Our aim was to co-modify adipose-derived stem cells (ADSCs) with PDE5-siRNA and iNOS gene, providing enough NO and cGMP for cavernosum smooth muscle relaxing, which might be more effective than PDE5/iNOS single gene and serve for future ED therapy by autograft into corpus cavernosum. METHODS ADSCs were isolated and cultured from the inguinal region of SD rat in vitro. The characterization of ADSCs was arrayed by flow cytometry and inducing in the adipogenic, osteogenic, myogenic, endothelial and neurogenic differentiation. Construct recombinant adeno-associated viruses carrying PDE5-siRNA and iNOS gene. ADSCs were co-transfected with the two recombinant adeno-associated viruses, and the expressions of PDE5 and iNOS gene in ADSCs were detected by realtime RT-PCR, western blotting assay. Nitric oxide and cGMP were determined by Nitrate/Nitrite Assay Kit cultured with 10mM L-arginine containing medium for 24 hours and ELISA Kit cultured with 10μM sodium nitroprusside containing medium for 2 minutes, respectively. RESULTS The cultured cells expressed CD49d, CD73, CD90, CD105 and CD106, lacked CD31, CD34 and CD45. Adipogenic, osteogenic, myogenic, endothelial and neurogenic differentiation cells can be stained with Oil Red O, Alizarine Red, α-SMA, vWF and NeuN respectively, and the negative control was negative. PDE5-siRNA worked at 3 days, reached a peak at 5 to 7 days, and decreased at 14 days. The expression level of PDE5 gene was down-regulated by more than 70 percent from 3 to 14 days, significantly lower than negative control (P<0.001). iNOS gene began to express at 24 hours, reached a peak at 4 to 5 days, and decreased at 7days. There was an overexpression of iNOS gene in ADSCs modified with iNOS (P<0.001). The level of NO was greatly higher than negative control (P<0.01), but no greater than iNOS single gene modified control (P>0.05). The concentration of cGMP was significantly greater than PDE5-siRNA/iNOS single gene modified control (P<0.01). The activation effects of PDE5-siRNA and iNOS genes on NO/cGMP signal pathway in ADSCs were kept at least 2 weeks. CONCLUSIONS ADSCs modified by PDE5-siRNA and iNOS genes could produce enough NO and cGMP, which is more effective than single gene.
