Evaluation of vascular endothelial function and related risk factors in gestational diabetes mellitus patients with echo-tracking technique

Abstract:Objective: To evaluate vascular endothelial function before vascular complications appearing in gestational diabetes mellitus (GDM) patients by using echo-tracking (ET) technique, and to observe related risk factors. Methods: Fifty GDM patients (GDM group) and 50 normal pregnant women (control group) were collected, body mass index (BMI) before pregnancy, fasting blood glucose (FBG), blood glucose 1 h after meal (P1hPG) and 2 h after meal (P2hPG), total cholesterol (TC), triglyceride (TG) and insulin resistance index (IR) were compared. The brachial artery was detected with ET, the parameters of vascular endothelial function were obtained and compared between the two groups. Taking BMI, FBG, P1hPG, P2hPG as independent variables, stiffness parameter (β), pulse wave velocity (PWVβ), pressure-strain elastic modulus (Eρ), arterial compliance (AC) as dependent variables, stepwise linear regression analysis was made. Results: Compared with those in control group, β, Ep, PWVβ, BMI, FBG, P1hPG, P2hPG, TC, TG and IR increased, but AC decreased in GDM group (all P<0.05). There was significant relationship between IR and β, FBG and β, IR and PWVβ, TG and Eρ, BMI and AC (all P<0.05). Conclusion: Vascular endothelial dysfunction may exist before vascular complications appearing in GDM patients. Effective adjustment and control of body mass, FBG, TG and IR before pregnancy are important to prevent and cure vascular endothelial dysfunction in GDM patients.
