Abstract:Mental health conditions such as depression is a rapidly rising epidemic and a major contributor to the overall global burden of disease. In addition to the direct medical costs and indirect economic costs that falls into the traditional boundary of disease burden estimation, many social costs associated with depression are hidden yet important. This paper provides empirical evidence on the existence of two hidden costs associated with depression: negative impact on social trust and life satisfaction. Based on the data obtained from 2012 China Family Panel Studies, our estimated results indicate that individuals who have a high tendency for depression or depressive symptoms are less likely to trust other people, and they also have significantly lower life satisfaction than their counterparts who are relatively mentally healthy. Given that trust is an important component of social capital, which in turn is an important input to foster economic growth in general and innovation in particular, the reduction in trust induced by the increasing prevalence of depression imposes a significant cost to the society in terms of poor economic performance. Similarly, as life satisfaction has been widely recognized as an important measure of well-being, our study also highlights that the increase in the prevalence of depression leads to a reduction in the well-being that individual can enjoy. All these costs are real, but did not receive sufficient attention in the previous research. The contribution of our research is to shed light on the existence of these hidden costs and to quantify the magnitude of such costs in the context of China.

Abstract:BACKGROUND:Trust is important for health at both the individual and societal level. Previous research using Western concepts of trust has shown that a high level of trust in society can positively affect individuals' health; however, it has been found that the concepts and culture of trust in China are different from those in Western countries and research on the relationship between trust and health in China is scarce.METHOD:The analyses use data from the national scale China General Social Survey (CGSS) on adults aged above 18 in 2005 and 2010. Two concepts of trust ("out-group" and "in-group" trust) are used to examine the relationship between trust and self-rated health in China. Multilevel logistical models are applied, examining the trust at the individual and societal level on individuals' self-rated health.RESULTS:In terms of interpersonal trust, both "out-group" and "in-group" trust are positively associated with good health in 2005 and 2010. At the societal level, the relationships between the two concepts of trust and health are different. In 2005, higher "out-group" social trust (derived from trust in strangers) is positively associated with better health; however, higher "in-group" social trust (derived from trust in most people) is negatively associated with good health in 2010. The cross-level interactions show that lower educated individuals (no education or only primary level), rural residents and those on lower incomes are the most affected groups in societies with higher "out-group" social trust; whereas people with lower levels of educational attainment, a lower income, and those who think that most people can be trusted are the most affected groups in societies with higher "in-group" social trust.CONCLUSION:High levels of interpersonal trust are of benefit to health. Higher "out-group" social trust is positively associated with better health; while higher "in-group" social trust is negatively associated with good health. Individuals with different levels of educational attainment are affected by trust differently.

Abstract:Recent research documents increasing unhappiness (even depression) among Chinese adults despite the rapid economic growth in China. This paper explains this discrepancy by exploring the relationship between relative economic status and mental health (measured by Center for Epidemiologic Studies Depression Scale score) among the middle-aged and elderly in China. In measuring relative economic status, we use five reference groups for comparison (i.e., relatives, colleagues, schoolmates, neighbors or villagers, and people in the same city or county). Using data from 2013 China Health and Retirement Longitudinal Study, we find that individuals with relatively lower economic status are more likely to be mentally depressed. The relationship differs across reference groups for comparisons, with the larger impacts seen from the comparison against acquaintances, while the impact of comparison to regional average is the smallest despite its frequent use in the literature. The relative economic status-mental health gradient is asymmetric; that is, the negative impact of worse relative economic status is comparatively larger and more significant than the positive impact of better relative economic status. Our results imply that reducing wealth gaps within groups of similar backgrounds can be more effective in improving people's mental health than a general reduction of wealth inequality within the population.

Abstract:Life satisfaction in China declined dramatically precisely at the time of its unprecedented economic growth and poverty reduction. We posit that a "progress paradox" is among the possible channels explaining these contrasting trends. Using data from the Chinese Livelihood Survey, we explore the role of reported physical and psychological health. The standard correlates of life satisfaction such as age, income, and health hold. In addition, we find that those with insufficient rest and leisure are significantly less satisfied. Urban, educated respondents are more likely to report depression, while rural and uneducated respondents are more satisfied with their lives and are less likely to report poor mental health. We also find that insufficient rest, stress, and low life satisfaction have a strong correlation with mental health problems. Given the gains in growth and poverty reduction in China, it is time to consider policies that focus on quality of life and mental illness. (C) 2017 Elsevier Ltd. All rights reserved.

Abstract:Due to its fast economic growth and lifestyle changes, China is experiencing a rapid epidemiological transition from communicable to noncommunicable diseases (NCDs). Mental disorder such as depression is an important yet often neglected NCD and is becoming a growing cause of disability, suicides, and disease burden. This paper provides the first nationally representative estimate of the medical cost attributable to depression and depressive symptoms among the adult population in China. On the basis of the 2012 China Family Panel Studies survey, our results indicate that these mental health conditions have significant impacts on the individual medical expenditure, and they jointly contribute to 14.7% of total personal expected medical spending in China, with depression and depressive symptoms accounting for 6.9% and 7.8%, respectively. Given that patients with mental illness face multiple psychological and institutional barriers in seeking appropriate treatment, the high depression-induced medical costs may be primarily driven by the cost-shifting effect from mental health care to general health care, as mental disorders often coexist with other NCDs such as diabetes and hypertension. As an implication, our study calls for an urgent reform of China's mental health and insurance systems to remove the policy-induced obstacles for the access to mental health care resources.
The Relationship Between Social Capital and Depressive Symptoms Among the Elderly in China: The Mediating Role of Life Satisfaction

Abstract:Background and aim: Depression has become a serious health and social issue in recent years in China. This study aims to explore the relationship between social capital and depressive symptoms among the elderly in China, with a particular focus on the mediating role of life satisfaction. Methods: The data of this study were sourced from the 2016 wave of China Family Panel Studies (CFPS), involving 1243 older adults aged 60 and above. A multiple linear regression model was used to explore the impact of social capital on depressive symptoms. Moreover, the add-on PROCESS macro for SPSS was employed to measure the mediating effect of life satisfaction on the relationship between social capital and depressive symptoms. Results: The regression results suggest that CES-D score was associated with trust (coefficient = -0.1013, p < 0.01). In addition, the protective role of trust was significantly stronger for older adults aged 70-79, women, the poorest 1/3, and the elderly who live in rural areas. Moreover, the mediation analysis results suggest that the effect of trust on depressive symptoms was fully mediated by life satisfaction. Conclusion: This study reveals that social capital has a positive effect on depressive symptoms among the elderly, and the positive health effect shows significant age, gender, income, and location inequalities. Furthermore, this study also provides new evidence indicating that life satisfaction fully mediates the relationship between social capital and depressive symptoms. Improving social capital could be a promising way for China to promote healthy aging in the future.

Abstract:This cohort study used the China Health and Retirement Longitudinal Study (CHARLS, 2015-2018) to investigate the effects of socioeconomic status and social capital to the incidence of depressive symptoms among middle-aged and older individuals in China, incorporating a sample size of 9949 participants. Socioeconomic status, social capital and other explanatory variables were collected in 2015, while depressive symptoms were assessed in 2018. Basic characteristics and social capital measures were compared between urban and rural residents using the chi-square test. Logistic regression was used to explore the relationship between socioeconomic status, social capital and depressive symptoms, and the Karlson, Holm, and Breen (KHB) method was employed to verify the mediating role of social capital. We reported persistent socioeconomic inequalities in depressive symptoms, with rural residents and the illiterate having 1.45 times and 1.34 times higher odds of depression. We ascertained social capital from both the cognitive and structural constructs, where we enriched the measurement of structural social capital from three specific dimensions, i.e., informal interaction, altruism, and formal social participation. We found that both cognitive and structural social capital were associated with lower incidence of depressive symptoms, where informal interaction had the largest effect. The mediation analysis further illustrated that informal interaction contributed most to explain 6 %-12 % of the socioeconomic inequalities in depressive symptoms. These results highlighted the unsatisfied mental wellbeing of the vulnerable older people living in rural areas. The finding suggested that older people may benefit more from personal interactions than formal participations. To fulfill the Health in All vision, government and social organizations should consider how to create opportunities to better integrate the older people into the community.

Abstract:Mental health (MH) differences remain an issue in contemporary Chinese society, with various factors influencing individuals' well‐being. This study investigates the complex connection between results related to psychological wellness and social issues, integrating cultural, psychological and technological perspectives. Through a quantitative research methodology, data were collected from 1000 individuals using a questionnaire survey and analysed using SPSS software. The study's findings shed light on the impact of socioeconomic status (SES) on MH stigma, help‐seeking behaviour and overall well‐being. The findings identify that lower SES was associated with heightened levels of MH stigma and decreased the search for assistance actions. Cultural beliefs emphasising MH stigma and societal expectations were identified as mediators in the connection involve SES and MH outcomes. Digital technology use emerged as a moderator, indicating that higher levels of technology utilisation were linked to reduced disparities in accessing MH resources. The novelty of this study lies in its comprehensive examination of the multifaceted factors influencing MH disparities within Chinese society. Additionally, psychological factors such as resilience and social support were observed to mitigate the negative lower SES's effect on MH consequences. The interaction between SES and cultural factors was found to predict MH outcomes, with lower SES exacerbating the effects of MH stigma and cultural barriers.

Abstract:China's fast economic growth in the past decades is accompanied by a rapid epidemiological transition from communicable to non-communicable diseases (NCDs). An important yet often neglected NCD is mental disorder, which accounts for 14% of global disease burden but has been paid relatively little research attention in China. This paper uses a nationally representative dataset to investigate the prevalence and correlates of depression and depressive symptoms among the adult population in China. Our results indicate that the prevalence rate of depression, estimated with the Center for Epidemiologic Studies Depression Scale (CES-D), is high (37.9% for depressive symptoms and 4.1% for depression) and unevenly distributed across regions and subpopulations. Specifically, women, older people and those who live in the central/western and rural areas are more likely to be depressed. We also find significant socioeconomic gradients in mental health: higher education and income levels are associated with lower likelihood of depression, especially among the lower socioeconomic groups. Our results indicate the urgent need for depression prevention and treatment in China (particularly in the economically less developed regions) through the expansion of primary mental health care resources and a reduction of socioeconomic inequalities.

Abstract:Social capital could protect mental health. We examined whether the COVID-19 context and province-level COVID-19 situation altered the longitudinal association between cognitive social capital (generalized trust, trust in neighbors, trust in local government officials, and reciprocity) and depression. Results from multilevel mixed-effects linear regression models showed that trust in neighbors, trust in local government officials, and reciprocity were more crucial in longitudinally reducing depression in 2020 than in 2018. Also, as compared with provinces where the COVID-19 situation was less poor, trust in local government officials in 2018 was more crucial in reducing depression in 2020 in provinces with a worse COVID-19 situation. Therefore, cognitive social capital should be taken into account for pandemic preparedness and mental health resilience.

Community Environment Perception on Depression: The Mediating Role of Subjective Social Class

Abstract:Depression has become a major social issue of global concern, which has seriously threatened the quality of an individual's life. Although the relationship between community environment and depression has aroused heated debate, the empirical research on the relationship between community environment perception and public depression is still relatively insufficient. Data for this study are from China Family Panel Studies in 2016, which were conducted by the institute of Social Science Survey, Peking University. This paper has tested group differences in the influence of community environment on public depression, as well as the mediating role of subjective social class between community environment perception and depression, so as to further explore the social psychological effect of community environment. The empirical study found that there are group differences in the impacts of community environment perception on depression. Specifically, men, rural residents, and people aged 60 and under are more likely to be depressed which were affected by the perception of community environment. Furthermore, we have found that the subjective social class can partly mediate community environment perception and depression. That is to say, the perception of community environment can induce depression by influencing the individual's subjective social class. Among them, community living environment and community public facilities have the greatest impact, community emotional attachment and community security situation have the second impact, and neighborhood mutual aid and neighborhood relationship have the least impact. In other words, the community environment is deeply endowed with a social psychological effect. To ameliorate the public's depression, it is necessary to consider the construction of community physical environment and the cultivation of harmonious community culture as powerful measures not to be ignored. In short, the important role of community environmental intervention in alleviating the public's depression caused by social class cognition deserves attention.
Understanding and Addressing the Treatment Gap in Mental Healthcare: Economic Perspectives and Evidence from China

Abstract:A common challenge faced by the healthcare systems in many low- and middle-income countries is the substantial unmet mental healthcare needs, or the large gap between the need for and the provision of mental healthcare treatment. This paper investigates the potential causes of this treatment gap from the perspective of economics. Specifically, we hypothesize that people with mental illness face4major hurdles in obtaining appropriate healthcare, namely the high nonmonetary cost due to stigma, the high out-of-pocket payment due to insufficient public funds devoted to mental health, the high time costs due to low mental healthcare resource availability, and the low treatment benefit due to slow technology diffusion. We use China as a study setting to show country-specific evidence. Our analysis supports the above theoretical argument on the 4 barriers to access, which in turn sheds light on the effective approaches to mitigate the treatment gap. Four policy options are then discussed, including an information campaign for mental health awareness, increasing public investment in primary mental healthcare resources, transforming the healthcare system towards an integrated people-centered system and capitalizing on e-health technologies.
SunForum: Understanding Depression in a Chinese Online Community

Abstract:More than 350 million people worldwide suffer from depression. Major depressive disorder has a hugely negative impact on psychological well-being, work, and family life. Yet culture may shape how depressed patients interpret their symptoms, choose treatments, and behave. This paper reports a case study, including participant observations and interviews, of the Chinese online depression community, SunForum. Our findings reveal that Chinese cultural beliefs (e.g., the power of inner self-control) and Chinese beliefs about traditional medicine (e.g., the integrated body-mind relationship) significantly affect patients' understandings of depression, illness management, and social interactions. These beliefs create problems of understanding depression in society - including family members, friends, co-workers, and others - and present various challenges for depressed patients who can become marginalized, suffer discrimination, and lose their jobs. We draw implications for how Chinese society as a whole may respond to the misunderstanding of mental illness and the raising of public awareness. We also propose specific social media design to support depressed patients as they seek online information and social support.
Prevalence and associated factors for depressive symptoms among the general population from 31 provinces in China: The utility of social determinants of health theory

Abstract:Background Depression is one of the most common types of mental disorders. Guided by the theory of social determinants of health (SDH), the study aimed to assess the prevalence of depressive symptoms and to identify factors related to depressive symptoms in the general population of China. Methods A cross-sectional, online survey was conducted among 101,392 residents from 31 provinces of mainland China from January to March 2019, and 97,126 survey responses were included in the final analysis. Multilevel linear regression models were used to identify SDH associated with depressive symptoms. Results The prevalence of depressive symptoms (PHQ-9 scores ≥10) in Chinese residents was 15.81 %. The results of the multilevel analysis demonstrated that depressive symptoms were affected by various factors on five levels, including individual characteristics, behavioral lifestyle, community support network, social structural factors, and macro social factors. Limitations The cross-sectional design of the study makes it difficult to establish causality between variables. Conclusions The prevalence of depressive symptoms is high among general population in China. According to the theory of SDH, the study shows that the depressive symptoms are complex and involves all areas of social life. Therefore, adopting a multi-level, cross-sectoral intervention approach will be instrumental to improving the mental health of residents in China.

Keeping Up with the Wangs: Individual and Contextual Influences on Mental Wellbeing and Depressive Symptoms in China

Abstract:BACKGROUND:In recent decades, China has experienced dramatic changes to its social and economic environment, which has affected the distribution of wellbeing across its citizens. While several studies have investigated individual level predictors of wellbeing in the Chinese population, less research has been done looking at contextual effects. This cross-sectional study looks at the individual and contextual effects of (regional) education, unemployment and marriage (rate) on individual happiness, life satisfaction and depressive symptomatology.METHODS:Data were collected from over 29,000 individuals (aged 18 to 110, 51.91% female) in the China Family Panel Studies, and merged with county level census data obtained from the 2010 China Population Census and Statistical Yearbook. To explore contextual effects, we used multilevel models accounting for the hierarchical structure of the data.RESULTS:We found that a one-year increase in education was associated with a 0.17% increase in happiness and a 0.16% decrease in depressive symptoms. Unemployed men were 1% less happy, 1% less satisfied with life and reported 0.84% more depressive symptoms than employed men while minimal effects were seen for women. Single, divorced and widowed individuals had worse outcomes than married individuals (ranging from 2.96 to 21% differences). We found interaction effects for education and employment. Less educated individuals had greater happiness and less depressive symptoms in counties with higher average education compared to counterparts in less educated counties. In contrast, more educated individuals were less satisfied with life in more educated counties, an effect that is possibly due to social comparison. Employed individuals had lower life satisfaction in areas of high unemployment, while levels were constant for the unemployed. A 1% increase in county marriage rate was associated with 0.33 and 0.24% increases in happiness and life satisfaction respectively, with no interactions. We speculate that this effect could be due to greater social cohesion in the neighbourhood.CONCLUSIONS:Our results show that policies designed to improve employment and marriage rates will be beneficial for all, while interventions to encourage positive social comparison strategies may help to offset the negative effects of increasing neighbourhood average education on the highly educated.

A total of 26,464 individuals were sampled in a weighted manner to represent the entire country. Univariate and multivariate analysis were used to explore the risk factors for depression and the odds of depression in people with different characteristics.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p><strong>:</strong> A total of 76.6% of adult residents were mentally healthy. Females had higher rates of depression than males (OR=1.515). There was an "inverted U-shaped" distribution between age and risk of depression. Higher levels of education and better physiological health were associated with a lower risk of depression. Self-evaluated income and social status were not linearly related to the risk of depression but rather had a "U-shaped" distribution. For social capital, better social relationships and a higher level of social trust were associated with a lower risk of depression. Nonsmokers had a lower risk of depression than smokers (OR=0.818).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Limitations</h3><p><strong>:</strong> All items were self-reported and some results depend on single item responses. This is a cross-sectional survey so that causative interpretations cannot be drawn.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p><strong>:</strong> These findings suggest potential public health measures whose efficacy needs to be tested and confirmed.</p>

Abstract:In recent decades, China’s rapid economic growth has substantially improved average living standards; however, this has not been accompanied by greater happiness among the Chinese population. This phenomenon is known as the Easterlin Paradox (i.e., there is no link between a society’s economic development and its average level of happiness) in Western countries. This study examined the effects of subjective social class on subjective well-being and mental health in China. Consequently, we found that individuals in a relatively low social class had lower levels of subjective well-being and mental health; self-class discrepancy partially explains the relationship between subjective social class and subjective well-being and fully explains the relationship between subjective social class and mental health; and subjective social mobility moderates the path from self-class discrepancy to subjective well-being and mental health. These findings suggest that enhancing social mobility is an important method for reducing class differences in subjective well-being and mental health. These results have important implications, indicating that enhancing social mobility is an important method for reducing class differences in subjective well-being and mental health in China.

The Impact of Psychological Capital and Social Capital on Residents' Mental Health and Happiness During COVID-19: Evidence From China

Abstract:Objective: This paper studies the mediating and interactive effects of social capital on psychological capital and the feeling of happiness from the impact of COVID-19. Since its emergence, the COVID-19 pandemic has taken a toll on people's mental health and affected their hopes for the future. Lifestyle and economic conditions have also been affected and have subsequently impacted people's sense of confidence in life. This could increase the likelihood of many people developing mental health issues, such as anxiety or depression. Therefore, it is vital to study the influence of psychological capital and social capital on people's subjective psychology and happiness experiences. Materials and Methods: Using an ordered probit model, this paper studied the independent influence and interaction between psychological capital and social capital on people's happiness. The ordered probit model was chosen because subjective well-being (SWB) is an ordered variable. We further used structural equation modeling (SEM) to study the mediating effects of social capital on psychological capital and happiness. Results: The regression results showed that both psychological capital and social capital were significantly positively correlated with happiness when controlling for other factors. In addition, psychological and social capital significantly interacted, in which the psychological capital promotes the effect of social capital on happiness. Moreover, the effect of psychological capital on happiness was greater than that of social capital, demonstrating that happiness is more greatly influenced by subjective psychological experience. The interaction coefficient of psychological and social capital was also significant, showing that the two have mutually reinforcing effects on happiness. Finally, health, income class, real estate, stranger trust, age, and urban household registration had significant positive effects on happiness, while the view of money, being female, education had a negative relationship with happiness. The SEM results showed that the mediating effect of psychological capital on happiness was partly transmitted through social capital: the total effect of psychological capital on happiness was highly significant ( p < 0.0001), as was the total effect of social capital on happiness ( p < 0.0001); however, the coefficient for psychological capital was greater than that for social capital. Through heterogeneity analysis, we found that the relationship between psychological capital, social capital, and happiness was significantly positive in each sub-sample group. There was also a significant interaction between psychological and social capital for men, women, urban and rural residents, and higher education background sample groups. However, the interaction was not significant in the sample group without higher education. In addition, the relationship between the happiness of rural residents and their educational background and gender was not significant. Conclusion: We found that psychological and social capital have significant positive relationships and effects on happiness. Psychological capital demonstrated both direct and indirect influences on happiness, and further strengthens the influence of social capital on happiness. These results support a scheme to emphasize psychological support during the COVID-19 pandemic period to enhance the mental health of citizens.
