Abstract:Background: Obstructive sleep apnea (OSA) is a type of sleep-disordered breathing disease, with high prevalence and multiple complications. It seriously affects patients' quality of life and even threatens their lives. Early and effective treatment can significantly improve patients' health conditions. Objective: In this study, the main treatment methods, research hotspots and trends of OSA were summarized through bibliometric and visualization analysis. Methods: From the Web of Science Core Collection database, articles on the treatment of OSA from 1999 to 2022 were obtained. CiteSpace and VOSviewer were comprehensively used to visualization of journals, co-authorship of countries, institutions and authors, co-citation of references, keywords cluster and burst. Results: A total of 2,874 publications were obtained, of which 2,584 were concerned adults and 290 about children. In adults' research, Sleep and Breathing is the most published journal (280, 10.84%), the largest number of publications come from the United States (636,24.61%) and the University of Sydney (88, 3.41%), and Pepin JL is the most published author (48, 18.58%). In children's studies, International Journal of Pediatric Otorhinolaryngology is the most published journal (41, 14.14%), the maximum number of publications were also from the United States (123, 42.41%), with the University of Pennsylvania (20, 6.90%) and Marcus CL (15, 5.17%) being the most published institutions and authors. High-frequency keywords for adults' researches include positive airway pressure, oral appliance, surgery and positional therapy. On these basis, children's studies also focus on myofunctional therapy, rapid maxillary expansion and hypoglossal nerve Stimulation. Conclusion: Over the past two decades, research in the field of OSA therapeutics has experienced significant growth in depth and breadth. The author cooperation network has already established a solid foundation, while there is potential for further strengthening the cooperation network between countries and institutions. Currently, positive airway pressure and surgery are the primary treatments for OSA in adults and children. Future research will focus on multidisciplinary combination targeted therapy, which presents a key area of interest and challenge.

Abstract:Obstructive sleep apnea(OSA) refers to the complete or partial obstruction of the upper airway during sleep,resulting in repeated apnea and/or hypopnea,in turn leading to microarousal,intermittent hypoxia and excessive daytime sleepiness.OSA-induced chronic intermittent hypoxia,can cause sympathetic nervous excitement,endothelial dysfunction,systemic inflammation,oxidative stress and metabolic disorders,can induce or aggravate myocardial injury.Many studies have confirmed that OSA can cause myocardial injury markers increasing,and promote left ventricular remodeling and decreased cardiac function,and is an independent predictor of cardiovascular morbidity and poor prognosis.At present,the pathophysiological mechanism and development of myocardial injury caused by OSA remains to be further studied.The objective of this review was to discuss the clinical and basic research related to OSA and myocardial injury.

Abstract:Obstructive sleep apnea syndrome (OSAS) is a common sleep-disordered breathing disorder. Owing to the pathophysiological changes associated with it, it can affect the whole body. In recent years, domestic and foreign research studies have focused on the relationship between OSAS and systemic diseases. A number of studies have confirmed that OSAS is an independent risk factor for various cardiovascular and respiratory diseases. Based on the current status of knowledge regarding OSAS acquired through domestic and foreign studies, this paper summarizes the clinical features, preferred treatment methods, and related basic research of OSAS-related respiratory and circulatory diseases. It provides a basis for the clinical diagnosis and comprehensive treatment of OSAS.

Abstract:Objectives: The purpose of this study was to systematically analyze the studies of hypertension associated with obstructive sleep apnea to assess the current status and hot spots in this field. Methods: We searched the Web of Science Core Collection for publications related to hypertension associated with obstructive sleep apnea published before July 3, 2021. Bibliometric analyses and science mappings were carried out using the CiteSpace 5.8.R1 and Microsoft Office Excel 2019. CiteSpace 5.8.R1 was used to visualize the distribution of research fields, analyze co-occurring keywords and burst terms to detect trends and frontiers, and identify leading collaborations among countries, authors, and institutions. Microsoft Office Excel 2019 was used to make bar graphs, histograms and line graphs. Results: According to the search strategy, a total of 7263 published articles and reviews were retrieved. The research on hypertension associated with obstructive sleep apnea has been developing quickly at present. Sleep and Breathing was the most productive journal. The USA was a major producing country and Harvard Medical School was the most productive institution in this field. In the field of hypertension associated with obstructive sleep apnea, the main research hotspots were continuous positive airway pressure, cardiovascular disease, and obesity. Conclusions: The present study provides a new perspective for the study of hypertension associated with obstructive sleep apnea and valuable information for researchers to find potential partners and cooperative institutions, hot issues and research frontiers.

Abstract:Obstructive sleep apnea syndrome (OSAS) is a common breathing disorder in sleep in which the airways narrow or collapse during sleep, causing obstructive sleep apnea. The prevalence of OSAS continues to rise worldwide, particularly in middle-aged and elderly individuals. The mechanism of upper airway collapse is incompletely understood but is associated with several factors, including obesity, craniofacial changes, altered muscle function in the upper airway, pharyngeal neuropathy, and fluid shifts to the neck. The main characteristics of OSAS are recurrent pauses in respiration, which lead to intermittent hypoxia (IH) and hypercapnia, accompanied by blood oxygen desaturation and arousal during sleep, which sharply increases the risk of several diseases. This paper first briefly describes the epidemiology, incidence, and pathophysiological mechanisms of OSAS. Next, the alterations in relevant signaling pathways induced by IH are systematically reviewed and discussed. For example, IH can induce gut microbiota (GM) dysbiosis, impair the intestinal barrier, and alter intestinal metabolites. These mechanisms ultimately lead to secondary oxidative stress, systemic inflammation, and sympathetic activation. We then summarize the effects of IH on disease pathogenesis, including cardiocerebrovascular disorders, neurological disorders, metabolic diseases, cancer, reproductive disorders, and COVID-19. Finally, different therapeutic strategies for OSAS caused by different causes are proposed. Multidisciplinary approaches and shared decision-making are necessary for the successful treatment of OSAS in the future, but more randomized controlled trials are needed for further evaluation to define what treatments are best for specific OSAS patients.

The basic research in the center includes: epidemic survey of OSAHS and snoring in a Beijing population, setting up the cephalometric and MRI standard of airway and surrounding structures for non-snoring Chinese, aetology exploring OSAHS from views of airway size, craniofacial form, neck circumference and genioglossus muscle activity. Clinically, different dental appliances were developed. The treatment effects of the oral appliances for OSAHS patients were evaluated quantitatively. Two and three dimension changes of the airway and surrounding structures, genioglossus muscle activity, as well as snoring loudness following mandible advancing and opening were studied. Besides, research was expended to OSAHS in children recently. Under the support of National Nature Science Foundation 7 PhD and 3 MS students have graduated. More than 60 papers were published domestically and abroad and two awards were received from the National Ministry of Education and Chinese Medical Association in the past 10 years. This paper is a review of the past and current situation of dental research on OSAHS in China.

Abstract:Obstructive sleep apnea（OSA） is a common sleep-disordered disease which is characterized by repetitive narrowing or occlusion of the pharynx causing intermittent hypoxia, repetitive arousals, sleep deprivation, and excessive daytime sleepiness. OSA can contribute to atherosclerosis through direct and indirect mechanisms. Animal and clinical studies have shown a close relationship between OSA and atherosclerosis and its risk factors. However, current studies showed inconsistent results. In the future, further research both basic and clinical studies need to be fulfilled. Future studies are needed to investigate underlying mechanisms between OSA and atherosclerosis.
A Preliminary Study of Acoustic Characteristics of Snoring Sound in Patients with Obstructive Sleep Apnea/Hypopnea Syndrome (OSAHS) and with Simple Snoring

Abstract:Objective To investigate the acoustic characteristics of snoring sound in patients with obstructive sleep apnea hypopnea syndrome (OSAHS) and with simple snoring. Methods 22 patients with OSAHS and 15 with simple snoring were included in this study. Natural overnight snoring was digitally recorded and portable sleep monitoring was performed simultaneously.10 snores,which were the 1st snores after 10 cycles of obstructive apnea,from each patient in OSAHS group,and 10 snores from each patient in simple snoring group were analyzed in the time and frequency domains.Results The sound waves of snoring in the two groups exhibited different patterns both in the time and frequency domains. The snoring spectrum of patients with simple snoring showed distinct fundamental-harmonic structures which were not clear in patients with OSAHS.The central frequency of the patients with OSAHS was higher,and 800 Hz power ratio was lower than those of the patients with simple snoring. In the OSAHS group,the central frequency of the patients with moderate-to-severe OSAHS was higher,and 800 Hz power ratio was lower than those of the patients with mild OSAHS. The differences of the two parameters were of statistical significance. Conclusion The snoring sounds in patients with OSAHS and with simple snoring have different characteristics in time and frequency domains,indicating that it is feasible to research the OSAHS by way of snore monitoring and analyzing technique.
Clinical and Research Solutions to Manage Obstructive Sleep Apnea: A Review

Abstract:Obstructive sleep apnea (OSA), a common sleep disorder disease, affects millions of people. Without appropriate treatment, this disease can provoke several health-related risks including stroke and sudden death. A variety of treatments have been introduced to relieve OSA. The main present clinical treatments and undertaken research activities to improve the success rate of OSA were covered in this paper. Additionally, guidelines on choosing a suitable treatment based on scientific evidence and objective comparison were provided. This review paper specifically elaborated the clinically offered managements as well as the research activities to better treat OSA. We analyzed the methodology of each diagnostic and treatment method, the success rate, and the economic burden on the world. This review paper provided an evidence-based comparison of each treatment to guide patients and physicians, but there are some limitations that would affect the comparison result. Future research should consider the consistent follow-up period and a sufficient number of samples. With the development of implantable medical devices, hypoglossal nerve stimulation systems will be designed to be smart and miniature and one of the potential upcoming research topics. The transcutaneous electrical stimulation as a non-invasive potential treatment would be further investigated in a clinical setting. Meanwhile, no treatment can cure OSA due to the complicated etiology. To maximize the treatment success of OSA, a multidisciplinary and integrated management would be considered in the future.
Snoring, Inflammatory Markers, Adipokines and Metabolic Syndrome in Apparently Healthy Chinese

Abstract:Objective: Chronic low-grade inflammation and adipokines dysregulation are linked to mechanisms underscoring the pathogenesis of obesity-related metabolic disorders. Little is known about roles of these cytokines on the association between snoring and metabolic syndrome (MetS). We aimed to investigate whether a cluster of cytokines are related to snoring frequency and its association with MetS in apparently healthy Chinese.Methods: Current analyses used a population-based sample including 1059 Shanghai residents aged 35-54 years. Self-reported snoring frequency was classified as never, occasionally and regularly. Fasting plasma glucose, lipid profile, insulin, C-reactive protein, interleukin-6, interleukin-18, lipopolysaccharide binding protein, high-molecular-weight adiponectin and leptin were measured. MetS was defined by the updated National Cholesterol Education Program Adult Treatment Panel III criteria for Asian-Americans.Results: Overweight/obese subjects had significantly higher prevalence of regular snorers than their normal-weight counterparts (34.8% vs. 11.5%, P< 0.001). Regular snoring was associated with unfavorable profile of inflammatory markers and adipokines. However, those associations were abolished after adjustment for body mass index (BMI) or waist circumference. The MetS risk (multivariate-adjusted odds ratio 5.41, 95% confidence interval 3.72-7.88) was substantially higher in regular snorers compared with non-snorers. Controlling for BMI remarkably attenuated the association (2.03, 1.26-3.26), while adjusting for inflammatory markers and adipokines showed little effects.Conclusion: Frequent snoring was associated with an elevated MetS risk independent of lifestyle factors, adiposity, inflammatory markers and adipokines in apparently healthy Chinese. Whether snoring pattern is an economic and noinvasive indicator for screening high-risk persons needs to be addressed prospectively.

Abstract:The purpose of this study was to analyze acupuncture treatment methods and acupoints used to treat obstructive sleep apnea (OSA) or obstructive sleep apnea-hypoapnea syndrome (OSAHS). The data were retrieved from January 2010 to May 2022 from the China National Knowledge Infrastructure database. The search terms included “adult,” “obstructive sleep apnea,” “obstructive sleep apnea hypoapnea syndrome,” “acupuncture,” and “electro-acupuncture.” Clinical trials for acupuncture treatment of OSA or OSAHS were included in this review (4 non-randomized controlled studies, 1 was a case report, and 10 randomized controlled studies). For OSA and OSAHS treatment, the acupoints that were most frequently used included REN23, LU7, ST40, EX9, LI11, and DU20. Compared with the control or Western treatment group, the treatment outcome measures of participants in the acupuncture treatment group significantly improved. In some studies, participants in the acupuncture group did not have side effects and the treatment was cost-effective. The data analyzed in this review suggest that acupuncture is an effective treatment for OSA or OSAHS.
Acoustic Analysis of Snoring in the Diagnosis of Obstructive Sleep Apnea Syndrome: A Call for More Rigorous Studies.

Abstract:BACKGROUND Snoring is a common symptom of obstructive sleep apnea syndrome (OSA) and has recently been considered for diagnosis of OSA. OBJECTIVES The goal of the current study was to systematically determine the accuracy of acoustic analysis of snoring in the diagnosis of OSA using a meta-analysis. METHODS PubMed, Cochrane Library database, and EMBASE were searched up to July 15, 2014. A systematic review and meta-analysis of sensitivity, specificity, and other measures of accuracy of acoustic analysis of snoring in the diagnosis of OSA were conducted. The median of apneahypopnea index threshold was 10 events/h, range: 5-15 or 10-15 if aforementioned suggestion is adopted. RESULTS A total of seven studies with 273 patients were included in the meta-analysis. The pooled estimates were as follows: sensitivity, 88% (95% confidence interval [CI]: 82-93%); specificity, 81% (95% CI: 72-88%); positive likelihood ratio (PLR), 4.44 (95% CI: 2.39-8.27); negative likelihood ratio (NLR), 0.15 (95% CI: 0.10-0.24); and diagnostic odds ratio (DOR), 32.18 (95% CI: 13.96-74.81). χ(2) values of sensitivity, specificity, PLR, NLR, and DOR were 2.37, 10.39, 12.57, 3.79, and 6.91 respectively (All p > 0.05). The area under the summary receiver operating characteristic curve was 0.93. Sensitivity analysis demonstrated that the pooled estimates were stable and reliable. The results of publication bias were not significant (p = 0.30). CONCLUSIONS Acoustic analysis of snoring is a relatively accurate but not a strong method for diagnosing OSA. There is an urgent need for rigorous studies involving large samples and single snore event tests with an efficacy criterion that reflects the particular features of snoring acoustics for OSA diagnosis.
