Relationship Between Vascular Endothelial Function and C-reactive Protein with Gestational Diabetes Mellitus and Intervening Effect of Insulin

Abstract:Objective The relationship between vascular endothelial function and inflammation state by measured branchial artery flow-mediated dilation(FMD) and serum C-reactive protein level in gestational diabetes mellitus was studied to investigate the mechanisms of insulin effect on improved vascular endothelial function by assessing CRP and FMD both before and after insulin therapy.Methods Thirty-one pregnant women with gestational diabetes mellitus(GDM) and 27 normal healthy pregnant woman received routine physical examination,laboratory assessment and vascular ultrasound examination before and after their 3 months insulin treatment.Hs-CRP was used for evaluating systemic inflammation and high resolution ultrasound examining branchial artery endothelium-dependent flow-mediated vasodilation was used to evaluating vascular endothelial function.Results Significant differences in FMD and CRP levels between gestational diabetes mellitus and normal controlled subjects were observed.FMD and CRP levels were improved significantly after treatment with insulin in the gestational diabetic women.Using Pearson correlation analysis,we found that there existed a negative correlation between FMD and CRP,SBP,FPG,TC,DBP,HOMA-IR,LDL-C and HbA1C in patients with GDM.Using multiple linear stepwise regression analysis,it was found that CRP,HOMA-IR,FPG,TC and SBP can be entered into the regression equation.Conclusions There exist an artery endothelium-dependent flow-mediated vasodilation dysfunction and an elevated CRP level in patients with gestational diabetes mellitus.There exists a negative correlation between FMD and CRP in patients with gestational diabetes mellitus which suggests the negative correlation between vascular endothelial function and inflammation.At certain level,insulin may improve vascular endothelial function by its anti-inflammation activities in patients with GDM.
