Nigel Hawkes reports on how standardised mortality ratios at Mid Staffordshire NHS Foundation Trust came to conceal the hospital’s failings The new NHS in England aims to empower the patient and shame failing hospitals by measuring and publishing outcomes. Open data will make health providers honest and patients better able to choose between them. How simple is that? Rather too simple, to judge by evidence presented to the public inquiry into Mid Staffordshire NHS Foundation Trust, which has shown that the old NHS had ingenious ways of burnishing its outcomes and concealing its failings. Even death, the most unambiguous outcome of all, lost its sting when changes in coding practice resulted in many deaths being excluded from the calculation of hospital mortality ratios. Whether this amounted to actual dishonesty may never be determined. In his final submission Tom Kark, counsel to the inquiry, suggested that it may not be necessary for Robert Francis, the inquiry’s chair, to make any finding of fact over claims that Mid-Staffs and two other West Midlands trusts engaged in a plan to lower their apparent mortality ratios by changing the way patients were coded.1 Nor did Kark reach any firm conclusion over the suggestion that West Midlands Strategic Health Authority (SHA) sought to muddy the waters by commissioning a critical report into mortality measures, published in the BMJ, from academics known to disbelieve in the method. Both these charges were made in evidence to the inquiry by Professor Brian Jarman, the father of the hospital standardised mortality ratio (HSMR, see box), which has been used for the past decade by the health analytics company Dr Foster to compare hospital performance. Abstract:Whether seeking outpatient health maintenance, emergency care, inpatient management, or critical care, the preparation, skill, and competency of the clinician providing your care is paramount. When there is no preexisting relationship, one instead relies on education, credentialing, and licensing bodies to assess the clinician ability to render care that meets the existing standard before entering independent practice. One underlying assumption was that there was time spent during training that afforded regular and direct evaluation of knowledge and skills, including critical thinking. In complex care environments such as the ICU life-saving decisions and procedures are often time-sensitive events. The Clinical Teacher. 2014;11(6):425-428. we developed a novel approach to aiding final-year medical students prepare for this. A simulated teaching programme allowed students to experience the pressures of working on-call. We hoped to imitate stressors within a safe environment. Students were each given a bleep for an hour. Supervisors role-playing a concerned nurse "bleeped" the students. Each task was held in a folder on different wards (no patient interaction or information was involved). They were relatively simple and designed to stimulate resourcefulness, communication and triage skills. Various resources were available including the number for the medical registrar, played by supervisors. The final station was always the unwell patient aimed at drawing the student immediately to this scenario. A facilitated feedback session explored students' positive and negative experiences, concerns and coping mechanisms. Over the three years of this running, results were resoundingly positive with students taking great confidence from the programme. During the open feedback session, students valued using open wards and having to navigate in an unfamiliar hospital as a realistic preparation for next year. Being on-call is an inevitable part of a junior doctor's work and we believe there is scope for better preparation within undergraduate training. We have developed an effective and sustainable simulation that has shown excellent results. Due to the positive reaction and low maintenance of the project, we aim to cement our teaching programme as a permanent feature for undergraduate students at Blackpool Victoria Hospital.

Abstract:This column presents a way of conceptualizing the clinical practice of medicine including psychiatry within the framework of the scientific method. The goal is to aid practicing clinicians as well as trainees. This conceptual framework will improve the care of patients as it applies a discipline relative to giving time-limited trials of the various treatments available and then an assessment of whether the treatment worked adequately or not and what to do in the latter case. In this way, this approach should decrease the risk of excessive multiple medication use to treat a specific patient. Incorporating this conceptual framework early in the training of mental health care prescribers would be desirable.

Abstract:Introduction: An essential aim of courses in evidence-based medicine (EBM) is to improve the skills for reading and interpreting medical literature adequately. Regarding the conceptual framework, it is important to consider different educational levels. Aim: Our primary aim was to investigate the applicability of different instruments for the assessment of methodological study quality by 3rd grade students after short courses in EBM. Our secondary outcomes were agreement with expert assessments and student's knowledge and competences. Methods: We conducted four short courses in EBM of 90 minutes each for health care management and medical students focused on critical appraisal of the literature. At the end, the students assessed five publications about randomized controlled trials (RCTs) using five different instruments; the results were compared to expert assessments. Results: In total, 167 students participated in our EBM courses. Students' assessments showed a non-systematic over- and underestimation of risk of bias compared to expert assessments with no clear direction. Agreement with expert assessments ranged between 66% to over 80%. Across RCTs, evidence was found that the choice of instrument had an impact on agreement rates between expert and student assessments (p=0.0158). Three RCTs showed an influence of the instrument on the agreement rate (p<0.05 each). Discussion: Our results contrast sharply with those of many other comparable evaluations. Reasons may be a lack of students' motivation due to the compulsory courses, and the comparison to a reference standard in addition to self-ratings causing objectivity. Conclusion: Undergraduates should become familiar with the principles of EBM, including research methods, and the reading of scientific papers as soon as possible. For a deeper understanding, clinical experience seems to be an indispensable precondition. Based on our results, we would recommend an integration of lectures about EBM and critical appraisal at least twice during studies and with greater intensity shortly before graduation.

Abstract:How realistic are the predictions of the Five Year Forward View ? Since he published his Five Year Forward View in October, Simon Stevens has been walking on water. The chief executive of NHS England is (temporarily) able to do no wrong. Just as Melanesian islanders attribute magical powers to charismatic outsiders, Stevens has acquired a cargo cult of his own, deliciously lampooned in a series of tweets by the journalist Andy Cowper under the hashtag #SimonStevensfacts. Here’s a sample. “Hot? Bothered? Chant NHS England three times and Simon Stevens will appear with a chilled bottle of mineral water.” Impressive, but not quite as awe inspiring as another fact unearthed by Cowper. “Simon Stevens mined the Large Hadron Collider’s 27-km particle accelerator tunnels in a lunch break, using a plastic spoon.” Joking aside, it does seem that Stevens can currently get away with things that would bring derision on the heads of ordinary folk. For example, he has redefined the intervention process designed to rescue failing NHS services as a “success regime” rather than a “failure regime” to avoid demotivation and stigma. If his predecessor, David “Ironman” Nicholson, had tried something like this, laughter would have rolled irresistibly across the land. From Stevens it’s pure gold. Abstract:<b>SYNOPSIS:</b> Randomized controlled trials (RCTs) are ubiquitous in medicine and have facilitated great strides in clinical care. However, when applied in sport, RCTs have limitations that hinder implementing effective interventions in the real-world clinical setting. Pragmatic clinical trials offer some solutions. Yet due to the competitive, high-pressure nature of sport at the individual, team, and governing body level, RCTs are likely infeasible in certain sport settings. The small number of athletes at the elite team level, along with the potential financial consequences of randomizing at the individual athlete and team level, also restricts study power and feasibility, limiting conclusions. Consequently, researchers may need to "think outside the box" and consider other research methodology, to help improve athlete care. In this Viewpoint, we detail alternative study designs that can help solve real-world problems in sports medicine and performance, while maintaining robust research standards and accounting for the challenges that RCTs pose.
