Abstract:This study aimed to present the safety and efficiency of in situ laser fenestrated thoracic endovascular aortic repair (f-TEVAR) with distal restrictive covered stent in treating aortic dissections. The retrospective study was conducted of 33 patients who underwent f-TEVAR with distal restrictive covered stent from February 2017 to June 2019. Indications for the distal restrictive covered stent procedure included distal oversizing >20%, radiologic evidence of true lumen compression, long-segment lesions that cannot be covered by a single stent graft, and arch lesions that need to eliminate tortuosity of the proximal stent graft. Technique success, aortic remodeling, and clinical outcomes, especially rate of stent graft-induced new entry (SINE) and spinal cord ischemia, were evaluated. There were 33 patients, including 25 type B and 8 type A aortic dissections, who underwent TEVAR combined with in situ laser fenestration of involved aortic arch branches. The technical success rate was 100%. The 30-day mortality rate was 0%. The taped covered stent has an average length of 59.5 mm (13.6-08.3 mm) from distal end of landing zone to celiac trunk. One-month postoperative computed tomography angiography showed that 90.9% (30 patients) had complete false lumen thrombosis in the segment of aortic coverage, and true lumen diameters were increased in 46.5% ± 14.7% (mean ± standard deviation) and 45.7% ± 16.1% of patients in the proximal stent graft and distal restrictive covered stent segments, respectively. During follow-up (1-21 months; mean, 7.5 ± 5 months), no distal SINE and endoleak were observed. Two cases of spinal cord ischemia were encountered; both patients recovered after conservative treatment, without permanent paraplegia. The f-TEVAR with distal restrictive covered stent is a safe and feasible approach to promote aortic remodeling and to reduce the risk of SINE. Furthermore, the distal restrictive covered stent does not increase the rate of spinal cord ischemia or paraplegia. However, these results need to be confirmed in a larger series of patients with longer follow-up.
