Abstract:Objective:To assess the efficacy of two tocolytic agents, ritodrine hydrochloride and atosiban, for inhibiting preterm labour.Methods:The clinical data of 284 patients diagnosed with preterm labour, hospitalised and delivered in Women′s Hospital School of Medicine Zhejiang University from December 2018 to December 2020 were retrospectively reviewed.They were divided into three groups which are the intravenous ritodrine hydrochloride group, the atosiban group, and the alternative group according to the use of tocolysis.Screening for statistically significant outcome indicators by group comparison and controlling the confounding factors by Logistic regression, the maternal and neonatal outcomes were compared among three groups and quantitatively assessmented by corrected OR.Results:（1）Atosiban（OR 7.932,95%CI 1.722-36.526）group and alternative（OR 4.054,95%CI 1.649-9.965）group showed higher risks of giving birth before 28 weeks than ritodrine hydrochloride group（P＜0.01）.（2）Compared to ritodrine hydrochloride group, atosiban（HR 5.942,95%CI 1.736-20.335）group and alternative（HR 3.030,95%CI 1.565-5.865）group had less increase in the mean gestational age at delivery; the adverse drug reactions in atosiban group were less than those in ritodrine hydrochloride group and alternative group（P＜0.01）.（3）No apparent differences were showed between ritodrine hydrochloride and other two groups in live birth rate（P＞0.05）.（4）There were also no significant differences between ritodrine hydrochloride group and atosiban group in infant admission to neonatal intensive care unit（NICU）,neonatal respiratory distress syndrome（NRDS）,apnea, or need for mechanical ventilation.While there were higher event rates for these risks in alternative group compared with ritodrine hydrochloride group（P＜0.05）,OR=2.302（95%CI 1.149-4.612）,5.569（95%CI 2.380-13.033）,6.487（95%CI 2.564-16.409）,6.839（95%CI 2.896-16.152）.Conclusions:Monotherapy with ritodrine hydrochloride was more effective than monotherapy with atosiban and alternative therapy with these two medications for inhibiting preterm labour and prolonging the duration of pregnancy.However, atosiban had less adverse drug reactions compared with ritodrine hydrochloride.Suitable use of medications should be explored by further randomized controlled studies.

Abstract:Preterm labor, defined as contractions occurring every 5–10 minutes with cervical changes before the 37th week of estimated gestation, is a significant cause of perinatal mortality. This meta-analysis aims to evaluate the effectiveness and safety of magnesium sulfate compared to nifedipine in managing preterm labor. The systematic review protocol was registered with PROSPERO (CRD42023422419). Manuscripts published up to September 2024 were systematically searched in databases. Dichotomous data were pooled as odds ratios using a random-effects model with the Mantel-Haenszel method, while continuous data were analyzed as mean ± standard deviation values, mean differences, and standardized mean differences using inverse-variance fixed-effects analysis. Fifteen randomized clinical trials (RCTs) were included, enrolling a total of 2,186 pregnant women. Nifedipine showed a greater effect compared to a 4-gram IV dose of magnesium sulfate. However, the efficacy of these tocolytics in prolonging pregnancy by 48 hours did not significantly differ between nifedipine and a 6-gram IV dose of magnesium sulfate. Additionally, magnesium sulfate was associated with more adverse drug reactions. The moderate certainty of evidence found here requires confirmation in large, adequately powered RCTs.

Meta-analysis of nifedipine for inhibiting preterm labor

Abstract:OBJECTIVE To evaluate the curative effect and safety of nifedipine for inhibiting preterm labor.METHODS The relevant data bases were searched.The quality evaluation of studies and the data analysis followed the methods of the Cochrane Collaboration.Meta-analysis was performed by RevMan 5.0.23 software.RESULTS Eighteen studies were included.The meta-analysis showed: ①nifedipine was superior to basic treatments in No.of prolonging pregnancy more than 48 h/72 h/7 d,neonatal weight and diseases,but inferior in adverse events in pregnant women.②Compared with magnesium sulfate,nifedipine had shorter effectual time(RR-0.48,95%CI-0.88 to-0.08),higher tocolysis rate(RR 1.12,95%CI 1.01 to 1.25) and less adverse events in pregnant women(RR0.58,95%CI 0.46 to 0.73).③Compared with β2 adrenergic agonists,nifedipine had less adverse events in pregnant women(RR 0.37,95%CI 0.27 to 0.50).④nifedipine had shorter effectual time but higher adverse events rate than atosiban.CONCLUSION Nifedipine was superior to basic treatments in tocolysis rate,neonatal weight and diseases,but inferior in adverse events in pregnant women.Compared with other tocolytics,nifeidpine had shorter effectual time than magnesium sulfate and atosiban,higher tocolysis rate than magnesium sulfate,lower adverse events rate than magnesium sulfate and β2 adrenergic agonists,but higher adverse events rate than atosiban.
Effects of intrathecal magnesium sulfate in severe pre- eclampsia patients undergoing cesarean section

Abstract:AIM To study the effects of intrathecal magnesium sulfate on anesthesia in severe pre-eclampsia patients undergoing cesarean section. METHODS Sixty severe pre- eclampsia primiparas undergoing caesarean section were included and randomly divided into two groups(n = 30). The patients in the magnesium group were received spinal anesthesia with 0.5% bupivacaine 2 m L + sufentanil 2.5 μg + magnesium sulfate 50 mg and the other patients in the control group received bupivacaine and sufentanil only. Intra- operative pain was rescued by intravenous fentanyl 1 μg·kg-1. Postoperative pain was rescued by patient control intravenous analgesia(PCIA). Onset, duration of sensory and motor block, duration of spinal anesthesia and postoperative analgesia requirements were studied. The adverse drug reactions and the Apgar score of infants were observed.RESULTS The duration of spinal anesthesia((233.4 ± 17.2) min vs.(192.4 ± 12.4) min) and motor block((204.8 ± 20.1) min vs.(173.6 ± 17.5) min) were significantly longer in the magnesium group compared with the control group(P < 0.05). The VAS at 4 h time point after cesarean delivery in the magnesium group was significantly lower than that in the control group(P < 0.05). Fentanyl requirement for 24 h following surgery was significantly lower in the magnesium group than in the control group(( 157.3 ± 45.5) μg vs.( 243.5 ±48.7) μg, P < 0.05). The incidence rate of adverse drug reactions, such as hypotension, bradycardia,nausea, vomiting and pruritus, was similar between the two groups(P > 0.05). The Apgar score at 1 min and5 min after babies’ delivery was above seven points in the two groups. CONCLUSION In parturients with severe pre- eclampsia undergoing caesarean delivery, the addition of intrathecal magnesium sulfate to bupivacaine and sufentanil prolongs the duration of analgesia and reduces postoperative analgesic requirements without additional adverse drug reactions.

Abstract:Aim: To compare the efficacy and safety of magnesium sulphate and oral nifedipine in acute tocolysis of preterm labor. Study design: Randomized controlled trial. Place and duration of study: Department of Obstetrics & Gynecology, DHQ Hospital, Mirpur from 19th October 2019 to 18th April 2020. Methodology: This study included 178 pregnant women with singletons (18 to 40 years of age) assessed through ultrasonography and presented preterm labor. Pregnant women having fetal growth restriction, fetal death, severe preeclampsia, fetal distress, hyperthyroidism,any maternal contraindication for the use of tocolytic drugs, cardiovascular disease, abruptio placentae or placenta-previa were placed in the seclusion criteria of the study. In the group A, oral loading dose of nifedipine 20mg was given. While in group B, magnesium sulphate was injected intravenously with a loading dose of 4 grams over 15 minutes followed by maintenance dose of 2-3 grams/hr. All patients in both groups were evaluated by the researcher herself for prolongation of pregnancy at 48 hours after the start of treatment and efficacy and safety was noted. Results: Efficacy was seen in 80 (89.89%) in group A (Oral nifedipine) and 67 (75.28%) in group B (magnesium sulfate) with p0.019. Safety was seen in 72 (80.90%) in group B (magnesium sulfate) and 83 (93.26%) in group A (oral nifedipine) with p 0.014. Practical Implication: The administration of corticosteroids and also assist in reducing mortality and morbidity related with preterm labor events. Conclusion: This study concluded that oral nifedipine is efficacious and safe than magnesium sulphate for acute tocolysis of preterm labour. Abstract:Background: Hypertensive disorders of pregnancy are the most frequently occurring medical condition during pregnancy, resulting in fetal and/or maternal morbidity and mortality. This meta-analysis compared the efficacy and safety of nifedipine with other antihypertensive medications used in hypertensive disorders of pregnancy. Methodology: A comprehensive search was performed using PubMed, Cochrane Central Register of Controlled Trials (CENTRAL), and Google Scholar. The meta-analysis was carried out using Review Manager Software, and the pooled effect estimate was generated as standardized mean difference and odds ratio with 95% confidence interval and two-sided P -value. Results: The meta-analysis was comprised of 22 randomized control trials with 2595 participants. It was found that meantime and number of doses required to achieve target blood pressure were lower in the nifedipine group ( P < 0.05). Even though it is statistically insignificant, fetal APGAR (Appearance, Pulse, Grimace, Activity, and Respiration) scores less than seven favors nifedipine intervention. Furthermore, none of the fetal or maternal secondary outcomes were found significant. Conclusion: Nifedipine was found to be more effective than other antihypertensive medications to reduce blood pressure, particularly in patients with severe hypertension. However, future clinical studies, including real-world data are necessary to establish the safety profile of nifedipine concerning the fetal outcomes in hypertensive pregnant women.

Abstract:Background: Preterm labor poses significant challenges to obstetric practice, with preterm births contributing to a substantial portion of infant morbidity and mortality worldwide. Although various pharmacological agents are employed to manage preterm labor, the effectiveness of different therapeutic combinations remains a topic of ongoing research. Calcium channel blockers like nifedipine are increasingly favored for their efficacy in reducing uterine contractions, while progestins have been shown to reduce the recurrence of preterm births. Objective: The objective of this study was to compare the efficacy of nifedipine monotherapy versus the combination of nifedipine with progesterone depot in the management of preterm labor, evaluating the cessation of uterine contractions within 48 hours as the primary outcome. Methods: This randomized controlled trial was conducted at the Department of Obstetrics & Gynecology, Unit III, at Ganga Ram Hospital, Lahore, with a sample size of 92 patients experiencing preterm labor between 28 and 36 weeks of gestation. Patients were randomly assigned to Group A (nifedipine monotherapy) or Group B (nifedipine plus progesterone depot). Group A received an initial dose of 20 mg nifedipine followed by 20 mg twice daily for two days. Group B received the same nifedipine regimen with a single intramuscular injection of 250 mg of 17-alpha-hydroxyprogesterone caproate. The efficacy of the treatment was defined as the cessation of uterine contractions within 48 hours. Data were analyzed using SPSS version 25, with significance set at p ≤ 0.05. Results: The study found that 71.74% (33/46) of patients in Group B achieved cessation of uterine contractions within 48 hours compared to 47.83% (22/46) in Group A. This difference was statistically significant (p = 0.019), indicating that the addition of progesterone depot to nifedipine enhanced its effectiveness in managing preterm labor. Conclusion: The combination of nifedipine with progesterone depot proved significantly more effective than nifedipine alone in the cessation of uterine contractions associated with preterm labor. This combined therapy approach presents a promising strategy for improving maternal and neonatal outcomes in the management of preterm labor
Four kinds of tocolytic therapy for preterm delivery: Systematic review and network meta-analysis

Abstract:What is known and objective: Premature birth affects more than 15 million infants, as well as mothers and families around the world. With the relaxation of the two-child policy, the problem of premature birth has become relatively prominent in China. According to statistics, China had a birth population of 15.23 million in 2018, with a considerably large number of premature births. This study aims to evaluate the efficacy and safety of tocolysis in the treatment of preterm delivery, provide clinical evidence for medical staff and promote the self-management of patients with premature births. Methods: Four English databases (PubMed, Embase, Cochrane Library and Web of Science) were retrieved by computer, the retrieval time was from the establishment of each database to November 2021, and the randomized controlled trials for the treatment of preterm delivery were screened according to the pre-set natriuretic exclusion criteria. After literature screening, data selection and risk of bias evaluation were independently conducted by two researchers. R 4.1.1 and Stata 17.0 software were used for statistical analysis. Results and discussion: A total of 44 RCTs were included, including 6939 patients. The results of network meta-analysis reveal that in terms of effectiveness, indomethacin was the most effective intervention measure, followed by nifedipine, and the difference was statistically significant; regarding safety, nifedipine was the safest intervention measure, followed by indomethacin, and the difference was statistically significant; and in respect of adverse reactions, ritodrine had the highest probability, and the difference was statistically significant. What is new and conclusion: Nifedipine may be better for delayed delivery and less likely to produce adverse pregnancy outcomes, followed by indomethacin. Limited by the number and quality of recipient studies, the aforementioned conclusions need to be verified through more high-quality studies. At the same time, the focus should be on patients with twin pregnancy and patients with clinical manifestations of extreme preterm delivery.

Abstract:To investigate the treatment of preterm labor with the beta-adrenergic agonist ritodrine.126 women with preterm labor were randomly assigned to intravenous infusion of ritodrine or magnesium sulfate treatment. Using minimal effective dosage for inhibition of uterine activity, terminating therapy on time, and repeating the i.v. infusion if contraction restarted were the strategy for treatment.Ritodrine was significantly more effective both in the treating period for suppression of contraction (2.33 +/- 0.63 hr for initiation of inhibiting uterine activity, 9.38 +/- 3.88 days of accumulative treating period) and prolongation of gestation (4.81 +/- 2.83 wks) than that of the magnesium sulfate (P < 0.001, 0.005 and 0.01 respectively), and had a term labor of 73.44%. There was no serious cardiac and pulmonary complications in the ritodrine group.The present study has shown that ritodrine as a tocolytic agent is highly effective for the management of preterm labor and relative safe for mother and fetus.
Different dosage regimens of nifedipine, labetalol, and hydralazine for the treatment of severe hypertension during pregnancy: a network meta-analysis of randomized controlled trials

Abstract:Objectives: This network meta-analysis aimed to compare the efficacy and safety of intravenous (IV) hydralazine, oral nifedipine, and IV labetalol with different dosage regimens in the treatment of severe hypertension during pregnancy. Methods: A comprehensive literature search was performed on PubMed, Embase, the Cochrane Library, and ClinicalTrials.gov for randomized controlled trials (RCTs) exploring the effects of hydralazine, nifedipine, and labetalol in the treatment of severe hypertension during pregnancy. Results: A total of 21 RCTs with 2183 patients comparing 7 regimens (oral nifedipine 50,60,90 mg; hydralazine 15,25 mg; and labetalol 220,300 mg) were identified. Compared with IV labetalol 300 mg, nifedipine 50,60, and 90 mg significantly improved the successful treatment rate of severe hypertension during pregnancy, nifedipine 50 and 90 mg and IV hydralazine 25 mg required significantly fewer doses to achieve target blood pressure (BP), and nifedipine 50 mg took significantly shorter time to achieve target BP. Subgroup analysis showed that only nifedipine 50 mg tablets, not capsules, required a significantly shorter time and fewer doses to achieve target BP than IV labetalol 300 mg. Moreover, nifedipine 60,90 mg showed superior effectiveness than IV hydralazine 15,25 mg in the successful treatment rate of severe hypertension during pregnancy. SUCRA analysis suggested that nifedipine 50,60,90 mg as the better regimens with the lower rates of overall ADR and neonatal complications. Conclusion: These findings demonstrated the superiority of oral nifedipine 50,60,90 mg, especially oral nifedipine 50 mg tablets, in the treatment of severe hypertension during pregnancy than IV labetalol 300 mg, while oral nifedipine 60,90 mg also showed superiority in the successful treatment rate of severe hypertension during pregnancy than IV hydralazine 15,25 mg. However, the limitations of the underlying data indicate that future large-scale and rigorous RCTs are needed to confirm such findings.
Impact of Nifedipine on pregnant women with threatened premature labor and their fetuses

Abstract:Objective To investigate the reliability and safety of Nifedipine for pregnant women with threatened premature labor, and observe their effects on newborns. Methods One hundred and fifty pregnant women with threatened premature labor and fetuses in our hospital from June 2011 to June 2012 were enrolled in this study. The patients were divided into two groups: the study group and the control group. Patients in the study group were treated with Nifedipine, while those in the control group were treated with ritodrine hydrochloride. The onset time, duration, fetus protection success rate, blood pressure, heart rate, costs and adverse reactions were observed. Results The time of the uterine contraction inhibited was significantly shorter in the study group than the control group (P<0.05). The inhibition maintain time was significantly longer in the study group than the control group (P<0.05). The effective rate was 93.33% in the study group and 90.67% in the control group, with statistically significant difference between the two groups (P>0.05). The blood pressure of the two groups was within the normal range, with no statistically significant difference (P>0.05). Heart rate in patients of the control group was significantly higher than the study group (P<0.05). The Apgar score of the preterm children in the study group was significantly higher than that in the control group (P<0.05). No statistically significant difference was found in the weight of preterm children and admission to Neonatal Intensive Care Unit (NICU), P>0.05. Drug side effects in the study group was slightly less than those in the control group, with no statistically significant difference (P>0.05). The costs of the study group was significantly lower than those in the control group (P<0.05). Conclusion For treating threatened premature labor, Nifedipine can effectively shorten the onset time, extend the maintenance time, and reduce side effects, which can be actively promoted in clinical practice.

Abstract:The aim of our study was to compare the efficacy and safety of nifedipine and progesterone for maintenance tocolysis after arrested preterm labour, in prolonging pregnancy and preventing recurrence of preterm labour. This study was a randomised comparative study conducted on 110 pregnant women with arrested preterm labour, randomised to receive either nifedipine 20 mg Q 8-hourly or progesterone 400 mg daily for maintenance tocolysis. Other than demographic parameters, obstetric parameters like previous history of abortions or preterm deliveries, gestational age, cervical dilatation and effacement, ultrasound measured cervical length at admission, were noted. Outcome measures studied were mean prolongation of pregnancy, mode of delivery, neonatal outcome and side-effects of both the drugs. We found that there was no significant difference in the demographic profile, parity, number of abortions, previous preterm deliveries, gestational age, cervical dilatation and effacement at admission between the two groups. A total of 10% of the patients in the nifedipine group and 61% of the patients in the progesterone group delivered at term (p value 0.000). The mean prolongation of pregnancy in the nifedipine group was 16.63 days and 40.14 days in the progesterone group which was significant (p = 0.000). Neonates in the progesterone group had better birth weight, better Apgar scores at 1 and 5 min, lesser need for ventilation and significantly lesser composite morbidity. Nifedipine was associated significantly with side-effects. We conclude that when compared with nifedipine, progesterone significantly prolongs pregnancy in women with arrested preterm labour with better neonatal outcomes and fewer side-effects.

Abstract:Objective We conducted a systematic review and meta -analysis to evaluate the fetomaternal outcomes after the administration of norepinephrine or phenylephrine for the treatment of post spinal hypotension in preeclamptic women undergoing a cesarean section. Data sources We searched on PubMed, Embase, Scopus, Cochrane CENTRAL, and clinicaltrials.gov from inception till June 2024. Study selection Randomized controlled trials of preeclamptic women receiving norepinephrine or phenylephrine for post spinal hypotension were included. Data extraction and synthesis Two reviewers extracted data onto an Excel spreadsheet. R version 4.4 was used for statistical analysis. Risk ratios (RR) and their 95% confidence intervals (CIs) were calculated and pooled using the random effects model. Cochrane's risk of bias (RoB 2) tool was used for quality assessment. This review has been registered with PROSPERO (CRD42024532740). Results A total of 4 trials, comprising 413 participants, were included in this review. 206 patients received norepinephrine, while 207 received phenylephrine. The incidence of maternal bradycardia was significantly lower in the norepinephrine group compared with the phenylephrine group (RR = 0.25, 95 % CI = 0.16 to 0.39, p < 0.01). There were no statistical differences in other maternal outcomes or in the umbilical artery and umbilical vein blood gas analysis values. We also analyzed adverse events such as nausea (RR = 1.00, 95 % CI: 0.62 to 1.60, p = 1.00) and vomiting (RR = 0.99, 95 % CI: 0.89 to 1.11, p = 0.61), but they did not show a significant association with any group. All the trials had a moderate or low risk of bias. Conclusion Bolus doses of NE and PE for the treatment of post-spinal hypotension in preeclamptic women undergoing cesarean sections were found to exhibit comparable neonatal outcomes. However, NE provided superior maternal safety due to a lower incidence of bradycardia compared to PE.

Abstract:ObjectiveTo determine the effectiveness of an oral versus sublingual loading dose of nifedipine for tocolysis. MethodsAn open, randomized clinical trial conducted between March 1, 2013, and April 31, 2014. Participants were pregnant women with a diagnosis of premature labor, single live fetus, topical pregnancy, gestational age 24–36 weeks, normal fetal vitality, cervical dilatation less than or equal to 4 cm, cervical effacement less than or equal to 80%, and intact amniotic membranes. They were randomized into two groups, oral and sublingual nifedipine, 20 mg loading dose, repeated every 30 minutes (maximum dose of 60 mg). The primary endpoint was the time until tocolysis and the secondary endpoints were the effectiveness of tocolysis within 90 minutes, 12 hours, and 48 hours; premature delivery within 48 hours; and maternal hemodynamic parameters and side effects. ResultsThere were 80 patients randomized to oral (n=40) and sublingual (n=40) nifedipine. The time required for tocolysis was significantly less with sublingual nifedipine (160 minutes vs 340 minutes; P=0.0003). Sublingual nifedipine was also more successful than oral nifedipine at inhibiting premature labor within 90 minutes (n=8 [20.0%] vs n=1 [2.5%], P=0.014) There was no statistically significant difference between the groups for the other secondary endpoints. Conclusioncompared with oral administration, a sublingual loading dose of nifedipine resulted in faster tocolysis in patients with premature labor.

Abstract:Background: A prospective study was conducted to compare the effectiveness of Nifedipine and Isoxsuprine in suppression of preterm labour pain as tocolytics drug. As preterm labour pain is major contributor for perinatal morbidity and mortality. The aims of this study were to assess the effect of nifedipine and isoxsuprine in threatened preterm labour with the aim of preventing preterm birth and its sequelae.Methods: This study was conducted on 100 patients coming to Pannadhay Rajkiya Mahila Chikitsalaya, RNT Medical College, Udaipur and attending OPD and IPD with complain of uterine contractions between 28-36 weeks of gestation.Results: Nifedipine was more effective than isoxsuprine hydrochloride as tocolytic agent.Conclusions: There is high incidence of preterm labour in India which leads to neonatal morbidity and mortality. Nifedipine is a better tocolytic drug compared to isoxsuprine hydrochloride.

Abstract:Preterm delivery presents the main cause of neonatal morbidity and mortality worldwide. The rate of preterm delivery is 12% to 13% in the United States, of which 29% concerns preterm deliveries before 34 weeks of gestation. Basic parameter of prevention strategy is implementation of tocolytic therapy in cases of threatened preterm labor. Several therapeutic approaches have been proposed, among which betamimetic agonists, calcium channel blockers, magnesium sulfate, oxytocin receptor blockers, nitrates, and prostaglandin inhibitors, whereas new alternatives such as usage of thiocolchicoside have also been reported. This article is one among few that aims to review the comparative effectiveness of various tocolytic agents regarding prevention of preterm delivery, impact on perinatal morbidity and mortality, neonatal health status, and maternal complications. Main conclusions of recent randomized control trials and meta-analyses are summarized to assess about which agents consensus already exists on their effectiveness, which agents should be further studied to achieve conclusions, as well as those that are rather unlikely to have significant tocolytic impact or any other benefit on neonatal outcome.

Abstract:Objective: The purpose of the study was to observe and compare the effects of ritodrine hydrochloride and magnesium sulfate on maternal fluid dynamics. Study design: Fourteen women in preterm labor were prospectively studied during tocolytic therapy with either ritodrine hydrochloride or magnesium sulfate. The cardiovascular and renal effects of a pretreatment crystalloid infusion were compared with those observed during tocolytic therapy. Profile analysis and repeated measures of variance were used to analyze the data. Results: Ritodrine hydrochloride was associated with decreased colloid osmotic pressure, hematocrit, and serum proteins and increased maternal and fetal heart rates. Arginine vasopressin levels increased during the first 2 hours of therapy, then returned to baseline. Sodium excretion was reduced and there was marked fluid retention. Intravenous magnesium sulfate also resulted in a reduction of colloid osmotic pressure, but hematocrit, serum protein concentration, arginine vasopressin, maternal and fetal heart rates, and mean arterial pressure were minimally affected. Sodium excretion increased to a maximum at 6 to 8 hours of treatment, then returned to baseline. A positive fluid balance was also noted in magnesium sulfate-treated patients but to a lesser degree than with ritodrine. Conclusions: Sodium retention appears to be the primary cause of plasma volume expansion in ritodrine-treated patients, whereas volume expansion during magnesium sulfate therapy is probably related to intravenous overhydration. In the absence of risk factors for pulmonary capillary membrane injury, available evidence supports volume overload as the principal mechanism for pulmonary edema during tocolytic therapy.
Efficacy and safety of Nicardipine in treatment of severe hypertensive disorder complicating pregnancy

Abstract:Objective To evaluate the clinical efficacy and safety of nicardipine in treatment of hypertensive disorder complicating pregnancy. Methods Sixty-two women with severe hypertensive disorder complicating pregnancy registered to three hospitals between March 2009 and March 2010 were included in this study.These patients were divided into two groups randomly to receive treatment with labetalol(n=30) or nicardipine(n=32).Changes of blood pressure before and after treatment,perinatal outcomes and side effects of medication were observed in both groups. Results In nicardipine group,the target levels of blood pressure were achieved at 1 hour after medication.The medication was shifted to oral use in 13 cases,which 7 patients required treatment in combination with other antihypertensive drugs.In the labetalol group,the target levels of blood pressure were achieved at 4 hours after medication.The medication was shifted to oral use in 9 cases,which 11 patients required treatment in combination with other antihypertensive drugs.There were a significant difference in antihypertensive effects between two groups(P<0.05).The birth weight and placenta weight in nicardipine group were significantly greater than those in labetalol group(P<0.05).There was no statistic difference in changes of heart rate before and after treatment in two groups(P>0.05). Conclusion Nicardipine may provide effective control of blood pressure and better pregnancy outcome for women with severe hypertensive disorder complicating pregnancy.
Nifedipine or amlodipine? The choice for hypertension during pregnancy: a systematic review and meta-analysis

Abstract:Background: There is a lack of sufficient evidence regarding efficacy and safety of amlodipine on treating hypertension during pregnancy. Objective: To compare antihypertensive efficacy, pregnancy outcome and safety of amlodipine with nifedipine on hypertension during pregnancy. Methods: A systematic search of PubMed, Embase, Cochrane Library, clinicaltrials.gov, Chinese National Knowledge Infrastructure, Wanfang Database and China Biology Medicine disc of randomized controlled trials (RCTs) up to April l5, 2021 was conducted on RCTs comparing amlodipine to nifedipine for the treatment of hypertension during pregnancy. Screening, data extraction, and quality assessment were done by two independent reviewers. To estimate relative effects from all available evidence, a meta-analysis was conducted. Results: Seventeen RCTs were included. Amlodipine was found the efficacy is slightly superior to nifedipine on treating hypertension during pregnancy (RR 1.06, 95% CI 1.01 to 1.10) with a decreased risk for maternal side effects (RR 0.42, 95% CI 0.29 to 0.61). Subgroup analysis found amlodipine can get a better control on SBP (RR - 11.68, 95% CI - 17.98 to - 5.37) and DBP (RR - 7.44, 95% CI - 13.81 to - 1.06) compared with intermediate-/long-acting nifedipine. In addition, there was no difference between amlodipine and nifedipine on pregnancy outcomes including caesarean section, premature labour, placental abruption, FGR, fetal distress, neonatal asphyxia. Conclusions: Given the results of this systematic review and meta-analysis, amlodipine can be effectively and safely used for hypertension during pregnancy.
