Abstract:Abstract Background Coronavirus disease 2019 (COVID-19) was a new emerging disease with high infectiousness. Its diagnosis primarily depended on real-time polymerase chain reaction (RT-PCR) results. This study investigated epidemiological, clinical, and radiological characteristics of COVID-19 with negative RT-PCR results before confirmation. Methods Patients with COVID-19 were enrolled and divided into 2 groups: a negative group with negative RT-PCR results before confirmation and a positive group with positive results at the first detection. Epidemiological and clinical features were compared. Dynamic chest computerized tomography (CT) images of the negative group were evaluated. Results Ninety-nine laboratory-confirmed patients with COVID-19 including 8 patients (8%) with negative RT-PCR results were included. Patients from the negative group had similar epidemiological features: the average age (50.25 ± 13.27 years in the negative group and 53.70 ± 16.64 years in the positive group) and gender distribution (males made up 50% of the negative group and 62.6% of the positive group) were comparable. No significant differences were observed in clinical symptoms between the 2 groups. We found that fever was the most common symptom for both groups, followed by cough, expectoration, chest distress, fatigue, and gastroenterological symptoms. Moreover, ground-glass opacities and consolidations were the main manifestation in chest CT of patients with COVID-19 with or without confirmed RT-PCR results. Conclusions Regardless of initial RT-PCR results, patients with COVID-19 had similar epidemiological, clinical, and chest CT features. Our study suggests value from early chest CT scans in COVID-19 screening and dynamic significance of radiology in disease monitoring should guide clinical decisions.

Abstract:OBJECTIVE. Confronting the new coronavirus infection known as coronavirus disease 2019 (COVID-19) is challenging and requires excluding patients with suspected COVID-19 who actually have other diseases. The purpose of this study was to assess the clinical features and CT manifestations of COVID-19 by comparing patients with COVID-19 pneumonia with patients with non-COVID-19 pneumonia who presented at a fever observation department in Shanghai, China. MATERIALS AND METHODS. Patients were retrospectively enrolled in the study from January 19 through February 6, 2020. All patients underwent real-time reverse transcription-polymerase chain reaction (RT-PCR) testing. RESULTS. Eleven patients had RT-PCR test results that were positive for severe acute respiratory syndrome coronavirus 2, whereas 22 patients had negative results. No statistical difference in clinical features was observed (p > 0.05), with the exception of leukocyte and platelet counts (p < 0.05). The mean (± SD) interval between onset of symptoms and admission to the fever observation department was 4.40 ± 2.00 and 5.52 ± 4.00 days for patients with positive and negative RT-PCR test results, respectively. The frequency of opacifications in patients with positive results and patients with negative results, respectively, was as follows: ground-glass opacities (GGOs), 100.0% versus 90.9%; mixed GGO, 63.6% versus 72.7%; and consolidation, 54.5% versus 77.3%. In patients with positive RT-PCR results, GGOs were the most commonly observed opacification (seen in 100.0% of patients) and were predominantly located in the peripheral zone (100.0% of patients), compared with patients with negative results (31.8%) (p = 0.05). The median number of affected lung lobes and segments was higher in patients with positive RT-PCR results than in those with negative RT-PCR results (five vs 3.5 affected lobes and 15 vs nine affected segments; p < 0.05). Although the air bronchogram reticular pattern was more frequently seen in patients with positive results, centrilobular nodules were less frequently seen in patients with positive results. CONCLUSION. At the point during the COVID-19 outbreak when this study was performed, imaging patterns of multifocal, peripheral, pure GGO, mixed GGO, or consolidation with slight predominance in the lower lung and findings of more extensive GGO than consolidation on chest CT scans obtained during the first week of illness were considered findings highly suspicious of COVID-19.

CT scans are considered the best imaging method for early detection and accurate diagnosis of the disease. They are essential for clinical classification, treatment selection, monitoring disease progression, assessing risk factors, and predicting patient prognosis. This article aims to enhance the understanding and diagnostic capabilities of clinical and imaging physicians by reviewing the epidemiology, pathological changes, clinical manifestations, and CT features of COVID-19.

Abstract:Abstract Background: Confirmed cases of coronavirus disease 2019 (COVID-19) is still increasing, detailed analysis of confirmed cases may be beneficial for disease control.Methods: To describe the clinical and radiological findings of patients confirmed with COVID-19 infection in Haikou, China.Results: A total of 67 patients confirmed with COVID-19 infection were included in this study. 50 were imported cases. Most infected patients presented with fever and cough. The typical CT findings of lung lesions were bilateral, multifocal lung lesions (52[78%]), with subpleural distribution, and more than two lobes involved (51[78%]). 54 (81%) patients of COVID-19 pneumonia had ground glass opacities. Consolidation was in 30 (45%) patients, crazy paving pattern or interlobular thickening in 17 (25%), adjacent pleura thickening in 23 (34%) patients. Additionally, baseline chest CT did not reveal positive CT findings in 7 patients (23%), but 3 patients presented unilateral ground glass opacities at follow-up. Importantly, the follow-up CT findings were fitted well with the clinical outcomes.Conclusions: Chest CT could be used as an important tool for early diagnosis of COVID-19, monitoring the disease evolution, judging the treatment effectiveness and predicting the clinical outcomes.

Abstract:Purpose: The main objective of this study was to decipher the general epidemiology, clinical characteristic, laboratory finding and chest computed tomography (CT) imaging features of the novel coronavirus disease (COVID-19) patients whose initial detection of severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2)is negative. Methods: Nearly 100 confirmed cases admitted to The Fifth Affiliated Hospital of Sun Yatsen University from 18th January to 26th February, 2020, were screened. Clinical records, laboratory results and CT images were reviewed for nine COVID-19 patients with initially negative RT-PCR detection. Results: Fever and cough were common, and one patient merely present gastrointestinal symptoms. Increasing CRP and decreasing ALB were showed in nearly half of the patients among negative detection and return to normal level after real time polymerase chain reaction (RT-PCR) results converted from positive to negative. Left lower lobe was affected nearly in all the patients. A patient received oxygen support timely according his high Mulbsta score. Conclusion: Our study elucidated on the clinical features of hospitalized patients with initially negative detection of SARS-CoV nucleic acid. Patient merely with symptoms associated with digestive system should be screened for COVID-19. CT scan and repeated RT-PCR are two powerful diagnostic tools. Mulbsta score assessing in the early stage enhances the confidence of severity evaluation in physician.

Abstract:Purpose: To report CT features of coronavirus disease-2019 (COVID-19) in patients with various disease severity. Methods: The CT manifestations and clinical data of 73 patients with COVID-19 were retrospectively collected in 6 hospitals from Jan 21 to Feb 3, 2020. We analyzed the initial and follow-up CT features of patients with disease severity, according to the Guidelines for the Diagnosis and Treatment of New Coronavirus Pneumonia. Results: Six patients (8%) were diagnosed as mild type pneumonia; these patients had no obvious abnormal CT findings or manifested mild changes of lung infection. All 43 patients (59 %) with common type presented unique or multiple ground-glass opacities (GGO) in the periphery of the lungs, with or without interlobular septal thickening. In the 21 patients (29 %) with severe type, extensive GGO and pulmonary consolidation were found in 16 cases (16/21, 76 %) and 5 cases (24 %), respectively. An extensive "white lung", with atelectasis and pleural effusion were found in critical type patients (3, 4%). On the resolutive phase of the disease, CT abnormalities showed complete resolution, or demonstrated residual linear opacities. Conclusions: Different CT features are seen according to disease severity, which can help COVID-19 stratification.

Abstract:Coronavirus disease 2019 (COVID-19) has rapidly swept across the world. This study aimed to explore the relationship between the chest CT findings and clinical characteristics of COVID-19 patients.Patients with COVID-19 confirmed by next-generation sequencing or RT-PCR who had undergone more than 4 serial chest CT procedures were retrospectively enrolled.This study included 361 patients – 192 men and 169 women. On initial chest CT, more lesions were identified as multiple bilateral lungs lesions and localised in the peripheral lung. The predominant patterns of abnormality were ground-glass opacities (GGO) (28.5%), consolidation (13.0%), nodule (23.0%), fibrous stripes (5.3%) and mixed (30.2%). Severe cases were more common in patients with a mixed pattern (21.1%) and less common in patients with nodules (2.4%). During follow-up CT, the mediumtotal severity score (TSS) in patients with nodules and fibrous strips was significantly lower than that in patients with mixed patterns in all three stages (p < .01).Chest CT plays an important role in diagnosing COVID-19. The CT features may vary by age. Different CT features are not only associated with clinical manifestation but also patient prognosis.The initial chest CT findings of COVID-19 could help us monitor and predict the outcome.Nodules were more common in non severe cases and had a favorable prognosis.The mixed pattern was more common in severe cases and usually had a relatively poor outcome.

Abstract:Background: To study the clinical features and CT manifestations of patients with confirmed 2019-nCoV pneumonia and compare between patients with and without 2019-nCoV infection among all the suspects during the pandemic period.Methods: The patients were enrolled from Jan 20 to Feb 1, 2020, at the fever observation department. All the patients with suspected 2019-nCoV infection were tested by RT-PCR. All the clinical data, laboratory results, imaging findings on CT was retrospectively reviewed.Findings: Seven patients were confirmed 2019nCoV positive while 21 patients were negative. No statistical difference was observed in all the clinical features between these 2 groups (p> 0.05). All the positive patients had pulmonary infiltrations, located predominantly in the peripheral zone compared with negative group (100% vs. 43.75%, p=0.02). The number of infiltrated lung lobes and segments in the positive group were higher than that in the negative group (5/12 vs. 2/4, p=0.00/0.00), and more right lower lung segments were affected in the positive group although no significant difference was observed (71.43% vs. 28.57%, p=0.08). Air bronchogram was more frequent in the positive group (100% vs. 19.50%, p=0.00), and the tree-in-bud sign was less common (14.29% vs. 19.05%, p=0.00).Interpretation: The imaging pattern of multifocal patchy peripheral pure ground glass opacity (GGO), mixed GGO or consolidation predominant in the lower lung, with more extensive GGO than consolidation in CT scans within the first week of illness is highly suspicious of 2019-nCoV infection during the outbreak, which may be crucial for early treatment and prompt isolation.Funding: No funding.Declaration of Interest: The authors of this study declare that they each have no conflict of interest.Ethical Approval: This retrospective study was approved by our Institutional Review Board (KY2020-26). Abstract:BackgroundLittle is known about COVID-19 outside Hubei. The aim of this paper was to describe the clinical characteristics and imaging manifestations of hospitalized patients with confirmed COVID-19 infection in Wenzhou, Zhejiang, China.MethodsIn this retrospective cohort study, 149 RT-PCR confirmed positive patients were consecutively enrolled from January 17th to February 10th, 2020 in three tertiary hospitals of Wenzhou. Outcomes were followed up until Feb 15th, 2020.FindingsA total of 85 patients had Hubei travel/residence history, while another 49 had contact with people from Hubei and 15 had no traceable exposure history to Hubei. Fever, cough and expectoration were the most common symptoms, 14 patients had decreased oxygen saturation, 33 had leukopenia, 53 had lymphopenia, and 82 had elevated C reactive protein. On chest computed tomography, lung segments 6 and 10 were mostly involved. A total of 287 segments presented ground glass opacity, 637 presented mixed opacity and 170 presented consolidation. Lesions were more localized in the peripheral lung with a patchy form. No significant difference was found between patients with or without Hubei exposure history. Seventeen patients had normal CT on admission of these, 12 had negative findings even10 days later.InterpretationMost patients presented with a mild infection in our study. The imaging pattern of multifocal peripheral ground glass or mixed opacity with predominance in the lower lung is highly suspicious of COVID-19 in the first week of disease onset. Nevetheless, some patients can present with a normal chest finding despite testing positive for COVID-19. Funding: We did not receive any fundings.

Abstract:The novel coronavirus 2019 (2019 nCoV), appeared in Wuhan in December 2019, can cause a novel coronavirus pneumonia (Corona Virus Disease 2019, COVID-19). COVID-19 is highly infectious and easy to infect people. The epidemic has gradually spread to all parts of the country. In order to provide a basis for clinical diagnosis, this study retrospectively analyzed the imaging characteristics, evolution and related imaging manifestations of COVID-19 patients in different stages of the disease. The results suggest that the imaging findings of 48 COVID-19 patients from Hengyang, Hunan Province are comparable in different stages of the disease. Chest CT showed no pneumonia in one mild patient. Chest CT findings of moderate type (n=38) and severe type (n=9) had comparable characteristics. The main manifestations were ground-glass opacity (GGO) (18/38, 47.37%; 1/9, 11.11%), and GGO with consolidation (16/38, 42.11%; 5/9, 55.56%), which respectively presented in bilateral lungs (34/38, 89.47%; 9/9, 100.00%), and multi-lobe distribution (involving 5 lobes) (17/38, 44.74%; 8/9, 88.89%). After treatment, 28 patients were isolated for 14 days and returned to the hospital for re-examination; among them, the pulmonary lesion was completely absorbed in 15 moderate patients, while 13 patients mainly manifested as GGO. The CT imaging findings of patients with COVID-19 can detect the lesions early, observe the scope of the lesions, evaluate the severity of the lesions, and assist the clinician in completing rapid isolation, diagnosis and treatment. At the same time, it can help to understand the performance of COVID-19 in different stages and dynamically detect changes in the patient's condition.
Coronavirus Disease 2019 (COVID-19): Chest CT Characteristics Benefit to Early Disease Recognition and Patient Classification—a Single Center Experience

Abstract:Background The current outbreak of coronavirus disease 2019 (COVID-19), epi-centered in Wuhan, Hubei Province of the China, has become a global health emergency. Several studies from China have recently provided the evidence of epidemiological, clinical, laboratory, and outcomes of COVID-19 patients. Investigation on the role of chest CT in patient screening and management course in a large cohort remains paucity. Methods This was a retrospective observational study based on the data collected between January 19 and 2020 to February 15, 2020. A clinic workflow using chest CT and RT-PCR assay to screen suspected patient was reviewed. Clinical data were evaluated and patients were classified to mild, common, severe and critical group. Chest CT characteristics of each patient were evaluated and a CT scoring system was applied to grade the lung involvement. Results Of 98 enrolled patients, 1, 29, 51 and 17 were clinically classified into mild, common, severe and critical group, respectively. Eighty-three patients (84.7%) demonstrated ground-glass opacity (GGO), 76 patients (77.5%) demonstrated consolidation and 18 patients (18.4%) demonstrated crazy-paving pattern on chest CT. Based on the CT scoring, 2, 35, 55 and 6 patients were categorized to grade 0, grade 1, grade 2 and grade 3, respectively, which significantly consistent with clinical classification (kappa =0.638, P﹤0.05). Twenty-nine patients admitted from fever clinic, with an average interval of 1.2 days (range, 0–4 days) between CT examination and onset of symptom. Three of these patients had negative initial RT-PCR result while abnormalities displayed on the initial chest CT. Conclusions Peripheral lung distributed GGO and consolidation, without subpleural sparing, are the most common manifestations on chest CT of COVID-19. Abnormalities on chest CT can occur in an early stage of COVID-19, even when RT-PCR assay negative, which may help to early recognition and rapid diagnosis of this disease.

Abstract:ObjectiveTo investigate the clinical characteristics and CT features of novel coronavirus pneumonia (COVID-19).MethodsThe clinical and CT data of 13 cases of COVID-19 diagnosed by nucleic acid test in our hospital were retrospectively analyzed. The CT features focused on the density, number, distribution, location and morphology of the lesions, as well as pleural thickening, pleural effusion, mediastinal lymph node enlargement or other accompanying signs.ResultsThere were 9 males and 4 females, aged 31-67 years, with an average age of 49±12 years. Common clinical symptoms included fever (8/13), dry cough or cough (3/13), accompanied by diarrhea, nausea and vomiting in 1 case (1/13). Lymphocyte count in peripheral blood decreased in 2 cases (2/13) and CRP increased in 5 cases (5/13). 10 cases (10/12) have multiple lesions, 11 cases (11/12) involve the dorsal and posterior basal segment of the lower lobe bilateral lung. Peripheral distribution (located in subpleural or subpleural area of interlobular fissure) was dominant in 12 cases (12/12), and distribution along the bronchial tree in 10 cases (10/12), There were 10 cases (10/12) with mass and patchy morphology, 9 cases (9/12) with grid shadow. The density was mainly ground-glass opacity lesions (11/12 cases), thickening of the adjacent bronchial bundle was observed in 9 cases (9/12), Pleural thickening or traction deformation (including interlobular fissure distortion) was observed in 8 cases (8/12).ConclusionThe clinical features of COVID-19 are fever and increased CRP. The imaging features is peripheral GGO, and HRCT of the chest can detect the pulmonary changes of COVID-19 patients at an early stage.

Abstract:In this retrospective study, chest CTs of 121 symptomatic patients infected with coronavirus disease-19 (COVID-19) from four centers in China from January 18, 2020 to February 2, 2020 were reviewed for common CT findings in relationship to the time between symptom onset and the initial CT scan (i.e. early, 0-2 days (36 patients), intermediate 3-5 days (33 patients), late 6-12 days (25 patients)). The hallmarks of COVID-19 infection on imaging were bilateral and peripheral ground-glass and consolidative pulmonary opacities. Notably, 20/36 (56%) of early patients had a normal CT. With a longer time after the onset of symptoms, CT findings were more frequent, including consolidation, bilateral and peripheral disease, greater total lung involvement, linear opacities, "crazy-paving" pattern and the "reverse halo" sign. Bilateral lung involvement was observed in 10/36 early patients (28%), 25/33 intermediate patients (76%), and 22/25 late patients (88%).

Abstract:Background: Coronavirus disease 2019 (COVID-19) is highly contagious and has claimed more than one million lives, besides causing hardship and disruptions. The Fleischner Society has recommended chest X-ray (CXR) in detecting cases at high risk of disease progression, for triaging suspected patients with moderate-to-severe illness, and for eliminating false negatives in areas with high pre-test probability or limited resources. Although CXR is less sensitive than real-- time reverse transcription-polymerase chain reaction (RT-PCR) in detecting mild COVID-19, it is nevertheless useful because of equipment portability, low cost and practicality in serial assessments of disease progression among hospitalized patients. Objective: This study aims to review the typical and relatively atypical CXR manifestations of COVID-19 pneumonia in a tertiary care hospital. Methods: The CXRs of 136 COVID-19 patients confirmed through real-time RT-PCR from March to May 2020 were reviewed. A literature search was performed using PubMed. Results: A total of 54 patients had abnormal CXR whilst the others were normal. Typical CXR findings included pulmonary consolidation or ground-glass opacities in a multifocal, bilateral peripheral, or lower zone distribution, whereas atypical CXR features comprised cavitation and pleural effusion. Conclusion: Typical findings of COVID-19 infection in chest computed tomography studies can also be seen in CXR. The presence of atypical features associated with worse disease outcome. Recognition of these features on CXR will improve the accuracy and speed of diagnosing COVID-19 patients.

Abstract:In this retrospective case series, chest CT scans of 21 symptomatic patients from China infected with the 2019 Novel Coronavirus (2019-nCoV) were reviewed with emphasis on identifying and characterizing the most common findings. Typical CT findings included bilateral pulmonary parenchymal ground-glass and consolidative pulmonary opacities, sometimes with a rounded morphology and a peripheral lung distribution. Notably, lung cavitation, discrete pulmonary nodules, pleural effusions, and lymphadenopathy were absent. Follow-up imaging in a subset of patients during the study time window often demonstrated mild or moderate progression of disease as manifested by increasing extent and density of lung opacities.

Abstract:Coronavirus disease 2019 (COVID-19) is a transmissible respiratory disease that was initially reported in Wuhan, China in December 2019. With the alarming levels of COVID-19 spread worldwide, the World Health Organization characterized COVID-19 as a pandemic. Over the past several months, chest CT has played a vital role in early identification, disease severity assessment, and dynamic disease course monitoring of COVID-19. The published data has enriched our knowledge on the etiology, epidemiology, clinical manifestations, and pathologic findings of COVID-19. Additionally, as the imaging spectrum of the disease continues to be defined, extrapulmonary infections or other complications will require further attention. This review aims to provide an updated framework and essential knowledge with which radiologists can better understand COVID-19.

Abstract:In December 2019, an outbreak of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection occurred in Wuhan, Hubei Province, China and spread across China and beyond. On February 12, 2020, WHO officially named the disease caused by the novel coronavirus as Coronavirus Disease 2019 (COVID-19). Since most COVID-19 infected patients were diagnosed with pneumonia and characteristic CT imaging patterns, radiological examinations have become vital in early diagnosis and assessment of disease course. To date, CT findings have been recommended as major evidence for clinical diagnosis of COVID-19 in Hubei, China. This review focuses on the etiology, epidemiology, and clinical symptoms of COVID-19, while highlighting the role of chest CT in prevention and disease control.

Abstract:OBJECTIVE. The purpose of this study was to investigate 62 subjects in Wuhan, China, with laboratory-confirmed coronavirus disease (COVID-19) pneumonia and describe the CT features of this epidemic disease. MATERIALS AND METHODS. A retrospective study of 62 consecutive patients with laboratory-confirmed COVID-19 pneumonia was performed. CT images and clinical data were reviewed. Two thoracic radiologists evaluated the distribution and CT signs of the lesions and also scored the extent of involvement of the CT signs. The Mann-Whitney U test was used to compare lesion distribution and CT scores. The chi-square test was used to compare the CT signs of early-phase versus advanced-phase COVID-19 pneumonia. RESULTS. A total of 62 patients (39 men and 23 women; mean [± SD] age, 52.8 ± 12.2 years; range, 30-77 years) with COVID-19 pneumonia were evaluated. Twenty-four of 30 patients who underwent routine blood tests (80.0%) had a decreased lymphocyte count. Of 27 patients who had their erythrocyte sedimentation rate and high-sensitivity C-reactive protein level assessed, 18 (66.7%) had an increased erythrocyte sedimentation rate, and all 27 (100.0%) had an elevated high-sensitivity C-reactive protein level. Multiple lesions were seen on the initial CT scan of 52 of 62 patients (83.9%). Forty-eight of 62 patients (77.4%) had predominantly peripheral distribution of lesions. The mean CT score for the upper zone (3.0 ± 3.4) was significantly lower than that for the middle (4.5 ± 3.8) and lower (4.5 ± 3.7) zones (p = 0.022 and p = 0.020, respectively), and there was no significant difference in the mean CT score of the middle and lower zones (p = 1.00). The mean CT score for the anterior area (4.4 ± 4.1) was significantly lower than that for the posterior area (7.7 ± 6.3) (p = 0.003). CT findings for the patients were as follows: 25 patients (40.3%) had ground-glass opacities (GGO), 21 (33.9%), consolidation; 39 (62.9%), GGO plus a reticular pattern; 34 (54.8%), vacuolar sign; 28 (45.2%), microvascular dilation sign; 35 (56.5%), fibrotic streaks; 21 (33.9%), a subpleural line; and 33 (53.2%), a subpleural transparent line. With regard to bronchial changes seen on CT, 45 patients (72.6%) had air bronchogram, and 11 (17.7%) had bronchus distortion. In terms of pleural changes, CT showed that 30 patients (48.4%) had pleural thickening, 35 (56.5%) had pleural retraction sign, and six (9.7%) had pleural effusion. Compared with early-phase disease (≤ 7 days after the onset of symptoms), advanced-phase disease (8-14 days after the onset of symptoms) was characterized by significantly increased frequencies of GGO plus a reticular pattern, vacuolar sign, fibrotic streaks, a subpleural line, a subpleural transparent line, air bronchogram, bronchus distortion, and pleural effusion; however, GGO significantly decreased in advanced-phase disease. CONCLUSION. CT examination of patients with COVID-19 pneumonia showed a mixed and diverse pattern with both lung parenchyma and the interstitium involved. Identification of GGO and a single lesion on the initial CT scan suggested early-phase disease. CT signs of aggravation and repair coexisted in advanced-phase disease. Lesions presented with a characteristic multifocal distribution in the middle and lower lung regions and in the posterior lung area. A decreased lymphocyte count and an increased high-sensitivity C-reactive protein level were the most common laboratory findings.
