Abstract:Within the last decade, deceased organ donation and transplantation in China have made great progress, but there is still much room for improvement. Inefficient communication with the family of potential organ and tissue donors is one of the major reasons for donor loss. In this study, the current progress of the deceased organ donation program in China was summarized, with a particular focus on the analysis of problems and solutions for achieving best practices in communication with the family of potential organ and tissue donors.This paper is in compliance with the Helsinki Congress and the Declaration of Istanbul.
Investigation of family attitudes toward organ donation after citizen death in China

Abstract:Objective The purpose of this study was to investigate the attitudes trend and characteristics of family members of potential donation after cardiac death(DCD)donors in China.Methods This largest single center cohort study was performed by an interview or by telephone using a questionnaire. The family members of potential DCD donors were recruited from the first affiliated hospital, medical college of Xi'an Jiaotong university located in a metropolitan area of northwest of China. The12-item attitude questionnaire was specifically developed from the literature review and coordinator, physician, and donor's family feedback. The participants were asked to rate the queries on a5-point Likert intensity scale.Results The174 participants included56(32.2%) women and118 (67.8%) men. Most of people were aged between41 and50 years old(n=63,36.2%),31 and40 years old(n=59,33.9%), and less than30 years old(n=36,20.7%). The top five attitudes of participants were the best person to suggest organ donation to a family was ranked as the DCD coordinator of Red Cross Organization(RCO)(n=160, 92%), donor was a hero(n=143,82.2%), honor to be a donor's family member(n=136,78.2%), improved relationship with colleagues(n=124,71.3%), and with recipient after donation(n=12370.7%). The best person to suggest organ donation to a family was ranked as the coordinator of RCO(n=160,92%), doctor unrelated to transplantation(n=104,59.8%), social worker(n=36,20.7%), and doctor related to transplantation(n=25, 14.4%). The top two reasons for non-consent to donation were the family insisted on intact body after patient death, and did not want to have surgery again(n=51,41.5%), and feared that they would be misunderstood by neighbors, relatives and friends about donation(n=28,22.8%).Conclusion This study revealed initial attitudes toward DCD organ donation in China. Some data afford insight to the decision-making procedure. The concerns of potential DCD donors and their families may help professionals provide better interventions in the future.

Abstract:China is experiencing an organ shortage crisis. We experimentally test the effectiveness of the family-priority allocation rule on organ donation and argue that such incentive would be highly motivating in Chinese family-oriented culture. Results of our experiment show that introducing the family-priority rule can not only increase donor registration but also promote family consent. Such priority rule would be particularly effective to increase deceased organ donation in China, as it will significantly promote donor registration and meanwhile generate a consistent higher family consent rate in a more family-orientated culture.

Abstract:Organ donation is recognized as the core part of organ transplantation and its distribution often raises ethical controversy .Organ donor ' s lineal consanguinity priority included in Chinese organ donation principles has extreme ethical values and social benefits .Stating"organ donors and their lineal consanguinity priority"as"family priority principle" is much more accorded with China ' s national conditions .Beginning with the four principles of"benefit, respect and autonomy , no harm, and justice", this paper defended family priority principle in our coun-try , in order to enhance public trust in organ donation distribution system of our country and promote the further de -velopment of organ transplantation .

Abstract:Influenced by traditional culture and lack of knowledge among Chinese, China has a low rate of organ donation after death. The aims of this study were to assess the attitude toward deceased organ donation among medical students in China and to identify the possible factors related with this attitude.An anonymous, self-administered questionnaire comprising 15 questions was distributed to 1500 first-to-fifth grade medical students, with 300 students in each grade.The response rate was 90.9% (n = 1363). The level of knowledge about organ donation and the level of knowledge about the transplantation law were positively related with the seniority (P < .001). The attitude was favorable in 62.7% (n = 855) and against in 37.3% (n = 508) of the respondents. The attitude was related with family discussion about organ donation (P = .002), experience of blood donation (P = .004), willingness to accept a donated deceased organ (P < .001), and the decision of donating the organs of a family member (P < .001). No evidence of relation was detected for age, sex, grade, being an only child, coming from a city or village, knowledge of organ transplantation, knowledge of the transplantation law, and experience of organ donation and transplantation.Most medical students in China had a favorable attitude toward deceased organ donation. This attitude was positively related with family factors, experience of blood donation, and willingness to accept a donated deceased organ. This attitude did not increase with seniority and a rising level of medical knowledge.
Factors Influencing Families' Decision-Making for Organ Donation in Hunan Province, China

Abstract:Background: In 2010, the Chinese government started a pilot project of donation after Chinese citizens' death. Seeking the consent of the families of potential donors is an important task in the organ donation process. Objective: We aimed to explore the factors that influence the families' decision-making for organ donation. Methods: We performed a content analysis of 30 semistructured in-depth interviews with immediate family members who made the decision for an organ donation. The interviewees were asked questions such as how the decision to donate was made, what factors influenced the decision, and whether the decision was rejected by relatives and friends. Results: The interviewees were the donors' parent (15; 50%), spouse (9; 30%), brother (3; 10%) or son (3; 10%), including 18 men and 12 women, with a mean age of 41.6 (standard deviation, 10.7) years; rural households accounted for 83.3%. Through analyzing the interview notes, 17 factors ware mentioned by interviewees, and 3 types of decision-making in organ donors' families were obtained. Then, the factors affecting the decisions of the donor families were classified into 3 categories and 11 subcategories. Conclusions: Factors influencing the families' decision-making for organ donation were analyzed preliminarily in this study.

The top two reasons for non-consent to donation were that the family insisted on intact body after patient death and did not want to have surgery again (n = 51, 41.5%), and feared that they would be misunderstood by neighbors, relatives, and friends about donation (n = 28, 22.8%).CONCLUSIONS:This study revealed initial attitudes toward DCD in China. Some data afford insight into the decision-making procedure. The concerns of potential DCD donors and their families may help professionals provide better interventions in the future.

Abstract:Organ transplantation is the most effective treatment for end stage organ failure,and voluntary donation after citizen′s death is the only source of transplanted organ in China.Clinically,transplantation organ protection technology plays a critical role in improving the quality of transplantation organs and the prognosis of recipients.On the basis of domestic and worldwide basic research and clinical practice and according to the Oxford evidence classification and GRADE system,the experts organized by Organ Transplantation Doctor Branch of Chinese Medical Doctor Association,Organ Transplantation Group in Surgery Branch of Chinese Medical Association and China Liver Transplant Registry Scientific Committee had compiled and published the Chinese Expert Consensus on Organ Protection of Transplantation(2016 edition) for liver,kidney,pancreas,small intestine,heart,lung transplantation organs.With the support of China Liver Transplant Registry,National Center for Healthcare Quality Management in Liver Transplant,National Quality Control Center for Donated Organ Procurement,National Clinical Research Center for Orthopedics & Sports Medicine & Rehabilitation and National Trauma Medical Organ Protection Committee and combined with recent domestic and worldwide clinical practice and research progress for organ transplantation and organ protection, the Chinese Expert Consensus on Organ Protection of Transplantation(2022 edition)(hereinafter referred to as the consensus) has been formulated in 2022.This edition of the consensus focuses on updating the technical progress and evidence-based medicine of organ procurement,organ preservation,organ transport,and quality evaluation in clinical practice.Additionally,protection of composite tissue for transplantation,mainly including limb graft,has also been included.The organ protection strategy recommended in this consensus aims to promote scientific and standardize clinical organ transplantation work.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Objective</h3><p>This systematic review synthesizes qualitative evidence on the experiences of donor families after consent to organ donation.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>This robust, qualitative systematic review included an exhaustive search of electronic databases, including PubMed, Scopus, EBSCO PsycINFO, and CINAHL Complete. Manual searches of reference lists and gray literature were conducted to achieve a comprehensive identification of all relevant research. A qualitative study design served to capture the experiences of donor families after organ donation consent.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>A total of 6 articles that met the eligibility criteria were identified and included in this review. Three key themes emerged from the primary research: 1. family members were ambivalent about consent due to the ambiguity of brain death; 2. conversations about donation requirements proved uncomfortable; and 3. support is needed after donation.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3><p>Family members demonstrated ongoing ambivalence and distress that lasted for weeks after organ donation due to the ambiguity of brain death. Some family members were unhappy with having been approached for a conversation about organ donation. Donor families were not always able to deal with the difficulties they faced after their decision about organ donation. Health care professionals should provide ongoing care and updated information to family members. This review helped to identify family members' needs for both psychological and financial assistance.</p>

Analysis on the Application of Grief Counseling for Relieving Donor Family′s Grief

Abstract:Objective:To explore the effect of grief counseling for relieving donor family′s grief. Methods:From September 2012 to February 2015, 180 families of potential organ donors, who met the class III standard of China, were invited to participate in this study. The grief score was evaluated using questionnaire before and after grief counseling. Results:All of 180 potential organ donor′s families had different level of sadness. The grief was significantly reduced after grief counseling and the score was significantly lower than before ( P<0 . 05 ) . Sixty-five cases agreed to donate organ and 60 cases succeed. Conclusion:Grief counseling for potential organ donor′s families could relieve their grief effectively. This method is beneficial for communication of organ donation and pro-moting donation career of China.
The General Public New Views on Deceased Organ Donation in China

Abstract:The organ donation in China has developed rapidly since fully launched donations after citizens death in 2015. This study was conducted to evaluate how the Chinese general public views changed on deceased organ donation, and to improve the donation process.A total of 110 eligible studies, including 103, 410 individuals, were selected to analyze through searching PubMed, CBMdisc, CQVIP, CNKI, and Wanfang Data from Jan 1, 1990 to May 31, 2019. The pooled proportions (and 95% CIs) of cognition, attitudes and willingness related to organ donation were calculated using the Freeman-Tukey double arcsine transformation.The pooled proportions of knowing about organ donation and willing to donate increased from 84.6% (73.0-93.4) and 32.4% (23.9-41.6) before 2015, to 86.4% (74.5-95.1) and 39.9% (32.8-47.2) after 2015, respectively. The willingness to posthumous organ donation for cornea, heart, kidney, and liver had a significant improvement. Especially, the proportion of willingness to donate cornea increased to 56.0% (43.3-68.3) after 2015, from 39.2% (31.2-47.4) before 2015. However, although 69.7% (62.7-76.4) of participants approved the deceased organ donation, only 35.6% (29.7-41.8) and 43.9% (37.2-50.8) were willing to donate their own and relatives organs postmortem, respectively. The leading reasons for refraining from donating organs postmortem were distrusting the medical professionals (49.8%, 35.2-64.4) and traditional Chinese values (40.6%, 32.4-49.0). Popularizing knowledge about organ donation (61.5%, 45.7-76.1), humanitarian aid (57.1%, 48.8-65.3), and priority of using donated organs for relatives (53.1%, 30.8-74.7) were the applauded strategies to improve the willingness to posthumous organ donation.The willingness toward posthumous organ donation has a significant improvement among Chinese general public since 2015, however, several important measures still need to be taken to promote the favorable attitudes and willingness toward organ donation.

Abstract:Under active exploration and practice of several generations of organ transpant workers,organ transplantation in China has developed rapidly since 2000 and achieved remarkble results.Techniques of transplantation become mature,and liver transplantation and renal transplantation have reached international advanced level,which benefit massive patients.With high attention and promotion of national government and health administrative departments,structural changes have occurred to source of national organ transplantation to form a "China Model".The donation after citizen's death has become the new normal in the organ transplantation in China.Human organ donation work promots the rapid development of transplantation in China,but it also faces challenges of the donor organ shortage and quality improvement.Under the new situation,health,family planning and medical reform work of the national " 13th Five Year Plan" puts forward new requirements on human organ donation and transplantation.Combined with practical work,this article gives a few opinions on how to scientifically further improve the quality and quantity of organ donation and transplantation and directions of future works.
A mixed-methods study of emotional support for families of organ donors in Hunan Province, China

Abstract:Background: Family consent is a prerequisite for the organ donation of the deceased in China. However, a large number of donors are individuals who died due to accidental injuries or unanticipated diseases, which means that most of the families of such donors have just experienced the sudden death of their loved one and have to make a donation decision in a short time. This decision may cause psychological stress and some psychological damage to the minds of relatives of the donors. In addition, cultural sensitivity also has largely caused the relatives of donors inner conflicts and contradictions. And sometimes organ donation may still be stigmatized. However, have they received any emotional support and what is their emotional support needs are some questions that need to be answered. Therefore, this study aims to investigate the emotional support, influencing factors, and needs of the family members of organ donors in Hunan Province, China. Materials and methods: This is mixed-methods research that combines quantitative and qualitative research methods. A cross-sectional survey was conducted among 102 donor families using a questionnaire to investigate their emotional support status. To further understand their emotional support needs, 12 donor families participated in the semi-structured interview. Results: The results confirmed that: (1) A total of 67.7% of the 102 respondents received emotional support or psychological comfort. Thus, only a small number of respondents (31.4%) felt respected by the public. (2) Emotional support came mainly from immediate family members (73.91%), and official organizations such as the Red Cross (43.48%). (3) Marital status, health status, occupation, and coping style can affect the emotional support of the donor families (p < 0.05). (4) Interview showed that the families of donors need emotional support and psychological aid from psychological professionals mostly. And they also wish to receive the understanding and respect of the public. Conclusion: Most families of organ donors received emotional support from family, Red Cross, and friends, but only a minority of families of donors reported receiving respect from the public after the donation. And families of donor showed a strong need for emotional support and professional psychological aid from institutions.

Abstract:Aims and ObjectivesTo explore the acceptances and associated influences of organ donation in mainland China. BackgroundThe shortage of organ donors has limited the development of organ transplantation in China. It is important to recognise the target population who has high intention to donate their organs may change the status. DesignWe conducted a cross-sectional, multi-stage sampling study collected demographic data and individuals' willingness to accept organ donation. MethodsA stepwise linear regression analysis was adopted to evaluate the factors related to the attitudes toward organ donation. ResultsWe collected 11,031 valid samples for the survey. The willingness to donate organs among Chinese residents averaged 56.93 points. To be specific, males (beta = -.03), religious believers (beta = -.01) and parents with a different number of children (all: beta = -.04) are less willing to donate their organs. Respondents who live in an urban area (beta = .03), have higher education level (High school or junior college beta = .04, Bachelor degree or above beta = .09), feel anxious (mild, moderate beta = .02), feel pressured (moderate, severe beta = .08), have higher scores of the Short-Form Health Literacy Instrument (HLS-SF12) (beta = .31), The Self-Management Scale (SHMS) (beta = .16), EuroQol Five Dimensions Questionnaire (EQ-5D) (beta = .04) and EuroQol Visual Analogue Scale (EQ-VAS) (beta = .24), are more positive to donate. ConclusionsThis study firstly discusses the public acceptance of organ donation through a nationwide sample around China. In this study, we discovered that Chinese residents' acceptance level of organ donation and that gender, house, anxiety, pressure, social support and health literacy were the main influencing factors on residents' attitudes. Relevance to Clinical PracticeTo figure out the Chinese public acceptance and its influencing factors of organ donation can help nurse transplant coordinators to recognise the target population and the obstacles of organ donation. Patient or Public ContributionAt the phase of collecting data, participants were recruited to fill the questionnaires.

Abstract:Objective:To discuss the ethical issues of living organ donation in China systematically and comprehensively,from the perspective of society,institution,family,individual and other level.Methods:Literature research and expert interviews were used as main research methods,supplemented by group discussion,in-depth interviews and other methods.Results:In the development of living organ donation in China,from the social level,the relevant laws and regulations,civic education and supporting measures had problems.From the institutional level,hospital human organ transplant ethics committee also had some deficiencies in staff composition,review capacity and supervision.On the individual level,donors and receptors of living organ donation and related medical personnel also had their own ethical dilemma.Conclusion:It is beneficial to fully promote living organ donation and fully realize the purpose of living organ transplantation for the benefit of mankind through solving the ethical issues from social,institutional and individual levels.

Abstract:The shortage of organs is a worldwide challenge for transplantation. To alleviate such organ shortage and keep pace with the world’s development and experience on organ transplantation, the pilot program of organ donation after citizen death (DCD) has been carried out in China, with support and attention from the Chinese government. From 2010, with the joint efforts of the government and medical workers for these years, a series of laws, regulations and related process management have been formulated, and great achievements have been made in DCD work. Currently, the main source of organs in China has come from DCD. However, some difficulties still restrict organ donation rates. Firstly, resistance to organ donation in China is often due to the conventional view among citizens. Secondly, some medical workers do not fully understand the definition and diagnosis criteria of brain death and therefore do not uphold and promote DCD work. Thirdly, the existing laws and institutions for organ transplantation fail to implement and remain defective. Nevertheless, China has made a firm and strong stride in DCD work. In order to carry out DCD work better, Chinese government, people and medical workers have to do much more.

Abstract:Because of altruism and freedom of living organ donation,medical law should strictly regulate living organ donor's right to informed consent,in order to protect their lives and health.In terms of the physician's obligation of notification,we should adopt a compromise between the rational physician and the specific patient standards,and specify the extent of the physician's specific notification.In terms of the expression of patient consent,the ability to identification should be taken as the judgment standard of patients' capacity to consent.Furthermore,the form of informed consent should be standardized,and additional medical excuse system should be established.In addition,the donors have the right to withdraw their consent,but should take the responsibility for the damages to the receivers.

Abstract:Donation after cardiac death (DCD) means organ harvest and donation cannot be carried on until the requirement of death criteria using cardiopulmonary arrest is reached.Great success has been achieved since DCD was promoted widely in China.However,some problems were encountered during the implementation of DCD and there also exists many ethical controversies.The ethical principles of informed consent and no harm mentioned in " Donation after Cardiac Death Work Instructions of China" should be strictly adhered to,while justice,fairness and openness must be ensured in organ allocation.Appropriate compensation should be given to DCD donor and his family,and the utilization of DCD should be further improved.All of these ethical principles will help to promote the healthy development of organ donation and transplantation in China.
The difference in the attitude of Chinese and Japanese college students regarding deceased organ donation

Abstract:Background: Under the influence of traditional oriental culture, the lack of organ donation is especially serious in China and Japan. The aim of this study was to compare Chinese and Japanese college students' attitudes and analyze contributing factors toward deceased donation. Methods: An anonymous, self-administered questionnaire comprising 15 questions was distributed to approximately 400 college students at Liaoning University, China, and Kyushu University, Japan. Statistical analysis used SPSS software. Results: Japanese students' attitude towards deceased organ donation was more favorable than that of Chinese students (43.6% versus 35.9%, P = .001). Several factors contributed to positive responses by students from both countries: family perspective on organ donation and transplantation; decision to donate to family members; prior blood donation; living liver or kidney donation; possibility of needing a transplant; and willingness to receive a deceased or a living donor organ. Conclusions: More efforts must emphasize awareness and up-to-date knowledge regarding organ donation among citizens and should be undertaken by the Chinese and Japanese governments.
