Solitary Fibrous Tumors of the Central Nervous System: Clinical Features and Imaging Findings in 22 Patients

Abstract:Introduction Solitary fibrous tumor (SFT) is a rare mesenchymal neoplasm originating in the central nervous system (CNS), with imaging features currently not well known. The purposes were to describe and characterize clinical features and imaging findings of CNS SFT. Methods We retrospectively reviewed computed tomographic (CT; n = 10) and magnetic resonance (MR) images (n = 18) of 22 patients with SFT (13 males and 9 females; mean, 47.6 years) with associated clinical records. Results Each lesion was found as a solitary, well-defined mass, ranging in size from 12 to 70 mm (mean, 38 mm). The tumor shape was roundlike in 16 cases (72.7%) and irregular in 6 cases (27.2%). The cerebellopontine angle zone was the most affected area (n = 6). On precontrast CT scans, 10 cases showed predominantly hyperattenuation (n = 9) and isoattenuation (n = 1). No lesion contained calcification, and 2 cases showed bone invasions. All 18 tumors examined by MR imaging showed homogeneous hypointensive (n = 5) or isointensive (n = 7) signal intensity and heterogeneous mixed isointense and hypointense signal intensity (n = 6) on T1-weighted images, whereas most tumors were predominantly isointense (n = 13) and hypointense (n = 4) to the cortex on T2-weighted images; on postcontrast CT and MR images, enhancement was marked homogeneous (n = 10) or heterogeneous (n = 12). Fourteen tumors had thickening of the meninges adjacent to the tumor. Conclusions Although SFT is a rare neoplasm in the CNS, it should be considered in the differential diagnosis. The most affected area is the cerebellopontine angle zone. Solitary fibrous tumor tends to have some imaging features, such as high attenuation on CT, isointense to hypointense signal intensity on MR images, and marked enhancement.

Abstract:Objective To investigate the imaging characteristics of solitary fibrous tumor(SFT).Methods This study included 11 cases with SFT proved by pathological results.The imaging manifestations were retrospectively reviewed and analyzed in comparison with surgical and pathological results. Among them,8 patients underwent CT plain scan (1/8) or enhanced scan (7/8),4 patients underwent MR plain scan (2/4) or enhanced scan (2/4),and only 1 patient underwent both CT and MR scan.Results ( 1 )SFT were located in the thoracic cavity(2/11 ),abdominal and pelvis(5/11 ),and somatic soft tissues (4/11 ).The average tumor size was 11.8 cm ( 2.5 to 23.0 cm).Among them,4 tumors were round or ellipse,and 7 tumors were irregular.Eight tumors had well-defined contours,and the others showed illdefined contours.(2)On CT scan,all SFTs showed inhomogeneous density with low density areas.Calcification was seen in I case,and hemorrhagic foci were detected in 2 cases.(3)On MR scan,all SFTs showed intermediate signal intensity on T1 WI.Three tumors were slightly hyperintense and the other was hypointense on T2WI.All of them were rich of vascular signal voids.Three tumors,including 2 tumors with scattered long T1 and T2 signal and 1 tumor in the liver with hyperintense haemorrhage,displayed inhomogeneous mixed signal.One tumor in the orbit showed homogeneous signal.(4)On enhanced scan,9 tumors showed irregular enhancement with multiple circuitous vessels in the arterial phase,and continuous or progressive enhancement with patch non-enhanced areas in the venous phase.( 5 ) Histologically,SFTs were composed of juxtaposed hyper- and hypo-cellular spindle cells,dense collagenous stromas and numerous thin-walled blood vessels with a staghorn configuration.Conclusion The possibility of SFT should be considered when a single soft tissue mass with sharp border,inhomogeneous density is detected,especially with inhomogeneous enhancement maintaining in the venous phase.

Abstract:The aim of the present study was to summarize the clinical manifestations, diagnosis, treatment, and prognosis of solitary fibrous tumor (SFT). In total, 47 cases of SFTs diagnosed by postoperative pathology between January 2002 and September 2014 were retrospectively reviewed, and the general information, clinical manifestations, imaging techniques, treatment, pathology and follow-up findings were analyzed. Of the 47 patients, clinical characteristics were collected in 37 cases (18 men and 19 women; mean age, 44.1 years; age range, 13-72 years). The maximum diameters of the tumors were 1.5-25 cm, with a mean diameter of 8.8 cm. The symptoms were various and non-specific. Imaging examinations following iodinated contrast administration showed the SFTs to be well-defined, cystic or solid mass and enhanced. On color Doppler ultrasound, SFTs were described as hypoechoic, clear, irregular masses. All patients underwent surgical resection, and SFT was diagnosed by postoperative pathological and immunohistochemical examination. Of the 47 patients, 25 received complete follow-up of 5-130 months, with a median follow-up period of 35.2 months, that included a color Doppler ultrasound or computed tomography (CT) scan every 6-12 months. At the end of the follow-up period all patients were alive and healthy, with the exception of one patient, who presented with recurrence 15 months after surgery. The findings of the present study showed SFT to be a rare systemic disease with no particular clinical manifestations. In the cases reviewed in the present study, CT, magnetic resonance imaging scans and color Doppler ultrasound were important for the diagnosis of SFT, while the definitive diagnosis relied on pathological and immunohistochemical examinations. Surgery, the primary treatment for SFT, was performed, and, following complete removal of the tumor, the prognosis was favorable.

Abstract:Objective To discuss the clinical features,diagnosis,differential diagnosis,treatment principles and prognosis of solitary fibrous tumor(SFT)in the head and the neck.Method 6 cases of head and neck SFT were retrospectively analyzed in clinical features,imaging findings,surgical techniques and postoperative HE staining immuno-histochemical characteristics of chemistry and prognosis.Results All were primary healing without significant complications.In a follow-up of 12-28 months,recurrence was not found in 5 patients.1 patient lost follow-up.Post-operative pathology diagnosis was SFT and of all tumor cells were immune-histochemically positive for CD34 and Vim.Conclusion SFTs in the head and the neck are rare,and most are clinically benign,occasional with recurrence and metastasis of intermediate tumor.Diagnosis must depends on immune-histochemistry of the whole specimen.Complete radical resection is the first choice of treatment.Long-term close follow-up is mandatory because of aggression in the small number of case.

Abstract:Objective To assess the clinical,histopathological and immunohistochemical characteristics of the solitary fibrous tumor(SFT),and to discuss its origin and differential diagnosis.Methods Twenty-seven cases of solitary fibrous tumor were analyzed by clinical and histological observation,and immunohistochemical staining of CD34,Vim,CD99,Bcl-2,Des,SMA,CK,EMA,CD68,CD-117,S-100 protein was examined.Results In 27 cases SFT lesions were found in lung,thoracic cavity,abdomen,mediastinum,fossa orbitalis,conjunctiva,hypoglossis,cervical vertebrae,forefinger,pelvis,groin,spermatic cord,scrotum,thigh and so on.The size of tumors ranged from 0.8 cm×0.6 cm×0.5 cm～30 cm×20 cm×8 cm.The majority of patients had some symptoms due to compression from the large tumor.Most cases,separated by strands of rope-like collagen,showed haphazard growth pattern of short spindle cells presenting a storiform,fascicularor or spiral pattern in some areas.Immunohistochemically overexpresstion of CD34,Vim,CD99 was observed in all cases;positive Bcl-2 was showed in 23 cases,CD117 was detected focally in 2 cases;SMA and Des were detected focally in 1 case;CK,EMA,CD68 and S-100 proteins were negative.Twenty-one patients were followed up for 3~92 months and all were alive without recurrency or metastasis.Conclusions The diagnosis of SFT is based on histopathology and immunohistochemistry.Patients with SFT have a good prognosis after tumor removed completely.

Abstract:Objective: To investigate the clinical and pathological features and imaging manifestations of SFT (solitary fibrous tumor), and further to define the differiciation between the benign and malignant SFTs. Methods: The imaging findings were retrospectively analyzed and compared with the corresponding pathological results in 18 patients with SFT confirmed pathologically. Results: Of the 18 patients, 2 had postoperative recurrent or metastatic tumor, 3 had malignant SFT. The X-ray and MSCT (multi-slice computed tomography) scan images showed oval or lobulated masses with discrete margins except one malignant tumor. The tumor density was clear or nearly uniform. MRI (magnetic resonance imaging) examination showed that the tumors had intermediate signal intensity on T1WI (T1-weighted image) and heterogeneous signal intensity with flow voids on T2WI (T2-weighted image). All tumors showed heterogeneously intermediate or intense enhancement with geographic pattern. Intralesionally and paraleisonally prominent feeding vessels could be seen. The results of immunohistochemistry demonstrated that CD34 was positive in 18 patients; Bcl-2 and vimentin were positive in 17 patients; S100 was negative in 15 patients; cytokeratin and desmin were negative in 18 patients; Ki-67 expression was less than 5% in 15 benign tumors and more than 5% in 3 malignant tumors. Conclusion: There are some specific imaging features which are helpful to the differential diagnosis for SFT. These specific imaging features include the rich feeding vessels, the signal intensity on T2WI and the different enhancements associated with the tissue compositions of the tumor. However, making a definitive diagnosis still depends on pathology and immunohistochemistry. Copyright © 2013 by TUMOR.

Abstract:Background Intracranial solitary fibrous tumors (SFTs) are rare mesenchymal neoplasms, often challenging to diagnose due to their resemblance to meningiomas and other central nervous system tumors. While advancements in molecular genetics have aided in classification, diagnostic nuances and optimal management strategies remain areas of interest. Materials and methods This retrospective study analyzed 11 cases of intracranial SFTs treated at a neurosurgical centre in India between February 2020 and January 2024. Clinical data, radiological findings, histopathological features, and follow-up details were reviewed. Immunohistochemistry, particularly STAT6, facilitated diagnosis confirmation. Results The median age of presentation was 32 years, with a male predominance. Headache was the most common presenting symptom, often leading to misdiagnosis as meningiomas on radiological imaging. Histologically, SFTs exhibited spindle to ovoid cells with staghorn vessels and collagenized stroma, posing challenges in differential diagnosis. WHO grading predominantly revealed grade 1 tumors, though recurrence occurred, emphasizing the importance of long-term follow-up. Immunohistochemistry, particularly STAT6, played a pivotal role in distinguishing SFTs from other entities. Conclusion Intracranial SFTs present diagnostic challenges due to overlapping features with other tumors, warranting a comprehensive approach integrating clinical, radiological, and histopathological findings. Immunohistochemistry, particularly STAT6, emerges as a valuable diagnostic tool. Long-term follow-up is essential for monitoring recurrence and potential malignant transformation. Further research is needed to delineate optimal treatment strategies, including the role of radiotherapy in SFT management.

Abstract:Solitary fibrous tumor (SFT) is a spindle cell tumor of mesenchymal origin that typically occur in pleura tissue. Ocular SFT as a rare disease, which may occur in a variety of areas, including eyelid, intro-orbital, intra-bulbar. Ocular SFT is mainly seen in adults, monocular onset. The clinical features of the patients often present with eyelid swelling, proptosis, vision disturbances and a palpable mass. CT and MRI revealed a well-circumscribed ovoid mass with intensely enhancement, because of the prominent blood vascular channels within the tumor. The classic histopathological feature of SFT is ovoid to spindle shaped cell growing in a haphazard manner. Background stroma was fibro-collagenous and richly "storiform pattern and staghorn" vessels. A diffuse and intense staining of CD34 and STAT6 are important diagnostic clue favoring SFT. CD99, vimentin and Bcl-2 are also frequently positive in SFT. Scientists have recently identified NAB2-STAT6 gene fusions in the vast majority of SFT, which may promote the formation of SFT. Radical surgical resection seemed to be the most important treatment segment. When the lesion is not completely cleared or in recurrent and malignant cases, postoperative radiotherapy can be used as an adjuvant therapy. SFT as an intermediate tumor, its biological behavior is between benign and malignant. Solitary Fibrous Tumors in Abdomen and Pelvis:Imaging Characteristics and Radiologic-Pathologic Correlation

Abstract:AIM:To describe the imaging features of solitary fibrous tumors(SFTs)in the abdomen and pelvis,and the clinical and pathologic correlations.METHODS:Fifteen patients with pathologically confirmed SFTs in the abdomen and pelvis were retrospectively studied with imaging techniques by two radiologists in consensus.Patients underwent unenhanced and contrast-enhanced imaging,as follows:3 with computed tomography(CT)and magnetic resonance imaging(MRI)examination,8 with CT examination only,and 4 with MRI examination only.Image characteristics such as size,shape,margin,attenuation or intensity,and pattern of enhancement were analyzed and correlated with the microscopic findings identified from surgical specimens.In addition,patient demographics,presentation,and outcomes were recorded.RESULTS:Of the 15 patients evaluated,local symptoms related to the mass were found in 11 cases at admission.The size of the mass ranged from 3.4 to 25.1cm(mean,11.5 cm).Nine cases were round or oval,6were lobulated,and 10 displaced adjacent organs.Unenhanced CT revealed a heterogeneous isodense mass in 7 cases,homogeneous isodense mass in 3 cases,and punctuated calcification in one case.On MRI,most of the lesions(6/7)were heterogeneous isointense and heterogeneous hyperintense on T1-weighted images and T2-weighted images,respectively.All tumors showed moderate to marked enhancement.Heterogeneous enhancement was revealed in 11 lesions,and 7of these had cysts,necrosis,or hemorrhage.Early nonuniform enhancement with a radial area that proved to be a fibrous component was observed in 4 lesions,which showed progressive enhancement in the venous and delayed phase.No statistical difference in the imaging findings was observed between the histologically benign and malignant lesions.Three patients had local recurrence or metastasis at follow-up.CONCLUSION:Abdominal and pelvic SFTs commonly appeared as large,solid,well-defined,hypervascular masses with variable degrees of necrosis or cystic change that often displaced adjacent structures.
Retrospective study of 13 maxillofacial solitary fibrous tumors in 5 years

Abstract:Objective To explore the diagnostic methods and therapeutic principles of maxillofacial solitary fibrous tumors (SFT) through a retrospective analysis of clinical cases.Methods From 2009 to 2014,patients with maxillofacial SFT were counted in Shanghai No.9 Hospital oral -pathology database.The clinical data,imaging data and diagnostic methods were analyzed.Results 13 patients were selected in this study,whose post -operative pathology diagnoses were SFT with no typical clinical specificities,and their tumors were significantly enhancing masses according to the CT data.The immunohistochemistry of neoplasm cells were all positive for CD34 and Vim,and negative for Des.The wound healings were fine without any complications in 13 patients.During the post -operative follow -up period of 3 to 60 months,recurrences were found in 2 patients whose first pathological results were malignant SFT.After a reoperation,recurrences didn′t happen again in these 2 patients.Conclusions SFT was a rare disease in the maxillofacial region,and only the immunohistochemistry of the whole specimen would confirm the diagnosis.Complete radical resection was the first choice,long-term close follow -up was mandatory to control the recurrences,and the patient′s prognosis was usually satisfactory.

Abstract:Solitary Fibrous Tumors (SFTs) are uncommon mesenchymal tumors of fibroblastic/myofibroblastic origin that stem from various anatomical sites. Most SFTs are asymptomatic and noticed incidentally by various imaging modalities. Although SFTs were initially identified in the pleura and erroneously considered to originate solely from serosal layers, extrapleural SFTs have been reported more commonly than their pleural counterparts. Imaging features are similar in different anatomical locations and are mainly related to the tumor's size and collagen content, characteristically displaying low signal intensity on Magnetic Resonance Imaging (MRI). Smaller tumors typically exhibit uniform enhancement, yet necrotic regions may become evident as the tumor size increases, resulting in heterogeneous enhancement. Less than 30% of SFTs demonstrate unfavorable clinical outcomes regardless of their histological features, warranting surgery as the preferred treatment with long-term follow-up. In this article, we have reviewed the clinical manifestations and imaging features of SFTs, discussed their differential diagnosis based on anatomical site, and provided diagnostic pearls.
Pathological and Imaging Analysis of Solitary Fibrous Tumors

Abstract:Objective To study the pathological and imaging features of the solitary fibrous tumors (SFT).Materials and Methods The imaging findings of pleural or abdominal SFT in 10 patients were retrospectively analyzed, and a comparison with histopathological and immunohistochemical results was made. Results (1) SFT predominantly occurred in the thorax, being manifested on plain film as a smooth margined solitary mass. (2) On CT scans, cystic degeneration area and marked enhancement of tumor's parenchyma were demonstrated. (3) On T 2WI, most tumors were hypointense, and larger lesions displayed low high mixed signal with signal void of flow. (4) Microscopically, SFT was composed of spindle cells arranged in different patterns, with CD 34 being immunohistochemical positive. Conclusion SFT is a soft tissue tumor originated from mesenchyma. The possibility of SFT should be considered when a soft tissue mass with sharp border is seen on imaging examination. However, the verification of the diagnosis should be based on the histopathological and immunohistochemical examinations.
Diagnosis and Differential Diagnosis for Solitary Fibrous Tumor in the Abdomen and Pelvis by CT

Abstract:OBJECTIVE:To study the CT features for solitary fibrous tumor (SFT) in the abdomen and pelvis and to improve the diagnostic accuracy.  Methods: Fourteen patients with SFT were collected in our hospital from January, 2011 to December, 2015. Characteristic of images were analyzed and compared for 10 SFT, which located outside the abdominal organs with extragastrointestinal stromal tumors (EGIST), leiomyosarcoma, and schwannoma.  Results: Necrosis and cystic formation were frequently present in SFT in the abdomen and pelvis. CE-CT showed serpentine vessels along the periphery, while pattern of enhancement was map-like inhomogeneous progressive. Comparing with the EGIST or schwannoma, the difference of CT value in non-contrast and the arterial phase were statistically significant (P<0.05). The numbers of peritumoral circuity vessel were significantly different between SFT and EGIST (χ²=18.27, P<0.008) or between SFT and schwannoma (χ²=19.25, P<0.008). Comparing with the leiomyosarcoma or schwannoma, SFT located outside the abdominal organs. We found that tumor necrosis rate was significantly different between SFT and leiomyoscarcoma (χ²=8.00, P<0.008).  Conclusion: SFT in the abdomen and pelvis show certain CT characteristics. The CT value in non-contrast and at the arterial phase, tumor necrosis rate, and serpentine vessels along the periphery were pivotal in differentiating SFT from leiomyosarcoma, EGIST and schwannoma.
Clinical characteristics and prognosis of orbital solitary fibrous tumor in patients from a Chinese tertiary eye hospital

Abstract:Background: Solitary fibrous tumor (SFT) is predominant within the pleura but very rare in the orbit, which is why the diagnosis of orbital SFT poses challenges in clinical practice. Accordingly, an integrated approach that incorporates specific clinical features, histological, histopathological, and immunohistochemical (IHC) examinations, and molecular analyses is warranted. Aim: To retrospectively explore the clinical and imaging characteristics, treatment, outcomes of a series of patients with orbital SFT. Methods: We conducted a retrospective review of a series of patients diagnosed with a histopathologic orbital SFT treated at a single institution. All data on demogra/phics, clinical characteristics, imaging, treatment, postoperative histopathological and IHC examinations, and prognosis were collected. Results: In total, 13 patients were enrolled, 7 (53.8%) of whom had the tumor located in the superomedial quadrant of the orbit. Computed tomography revealed a solitary ovoid lesion in 10 (76.9%) patients and irregular lesion in 3 (23.1%) patients. Magnetic resonance imaging results were as follows: On T1 weighted images, 3 (23.1%) patients had hypointense mixed signals, whereas 10 (76.9%) patients showed isointense mixed signals; on T2 weighted images (T2WI), 3 (23.1%), 4 (30.8%), and 6 (46.2%) patients exhibited hypointense mixed, isointense mixed, and hyperintense signals, respectively. Notably, 12 (92.3%) patients showed significant enhancement, whereas there were patchy slightly enhanced areas in the tumor. All patients were treated by surgery. IHC analysis demonstrated that the tumor cells were immunoreactive for CD34, CD99, STAT-6, and vimentin in all patients. The lesions showed Ki-67 positivity < 5% in 1 (7.7) patient, 5%-10% in 10 (76.9%), and > 10% in 2 (15.4%). Two (15.4%) patients exhibited tumor recurrence. Conclusion: The clinical manifestations and radiologic characteristics of orbital SFT are diverse and not specific. Accurate diagnosis and treatment require detailed radiological and histopathological/IHC evaluation.
