THE INTERVENTIONAL THERAPY FOR THE DORMANT MASS AFTER HEPATECTOMY IN HEPATOCELLULAR CARCINOMA

Abstract:Objective To investigate the role of chemoembolization for the treatment of hepatocellular carcinomaafter hepatectomy with low dose and low flow rate perfusion vla superselective cannulation.Methods Among sixty-seven patients with hepatocellular carcinoma receiving transcatheter arterial chemoembolization(TAE)therapy on 40th~120th day after operation,fourteen latent lesions were detected by digital substraction angiography(DSA)examination in thirteen patients,which were not detected by B-ultrasound(BUS),computerized tomography(CT) and magnetic resonance imaging(MRI)examination.The diameter of lesions is from 0.4~0.8 cm,average 0.6 cm,alpha-fetoprotein(AFP) value was more than 400μg/ml in two patients,200~400μg/ml in four patients,80~200μg/ml in five patients and less than 25μg/ml in two patients.Thirteen patients were treated by TAE with low dose and low flow rate continuous perfusion via superselective cannulation.Result thirteen patients were followed up by abdomen CT examination 2~4 weeks after first TAE.Image of CT showed well accumulation of iodized oil within hepatocellular carcinoma,which was the same as the DSA image.After 1~3 times treatment,the lesions reopt stable.In eleven patients,AFP values recovered to normal.Negative of AFP in two patients was still negative after therapy.Following up 25~51 months,no new lesion was discovered.Conclusion Early diagnosis and early therapy for latent hepatic tumor after operation play an active role in prolonging life of patients with hepatocellular carcinoma.Hepatic DSA and iodized oil CT examination are the most sensitive way to discover latent lesion.Chemoembolization with low dose and low flow rate continuous perfusion via superselective cannulation is a positive and effective method to therapy latent hepatocellular carcinoma.
