Abstract:Within the last decade, deceased organ donation and transplantation in China have made great progress, but there is still much room for improvement. Inefficient communication with the family of potential organ and tissue donors is one of the major reasons for donor loss. In this study, the current progress of the deceased organ donation program in China was summarized, with a particular focus on the analysis of problems and solutions for achieving best practices in communication with the family of potential organ and tissue donors.This paper is in compliance with the Helsinki Congress and the Declaration of Istanbul.
Factors Influencing Families' Decision-Making for Organ Donation in Hunan Province, China

Abstract:Background: In 2010, the Chinese government started a pilot project of donation after Chinese citizens' death. Seeking the consent of the families of potential donors is an important task in the organ donation process. Objective: We aimed to explore the factors that influence the families' decision-making for organ donation. Methods: We performed a content analysis of 30 semistructured in-depth interviews with immediate family members who made the decision for an organ donation. The interviewees were asked questions such as how the decision to donate was made, what factors influenced the decision, and whether the decision was rejected by relatives and friends. Results: The interviewees were the donors' parent (15; 50%), spouse (9; 30%), brother (3; 10%) or son (3; 10%), including 18 men and 12 women, with a mean age of 41.6 (standard deviation, 10.7) years; rural households accounted for 83.3%. Through analyzing the interview notes, 17 factors ware mentioned by interviewees, and 3 types of decision-making in organ donors' families were obtained. Then, the factors affecting the decisions of the donor families were classified into 3 categories and 11 subcategories. Conclusions: Factors influencing the families' decision-making for organ donation were analyzed preliminarily in this study.

Abstract:Aims and objectives: To explore the acceptances and associated influences of organ donation in mainland China. Background: The shortage of organ donors has limited the development of organ transplantation in China. It is important to recognise the target population who has high intention to donate their organs may change the status. Design: We conducted a cross-sectional, multi-stage sampling study collected demographic data and individuals' willingness to accept organ donation. Methods: A stepwise linear regression analysis was adopted to evaluate the factors related to the attitudes toward organ donation. Results: We collected 11,031 valid samples for the survey. The willingness to donate organs among Chinese residents averaged 56.93 points. To be specific, males (β = -.03), religious believers (β = -.01) and parents with a different number of children (all: β = -.04) are less willing to donate their organs. Respondents who live in an urban area (β = .03), have higher education level (High school or junior college β = .04, Bachelor degree or above β = .09), feel anxious (mild, moderate β = .02), feel pressured (moderate, severe β = .08), have higher scores of the Short-Form Health Literacy Instrument (HLS-SF12) (β = .31), The Self-Management Scale (SHMS) (β = .16), EuroQol Five Dimensions Questionnaire (EQ-5D) (β = .04) and EuroQol Visual Analogue Scale (EQ-VAS) (β = .24), are more positive to donate. Conclusions: This study firstly discusses the public acceptance of organ donation through a nationwide sample around China. In this study, we discovered that Chinese residents' acceptance level of organ donation and that gender, house, anxiety, pressure, social support and health literacy were the main influencing factors on residents' attitudes. Relevance to clinical practice: To figure out the Chinese public acceptance and its influencing factors of organ donation can help nurse transplant coordinators to recognise the target population and the obstacles of organ donation. Patient or public contribution: At the phase of collecting data, participants were recruited to fill the questionnaires.

The General Public New Views on Deceased Organ Donation in China

Abstract:The organ donation in China has developed rapidly since fully launched donations after citizens death in 2015. This study was conducted to evaluate how the Chinese general public views changed on deceased organ donation, and to improve the donation process.A total of 110 eligible studies, including 103, 410 individuals, were selected to analyze through searching PubMed, CBMdisc, CQVIP, CNKI, and Wanfang Data from Jan 1, 1990 to May 31, 2019. The pooled proportions (and 95% CIs) of cognition, attitudes and willingness related to organ donation were calculated using the Freeman-Tukey double arcsine transformation.The pooled proportions of knowing about organ donation and willing to donate increased from 84.6% (73.0-93.4) and 32.4% (23.9-41.6) before 2015, to 86.4% (74.5-95.1) and 39.9% (32.8-47.2) after 2015, respectively. The willingness to posthumous organ donation for cornea, heart, kidney, and liver had a significant improvement. Especially, the proportion of willingness to donate cornea increased to 56.0% (43.3-68.3) after 2015, from 39.2% (31.2-47.4) before 2015. However, although 69.7% (62.7-76.4) of participants approved the deceased organ donation, only 35.6% (29.7-41.8) and 43.9% (37.2-50.8) were willing to donate their own and relatives organs postmortem, respectively. The leading reasons for refraining from donating organs postmortem were distrusting the medical professionals (49.8%, 35.2-64.4) and traditional Chinese values (40.6%, 32.4-49.0). Popularizing knowledge about organ donation (61.5%, 45.7-76.1), humanitarian aid (57.1%, 48.8-65.3), and priority of using donated organs for relatives (53.1%, 30.8-74.7) were the applauded strategies to improve the willingness to posthumous organ donation.The willingness toward posthumous organ donation has a significant improvement among Chinese general public since 2015, however, several important measures still need to be taken to promote the favorable attitudes and willingness toward organ donation.

Abstract:Social support refers to the subjective and (or) objective influence of various social relationships on individuals, which has a certain influence on the negative emotions of the kidney transplant patients. But there are still significant differences among various studies, so we performed a meta-analysis to analyze the social support degree of kidney transplant recipients. This article searched and selected the relevant cross-sectional surveys from PubMed, Embase, VIP, CNKI, Wanfang, and CBM databases according to the inclusion and exclusion criteria and used the STROBE list combined with the observational research quality evaluation tools of Sanderson to conduct the quality appraisal. The "meta" and "metaphor" packages of the R software version 3.5.1 were used for the meta-analysis. A total of 17 studies with 2697 patients were included. The total scores of the social support and objective support of the renal transplant patients were abundant after the operation, indicating that the economic, physical, and emotional supports from the family, society, and the official organization are accepted. But the subjective support and support utilization degree were general. The support utilization was different among different genders, and female patients were lower than the males. In particular, the female patients relatively presented autism and the social support utilization degree was low. Medical staffs are needed to join the family, hospital, and society to create favorable conditions and improve the social support system and the utilization degree of the social support, thereby promoting the physical and mental health development of the patients.
Organ donation: Key factors influencing the younger generation's decision-making in China

Abstract:Background: The organ transplantation sector in China is facing a severe shortage of donors, and the organ donation rate needs to be increased. Since 2015, voluntary donation by citizens has become the only source of organs for transplantation in China. In recent years, there has been a relatively positive change in young people's attitudes toward organ donation after death. The aim of the study was to understand young people's perceptions and attitudes toward organ donation and the factors that influence them and can positively impact the promotion of organ donation. Methods: By analyzing relevant literature and legal texts, we developed a questionnaire. Information was obtained through questionnaires and interviews, and 501 valid questionnaires were returned from the target group. A chi-square test was used to examine whether there were significant differences in the willingness to organ donation among young people with different characteristics. A factor analysis was used to investigate the main factors influencing the different attitudes of young people toward organ donation, and a one-way ANOVA was used to examine whether young people with different characteristics were affected differently by different factors. Results: In our survey of young people aged 18–30 years, 99.2% of respondents knew about organ donation, 47.1% were willing to donate organs, and 15.2% understood that there were corresponding laws and regulations for organ donation. The study's findings showed that urban residents are more willing to be organ donators than rural residents; people with higher education levels have better awareness and are more willing to donate an organ; and people with religious beliefs are more likely to donate organs. The main factors that support the willingness of young people to donate are the social environment that provides support, their optimism in dealing with death, and their desire to realize their final value after death. The main factors for those unwilling to donate were low awareness or misconceptions about organ donation among individuals and their families and their attitudes toward death. As the people who took the questionnaire are probably interested in organ donation, the sample results will show a higher percentage of people who know about organ donation. We hope to discuss further with a larger and broader sample coverage to improve the estimates' validity and reflect the overall picture more accurately in a future study. Conclusion: Young people knew about organ donation but had a low depth of awareness. Household registration type, education level, and religious affiliation significantly correlate with people's willingness to donate. The supportive environment for organ donation in society and the correct understanding of the organ donation process and laws and regulations can influence people's willingness to donate.

Abstract:Influenced by traditional culture and lack of knowledge among Chinese, China has a low rate of organ donation after death. The aims of this study were to assess the attitude toward deceased organ donation among medical students in China and to identify the possible factors related with this attitude.An anonymous, self-administered questionnaire comprising 15 questions was distributed to 1500 first-to-fifth grade medical students, with 300 students in each grade.The response rate was 90.9% (n = 1363). The level of knowledge about organ donation and the level of knowledge about the transplantation law were positively related with the seniority (P < .001). The attitude was favorable in 62.7% (n = 855) and against in 37.3% (n = 508) of the respondents. The attitude was related with family discussion about organ donation (P = .002), experience of blood donation (P = .004), willingness to accept a donated deceased organ (P < .001), and the decision of donating the organs of a family member (P < .001). No evidence of relation was detected for age, sex, grade, being an only child, coming from a city or village, knowledge of organ transplantation, knowledge of the transplantation law, and experience of organ donation and transplantation.Most medical students in China had a favorable attitude toward deceased organ donation. This attitude was positively related with family factors, experience of blood donation, and willingness to accept a donated deceased organ. This attitude did not increase with seniority and a rising level of medical knowledge.

Abstract:In March 2010, China launched a pilot programme of deceased donor organ donation in 10 provinces and cities. However, the deceased donor donation rate in China remains significantly lower than in Spain and other Western countries. In order to provide incentive for deceased donor organ donation, five pilot provinces and cities have subsequently launched a financial compensation policy. Financial compensation can be considered to include two main forms, the 'thank you' form and the 'help' form. The 'thank you' form is an expression of gratitude on behalf of the Red Cross Society of China for consenting to donation. The 'help' form is social welfare support for needy families.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Objective</h3><p>This systematic review synthesizes qualitative evidence on the experiences of donor families after consent to organ donation.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>This robust, qualitative systematic review included an exhaustive search of electronic databases, including PubMed, Scopus, EBSCO PsycINFO, and CINAHL Complete. Manual searches of reference lists and gray literature were conducted to achieve a comprehensive identification of all relevant research. A qualitative study design served to capture the experiences of donor families after organ donation consent.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>A total of 6 articles that met the eligibility criteria were identified and included in this review. Three key themes emerged from the primary research: 1. family members were ambivalent about consent due to the ambiguity of brain death; 2. conversations about donation requirements proved uncomfortable; and 3. support is needed after donation.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3><p>Family members demonstrated ongoing ambivalence and distress that lasted for weeks after organ donation due to the ambiguity of brain death. Some family members were unhappy with having been approached for a conversation about organ donation. Donor families were not always able to deal with the difficulties they faced after their decision about organ donation. Health care professionals should provide ongoing care and updated information to family members. This review helped to identify family members' needs for both psychological and financial assistance.</p>

Abstract:Abstract Background The purpose of this study is to assess the level of knowledge, attitudes, and willingness to organ donation among the general public in China. Methods The study population consisted of 4274 participants from Eastern, Central and Western China. The participants’ knowledge, attitudes and willingness to organ donation were collected by a self-designed questionnaire consisting of 30 items. Knowledge is measured by 10 items and presented as a 10 point score, attitudes is measured by 20 items using a 5-step Likert scale and total score ranged between 0 and 80; while the willingness to donate is assessed as binary variable (0 = No; 1 = Yes). A logistic regression model was used to assess the association of knowledge and attitudes with willingness to organ donation, controlling for demographic and socioeconomic confounders. Results The questionnaire response rate was 94.98%. The mean score (± SD) of the general public’s knowledge to organ donation was 6.84 ± 1.76, and the mean score (± SD) of attitudes to organ donation was 47.01 ± 9.07. The general public’s knowledge and attitudes were the highest in Eastern China, followed by West and Central China. The logistic regression model indicated a positive association between knowledge and the willingness to organ donation (OR = 1.12, 95%CI: 1.08, 1.17; P < 0.001); attitudes were also positively potential determinant of more willingness to organ donation (OR = 1.08, 95%CI: 1.07, 1.09; P < 0.001). Conclusions Knowledge and attitudes were found to be positively associated with the Chinese general public’s willingness to organ donation. Knowledge about the concept of brain death and the transplant procedure may help raise the rate of willingness to organ donation.

Abstract:Background: The prevalence of antibodies for hepatitis A virus (anti‐HAV) in adolescents and young adults has decreased remarkably following the economic growth in Korea. As a result, this age group has a high risk for HAV infection paradoxically, and over 1500 cases of clinically overt hepatitis A occurred in 1998. Human isolates of hepatitis A virus (HAV) are categorized within four genotypes (I, II, III, and VII). In some geographic regions, closely related isolates cluster, suggesting endemic spread of the virus, while in other regions multiple genotypes circulate. Virtually no data are available with regard to the genetic relatedness of Korean strains of HAV.

Abstract:Objectives: The purpose of this study was to understand the knowledge, attitudes, and willingness of medical students in their clinical internships towards organ donation and to assess how traditional oriental culture plays a role. Methods: This study conducted an online cross-sectional survey using a cell phone application with a self-designed questionnaire based on the Delphi method. This study was conducted among medical students in their internships in hospitals in nearly 20 provinces of China. The questionnaire consisted of 17 main questions and a knowledge quiz. The results were analyzed with descriptive statistics using the software Microsoft Excel 2021 and SPSS 26. Results: A total of 225 medical students participated in this study, distributed among college, undergraduate, and postgraduate clinical internships. 96% of the medical students agreed with the value of organ donation, but the highest willingness to donate the whole body was only 38.22%. Among them, traditional culture, family values, and religious factors played important both positive and negative roles. Conclusion: Medical students have better knowledge about organ donation, but it rarely comes from professional education. Traditional concepts, family values, and religious factors play both a facilitating and hindering role, which can easily lead to moral dilemmas. Although it is not possible to make productive changes in the short term, we could still make improvements through educational changes, research breakthroughs, and institutional improvements.

Abstract:The shortage of organs is a worldwide challenge for transplantation. To alleviate such organ shortage and keep pace with the world’s development and experience on organ transplantation, the pilot program of organ donation after citizen death (DCD) has been carried out in China, with support and attention from the Chinese government. From 2010, with the joint efforts of the government and medical workers for these years, a series of laws, regulations and related process management have been formulated, and great achievements have been made in DCD work. Currently, the main source of organs in China has come from DCD. However, some difficulties still restrict organ donation rates. Firstly, resistance to organ donation in China is often due to the conventional view among citizens. Secondly, some medical workers do not fully understand the definition and diagnosis criteria of brain death and therefore do not uphold and promote DCD work. Thirdly, the existing laws and institutions for organ transplantation fail to implement and remain defective. Nevertheless, China has made a firm and strong stride in DCD work. In order to carry out DCD work better, Chinese government, people and medical workers have to do much more.

Abstract:In recent years, China has been facing a shortage of cadavers for teaching medical students. A better comprehension of the attitudes and factors influencing the general public's opinion toward body donation would be invaluable for planning and implementing body donation programs. Although altruistic attitude and attitudes toward death have received considerable attention in recent years globally, they have been largely understudied in China. Herein, the potential relationship between attitudes toward altruism and death, and willingness toward whole‐body donation in a sample of university students in Changsha City in China were analyzed. A multi‐stage sampling method was adopted to recruit 478 Chinese college students from two universities: the Medical College of Hunan Normal University (n = 272) and the College of Civil engineering of Hunan University (n = 206). The study participants were assessed by a sociodemographic questionnaire, the Death Attitude Profile‐Revised (DAP‐R‐C) questionnaire, and the altruism scale. Moreover, Chinese University students demonstrated moderate willingness to donate their bodies. The mean score of the willingness of study participants to donate their bodies was 3.138 ± 0.933 (5‐point Likert scale). Acceptance toward death, gender, and type of university were positive factors, whereas fear of death negatively affected willingness toward body donation. Regression analysis showed that factors, including gender (β = 0.237), type of university (β = 0.193), natural acceptance (β = 0.177), and fear of death (β = −0.160) influenced willingness toward body donation. Overall, the present study provides hitherto undocumented evidence on factors influencing willingness toward body donation among Chinese university students, which can assist in designing public awareness programs for encouraging body donation.

Abstract:Body donation has far-reaching significance for modern medical research and education. However, body donation in China lags far behind the demand. To assess the perception and attitude toward body donation, a survey of 2535 community residents was conducted in Changsha. The result showed that 89.5% of the respondents have heard about body donation through different sources, such as public media, medical college, and hospital. However, 61.8% of the respondents have limited knowledge of these body donation programs. The majority of respondents believed that body donation would contribute to researches in neuroanatomy, tumor biology, and ophthalmology, as well as anatomical education for medical students, and surgical training for clinicians/surgeons. Regarding the public's willingness to donate, 27.5% of respondents expressed a clear willingness. Further analysis revealed that people aged above 60 are less willing to donate. Compared with people having Confucianism funeral belief, those without the belief were 9.8 times more willing to donate. Furthermore, it was shown that respondents who had a good understanding of body donation were more willing to donate their bodies. Moreover, people thought body donation was beneficial to medical research and education were almost 10 times more willing to donate compared to those who thought it had no benefit. To promote body donation in China, greater efforts need to be made in promoting body donor programs and so increasing the public's perception toward body donation. Moreover, re-assessing and re-interpreting Confucianism beliefs regarding body donation also needs to be considered for future promotion of body donation in China and other East Asian countries.
The difference in the attitude of Chinese and Japanese college students regarding deceased organ donation

Abstract:Background: Under the influence of traditional oriental culture, the lack of organ donation is especially serious in China and Japan. The aim of this study was to compare Chinese and Japanese college students' attitudes and analyze contributing factors toward deceased donation. Methods: An anonymous, self-administered questionnaire comprising 15 questions was distributed to approximately 400 college students at Liaoning University, China, and Kyushu University, Japan. Statistical analysis used SPSS software. Results: Japanese students' attitude towards deceased organ donation was more favorable than that of Chinese students (43.6% versus 35.9%, P = .001). Several factors contributed to positive responses by students from both countries: family perspective on organ donation and transplantation; decision to donate to family members; prior blood donation; living liver or kidney donation; possibility of needing a transplant; and willingness to receive a deceased or a living donor organ. Conclusions: More efforts must emphasize awareness and up-to-date knowledge regarding organ donation among citizens and should be undertaken by the Chinese and Japanese governments.
Mediating effects of attitude on the relationship between knowledge and willingness to organ donation among nursing students

Abstract:Background: The current rate of organ donation in China falls significantly below the global average and the actual demand. Nursing students play a crucial role in supporting and promoting social and public welfare activities. This study primary aims to analyze the levels of knowledge, attitudes, willingness toward organ donation, and attitudes toward death among nursing students, and investigate the mediating role of attitude in the relationship between knowledge and willingness. The secondary aims to identify factors that may influence the willingness. Methods: A convenience sample of nursing students completed online-administered questionnaires measuring the level of knowledge, attitudes, and willingness toward organ donation before and after clinical internship. Spearman correlation and mediation analyses were used for data analyses. Results: Before the clinical internship, there were 435 nursing students who had not yet obtained their degrees and were completing their clinical internships. After the internship, this number decreased to 323. The mean score for knowledge before and after the clinical internship (7.17 before and 7.22 after, with no significant difference), the attitude (4.58 before and 4.36 after, with significant difference), the willingness (12.41% before and 8.67% after, with significant difference), the Death Attitude Profile-Revised (DAP-R) score (94.41 before and 92.56 after, with significant difference). The knowledge indirectly affected nursing students' willingness to organ donation through attitude. Knowledge had a direct and positive impact on attitudes ( β = 1.564). Additionally, nursing students' attitudes positively affected their willingness ( β = 0.023). Attitudes played a mediating role in the relationship between knowledge and willingness ( β = 0.035). Additionally, attitude toward death, fear of death, and acceptance of the concept of escape were found to be correlated with their willingness. Conclusion: Organ donation willingness was found to be low among nursing students. Positive attitudes were identified as a mediating factor between knowledge and willingness. Additionally, DAP-R was a related factor. Therefore, it is recommended to focus on improving knowledge and attitude, as well as providing death education to help nursing students establish a positive attitude toward death. These efforts can contribute to the promotion of organ donation.

Abstract:Objective To investigate the cognition towards organ donation,kidney transplantation and willingness to donate in driving license holders in China.Methods From January 2013 to March 2013,25 000 questionnaires were distributed to 25 transplant centers in China,which were the member units of the Kidney Transplantation Group of Chinese Urological Association.The survey on local driving license holders was performed by each center randomly.The questions in the questionnaire included demographic characteristics of respondents as well as their understanding of kidney transplantation and organ donation and personal willingness.All the questionnaires were collected to Peking University Third Hospital for final statistics and the results were analyzed by epidemiology specialists.Results Total 25 000 questionnaires were distributed and 12 807 collected (51.2％).The effective response rate of questionnaires was 95.3％ (12 205/12 807).74.4％ respondents supported voluntary organ donation.64.0％ advocated organ donation after death and 48.4％ wanted to be volunteers of organ donation.Conclusions Although most of the interviewee accept organ donation,less than half wish to be a volunteer of organ donation.The reasons for this are various and more propaganda will be needed to promote the voluntary organ donation in China.
