stereotactic radiosurgery (SRS) is an important weapon in the management of brain metastases. Single-fraction SRS is associated with local control rates ranging from approximately 70% to 100%, which are largely dependent on lesion and postoperative cavity size. The rates of local control and improved neurocognitive outcomes compared with conventional whole-brain radiation therapy have led to increased adoption of SRS in these settings. However, when treating larger targets and/or targets located in eloquent locations, the risk of normal tissue toxicity and adverse radiation effects within healthy brain tissue becomes significantly higher. Thus, hypofractionated SRS has become a widely adopted approach, which allows for the delivery of ablative doses of radiation while also minimizing the risk of toxicity. This approach has been studied in multiple retrospective reports in both the postoperative and intact settings. While there are no reported randomized data to date, there are trials underway evaluating this paradigm. In this article, we review the role of hypofractionated SRS in the management of brain metastases and emerging data that will serve to validate this treatment approach. Pertinent articles and references were obtained from a comprehensive search of PubMed/MEDLINE and clinicaltrials.gov.

Fractionated Stereotactic Radiation Therapy Using Volumetric Modulated Arc Therapy in Patients with Solitary Brain Metastases

Abstract:Purpose: To analyze retrospectively the clinical efficacy and safety for patients treated with fractionated stereotactic radiation therapy (FSRT) using volumetric modulated arc therapy. Methods: Between 2016 and 2017, 46 patients with solitary brain metastasis who underwent FSRT consisting of 25-40 Gy/5 fractions were recruited in this study. All targets within the same course received different prescriptions according to size. Toxicities were graded according to the Common Terminology Criteria for Adverse Events version 4.0. Results: The median follow-up was 11 months (3-53 months). The 6-month and 12-month local control rate calculated by Kaplan-Meier estimate was, respectively, 95% and 86%. Tumor diameter < 2.5 cm obtained 100% improved 12-month local control rate compared with 66% in those with ≥2.5 cm (P < 0.001). The 12-month local control calculated by Kaplan-Meier estimate was 95% in tumors with >30 Gy treatment and only 60% in tumors with ≤30 Gy treatment (P = 0.001). Multivariate analysis revealed that the prescription dose ≤ 30 Gy resulted in increased local failure (hazard ratio (HR), 0.14 (range, 0.019-0.95; P = .046)). Grade 3 or worse toxic effects were found in 5 (11%) patients, and no patient experienced surgical resection for symptomatic radioactive necrosis. Conclusions: FSRT for solid brain metastasis appears to have the advantages of a high rate of local control with a minimal risk of severe toxicity and deserves application in the clinical practice.

Abstract:Background: Studies on the prognostic factors for patients with brain oligo-metastasis treated with fractionated stereotactic radiotherapy (FSRT) usually focus on the size of metastatic tumor and radiation dose. Some inflammatory indicators have predictive value in non-small cell lung cancer (NSCLC) with brain metastasis receiving stereotactic radiotherapy. However, the prognostic value of inflammatory indicators in NSCLC patients with brain oligo-metastasis treated with FSRT, and their effect on radiotherapy dose is unknown. Methods: A total of 95 advanced NSCLC patients with brain oligo-metastasis who had undergone FSRT treatment at Ningbo Medical Center Lihuili Hospital between January 2015 and April 2022 were enrolled into the study. Neutrophil to lymphocyte ratio (NLR), platelet lymphocyte ratio (PLR), lymphocyte to monocyte ratio (LMR), tumor diameter and biologically effective dose (BED10) were analyzed using Chi-square test. Univariate and multivariate Cox regressions were used to identify predictors of survival. Results: Tumor diameter (< 2 cm), BED10 (≥ 48Gy) and LMR (≥ 4) were found to be independently associated with good intracranial local control survival (i-LCS) through multivariate analysis. The median i-LCS was longer in patients with 2 independent risk factors (tumor diameter ≥ 2 and LMR < 4) administered with BED10 > 53.6Gy compared with patients administered with BED10 ≤ 53.6Gy (20.7 months vs 12.0 months, P = 0.042). LMR ≥ 4 (P = 0.019) and positivity for driver gene mutations (P = 0.011) were independently associated with better overall survival (OS). Conclusions: LMR is an independent prognostic factor of i-LCS and OS in NSCLC patients with brain oligo-metastasis treated with FSRT. Patients with tumor diameter ≥ 2 and LMR < 4 should be treated with BED10 greater than 53.6Gy.

Abstract:Background and purpose:The incidence of brain metastasis is high in lung cancer and the survival time is short when this occurs.This study was done to provide referential data by analysis of survival for the patients with brain metastases from lung cancer in Zhongshan Hospital,Fudan University. Methods:The patients were diagnosed for brain metastasis from lung primary cancer by CT and MRI imaging from 2000 to 2005. The brain metastasis was treated through parallel fields radiotherapy with 6-8 MV photons. The dose was 40 Gy in the first 4 weeks for the whole brain plus a dose of 10-20 Gy in 1-2 weeks if only single brain metastasis existed.Results:① The average age was 60.4±8.4 years. The average survival time was 8.0±5.9months if KPS score was more than 50 and dose range was 40-60 Gy (mean =51 Gy).② Control of primary lung cancer and occurence of metastasis to the lung or bone were the factors influencing survival and prognosis.③ The survival time was longer for single brain metastases.Conclusions:The influential factors influencing survival and prognosis is primary lung tumor control and existence of metastasis to lung and bone.
Hypofractionated Stereotactic Radiation Therapy for Advanced Non-Small Cell Lung Cancer: Report on Clinical Outcome and Toxicity

Abstract:Currently, hypofractionated stereotactic radiation therapy for early stage non-small-cell lung cancer (NSCLC) has been achieved a significant curative effect. However, it is still being explored for advanced NSCLC. This retrospective study was to report on long-term follow-up results and tolerance for the treatment of advanced NSCLC. From 2009 to 2014, 27 patients (IIIA: 13, IIIB: 2, IV: 12) with advanced stage NSCLC treated with hypofractionated radiation therapy were analyzed. Four patients for postoperative radiation therapy, 6 patients received chemotherapy before radiation therapy. X knife treatment was given to 11 patients, PTV prescription dose is 35-55Gy/10 20f, γ knife treatment was given to 16 patients, PTV prescription dose is 21-50Gy/6-10f. Overall survival (OS) rates were calculated from the date of initial RT and were estimated using the Kaplan–Meier method. Subgroups analyses using the log-rank test. The Common Terminology Criteria for Adverse Events (CTCAE) version 3.0 was used as the standard for the evaluation of treatment toxicity. A P value of less than 0.05 was considered statistically significant. Median follow-up was 24 months (range, 6-72 months). The median OS was 24 months, with 1- and 3-year OS rates of 73.7% and 36.4%, respectively. Prior chemotherapy vs. radiation therapy alone had no significantly better OS (P=0.58). Female patients vs. male patients had a significantly better on OS (P=0.01).The percentages of Grade II acute erythema and Grade II chest pain were 4% and 7%, respectively. Three patients (11%) had Grade I late esophagitis toxicity. The percentages of Grade I and Grade II late chest pain were 4% and 7% respectively. Acute or late Grade III and above toxicity did not occur. Hypofractionated radiation therapy is a feasible methods and well-tolerated treatment for patients with advanced NSCLC. The results still need large-scale clinical trials to verify.
Analysis of Whole Brain Radiotherapy Plus Stereotactic Radiosurgery in Treatment of Brain Metastases from Lung Cancer

Abstract:Objective To assess the therapeutic effect of whole brain radiation therapy(WBRT) plus stereotactic radiosurgery(SRS) in the treatment of brain metastases from lung cancer. Methods Fifty-six patients with brain metastases from lung cancer were treated with WBRT(WBRT group,n=35) or WBRT plus SRS boost(WBRT+SRS boost group,n=21).The median dose of WBRT was 30 Gy in 10 fractions,and the median dose of SRS was 15 Gy in one fraction or 20 Gy in two fractions.The median survival,1-year survival rate,acute radioreaction and later complications were analysed between the two groups. Results The patients were followed up for 17 to 27 months(median,20 months).The median survival for WBRT group and WBRT+SRS boost group were 5.3 months(1 to 19 months) and 6 months(1 to 21 months),respectively,and the 1-year survival rate were 19.74% and 20.68%,respectively(P0.05).For the single brain metastasis,the median survival for WBRT group and WBRT+SRS boost group were 4.7 and 10.4 months,respectively,while no significant difference was found between the two curves of survival(P0.05).There was no significant difference in the prevalence of acute radioreaction between the two groups(P0.05),and no serious later complications were observed. Conclusion The median survival of WBRT plus SRS boost is better than that of WBRT alone for the treatment of single brain metastasis from lung cancer,while there is no significant difference between the two curves of survival.The whole brain irradiation dose of 30 Gy in 10 fractions plus SRS dose of 15 Gy in one fraction or 20 Gy in two fractions is safe and feasible.

Group A whole brain irradiation to 30 ?Gy/10 fractions over 2 weeks then boosted to 45?Gy/15 fractions over 3 weeks for patients with only one lesion in the brain without any extracranial involvement(14 cases).Group B whole irradiation to 39?Gy/13 fractions over 2.5 weeks for patients with more than one lesion in the brain also without any extracranial involvement (29 cases). Group C whole irradiation to 30 ?Gy/10 fractions over 2 weeks for patients with both extracranial and brain lesions(32 cases). Results The incidence of symptom relief (pain,nausea and vomiting) was 100% in all groups.The incidences of neurological sign relief in groups A、B、C were 100%(7/7),86%(12/14),93%(14/15). The overall median local control duration was 7.6 months (0.8-52.7 months) . The overall 1-and 2-year local control rates were 46% and 23%.The 1-and 2-year local control rates of groups A, B,C were 55% and 38%, 52% and 26%, 33% and 10%,respectively. The overall median survival was 9.6 months (0.8-52.8 months). The overall 1-and 2-year survival rates were 49% and 26%.The 1-and 2-year survival rates of groups A, B,C were 53% and 49%,50% and 23%,45% and 9%,respectively. Side effects: None of these patients showed serious acute side effects except one in group A who developed gastrointestinal bleeding and one in group C who had epileptic seizures during the second session of irradiation. Late effects:Two patients in group B showed impairment in thought coherence and behavior though without any tumor in the brain after 23,17 months of irradiation. The results of Cox regression in local control suggested that higher dose may give better local control duration (P=0.003). And another result of Cox regression in the overall survival showed that KPS and local control of brain tumor might influence the overall survival duration(P=0 .0 5 1,0 .056) .Conclusions This protocol gives satisfactory local control and local control duration without necessitating over-treatment in some patients.It warrants further study .

Abstract:Objective:To observe the efficacy and toxicity of whole-brain radiotherapy plus rotating Gamma Knife in patients with brain metastasis from lung cancer.Methods:All 52 patients suffering from lung cancers with brain metastasis were included in this study.The treatment consisted of concomitant whole brain radiotherapy(WBRT) with a dose of 30Gy in 10 fractions or 40Gy in 20 fractions.And then a dosage of 10-15Gy was added to the focus of metastasis by rotating Gamma Knife.Results:The overall response rate was 98.1%,CR 35 patients,PR 16,NC 1.Conclusion:The rotating Gamma Knife combined with whole brain radiation for brain metastasis from lung cancer is effective and can improve life quality.

Abstract:Background: Cavity stereotactic radiotherapy has emerged as a standard option following resection of brain metastases. However, the optimal approach with either single-fraction or hypofractionated stereotactic radiotherapy (HSRT) remains a significant question. Objective: To report outcomes for 5-fraction HSRT to the surgical cavity, based on contouring according to a recently reported international consensus guideline. Methods: Patients treated with cavity HSRT were identified from a prospective institutional database. Local brain control (LC), distant brain failure (DBF), leptomeningeal disease (LMD), and overall survival rates were determined. Univariate and multivariable analyses were performed on potential predictive factors. Results: One hundred thirty-seven cavities in 122 patients were treated at a median total dose of 30 Gy (range, 25-35 Gy). The median follow-up was 16 mo (range, 1-60 mo). Nonsmall cell lung cancer was the most common histology (44%), followed by breast cancer (21%). In 57% of surgical cavities, the preoperative tumor diameter was >3 cm. One-year LC, DBF, LMD, and overall survival rates were 84%, 45%, 22%, and 62%, respectively. Multivariable analyses identified colorectal (hazard ratio [HR] 4.1, P = .0066) and melanoma (HR 2.4, P = .012) metastases as predictors of local recurrence; preoperative tumor diameter >2 cm (HR 8.9, P = .012) and absence of targeted therapy (HR 4.4, P = .03) as predictors of DBF; and breast cancer histology (HR 2.1, P = .05) and subtotal resection (HR 2.6, P = .009) as predictors of LMD. Symptomatic radiation necrosis was observed in 7 patients (6%). Conclusion: High rates of LC were observed following this 5-fraction HSRT regimen. Superiority as compared to single-fraction SRS requires a randomized trial.

Abstract:Objective: To compare the short-term therapeutic effects, long-term survival and adverse effects between whole brain radiotherapy with targeted therapy and concomitant chemo-radiotherapy in treatment of non-small-cell lung cancer (NSCLC) with brain metastasis. Methods: The clinical data of 58 patients with NSCLC with brain metastasis were retrospectively analysed. Thirty-six patients received whole brain radiotherapy with targeted therapy (oral administration of gefitinib 250 mg or erlotinib 150 mg per day), and the other 22 patients received whole brain radiotherapy with concomitant chemotherapy (platinum-based regimen). Survival analysis was conducted in two groups with Kaplan Meier method and Logrank test, and the differences in therapeutic effects were explored between two groups. Results: The disease control rate in whole brain radiotherapy with targeted therapy group was significantly higher than that in concomitant chemo-radiotherapy group (66.7% vs 36.4%, P<0.05). The 1-year survival rate in whole brain radiotherapy with targeted therapy group was 68%, and the median duration of survival was 23.2 months. The 1-year survival rate in concomitant chemo-radiotherapy group was 41%, and the median duration of survival was 7.1 months. The median duration of survival in whole brain radiotherapy with targeted therapy group was significantly longer than that in concomitant chemo-radiotherapy group (P<0.05). Conclusion: Whole brain radiotherapy with targeted therapy is superior to concomitant chemo-radiotherapy in disease control rate and duration of survival for treatment of NSCLC with brain metastasis.
