Abstract:Chest CT has a potential role in the diagnosis, detection of complications, and prognostication of coronavirus disease 2019 (COVID-19). Implementation of appropriate precautionary safety measures, chest CT protocol optimization, and a standardized reporting system based on the pulmonary findings in this disease will enhance the clinical utility of chest CT. However, chest CT examinations may lead to both false-negative and false-positive results. Furthermore, the added value of chest CT in diagnostic decision making is dependent on several dynamic variables, most notably available resources (real-time reverse transcription-polymerase chain reaction [RT-PCR] tests, personal protective equipment, CT scanners, hospital and radiology personnel availability, and isolation room capacity) and the prevalence of both COVID-19 and other diseases with overlapping manifestations at chest CT. Chest CT is valuable to detect both alternative diagnoses and complications of COVID-19 (acute respiratory distress syndrome, pulmonary embolism, and heart failure), while its role for prognostication requires further investigation. The authors describe imaging and managing care of patients with COVID-19, with topics including (a) chest CT protocol, (b) chest CT findings of COVID-19 and its complications, (c) the diagnostic accuracy of chest CT and its role in diagnostic decision making and prognostication, and (d) reporting and communicating chest CT findings. The authors also review other specific topics, including the pathophysiology and clinical manifestations of COVID-19, the World Health Organization case definition, the value of performing RT-PCR tests, and the radiology department and personnel impact related to performing chest CT in COVID-19. ©RSNA, 2020.

Abstract:In the very last days of the year 2019a new virus of the coronavirus family, named Sars-Cov-2 [1] was identified as responsible for the outbreak of cases of human pneumonia in Wuhan, China, a condition named Corona Virus Disease of 2019 (COVID-19). This virus displayed a rapid worldwide spread, with rates significantly higher and with much more severe clinical manifestations than those of seasonal influenza virus. From the initial source in China the virus has progressively spread also to Europe, starting to heavily impact Northern Italy, and later to other European countries and the United States, which by the end of March 2020 has become the country with the largest number of documented cases. Since the virus is new and highly infectious, all humans are potentially susceptible. Moreover, the outbreak took place unexpectedly and in a densely populated Chinese city causing a rapid exponential growth in the number of cases. This led to a huge influx of patients to medical facilities demanding investigation and support for persisting high fever and respiratory compromise. Depending on the local population reaction and the rapidity of local authorities response in combating an infectious diffusive public health threat, different modalities of this epidemic can take place (https://www.youtube.com/watch?v=gxAaO2rsdIs) ([Fig. 1]). Unfortunately, not only in China, but also in Northern Italy, Spain, France and America, just to name some most hit countries, authority reaction and population restrictions were not sufficiently timely to avoid the exponential growth, producing a burst of cases. This has inevitably resulted in overwhelming the capacity of the healthcare systems, in particular hospital provision, and the ability to properly allocate patients to the appropriate clinical units. This has led, particularly in North Italy and Spain, to the risk of collapse of normally comprehensive and excellent health care systems. The sheer volume of sick patients has overrun the capability of the hospitals to provide the expected healthcare, creating a sort of war scenario.

Abstract:Coronavirus disease-2019 (COVID-19) is spreading throughout the world. Chest radiography and computed tomography play an important role in disease diagnosis, differential diagnosis, severity evaluation, prognosis prediction, therapeutic effects assessment and follow-up of patients with COVID-19. In this review, we summarize knowledge of COVID-19 pneumonia that may help improve the abilities of radiologists to diagnose and evaluate this highly infectious disease, which is essential for epidemic control and preventing new outbreaks in the short term.
Chest Radiograph (CXR) Manifestations of the Novel Coronavirus Disease 2019 (COVID-19): A Mini-review

Abstract:Background: Coronavirus disease 2019 (COVID-19) is highly contagious and has claimed more than one million lives, besides causing hardship and disruptions. The Fleischner Society has recommended chest X-ray (CXR) in detecting cases at high risk of disease progression, for triaging suspected patients with moderate-to-severe illness, and for eliminating false negatives in areas with high pre-test probability or limited resources. Although CXR is less sensitive than real-- time reverse transcription-polymerase chain reaction (RT-PCR) in detecting mild COVID-19, it is nevertheless useful because of equipment portability, low cost and practicality in serial assessments of disease progression among hospitalized patients. Objective: This study aims to review the typical and relatively atypical CXR manifestations of COVID-19 pneumonia in a tertiary care hospital. Methods: The CXRs of 136 COVID-19 patients confirmed through real-time RT-PCR from March to May 2020 were reviewed. A literature search was performed using PubMed. Results: A total of 54 patients had abnormal CXR whilst the others were normal. Typical CXR findings included pulmonary consolidation or ground-glass opacities in a multifocal, bilateral peripheral, or lower zone distribution, whereas atypical CXR features comprised cavitation and pleural effusion. Conclusion: Typical findings of COVID-19 infection in chest computed tomography studies can also be seen in CXR. The presence of atypical features associated with worse disease outcome. Recognition of these features on CXR will improve the accuracy and speed of diagnosing COVID-19 patients.

Abstract:With more than 900,000 confirmed cases worldwide and nearly 50,000 deaths during the first three months of 2020, the COVID-19 pandemic has emerged as an unprecedented healthcare crisis. The spread of COVID-19 has been heterogeneous, resulting in some regions having sporadic transmission and relatively few hospitalized patients with COVID-19 and others having community transmission that has led to overwhelming numbers of severe cases. For these regions, healthcare delivery has been disrupted and compromised by critical resource constraints in diagnostic testing, hospital beds, ventilators, and healthcare workers who have fallen ill to the virus exacerbated by shortages of personal protective equipment. While mild cases mimic common upper respiratory viral infections, respiratory dysfunction becomes the principal source of morbidity and mortality as the disease advances. Thoracic imaging with chest radiography (CXR) and computed tomography (CT) are key tools for pulmonary disease diagnosis and management, but their role in the management of COVID-19 has not been considered within the multivariable context of the severity of respiratory disease, pre-test probability, risk factors for disease progression, and critical resource constraints. To address this deficit, a multidisciplinary panel comprised principally of radiologists and pulmonologists from 10 countries with experience managing COVID-19 patients across a spectrum of healthcare environments evaluated the utility of imaging within three scenarios representing varying risk factors, community conditions, and resource constraints. Fourteen key questions, corresponding to 11 decision points within the three scenarios and three additional clinical situations, were rated by the panel based upon the anticipated value of the information that thoracic imaging would be expected to provide. The results were aggregated, resulting in five main and three additional recommendations intended to guide medical practitioners in the use of CXR and CT in the management of COVID-19.

Abstract:Abstract Background Coronavirus disease 2019 (COVID-19) was a new emerging disease with high infectiousness. Its diagnosis primarily depended on real-time polymerase chain reaction (RT-PCR) results. This study investigated epidemiological, clinical, and radiological characteristics of COVID-19 with negative RT-PCR results before confirmation. Methods Patients with COVID-19 were enrolled and divided into 2 groups: a negative group with negative RT-PCR results before confirmation and a positive group with positive results at the first detection. Epidemiological and clinical features were compared. Dynamic chest computerized tomography (CT) images of the negative group were evaluated. Results Ninety-nine laboratory-confirmed patients with COVID-19 including 8 patients (8%) with negative RT-PCR results were included. Patients from the negative group had similar epidemiological features: the average age (50.25 ± 13.27 years in the negative group and 53.70 ± 16.64 years in the positive group) and gender distribution (males made up 50% of the negative group and 62.6% of the positive group) were comparable. No significant differences were observed in clinical symptoms between the 2 groups. We found that fever was the most common symptom for both groups, followed by cough, expectoration, chest distress, fatigue, and gastroenterological symptoms. Moreover, ground-glass opacities and consolidations were the main manifestation in chest CT of patients with COVID-19 with or without confirmed RT-PCR results. Conclusions Regardless of initial RT-PCR results, patients with COVID-19 had similar epidemiological, clinical, and chest CT features. Our study suggests value from early chest CT scans in COVID-19 screening and dynamic significance of radiology in disease monitoring should guide clinical decisions.

Abstract:The global pandemic of coronavirus disease 2019 (COVID-19) has upended the world with over 6.6 million infections and over 391,000 deaths worldwide. Reverse-transcription polymerase chain reaction (RT-PCR) assay is the preferred method of diagnosis of COVID-19 infection. Yet, chest CT is often used in patients with known or suspected COVID-19 due to regional preferences, lack of availability of PCR assays, and false-negative PCR assays, as well as for monitoring of disease progression, complications, and treatment response. The International Atomic Energy Agency (IAEA) organized a webinar to discuss CT practice and protocol optimization from a radiation protection perspective on April 9, 2020, and surveyed participants from five continents. We review important aspects of CT in COVID-19 infection from the justification of its use to specific scan protocols for optimizing radiation dose and diagnostic information.Key Points• Chest CT provides useful information in patients with moderate to severe COVID-19 pneumonia.• When indicated, chest CT in most patients with COVID-19 pneumonia must be performed with non-contrast, low-dose protocol.• Although chest CT has high sensitivity for diagnosis of COVID-19 pneumonia, CT findings are non-specific and overlap with other viral infections including influenza and H1N1.

Abstract:As reported by Feng et al, chest CT has a pivotal role for the diagnosis and assessment of lung involvement in COVID-19 pneumonia (1). Nowadays CT protocols are used to estimate the pulmonary damage (1,2,3). Unfortunately, CT scanning is not available in all emergency departments. Lung US is a surface imaging technique greatly developed in the last decades and strongly recommended for acute respiratory failure (4). It is commonly used in the emergency department at the bedside for early diagnosis of pneumonia. It is a highly sensitive and specific technique considered as an alternative to chest radiography or CT scanning (5,6). We evaluated the role of lung US in patients who presented to our emergency department with COVID-19 pneumonia. Twelve patients (9 male and 3 female, mean age 63±13 years) with flu-like symptoms in the last 4–10 days and COVID-19 infection underwent bedside lung US and CT. Two patients had emphysema but without need of oxygen therapy at home. None of the patients had severe respiratory distress (PaO2/FiO2 257–376 mmHg).

Abstract:This document from the European Society of Radiology (ESR) and the European Society of Thoracic Imaging (ESTI) aims to present the main imaging features, and the role of CT scan in the early diagnosis of COVID-19, describing, in particular, the typical findings which make it possible to identify the disease and distinguish it from bacterial causes of infection, and to define which category of patients may benefit from CT imaging. The precautions that must be taken when performing scans to protect radiologists and technologists from infection will be described. The Value of Digital DR Bedside Chest Radiography and Chest CT Scanning in the Diagnosis of Severe Novel Coronavirus Pneumonia

Abstract:Objective: To investigate the applicability of DR Bedside chest radiography and chest computed tomography (CT) scanning for the imaging and clinical diagnosis of severe novel coronavirus pneumonia. Methods: Imaging and clinical data of 43 patients with novel coronavirus pneumonia who were admitted to Beijing Shijitan Hospital Affiliated to Capital Medical University from December 10, 2022 to January 10, 2023 were retrospectively analyzed. Joint diagnoses by multidisciplinary experts according to the clinical and laboratory diagnosis criteria confirmed that all the 43 enrolled patients were severely infected with novel coronavirus pneumonia. All 43 patients underwent digital DR Bedside chest radiography; of these, 6 patients underwent chest CT scanning as they had relatively mild symptoms. The time interval for chest X-ray reexamination was 1~5 days, with 1~4 reexaminations. The chest CT scan review interval was 1~4 days, with 2 reviews. The imaging findings of chest radiography and chest CT scanning were observed and analyzed. Results: Among 43 patients with severe novel coronavirus pneumonia, 25 cases showed the first digital DR Bedside chest radiographs showing plaques and solid shadows in both lungs, 6 cases of pleural effusion in both lungs, 18 cases of unilateral lung exudation and solid shadows, 4 cases of unilateral pleural effusion, 7 cases of cardiac enlargement combined with pulmonary edema, and 2 cases of suspected pulmonary tumor mass shadows. There were multiple ground glass shadows in both lungs in 6 patients who underwent chest CT scanning. Spot shadows, grid shadows, thickened interlobular septum, and thickened pulmonary vessels and subbronchus were identified in the lesion area. Double lung spot density increased in the chest radiographs; the shadow area expanded in 5 cases and pleural effusion increased in 2 cases. Reexamination of chest CT images showed that 4 cases of increased ground-glass shadow transformed into irregular patchy high CT value shadow, 1 case of new atelectasis and 1 case of new pleural effusion. Conclusion: Digital DR Bedside chest radiography and chest CT scanning are the primary imaging methods for the diagnosis of novel coronavirus pneumonia. In particular, the former plays an important auxiliary diagnostic role when chest CT scan cannot be performed in patients with severe disease, and can greatly assist in the later review and clinical evaluation of the disease during active treatment.

Abstract:In December 2019, a novel coronavirus pneumonia emerged in Wuhan, Hubei, China, and spread rapidly over the world (1). On January 20, 2020, China included the disease as a Class B infectious disease under the Law of the People’s Republic of China on the Prevention and Control of Infectious Diseases, with prevention and control measures for Class A (highest level) infectious diseases (2). On February 11, 2020, the virus which caused the epidemic was officially named as severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) by the International Committee on Taxonomy of Viruses. On the same day, the disease caused by SARS-CoV-2 was named as coronavirus disease 2019 (COVID-19) by the World Health Organization (WHO) (3). In February 2020, the Chinese Society of Radiology issued “a recommendation for Radiological Diagnosis of COVID-19 Pneumonia (first edition)”, in which the radiological examination protocol, imaging findings, staging and evolutions were stated, the essential roles of radiology in the diagnosis (suspected and clinically confirmed cases), patient follow-up and discharge were highlighted (4). Due to its easy availability, chest CT is emerging with significant advantages in implementing “early detection, early reporting, early isolation, early diagnosis, and early treatment” measures of COVID-19 control (5). However, there is still a lack of unified understanding and standards on CT based COVID-19 probability classification nor an objective and quantifiable interpretation of the result. By adopting a series of aggressive prevention and control measures (6-8), the epidemic situation of COVID-19 in Ningbo, Zhejiang Province, China, has been effectively curbed. However, the city epidemic control is now challenged with the overall work resumption and the ongoing immigration (imported) cases. Therefore, aiming to provide a guidance for disease prevention and treatment at various medical institutions, and to generate a standardized radiological diagnosis, this “chest CT based Viral Pneumonia Imaging Reporting and Data System (VP-RADS)” was initiated by radiology experts from the designated hospital for COVID-19 control in Ningbo, i.e., Hwa Mei Hospital, University of Chinese Academy of Sciences, and Ningbo Society of Radiology.

Abstract:In December 2019, an outbreak of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection occurred in Wuhan, Hubei Province, China and spread across China and beyond. On February 12, 2020, WHO officially named the disease caused by the novel coronavirus as Coronavirus Disease 2019 (COVID-19). Since most COVID-19 infected patients were diagnosed with pneumonia and characteristic CT imaging patterns, radiological examinations have become vital in early diagnosis and assessment of disease course. To date, CT findings have been recommended as major evidence for clinical diagnosis of COVID-19 in Hubei, China. This review focuses on the etiology, epidemiology, and clinical symptoms of COVID-19, while highlighting the role of chest CT in prevention and disease control.

Abstract:Coronavirus disease 2019 (COVID-19) is a transmissible respiratory disease that was initially reported in Wuhan, China in December 2019. With the alarming levels of COVID-19 spread worldwide, the World Health Organization characterized COVID-19 as a pandemic. Over the past several months, chest CT has played a vital role in early identification, disease severity assessment, and dynamic disease course monitoring of COVID-19. The published data has enriched our knowledge on the etiology, epidemiology, clinical manifestations, and pathologic findings of COVID-19. Additionally, as the imaging spectrum of the disease continues to be defined, extrapulmonary infections or other complications will require further attention. This review aims to provide an updated framework and essential knowledge with which radiologists can better understand COVID-19.

Abstract:Coronavirus disease 2019 (COVID-19) caused by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is spreading rapidly around the world, resulting in a massive death toll. Lung infection or pneumonia is the common complication of COVID-19, and imaging techniques, especially computed tomography (CT), have played an important role in diagnosis and treatment assessment of the disease. Herein, we review the imaging characteristics and computing models that have been applied for the management of COVID-19. CT, positron emission tomography - CT (PET/CT), lung ultrasound, and magnetic resonance imaging (MRI) have been used for detection, treatment, and follow-up. The quantitative analysis of imaging data using artificial intelligence (AI) is also explored. Our findings indicate that typical imaging characteristics and their changes can play crucial roles in the detection and management of COVID-19. In addition, AI or other quantitative image analysis methods are urgently needed to maximize the value of imaging in the management of COVID-19.
Coronavirus Disease 2019 (COVID-19): Chest CT Characteristics Benefit to Early Disease Recognition and Patient Classification—a Single Center Experience

Abstract:Background The current outbreak of coronavirus disease 2019 (COVID-19), epi-centered in Wuhan, Hubei Province of the China, has become a global health emergency. Several studies from China have recently provided the evidence of epidemiological, clinical, laboratory, and outcomes of COVID-19 patients. Investigation on the role of chest CT in patient screening and management course in a large cohort remains paucity. Methods This was a retrospective observational study based on the data collected between January 19 and 2020 to February 15, 2020. A clinic workflow using chest CT and RT-PCR assay to screen suspected patient was reviewed. Clinical data were evaluated and patients were classified to mild, common, severe and critical group. Chest CT characteristics of each patient were evaluated and a CT scoring system was applied to grade the lung involvement. Results Of 98 enrolled patients, 1, 29, 51 and 17 were clinically classified into mild, common, severe and critical group, respectively. Eighty-three patients (84.7%) demonstrated ground-glass opacity (GGO), 76 patients (77.5%) demonstrated consolidation and 18 patients (18.4%) demonstrated crazy-paving pattern on chest CT. Based on the CT scoring, 2, 35, 55 and 6 patients were categorized to grade 0, grade 1, grade 2 and grade 3, respectively, which significantly consistent with clinical classification (kappa =0.638, P﹤0.05). Twenty-nine patients admitted from fever clinic, with an average interval of 1.2 days (range, 0–4 days) between CT examination and onset of symptom. Three of these patients had negative initial RT-PCR result while abnormalities displayed on the initial chest CT. Conclusions Peripheral lung distributed GGO and consolidation, without subpleural sparing, are the most common manifestations on chest CT of COVID-19. Abnormalities on chest CT can occur in an early stage of COVID-19, even when RT-PCR assay negative, which may help to early recognition and rapid diagnosis of this disease.

Abstract:The ongoing outbreak of Coronavirus Disease 2019 (COVID-19) has spread rapidly throughout China and other countries, and has been characterized as a pandemic. With the strict prevention and control measures implemented by the Chinese government, the spread of the epidemic in China has come under preliminary control by the end of February, 2020, whereas the situation of some countries outside China is not optimistic and raises great public concern. In fighting COVID-19, radiologic examinations played a critical role in the early diagnosis of COVID-19, and could also help assess disease course and severity. There is an urgent need to share useful experience and effective measures internationally. This article outlines the collaborative actions and efforts by the Chinese radiology field and the situation of front-line health care workers in radiology departments to present the world with experiences and examples of Chinese radiology during the COVID-19 outbreak.

Abstract:The novel coronavirus infection, also known as COVID-19, can be detected, diagnosed, and differentiated using chest computed tomography (CT) scans. CT scans are considered the best imaging method for early detection and accurate diagnosis of the disease. They are essential for clinical classification, treatment selection, monitoring disease progression, assessing risk factors, and predicting patient prognosis. This article aims to enhance the understanding and diagnostic capabilities of clinical and imaging physicians by reviewing the epidemiology, pathological changes, clinical manifestations, and CT features of COVID-19.
