Abstract:A common challenge faced by the healthcare systems in many low- and middle-income countries is the substantial unmet mental healthcare needs, or the large gap between the need for and the provision of mental healthcare treatment. This paper investigates the potential causes of this treatment gap from the perspective of economics. Specifically, we hypothesize that people with mental illness face4major hurdles in obtaining appropriate healthcare, namely the high nonmonetary cost due to stigma, the high out-of-pocket payment due to insufficient public funds devoted to mental health, the high time costs due to low mental healthcare resource availability, and the low treatment benefit due to slow technology diffusion. We use China as a study setting to show country-specific evidence. Our analysis supports the above theoretical argument on the 4 barriers to access, which in turn sheds light on the effective approaches to mitigate the treatment gap. Four policy options are then discussed, including an information campaign for mental health awareness, increasing public investment in primary mental healthcare resources, transforming the healthcare system towards an integrated people-centered system and capitalizing on e-health technologies.

Abstract:Recent research documents increasing unhappiness (even depression) among Chinese adults despite the rapid economic growth in China. This paper explains this discrepancy by exploring the relationship between relative economic status and mental health (measured by Center for Epidemiologic Studies Depression Scale score) among the middle-aged and elderly in China. In measuring relative economic status, we use five reference groups for comparison (i.e., relatives, colleagues, schoolmates, neighbors or villagers, and people in the same city or county). Using data from 2013 China Health and Retirement Longitudinal Study, we find that individuals with relatively lower economic status are more likely to be mentally depressed. The relationship differs across reference groups for comparisons, with the larger impacts seen from the comparison against acquaintances, while the impact of comparison to regional average is the smallest despite its frequent use in the literature. The relative economic status-mental health gradient is asymmetric; that is, the negative impact of worse relative economic status is comparatively larger and more significant than the positive impact of better relative economic status. Our results imply that reducing wealth gaps within groups of similar backgrounds can be more effective in improving people's mental health than a general reduction of wealth inequality within the population.

Abstract:Over the past few years there have been considerable changes in China's mental health service system. This review provides an overview of the development of mental health services in China, including epidemiological data on psychiatric disorders, utilisation of mental health services, models of mental health service delivery, mental health resources and workforce, mental health policy framework and financial issues. We consider cultural and social factors including the involvement of family members in patient care, urbanisation and internal migration as well as the application of traditional Chinese medicine, which provides implications for mental health research and practice. Additionally, we also discuss major challenges and conclude by providing some specific recommendations on improving mental health services in China.

Socioeconomic inequalities and mental stress in individual and regional level: a twenty one cities study in China

Abstract:Objectives This study will examine explanatory variables including socioeconomic inequalities related to mental stress at both the individual and regional level. Methodology A cross-sectional multistage sampling process was used to obtain participants. Data on mental stress and individual socioeconomic status were gathered via face to face interview. Regional variables were retrieved from a national database. Multilevel logistic regression analysis was used to assess socioeconomic variances in mental stress. Results Among the 16,866 participants, 27.2% reported severe levels of mental stress (95% CI: 19.4%-35.1%). Multilevel regression analysis indicated that lower individual educational attainment and income, and lower regional Per Capita GDP was associated with mental stress. The results also indicated that managers, clerks, and professional workers manifested higher stress levels than those in other occupations. Conclusions Based on the results of this study individual and regional socioeconomic inequalities in China are associated with mental stress.

Abstract:Mental health (MH) differences remain an issue in contemporary Chinese society, with various factors influencing individuals' well‐being. This study investigates the complex connection between results related to psychological wellness and social issues, integrating cultural, psychological and technological perspectives. Through a quantitative research methodology, data were collected from 1000 individuals using a questionnaire survey and analysed using SPSS software. The study's findings shed light on the impact of socioeconomic status (SES) on MH stigma, help‐seeking behaviour and overall well‐being. The findings identify that lower SES was associated with heightened levels of MH stigma and decreased the search for assistance actions. Cultural beliefs emphasising MH stigma and societal expectations were identified as mediators in the connection involve SES and MH outcomes. Digital technology use emerged as a moderator, indicating that higher levels of technology utilisation were linked to reduced disparities in accessing MH resources. The novelty of this study lies in its comprehensive examination of the multifaceted factors influencing MH disparities within Chinese society. Additionally, psychological factors such as resilience and social support were observed to mitigate the negative lower SES's effect on MH consequences. The interaction between SES and cultural factors was found to predict MH outcomes, with lower SES exacerbating the effects of MH stigma and cultural barriers.

Abstract:The mental health implications of housing affordability have recently received research attentions in different countries. This study examines this issue in the context of urban China, where housing affordability has become one of the most pressing social issues and its health implications have aroused academic interests. We aim to enrich the literature by specifying the mental health implications of housing affordability for different population groups and geographical regions. We focus on homeowners living in urban China. Data from the 2016 Wave of the China Family Panel Studies are used for the study. Models are developed for the whole sample and for subsamples of different socioeconomic backgrounds or from different Chinese regions. We find that the mental health impacts of housing affordability are significantly different for different population groups. For examples, though males are in general mentally healthier than females, they are more likely to mentally suffer from housing unaffordability than females; unaffordable housing owners with non-agricultural Hukou are more likely to sacrifice mental health than those with agricultural Hukou, despite that the former are mentally healthier than the latter; housing unaffordability costs mental health for people living in the Western region, but not for those living in other regions of China. We argue that these group differences in the Chinese context may be attributed to a number of factors distinguishing China from other countries including the Hukou system and the large regional differences in social and economic development.

Depression Hurts, Depression Costs: the Medical Spending Attributable to Depression and Depressive Symptoms in China.

Abstract:Due to its fast economic growth and lifestyle changes, China is experiencing a rapid epidemiological transition from communicable to noncommunicable diseases (NCDs). Mental disorder such as depression is an important yet often neglected NCD and is becoming a growing cause of disability, suicides, and disease burden. This paper provides the first nationally representative estimate of the medical cost attributable to depression and depressive symptoms among the adult population in China. On the basis of the 2012 China Family Panel Studies survey, our results indicate that these mental health conditions have significant impacts on the individual medical expenditure, and they jointly contribute to 14.7% of total personal expected medical spending in China, with depression and depressive symptoms accounting for 6.9% and 7.8%, respectively. Given that patients with mental illness face multiple psychological and institutional barriers in seeking appropriate treatment, the high depression-induced medical costs may be primarily driven by the cost-shifting effect from mental health care to general health care, as mental disorders often coexist with other NCDs such as diabetes and hypertension. As an implication, our study calls for an urgent reform of China's mental health and insurance systems to remove the policy-induced obstacles for the access to mental health care resources.
The Prevalence of Depression and Depressive Symptoms among Adults in China: Estimation Based on a National Household Survey

Abstract:China's fast economic growth in the past decades is accompanied by a rapid epidemiological transition from communicable to non-communicable diseases (NCDs). An important yet often neglected NCD is mental disorder, which accounts for 14% of global disease burden but has been paid relatively little research attention in China. This paper uses a nationally representative dataset to investigate the prevalence and correlates of depression and depressive symptoms among the adult population in China. Our results indicate that the prevalence rate of depression, estimated with the Center for Epidemiologic Studies Depression Scale (CES-D), is high (37.9% for depressive symptoms and 4.1% for depression) and unevenly distributed across regions and subpopulations. Specifically, women, older people and those who live in the central/western and rural areas are more likely to be depressed. We also find significant socioeconomic gradients in mental health: higher education and income levels are associated with lower likelihood of depression, especially among the lower socioeconomic groups. Our results indicate the urgent need for depression prevention and treatment in China (particularly in the economically less developed regions) through the expansion of primary mental health care resources and a reduction of socioeconomic inequalities.
Sex ratios and mental health: Evidence from China

Abstract:While sex ratios (i.e., relative numbers of men and women) have been linked to various economic and social outcomes, how sex ratios affect mental health is underexplored. Using nationally representative data from the China Family Panel studies (CFPS) and Population Census, we evaluate the impact of sex ratios on mental health among Chinese men and explore potential mechanisms. Employing the instrumental variables (IV) approach where the One-Child Policy's mandated fertility limits and implementation are used as exogenous variations in local sex ratios, we find that higher local sex ratios increase depressive symptoms and probability of depression among Chinese men. The impact is stronger for men with lower levels of education and living in rural areas. Analyses of potential mechanisms show that higher sex ratios increase the likelihood of marriage delay and unemployment for men, and prolong working hours for the employed men. The findings are of direct relevance to the health and population policy in China.
Impact of House Price Growth on Mental Health: Evidence from China.

Abstract:BACKGROUND:Housing boom has raised global attention in the past two decades. A number of studies attempted to analyse the effect of house price increase. However, little is known about the health consequence as a result of housing boom, likely due to the scarcity of the data. The objective of this paper is to investigate the relationship between housing affordability and mental health as a result of house price increase.METHODS:Based on a panel dataset of 32 Chinese cities from January 2013 to December 2017, we used a fixed effect model adjusting for per capita disposable income to estimate the impact of house price growth rate on mental health, and applied the Instrumental Variable (IV) method to address the endogeneity problem.RESULTS:From both Ordinary Least Squares (OLS) and IV estimations, the results suggested that a one standard deviation increase in house price increase rate in the past three months is associated with a 0.443 standard deviation increase in people consulting with doctors about their mental disorders in the city. This effect does not vary by gender, but was more pronounced in residents older than 40 years.CONCLUSION:These results revealed the potential negative consequences in people's mental health due to house price increase, necessitate appropriate policy responses.

Abstract:Objective:This article aims to describe methodology and prevalence of mental disorder under the current status of mental health service in the new century,and to explore impact factors of prevalence of mental disorder for decreasing disease burden of mental disorder.Method:Large scale epidemiological studies on mental disorders were reviewed and the results of prevalence studies were summarized in the past decades.Results:There were different prevalence rates of mental disorders using different methodology in different regions.Needs of mental health service had increased,but resources and models of mental health services had not met the needs.Conclusions:There are challenges of improving methodology and reforming model of service in China.In order to reduce disease burden of mental disorder,mental health promotion should be advanced.
A Multilevel Cross-Sectional Study of Mental Disorder in Community Settings in Zhejiang Province

Abstract:OBJECTIVE:To investigate the prevalence of mental disorder through the 12-item general health questionnaire (GHQ-12) survey and its related factors in community settings in Zhejiang Province.METHODS:A muti-level model was analyzed using data of 14,632 respondents nested in counties (cities), townships (urban districts), villages (urban neighborhoods) from the epidemiological survey of mental illnesses above 15 years old.RESULTS:The mean score measured by GHQ-12 was (1.54 +/- 2.15), which was lower than that of samples from four cities of Guangzhou, Chongqing, Taiyuan and Hangzhou (U = 11.08, P < 0.01). The mental disorder clustered within county (city) level, which represented 4.3% (chi(2) = 3.94, P < 0.05) of the total variance including four levels. After controlling for the county level, the mental disorder was associated with age, marital status, residency patterns, education, occupation and household incoming. In particular, it was reported that higher household incoming was significantly associated with better mental disorder among population in community settings.CONCLUSION:The prevalence of mental disorder clustered at the county (city) level is relatively lower with vulnerable groups including divorced, widowed, unemployed, retired, lower-income ones in community settings in Zhejiang Province.

Abstract:Objectives Mental health has become a significant public health problem that impacts both economic and social development, with severe mental disorders (SMDs) being the top priority. Over recent years, Beijing, China, has introduced several policies to reduce the economic burden on patients with mental health disorders. The aim of this study was to investigate the current status and composition of patients' medical expenses following the introduction of multiple medical policies, explore the factors that may impact the utilisation of medical services and provide a reference and basis for subsequent policy improvements. Study design Multistage sampling was used to select a representative study population. A retrospective survey was used to collect patient information and data on medical expenses in 2019. Methods Descriptive statistics were applied to analyse the current status of patients' medical expenses, and a two-part model was used to examine the factors influencing healthcare utilisation and to model predicted expenses. Results Among 4940 participants, the average outpatient expenses of patients with SMD who incurred medical expenses were 8373.61 Yuan, and the average hospitalisation expenses were 81,594.05 Yuan. The out-of-pocket expenses were 29.22% of outpatient expenses and 8.13% of inpatient expenses. Factors such as age, household status, economic status, marital status, participation in the Community Free-Medication Service (CFMS) and the type of disease diagnosed influenced the differences in medical expenses and utilisation of services. Conclusions The medical expenses of patients with SMD in Beijing are high, but a number of introduced policies have effectively reduced these costs for patients. Future studies should focus on the impact of factors such as age, economic status, participation in the CFMS and the type of disease diagnosed on medical expenses.

Mental Health Challenges Raised by Rapid Economic and Social Transformations in China: a Systematic Review

Abstract:BackgroundWith the largest proportion of world's total population, the mental health pandemic in China and related challenges of treating mental disorders could influence the accomplishment of UN Sustainable Development Goals, which include mental health. Therefore, understanding China's current mental health status after rapid economic and social transformations, including urbanisation, industrialisation, globalisation and informatisation, could strengthen awareness of mental health, and aid the proposal of solutions to prevent and control mental disorders.MethodsWe did a systematic search of the literature using PubMed, Embase, PsycINFO, CNKI, and Wanfang databases to find relevant studies published between Jan 1, 1990, and Dec 31, 2018. Articles in English and Chinese were included. The search terms included “urbanisation”, “industrialisation”, “globalisation”, “informatisation”, “mental disorders[MeSH Term]” and “China”.FindingsDuring the past three decades, both the disease and economic burden of mental disorders have increased in China. Rapid urbanisation and industrialisation have resulted in increasing internal migration and left-behind families, who have poorer mental health status than their peers. The ageing population will lead to higher prevalence of mental and neurological disorders, such as depression and dementia. Adoption of unhealthy diets can directly affect mental health, as well as indirectly by facilitating intermediate risk factors such as obesity. Increased alcohol intake and smoking can induce increased alcohol and nicotine dependence, and even illicit drug abuse. Additionally, internet and social media use can not only increase internet or gaming addiction, but also facilitate exposure and delivery channels via the internet. The growth of urbanisation and manufactories has also increased air, light, and noise pollution that can further induce mental problems.InterpretationSocial and economic transformations have triggered increasing disease and economic burden of mental disorders. We propose three priorities to deal with these challenges: identify social and economic effects on mental health on the basis of high-quality qualitative and quantitative analysis, improve mental health awareness and literacy, and enhance the mental health-care system and promote implementation research. Lessons from China can be a great reference for low-and-middle-income countries (LMICs) and upper-middle-income countries in prevention and control mental disorders.FundingNational Natural Science Foundation of China.
Educational Mismatch and Mental Health: Evidence from China

Abstract:Education is one of the most important social determinants of mental health. Many studies have shown that overeducation has different and complex correlations with mental health status under different circumstances. However, few studies have compared overeducation and undereducation in the Chinese context to illustrate their different associations with mental health. Our study attempts to evaluate the potential association between educational mismatch and workers' mental health in China by subdividing educational mismatch into overeducation and undereducation. By using the 2020 Chinese Family Panel Survey (CFPS) data, we screened out 7225 valid cross-sectional samples and used an ordered logit model to identify the association between educational mismatch and mental health. The findings show that overeducation is strongly associated with better psychological status, while undereducation is significantly associated with worsening psychological status. The mediating effect test shows that income satisfaction has penalty and premium effects respectively in different contexts. In addition, we found that undereducation was not significantly associated with worse psychological status in the lower job satisfaction and stable employment groups, whereas overeducation was not significantly associated with better psychological status only in precarious employment. Our research provides evidence that developing policies and interventions to improve national education can help improve national health.

Abstract:Abstract Background: The prevalence of mental health problems in medical students has continuously increased and is higher than the prevalence of mental health problems in students with other majors, which could lead to undesirable consequences for the students and their future patients. In China, the current states of medical education and healthcare workplaces differ in certain areas from those in Western or other Asian countries. However, the mental health status of Chinese medical students has not been systematically analyzed. Objectives: The purpose of this meta-analysis was to summarize the prevalence of mental health problems in Chinese medical students. Methods: All cross-sectional studies that investigated the prevalence of any mental health problem among Chinese medical students were retrieved from the following databases: EMBASE, PubMed, PsycINFO, OVID, the Cochrane Database of Systematic Reviews, and the Cochrane Central Register of Controlled Trials. We accepted each individual trial's inclusion and exclusion criteria for participants. The National Institutes of Health Quality Assessment Tool for Observational Cohort and Cross-Sectional Studies was adopted to appraise the methodological quality of each study. RevMan (version 5.3) was used to analyze the data. Results: Ten cross-sectional studies involving a total of 30,817 Chinese medical students were included. The prevalence of depression, anxiety, suicidal ideation, and eating disorders were 29%, 21%, 11%, and 2%, respectively. Subgroup analysis revealed no significant differences in the prevalence of depression and suicidal ideation between genders and no significant difference in the prevalence of depression between individuals of different ages (20 years and older or younger than 20 years). Conclusions and implications of key findings: Chinese medical students have relatively high prevalence of depression, anxiety, and suicidal ideation but a low prevalence of eating disorders. Mental health problems in Chinese medical students should be taken seriously, and timely screening of and proper intervention in these mental health problems are highly recommended.

Abstract:Introduction: The shifting living and working conditions have profound impacts on the residents' mental health. However, current research in this field has not remarkable investigated. Methods: This study used the China Labor-force Dynamic Survey (CLDS) dataset from 2018 and relied on a regression model to examine the effects of the built environment, work environment, and subjective perception on the mental health of Chinese citizens. It also considers the circumstances of both migrants and local residents. Results: This study revealed significant correlations between mental health and greening space rate, road network density, commuting time, work feelings, community trust, economic satisfaction, and other factors. Additionally, the mental health of local residents was shown to be significantly affected by community security, while it shows no significance in migrants. Furthermore, a significant spatial autocorrelation was found in terms of mental health within the central and eastern regions of China. Discussion: The findings of this study offer valuable insights that can be used to facilitate measures aimed at improving the mental health of residents and promoting the development of healthy cities.

Abstract:With the socioeconomic development and the acceleration of the ageing process of the population, the incidence rates of mental disorders and psycho-behavioural problems have become higher and higher worldwide. So far, the disease burden caused by mental disorders has ranked second in the world.1 In China, the disease burden caused by mental disorders accounts for 13% of all non-communicable diseases burden.2 Hence, mental health has become a major public health problem and social problem. In order to promote mental health, the Chinese government has promulgated a series of policies and regulations, as well as reform measures. Moreover, the upcoming ‘China Brain Project’ will also focus on some mental disorders such as autism, depression and dementia to improve mental health research in China. ### Mental health services #### High prevalence of mental disorders and low consultation rate The epidemiological survey conducted in four provinces in China showed that the prevalence of mental disorders among adults in China was 17.5%, in which the prevalence of mood disorders (mainly depression), anxiety disorders and substance use disorders were 6.1%, 5.6% and 5.9%, respectively.3 According to the WHO report, the recognition rate of global mental disorders is around 50%, and the recognition rate in China is far below the world average. Taking depression as an example, the recognition rate of depression was only 21% in Shanghai, China. In addition, the rate of diagnosis and treatment of mental disorders is also relatively low, with an average of only around 150 people per 100 000 people receiving treatment for serious mental disorders. The treatment rate for serious mental disorders in high-income countries is about 17 times that of low-income countries.4 #### High proportion of refractory mental disorders and poor medications response The effectiveness of medications treating mental disorders varies among patients.
