Abstract:Objective:To value the effect of decompression of suction drainage on large mandibular cystic lesions.Methods:21 cases of large mandibular cystic lesions were performed decompression of suction drainage;the volume of the cavity and area of the cyst on a panoramic radiograph were measured preoperatively and postoperatively.Results:Observed from orth-panotomograph the density of bone increased significantly,with new bone formation and decreasing of the vulcancity of cysts.Conclusion:Decompression of suction drainage is an effective method to large mandibular cystic lesions.

Abstract:Objective:To investigate the clinical value of decompression in the treatment of jaw large cystic lesions.Methods:42 patients with jaw large cystic lesions were treated with decompression.Imaging examination was taken and compared before and after the treatment.The influence of frequency of cyst cavity washing on the effect of decompression was observed.Results:3 moths after de-compression the border of the cyst cavity became ambiguous and the size of the cavity reduced.12 months after decompression the size of cyst cavity was stable,when second surgery was taken to remove the left cyst wall.There was no recurrence of the cystic lesion after 1 ~2 year follow-up.Cyst cavity was washed everyday,but the effect and treatment course were not influenced by the daily washing times.Conclusion:Decompression is effective in the treatment of the jaw large cystic lesions.

Abstract:Purpose: To clinically evaluate the effect of decompression used for large mandibular cystic lesions and to explore its value in conservative mandibular surgery. Methods: Retrospect 139 cases of large mandibular cystic diseases, including odontogenic keratocyst (57%), basal cell nevus syndrome (8%), cystic ameloblastoma (21%) and other odontogenic cyst (14%), which were performed decompression from October 1999 to December 2006. The cystic space before and after the decompression was measured to evaluate the clinical effect, the recurrence rate was followed up. Results: All 139 cases gained a total effect rate of 86.4%. Furthermore, the odontogenic keratocyst had a better result than the cystic ameloblastoma. Among keratocyst cases, the unilocular type was more effective than multilocular type. However, the same groups of cystic ameloblastoma showed no significant difference in terms of effect rate. Conclusion: Decompression is a kind of ideal method for large mandibular cystic lesions.

Abstract:PURPOSE: To compare the clinical effects of fenestration and decompression technique,suction drainage and curettage for odontogenic cystic lesions of the jaws bones.METHODS: Odontogenic cystic lesions of jaw bones with a maximum diameter bigger than 3.5 cm were served as group one,and subdivided into group A(fenestration and decompression technique) and group B(suction drainage) according to the patients' desire.The lesions with a maximum diameter less than 3.5 cm were served as group two,and subdivided into group A(fenestration and decompression technique) and group B(curettage) according to the patients' desire.The evaluation for group one and two was done by measuring the cystic lesion during the treatment and comparing the speed of the reduction of the cystic area by panoramic view.Statistical analysis was performed after quantification for the lesions reduction using SPSS 11.0 software package.RESULTS: The healing of cystic cavity was good both in group A and group B of group one.After 6 months,the percentage of the reduction for the lesion volume was 63.66% and 83.52 %,respectively;the percentage of the reduction for the lesion area was 65.86% and 88.65 %.There was significant difference between them(P<0.05).In group B(surgery alone) of group two,after 6 months,the jaw bone had normal shape and the efficiency was 100%.In group A of group two,the efficiency was 60%.CONCLUSIONS: Suction drainage had a better effect for big jaw cystic lesions than fenestration and decompression technique.It is worthy of wide application.Surgical curettage is applicable to small odontogenic cystic lesions of the jaw bones.

Abstract:Objective To investigate the clinical value of cyst plug in decompression and suction drainage in the treatment of large jaw odontogenic cystic lesions.Methods Twenty four individualized cyst plugs made of Mucopren soft material were successfully applied to treat large jaw odontogenic cystic lesions.We also analyzed the advantage and complications of this kind of cyst plug.Results Totally twenty four cases of jaw large odontognenic lesions with the treatment of decompression or suction drainage have been reported to have pretty good result.The preparation and manufacturing of the individual cyst plug was simple,good looking,convenient,comfortable and reliable.The negative pressure of the night piece could hold for the whole night.The patient could simply adjust by cutting with scissors when they feel any discomfort.Conclusion The cyst plug made of Mucopren soft material is an ideal device for decompression and suction drainage in the treatment of large jaw odontogenic cystic lesions.
Clinical study of large mandibular odontogenic keratocyst treated by decompression

Abstract:PURPOSE: To clinically evaluate the effect of decompression used for large mandibular odontogenic keratocyst. METHODS: 61 cases with large mandibular odontogenic keratocyst underwent decompression from October 1999 to October 2004. All the patients were followed up for 6 months to 5 years. Clinical, X-ray and pathological examinations were performed for evaluation of the results. The changes in size of cysts before and after decompression were measured on panoramic X-ray films; the clinical effect and the recurrence rate were evaluated. SAS 6.12 software package was used for Chi-square test and Fisher exact test. RESULTS: After decompression, the sizes of the cysts reduced gradually, bone regeneration was noticed on radiographs. The displaced inferior alveolar neurovascular bundles returned to its normal position. The reposition of shifted teeth or bud was noticed, too. During the follow up period, no recurrence was found. The decompression period lasted from 6 months to 23 months, with a median time of 14 months. For clinical effect evaluation, a 91.8% complete response rate was obtained. Additionally, the single unicystic keratocyst had a better result than the multilocular keratocyst. However, different age groups showed no statistically significant difference in terms of efficiency. CONCLUSION: Decompression is a kind of ideal method for large mandibular odontogenic keratocyst. Abstract:Objective: To evaluate clinical curative effect on fenestration decompression in treatment of jaw cysts. Methods: Eighteen cases of jaw cysts in cyst fenestration decompression were done by CBCT pre-operative and 6 months post-operative, the cavity size and the variation of jaw around cysts was digitally measured. Results:There were statistical significance of the differences in the cavity size. Jaw shape gradually restored at 6 months post-operation. Conclu-sions: The fenestration decompression in jaw cyst has the advantages of simple operation, can effectively maintain the contour of the jaws, and reduced operation trauma.

However, some problems exist with its clinical application, such as the inappropriate determination of indication and the lack of standardized operational guidelines, resulting in treatment ineffectiveness or even failure. This paper aimed to summarize the clinical value of decompression for jawbone cystic lesions in terms of mechanism, scientific evidence, advancement, indications and effective evaluation by reviewing relevant literature and our clinical experience.

Abstract:PURPOSE:To evaluate the effectiveness of decompression as the primary treatment of odontogenic cystic lesions of the jaw involving factors that affect relative shrinking speed and bone regeneration. PATIENTS AND METHODS:A total of 32 patients with odontogenic cystic lesions of the jaw underwent decompression with customized thermoplastic resin stents. Clinical examinations and pre- and postdecompression panoramic radiographs were analyzed. RESULTS:The mean relative speed of shrinkage of radicular cysts (RCs; 3.37 cm(2)/month) was faster than those of keratocystic odontogenic tumors (KCOTs; 2.87 cm(2)/month) and unicystic ameloblastomas (UABs; 2.71 cm(2)/month). The relative shrinking size increased linearly in a time-dependent manner for KCOTs (r = 0.849, P < .001), RCs (r = 0.681, P = .319), and UABs (r = 0.146, P = .730); a similar relation was detected between the primary radiolucent area of cystic lesions before decompression and relative shrinking speed after decompression in KCOTs (r = 0.481, P = .032), RCs (r = 0.260, P = .673), and UABs (r = 0.370, P = .366), but patient age did not affect the relative speed of shrinkage (P > .05). Furthermore, the increase in bone density was more significant in RCs than in KCOTs (P = .026) and UABs (P = .012) after decompression. CONCLUSION:Decompression was effective in reducing odontogenic cystic lesions of the jaw and increasing bone density. For aggressive lesions, secondary definitive surgery was necessary.

Abstract:ve This study was to evaluate the effects of suction drainage of jaw cystic lesions on clinical symptoms and the rate of bone regeneration. Methods Fourteen cases of cystic lesions were treated with suction drainage. Clinical symptoms, radiographical and histopathological findings were evaluated before and after suction drainage. Results (1)The facial deformity was resolved completely 2 months postoperation. (2)The average decrease of cystic size was 50.9% and 88.1% in two and six months postoperation respectively. (2)The average thickness of new bone was 2.6mm and 4.84mm in two and six months postoperation respectively. Conclusion Suction drainage can rapidly correct the deformity caused by jaw cystic lesion and can promote the formation of new bone adjacent to the cystic lesion.
Pseudocysts of the Jaw: a Retrospective Study of 41 Cases from a Single Institution.

Abstract:Abstract Objective The purpose of this retrospective study was to investigate and compare the clinical, radiographic, pathological, pathogenesis, and therapeutic features of simple bone cysts (SBCs) and aneurysmal bone cysts (ABCs) of the jaw. Methods 35 patients with SBCs and 6 patients with ABCs who received treatment at the Department of Oral and Maxillofacial Surgery, Zhejiang University School of Medicine from 2017 to 2022 were followed up and reviewed retrospectively. Results The study included 41 patients, accounting for 2.14% of all jaw pathologies, with 35 patients having SBCs and 6 patients having ABCs; their average ages were 26.63 ± 13.62 years and 17.83 ± 7.88 years, respectively. The prevalence of SBC and ABC did not differ significantly by sex. The mandible was the most vulnerable area to be involved. Only 5.71% (2/35) of patients with SBCs and 16.7% (1/6) of patients with ABCs reported histories of previous trauma in the same region of the pseudocysts. A total of 42.86% (15/35) of SBC cases and 66.67% (4/6) of ABC cases had malocclusions. The radiographic features of pseudocysts varied in shape, were associated with the root, and unilocular or multilocular. All patients had curettage with or without bone graft or substitute implantation, and recurrences did not occur in 94.29% (33/35) of SBC patients and 100% (6/6) of ABC patients after a mean follow-up time of 26.23 ± 15.47 months and 21.67 ± 19.75 months, respectively. Conclusions Pseudocysts, including SBCs and ABCs, are benign osteolytic lesions without an epithelial lining that occur occasionally in the jaw, mostly in adolescents and young adults, and their incidence did not significantly differ by sex. The most vulnerable site of involvement is the mandible, and they are generally not overtly aggressive. Trauma has a less significant role in pseudocysts, but minor trauma, such as malocclusion, has the potential to influence pseudocyst development. The clinical presentation of pseudocysts lacks specificity, and most patients are asymptomatic and found incidentally during radiographs. Dental panoramic radiographs and CBCT cannot accurately distinguish between SBC and ABC, and the final diagnosis depends on pathological diagnosis. Curettage combined with bone grafting is currently the best treatment for both, with a 5.71% (2/35) recurrence rate for SBC and no recurrence found for ABC.
Conservative Treatment of the Teeth Involved by Large Cystic Lesion in Jaws

Abstract:PURPOSE: To study the clinical effect of preserving the teeth involved by large cystic lesion in jaws.METHODS: During curettage of the cystic lesion in the jaws,apicectomy was performed on 102 involved teeth which had received complete root canal therapy.All the patients were evaluated after operation with a follow-up of 7-28 months.RESULTS: Recurrence was not observed and a total of 98 teeth were successfully preserved,4 teeth were extracted due to severe loosening.CONCLUSION: The teeth involved by large cystic lesion in jaws could be successfully preserved by root canal therapy before operation and apicectomy during curettage,with no increase of postoperative recurrence.

Abstract:This paper summarizes the nursing care of 42 patients with huge mandibular cystic lesion treated by decompression and negative pressure suction.The key points in nursing were careful preoperative preparations,intensive postoperative care,proper guidance on the rinse of cyst and wear of obstructer,maintaining effective and continuous drainage with negative pressure.All the patients completed the whole process of treatment without postoperative complications.

Abstract:Objective: Cystic lesions of the jaw are common pathologies of chronic swelling of the jaw in oral and maxillofacial regions. Different treatment modalities have been described in the literature. However, the existence and proper treatment of these cysts remains a contentious topic. The aims of this review were to discuss the complexity of various surgical treatment and as factors with potential to influence outcome treatment. Finally, a practical and a rational clinical guideline for the management of such lesions have been suggested. Materials and Methods: A literature search without language limitation was performed in 2018 using MEDLINE, PubMed, Scopus, and Embase. Keywords for the search included the following terms: jaws cyst, cystic lesions, odontogenic cysts, cystic tumors, pseudocysts, treatments, therapy, wound healing, bone regeneration, and teeth involved cysts. Prospective or retrospective clinical studies with a sample size of n > 5 were evaluated and included in this review. The exclusion criteria were studies with unclear reporting of the treatment applied or outcome, nonhuman studies, case reports, letters, preface, comments, and cystic lesions associated to syndrome. After the full reading, 30 articles were included in the quantitative synthesis for the review. No meta-analysis could be performed due to the heterogeneity of the studies included. Clinical radiographic images were presented to illustrate the principles of some surgical treatments. Conclusion: Conservative surgery with primary closure defect (less than 4 cm) remains an initial approach, which reduces the morbidity of aggressive surgeries and obtains the complete bone healing before 24 months of postoperative. Marsupialization is considered as the most common option for the treatment of large cystic lesions when cases are carefully selected. Evocyst is an attractive new technique of obtaining complete bone defect healing within <3 months.
Dredging: A conservative surgical approach for treatment of large cystic lesions of the jaws

Abstract:Background: Treatment modalities of odontogenic cystic lesions of the jaws range from conservative to radical. "Dredging" may be considered the "middle path" between conservative and radical treatment options. It comprises three entities-decompression, followed by repeated scraping of the lesion, and finally, peripheral ostectomy providing margin clearance but without significant morbidity. Aim: To evaluate the outcome of "dredging" in the management of large cystic lesions of the jaws, without major resections and maintaining normal functions. Methodology: This prospective study was conducted on 30 patients. Inclusion criteria were odontogenic cystic lesions at least 2 cm in size either in the proximity of vital structures or in children with developing tooth germs or so large that upon enucleation, can result in pathological fracture. All underwent decompression followed by dredging performed at every 2 to 3 months intervals. Results: Drastic decrease in size of the lesion as measured on Cone Beam Computed Tomography (CBCT) and new bone formation was evident in all patients. The total Reduction Rate (RR) of the cystic lesions was 88.79% in 9.2 months. RR was highest in Calcifying epithelial odontogenic cyst (CEOC) followed by Unicystic Ameloblastoma (UA), Odontogenic Keratocysts (OKC), Dentigerous cyst (DC), and Radicular Cyst (RC) in CBCT. The mean speed of shrinkage was fastest in CEOC, followed by OKC, UA, RC, and DC after dredging. Shrinkage was higher in patients below 20 years of age (P 0.012) and patients with large cystic lesions (P 0.00447). Conclusion: The results suggest that dredging with regular, long-term follow-up can be a successful treatment method for large cystic lesions in young adults. This method includes the benefits of both conservative and radical treatment methods but lacks the disadvantages of either.
Combined orthodontic-surgical treatment for large jaw cysts with distal impacted teeth

Abstract:Objective:To summarize clinical experience of sixteen cases of large jaw cysts with distal impacted teeth treatment based on long-term follow-up,from the view of the orthodontist. Method:Sixteen cases of large jaw cysts with distal impacted teeth were collected from 2006 to 2014. The treatment consists of marsupialization of the cysts and preser-vation of permanent tooth.Obstructors were used after surgery to maintain the patency of the surgical opening.Panoramic ra-diograph was performed at the first,third and sixth month after sugery,as well as subsequent follow-up,to observe the posi-tion of impacted teeth. Orthodontic treatment or surgery assisted orthodontic traction was carried out if needed. Result:With the reducing of cyst cavity,return of distal impacted teeth were noticed in all 16 cases. The follow-up ranged from 1 to 5.5 years,with an average of 3 years. Further orthodontic treatment were also needed.The average orthodontic treatment duration was 2 years. Conclusion:Combined orthodontic-surgical treatment provides a new way for large jaw cysts with distal im-pacted teeth.
Decompression-first or direct enucleation: The choice of treatment for medium-sized odontogenic jaw cysts

Abstract:Background: The treatment of medium-sized odontogenic jaw cysts is inconsistent at present. Two main treatments, namely decompression and enucleation, are used overlappingly. This retrospective analysis aims to provide useful references for technique selection for future management of medium-sized odontogenic jaw cysts. Methods: Odontogenic cysts with lesion sizes ranging 2–4 cm were included. The clinical and radiological data of the patients were reviewed. Decompression-first and direct enucleation treatments were the two main surgical techniques. The preoperative parameters and postoperative outcomes were compared between the two groups. Results: Out of 69 patients included, 40 (58%) were in the decompression group and 29 (42%) in direct enucleation group. The logistic regression analysis of preoperative parameters demonstrated that the maximum lesion size and the chief surgeon's preference could affect the selection of surgical techniques for medium-sized cysts ( P 2.5 cm was the best cutoff value for predicting a decompression selection. Most postoperative outcomes differed significantly between the two groups ( P < 0.05). Lower-risk anesthesia, shorter hospitalization, tooth function protection, and fewer neurosensory impairments were decompression-favoring outcomes. However, more follow-up visits, more postoperative X-rays, and longer postoperative care were outcomes against decompression. The recurrence rate was low and did not differ significantly between the two groups ( P > 0.05). Conclusions: There is no apparent preference for treating medium-sized jaw cysts. The maximum lesion size is a moderate-impact factor for treatment selection. A tendency to prefer decompression-first with larger lesion size was found in medium-sized jaw cysts. The advantages of teeth preservation and low neurosensory impairment of decompression were verified in the medium-size jaw cysts. The burden of postoperative care should be considered when selecting decompression.
