Abstract:Purpose: The aim of the study was to assess vessel involvement and activity of Takayasu arteritis (TA) with whole-body magnetic resonance angiography (MRA) and vessel wall imaging.Methods: Whole-body MRA was performed in 42 patients with TA. In 23 of them, precontrast and postcontrast high-resolution vessel wall imagings were acquired. Three of 23 patients had follow-up imaging after medical treatment. According to TA activity criteria, these 26 vessel wall imagings were clinically divided into 2 groups: active (n = 18) and remissive group (n = 8). Vessel wall thickness and postcontrast signal intensity of vessel wall relative to adjacent muscle were analyzed.Results: Whole-body MRA yielded a diagnostic image of systemic and pulmonary artery. For vessel wall imaging, wall thickness of the active group (6.12 +/- 1.1 mm) was thicker than that of the remissive group (4.31 +/- 0.98 mm; P < 0.05). Postcontrast signal intensity ratio was significantly different between 2 groups (active vs remissive, 1.56 +/- 0.40 vs 1.17 +/- 0.21, P < 0.05). Based on the abnormalities revealed by combined whole-body MRA and vessel wall imaging, Lupi-Herrera's classification was modified in 8 of 23 patients.Conclusions: Whole-body MRA combined with vessel wall imaging had the potential for a comprehensive evaluation of disease extent and activity of TA.

Abstract:Background With the development of magnetic resonance (MR) technologies, whole-body 3D contrast-enhanced MR angiography (3D CE MRA) has become possible. The purpose of this study was to introduce and evaluate this technique in demonstration of various systemic arterial diseases.Methods Thirty-seven patients underwent whole-body 3D CE MRA using a 1.5T MR imager. The patients included were with clinically documented or suspected peripheral arterial occlusive disease (PAOD, n = 19), Takayasu arteritis (n = 8), polyarteritis nodosa (n = 1), Type B dissection (n = 4) and thoracic and/or abdominal aneurysm (n = 5). Sixty-eight surface coil elements were employed to encompass the whole body. Four 3D CE MRA stations were acquired successively through automatic table moving. A total scan range of 188 cm, covering the arterial tree from carotid artery to trifurcation vessels, was acquired. Overall image quality of each arterial segment and venous overlay were assessed and rated. The depiction of various systemic arterial diseases was evaluated and compared with other imaging modalities if available, including digital subtraction angiography (DSA), CT angiography, dedicated mono-station MRA.Results Whole-body 3D CE MRA was well tolerated by all patients. It yielded a detailed display of the arterial system with a short examination time. The image quality was considered diagnostic in 99.3% of the arterial segments. The remaining 0.7% of the arterial segments were considered non-diagnostic. In 7 of 19 patients with PAOD, whole-body MRA showed additional vascular narrowing apart from peripheral arterial disease. In 9 patients with vasculitis, whole-body MRA depicted luminal irregularity, narrowing or occlusion, aneurysm and collateral circulation involving multiple vascular segments. Whole-body MRA also clearly revealed the severity and extent of dissection and aortic aneurysm. In 20 cases the vascular pathologies demonstrated on whole body MRA were confirmed by other imaging investigations.Conclusions The whole-body MRA technique was non-invasive, quick and easy to perform. It was valuable for a comprehensive evaluation of vascular involvement of various systemic arterial diseases.
Value of Whole-Body Contrast-Enhanced Magnetic Resonance Angiography with Vessel Wall Imaging in Quantitative Assessment of Disease Activity and Follow-Up Examination in Takayasu’s Arteritis

Abstract:The aim of this study is to determine the value of whole-body contrast-enhanced magnetic resonance angiography(CE-MRI) with vessel wall imaging in quantitative assessments of Takayasu’s arteritis (TA) disease activity and follow-up examinations. Whole-body CE-MRI with vessel wall imaging (dark blood sequences) was performed in 52 TA patients and repeated in 15 patients after 6 months. Images were analyzed using quantitative scores. The distribution of Lupi-Herrera types (type III, 48.1 %; I, 40.4 %; II, 9.6 %; IV, 1.9 %) did not differ between active and inactive TA. Active vessel inflammation was found in seven patients diagnosed with inactive disease as Kerr scores and mainly involved the aortic arch, abdominal aorta, and ascending aorta. Quantitative MR scores were significantly higher in active TA (luminal stenosis 16.7 ± 5.3 vs. 4.2 ± 3.7, p < 0.01; wall thickening 7.2 ± 3.4 vs. 2.9 ± 2.3, p = 0.02; wall enhancement 8.7 ± 4.1 vs. 3.6 ± 2.4, p = 0.04) and positively correlated with Kerr scores, ITAS 2010, erythrocyte-sedimentation rate (ESR), and C-reactive protein (CRP) and pentraxin-3 (PTX-3) levels. At 6 months, the clinical symptoms, CRP level, and ESR improved significantly (p < 0.05) and wall enhancement decreased (6.7 ± 3.1 vs. 4.1 ± 2.1; p = 0.04), but the luminal stenosis (10.2 ± 4.3 vs. 8.8 ± 5.2; p = 0.12) and wall thickening (6.3 ± 3.8 vs. 5.8 ± 4.2; p = 0.27) remained unchanged. Whole-body CE-MRI with vessel wall imaging detected luminal changes and vessel wall inflammation in TA. Our MR scoring system enabled quantitative assessment of TA activity.
Magnetic Resonance Angiography and Multidetector CT Angiography in the Diagnosis of Takayasu's Arteritis: Assessment of Disease Extent and Correlation with Disease Activity

Abstract:Background: Takayasu's Arteritis (TA) is a large vessel vasculitis with diverse clinical presentations and arterial vascular bed involvement.It is characterized by chronic, nonspecific inflammation of all layers of the vessel wall which results in stenosis, occlusion, dilatation, or aneurysm formation in the involved blood vessels. Methods: The study included 36 patients of TA. All patients fulfilled the modified Ishikawa's diagnostic criteria for TA. All patients were evaluated for clinical presentation, angiographic findings, and severity of the disease. The disease activity was assessed based on Erythrocyte Sedimentation Rate (ESR) and C-Reactive Protein (CRP) and also by CT Angiography (CTA)/Magnetic resonance angiography (MRA) imaging. The angiographic types were classified based on the International TA Conference in Tokyo 1994 angiographic classification. Results: A total of 36 patients were included in the study, 86% were females and a mean age of 21.6 years. Hypertension (78%) was the most common clinical presentation. Type V was the most common angiographic type (42%) followed by type III (25%), type IV (14%), type IIb (11%), type I (5%) and type IIa (3%). Among the aortic arch branches the left subclavian artery (50%), right subclavian artery (38.8%), left vertebral artery (33.3%) and left common carotid artery (27.7%) were the most commonly involved arteries. Disease activity based on CT / MR imaging showed a significant statistical correlation with elevated ESR and positive CRP (p < 0.0001). Mediastinal lymphadenopathy was seen in 21 patients out of which 11 had active disease. However, no significant correlation was found between mediastinal lymphadenopathy and disease activity. Conclusion: TA presents with varied symptomatology and differing vascular involvement. CT/MR angiography is effective in diagnosis and accurately predicted the active stage of the disease.

Abstract:BACKGROUND:To measure and assess the disease activity of Takayasu arteritis (TA) quantitatively by magnetic resonance angiography (MRA).METHODS:Twenty-six Chinese TA patients were divided into an active group and an inactive group. All patients underwent delayed contrast-enhanced MRI. The vessels involved were categorized into type A (main branches of aorta and its arch) and type B (secondary or tertiary branches). Three quantitative scores were defined to gauge lumen stenosis, vessel wall thickness, and vessel wall enhancement, and compared between groups and types.RESULTS:The patients in the active group had more stenosis in left subclavian artery than those in the inactive group (14/16, 87.5% vs. 2/10, 20%; p<0.01), and greater vessel wall thickness in left common carotid artery (11/16, 68.75% vs. 1/10, 10%; p<0.01) and left subclavian artery (9/16, 56.25% vs. 0/10, 0%; p<0.01). The differences between active and inactive TA were significant in type A (lumen stenosis: 11.13 ± 6.17 vs. 4.00 ± 4.32; p<0.001; wall thickness: 8.00 ± 5.05 vs. 3.10 ± 3.31; p=0.01; wall enhancement: 6.94 ± 5.34 vs. 2.90 ± 4.36; p=0.05), and pronounced in type A+B (lumen stenosis: 13.31 ± 7.19 vs. 6.30 ± 5.60; p=0.02; wall thickness: 8.50 ± 5.29 vs. 3.40 ± 3.34; p=0.01; wall enhancement: 7.94 ± 6.55 vs. 3.20 ± 4.32, p=0.05, respectively). These three scores were moderately correlated to CRP, platelet count and fibrinogen levels (p<0.05).CONCLUSIONS:The MRI scoring system of lumen stenosis, wall thickness and wall enhancement could be a non-invasive approach to facilitate assessment in TA activity.

Abstract:Background: Determination of disease activity in Takayasu arteritis (TAK) is crucial for clinical management but challenging. The value of different magnetic resonance imaging (MRI) characteristics for the assessment of disease activity remains unclear. This study investigated the imaging findings of the thoracic aortic wall and elasticity by using a comprehensive 3.0 T MRI protocol. Methods: We prospectively enrolled 52 consecutive TAK patients. TAK activity was recorded according to the ITAS2010. All the patients underwent thoracic aortic MRI. The luminal morphology of the thoracic aorta and its main branches were quantitatively evaluated using a contrast-enhanced magnetic resonance angiography (MRA) sequence. The maximum wall thickness of the thoracic aorta, postcontrast enhancement ratio, and aortic wall edema were analyzed in each patient through pre- and post-enhanced TI-weighted and T2-weighted imaging. Pulse-wave velocity (PWV) of the thoracic aorta was calculated using a four-dimensional flow technique. Results: The majority of the 52 patients had type V disease (34.62%, 18/52). Among all the MRI indicators of the thoracic aorta, the area under the curve was the largest for the maximal wall thickness (0.804, 95% confidence interval [CI] = 0.667-0.941). The maximal wall thickness (93.33%, 95% CI = 68.1%-99.8%) exhibited the highest sensitivity with a cutoff value of 3.12 mm. Wall edema (84.00%, 95% CI = 63.9%-95.5%) presented the highest specificity. A positive correlation was noted between PWV and patients' age (r = 0.54,p < 0.001), disease duration (r = 0.52, p < 0.001), and the maximum wall thickness (r = 0.45, p = 0.001). Conclusions: MRI enabled the comprehensive assessment of aortic wall morphology and functional markers for TAK disease activity. Aortic maximal wall thickness was the most accurate indicator of TAK activity. The early phase was superior to the delay phase for aortic wall enhancement analysis for assessing TAK activity.

The spatial resolution was 1.6 x 1.0 mm and slice thickness was 1.5 mm for all stations. A total scan range of 188 cm was acquired. Overall image quality of each arterial segment and venous overlay were assessed. The depiction of various systemic arterial diseases was evaluated and compared, in 20 patients, with other imaging modalities. This WB 3D CE MRA yielded a detailed display of the arterial system with an average MR room time of 17.4 min. The image quality was considered diagnostic in 99.3% of the arterial segments. In 7 of 19 patients with PAOD, WB MRA showed additional vascular narrowing apart from peripheral arterial disease. In nine patients with vasculitis, WB MRA depicted luminal irregularity, narrowing or occlusion, aneurysm, and collateral circulation involving multiple vascular segments. WB MRA also clearly revealed the severity and extent of dissection and aortic aneurysm. In 20 cases where additional imaging investigations have been carried out, the vascular pathologies demonstrated by WB MRA agree with these additional imaging investigations.
Assessment of Takayasu's arteritis activity by ultrasound localization microscopy

Abstract:Background: Ultrasound localization microscopy (ULM) based on ultrafast ultrasound imaging of circulating microbubbles (MB) can image microvascular blood flows in vivo up to the micron scale. Takayasu arteritis (TA) has an increased vascularisation of the thickened arterial wall when active. We aimed to perform vasa vasorum ULM of the carotid wall and demonstrate that ULM can provide imaging markers to assess the TA activity. Methods: Patients with TA were consecutively included with assessment of activity by the National Institute of Health criteria: 5 had active TA (median age 35.8 [24.5-46.0] years) and 11 had quiescent TA (37.2 [31.7-47.3] years). ULM was performed using a 6.4 MHz probe and a dedicated imaging sequence (plane waves with 8 angles, frame rate 500 Hz), coupled with the intravenous injection of MB. Individual MB were localised at a subwavelength scale then tracked, allowing the reconstruction of the vasa vasorum flow anatomy and velocity. Findings: ULM allowed to show microvessels and to measure their flow velocity within the arterial wall. The number of MB detected per second in the wall was 121 [80-146] in active cases vs. 10 [6-15] in quiescent cases (p = 0.0005), with a mean velocity of 40.5 [39.0-42.9] mm.s-1 in active cases. Interpretation: ULM allows visualisation of microvessels within the thickened carotid wall in TA, with significantly greater MB density in active cases. ULM provides a precise visualisation in vivo of the vasa vasorum and gives access to the arterial wall vascularisation quantification. Funding: French Society of Cardiology. Magnetic resonance imaging of vascular changes in Takayasu arteritis

Abstract:We studied the thoracic aorta in 42 female patients with Takayasu arteritis by magnetic resonance imaging (MRI) with a 1.5-T superconducting instrument and compared the findings with those in 10 healthy female controls. Images in the transaxial and oblique sagittal planes were acquired by cardiac gating using a spin-echo pulse sequence. Images in the oblique plane were also obtained with cine MRI. Vascular changes were found in 41 (98%) of the patients. Dilatation of the ascending aorta was observed in 20 patients (48%), while stenotic changes were more characteristic in the descending aorta (16 patients, 38%). Moderate signal-intensity masses on the aortic wall were encountered in 21 patients (50%), suggesting mural thrombi. Turbulent blood flow was observed around all masses by cine MRI. HLA analysis revealed that patients carrying HLA-B52 typing showed these masses more frequently than those without this typing (P < 0.05). Cine MRI demonstrated aortic regurgitation in 20 patients (48%), all of whom showed dilated ascending aorta; furthermore, 14 (70%) of them were confirmed as carrying HLA-B52. Thus, MRI was a useful non-invasive tool not only for the evaluation of the morphological changes in the aorta and its branches, and of the hemodynamic disturbance in Takayasu arteritis, but also for follow-up of the morbid condition.

Abstract:Takayasu arteritis is a chronic, idiopathic, inflammatory disease that primarily affects large vessels, such as the aorta and its major branches and the pulmonary and coronary arteries. The non-specific inflammation of involved vessels usually leads to concentric wall thickening, fibrosis and thrombus formation. Diseased arteries become stenotic or occluded, undergo vascular remodelling or develop aneurysms. According to the involvement of arteries, six types of Takayasu arteritis are documented. The purpose of this pictorial review is to illustrate the various multidetector CT angiography appearances of Takayasu arteritis and to discuss the differential diagnosis.

Abstract:Objective: The goal of this study is to use superb microvascular imaging (SMI) to observe neovascularization in the carotid vessel wall to identify potential Takayasu's arteritis (TAK) inflammation markers. Methods: Bilateral carotid arteries from 96 patients with TAK were imaged by a Doppler ultrasound and SMI. The one-way analysis of variance (ANOVA) was used to document significant differences between the activity and inactivity stages of TAK and the factors closely related to its activity in the binary logistics regression equation. Clinical and laboratory data included age, gender, duration of disease, treatment history, NIH score, erythrocyte sedimentation rate (ESR), and high-sensitivity C-reactive protein. Imaging data included the arterial wall thickness, degree of lesion, SMI grade, and arterial aneurysm formation. Results: There were 45 patients in the active TAK stage and 51 in the inactive stage. The one-way ANOVA showed significant differences in SMI ( p = 0.001) and ESR ( p = 0.022) between the active and inactive groups. The binary logistics regression analysis showed that SMI was an independent risk factor for TAK activity (B = −1.505, S.E = 0.340, Wald = 19.528, OR = 0.222 95%, CI = 0.114–0.433, p < 0.01). Using SMI G1 or G2 as the cutoff values for the diagnosis of active TAK, the positive predictive value, sensitivity, and specificity were 60 and 86%, 84% and 56%, and 54% and 92%, respectively. Conclusion: The SMI grade is a potential marker of disease activity in patients with TAK.

A Novel Ultrasound-Based Score for Assessing Carotid Artery Activity in Takayasu's Arteritis

Abstract:OBJECTIVES:The role of ultrasonography for evaluating vessel wall inflammation in Takayasu's arteritis (TAK) is well-recognised; however, an effective approach for the quantitative assessment of disease activity remains lacking. This study aimed to develop a novel ultrasound-based score for determining TAK activity. METHODS:TAK patients with carotid artery involvement were prospectively followed-up for 6 months. Our proposed ultrasonographic activity score (ULTRAS, range between 0-12) consisted of wall thickness (TS, range between 0-8) and semi-quantitative echogenicity scores (ES, range between 0-4). The diagnostic performance of ULTRAS for disease activity was evaluated in terms of area under the receiver operating characteristic curve (AUC). Internal validation was subsequently performed. RESULTS:The patients were divided into training and validation groups (n=136 and 30. respectively). In the training group, 83 (61.0%) had active disease. At an optimal cut-off of 7, ULTRAS showed good diagnostic accuracy for active TAK (AUC, 0.88; 95% CI, 82-94). Improved diagnostic performance was achieved when combined with ESR (AUC, 0.91; 95% CI, 86-96) or CRP (AUC, 0.90; 95%CI, 86-95). In the verification group, the AUCs were 0.88, 0.95, and 0.92 for ULTRAS, ESR plus ULTRAS, and CRP plus ULTRAS, respectively. At post-treatment follow-up, the TS, ES, and ULTRAS paralleled the patients' disease remission and symptom recovery. At 3-month follow-up, an improvement in wall thickness of ≥0.3 mm correlated with symptom recovery in 50% of the patients. CONCLUSIONS:Our proposed ultrasound-based score carries the potential in the detection of active disease among TAK patients.

Abstract:Objective To investigate the current situation in Chinese rheumatologic physicians' clinical diagnosis and evaluation of Takayasu's arteritis (TA).Methods Nineteen rheumatology experts and three vascular surgery specialists in China were invited to make the nationwide investigation for the first time about the diagnosis and disease activity evaluation of TA in China,through the questionnaire survey on the internet.Weighted average was used to calculate the average scores of corresponding problems.Results Chinese experts mainly adopted 1990 American College of Rheumatology (ACR) classification criteria for clinical diagnosis of TA.In details,symptoms of age,limb claudication and amaurosis,signs including pulselessness or pulse weakening,vascular bruits,increasing bilateral pulse pressure and hypertension and acute phase reactants (APR) were critical to the clinical diagnosis of TA.Besides,noninvasive imaging examinations,such as computed tomography angiography (CTA),magnetic resonance angiography (MRA),vascular ultrasonography,and positron emission tomography (PET) were also of great importance.In the aspect of disease activity assessment,Chinese experts mainly used Kerr scoring tool.APR and noninvasive radiological examinations were considered with vital value.Some TA patients with carotid artery involvement were recommended using vascular ultrasonography,while others with pulmonary artery and thoracic/abdominal aorta trunk involvement were preferred CTA other than MRA.Conclusions APR and noninvasive imaging examinations were thought with great help to make clinical diagnosis and evaluation of TA for Chinese physicians.
Whole-body MR Angiography with 32-Receiving Channels and Parallel Imaging Techniques: a Preliminary Clinical Study

Abstract:Objective To preliminarily evaluate a novel whole-body 3D contrast-enhanced(CE) MR angiography(MRA) technique in the demonstration of various systemic arterial diseases.Methods(Thirty patients) underwent whole-body 3D CE MRA using a 1.5 T MR imager(Magnetom Avanto,Siemens AG) equipped with 32 receiving channels and integrated parallel acquisition techniques(iPAT).The patients included were with clinically documented peripheral arterial occlusive disease (PAOD,n=17),Takayasu arteritis(n=3),polyarteritis nodosa(n=1),Type B dissection(n=4),and thoracic and/or abdominal aneurysm(n=5).68 surface coil elements were employed to encompass the whole body.Four 3D CE MRA stations with each field-of-view of 50 cm were acquired successively through automatic table moving.The gadolinium contrast medium dose was 0.3 mmol/kg and a biphasic injection was employed with 0.15 mmol/kg injected at a rate of 1.3 ml/s and another 0.15 mmol/kg at a rate of 0.6 ml/s.A total scan range of 188 cm,covering the arterial tree from carotid artery to trifurcation vessels,was acquired.Overall image quality of each arterial segment was assessed.The depiction of various systemic arterial diseases was evaluated and compared to other available imaging modalities including X-ray angiography in nine patients,CT angiography in eight patients,and mono-station MRA in one patient.Results Whole-body 3D CE MRA was well tolerated by all patients.It yielded a detailed display of the arterial system with an average room time of 17.4 min.In seven of 17 patients with PAOD,whole-body MRA showed additional vascular narrowing apart from peripheral arterial disease.In four patients with vasculitis,whole-body MRA depicted luminal irregularity,narrowing or occlusion,aneurysm,and collateral circulation involving multiple vascular segments.Whole-body MRA also clearly revealed the severity and extent of dissection and aortic aneurysm.In cases with other available imaging techniques,the demonstration of vascular pathology all agreed except one case of overestimation of stenosis by whole body MRA.Conclusion Our results showed this 32-receiving-channel MR system with surface coil technology allowed a quick and easy-to-perform whole-body MRA.It was very valuable for a comprehensive evaluation of vascular involvement of various systemic arterial diseases.
Dark-Blood Computed Tomography Angiography Combined With Deep Learning Reconstruction for Cervical Artery Wall Imaging in Takayasu Arteritis

Abstract:Objective: To evaluate the image quality of novel dark-blood computed tomography angiography (CTA) imaging combined with deep learning reconstruction (DLR) compared to delayed-phase CTA images with hybrid iterative reconstruction (HIR), to visualize the cervical artery wall in patients with Takayasu arteritis (TAK). Materials and methods: This prospective study continuously recruited 53 patients with TAK (mean age: 33.8 ± 10.2 years; 49 females) between January and July 2022 who underwent head-neck CTA scans. The arterial- and delayed-phase images were reconstructed using HIR and DLR. Subtracted images of the arterial-phase from the delayed-phase were then added to the original delayed-phase using a denoising filter to generate the final-dark-blood images. Qualitative image quality scores and quantitative parameters were obtained and compared among the three groups of images: Delayed-HIR, Dark-blood-HIR, and Dark-blood-DLR. Results: Compared to Delayed-HIR, Dark-blood-HIR images demonstrated higher qualitative scores in terms of vascular wall visualization and diagnostic confidence index (all P < 0.001). These qualitative scores further improved after applying DLR (Dark-blood-DLR compared to Dark-blood-HIR, all P < 0.001). Dark-blood DLR also showed higher scores for overall image noise than Dark-blood-HIR (P < 0.001). In the quantitative analysis, the contrast-to-noise ratio (CNR) values between the vessel wall and lumen for the bilateral common carotid arteries and brachiocephalic trunk were significantly higher on Dark-blood-HIR images than on Delayed-HIR images (all P < 0.05). The CNR values were significantly higher for Dark-blood-DLR than for Dark-blood-HIR in all cervical arteries (all P < 0.001). Conclusion: Compared with Delayed-HIR CTA, the dark-blood method combined with DLR improved CTA image quality and enhanced visualization of the cervical artery wall in patients with TAK.

Abstract:Objective Few reports are seen on Takayasu arteritis(TA).This paper was to evaluate the CT presentations of TA and the role CT in its diagnosis and follow-up by analyzing the CT angiography(CTA) of the disease.Methods We retrospectively analyzed 27 cases of clinically confirmed TA,that underwent dual-source CTA of the neck,thorax and abdomen(n=7),neck and thorax(n=1),thorax and abdomen(n=8),abdomen(n=4),or whole body(n=7).Six of the patients were followed up by CT studies postoperatively.Results Of the 378 vascular segments evaluated,245 were found involved,with obvious thickening of the vascular walls,42 segments in 16 cases with severe stenosis and/or occlusion,20 segments in 15 cases with luminal extension,3 segments in 3 cases with aneurysm,and 11 cases with vascular wall calcification.Five patients exhibited pulmonary artery involvement and associated pulmonary hypertension,5 presented severe renal artery stenosis and/or occlusion and associated renal atrophy,and 19 showed collateral circulation.Of the 6 patients followed up by CT studies,4 underwent stent placement,and 2 bypass graft.Follow-up findings revealed stent-in restenosis in 1 case,but no abnormality in all the other 5.Conclusion Vascular thickening,stenosis or occlusion and associated collateral circulation,and poststenosis dilation are common CT findings of TA.Dual-source CTA plays a valuable role in the diagnosis and follow-up of TA.

Abstract:A 26-year-old woman with Takayasu's arteritis (TAK) experienced back and neck pain during tocilizumab (TCZ) treatment. The levels of C-reactive protein were normal, and ultrasonography revealed no significant changes. Diffusion-weighted whole-body imaging with background body signal suppression (DWIBS) showed signal enhancement in the walls of several arteries. Contrast computed tomography showed arterial inflammation in the same lesion. After increasing the dose of prednisolone and TCZ, all signal enhancements decreased and continued to decrease, as observed on days 76 and 132. Thus, DWIBS may be a novel imaging modality for assessing the disease activity of TAK, particularly during follow-up.

Detection of Mural Inflammation with Low B-Value Diffusion-Weighted Imaging in Patients with Active Takayasu Arteritis.

Abstract:OBJECTIVES:To evaluate the performance of low b-value diffusion-weighted imaging (DWI) for detection of inflamed vessels in active Takayasu arteritis (TA). METHODS:Forty patients with active TA involving the thoracic aorta and its super-aortic branches underwent low b-value (50 s/mm2) DWI, T2-weighted imaging (T2WI), and delayed enhancement T1-weighted imaging (DEI). Corresponding images on these 3 sequences at the same diseased level were evaluated qualitatively and quantitatively using Friedman and Kruskal-Wallis test, and the agreement between them in detection of inflamed vessels was assessed using Cochran's Q test. RESULTS:The overall image quality of DEI, DWI, and T2WI was scored 7.97 ± 1.15, 7.32 ± 1.73, and 6.51 ± 1.69 respectively. The score of DEI and DWI was higher than that of T2WI (p < 0.001). The quality of blood suppression was rated higher in DWI than T2WI and DEI (p < 0.001). Both the signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) of the diseased vessel walls measured on DEI and DWI were significantly higher than those on T2WI (p < 0.001). However, there was no significant difference in SNR and CNR between DEI and DWI (p = 0.283 and 0.063). In detection of mural inflammation, significant advantage was observed when comparing the findings from DEI/DWI to those from T2WI (p < 0.001). But no significant difference was found between the findings of DWI and DEI (p > 0.99). CONCLUSIONS:Low b-value DWI may be used as a promising alternative to DEI for detecting inflamed vessels in active TA. KEY POINTS:• Currently, the most widely used imaging modality in detection of mural inflammation is contrast-enhanced MRI. • Low b-value DWI is shown comparable to contrast-enhanced MRI and superior to T2WI in identifying mural inflammation in patients with active Takayasu arteritis. • Low b-value DWI is a fast and unenhanced MRI technique which may potentially replace contrast-enhanced MRI in identifying disease activity of Takayasu arteritis.
PET Vascular Activity Score for Predicting New Angiographic Lesions in Patients with Takayasu Arteritis: a Chinese Cohort Study.

Abstract:Objective To investigate the ability of F-18-fluorodeoxyglucose PET/CT to predict new lesions in Takayasu arteritis. Methods Eighty-two Chinese patients with newly diagnosed Takayasu arteritis were recruited. Their clinical characteristics, serum biomarkers and imaging results were recorded at baseline and every visit. They were followed up for at least 2 years. New angiographic lesions were evaluated by magnetic resonance angiography. Baseline PET vascular activity scores (PETVAS) for predicting new lesions were evaluated. Results At baseline, a moderate correlation was observed between PETVAS and ESR (r = 0.74, P < 0.01) and CRP level (r = 0.69, P < 0.01). Overall, 18 (22%) patients showed new lesions on imaging during a median follow-up time of 36 months. The median time to the first occurrence of new lesions was 18 months. Compared with patients without new lesions, the patients with new lesions included more female patients (67.2% vs 94.4%, P = 0.03), patients with higher ESR values (20 vs 49, P = 0.02) and patients with active disease (62.5% vs 94.4%, P < 0.01). Multivariate Cox regression analysis revealed PETVAS was an independent risk factor for new angiographic lesions (PETVAS >= 8, hazard ratio = 7.56; 95% CI 2.20, 26.01, P < 0.01) with adjustment of age, sex, chest pain, ESR and Physician Global Assessment. Furthermore, patients with PETVAS >= 8 at baseline were more likely to experience adverse events including arterial ischaemic events during the follow-up. Conclusion PETVAS showed good performance in predicting new lesions in Takayasu arteritis.
Correlation between Ultra-Wide-Field Retinal Imaging Findings and Vascular Supra-Aortic Changes in Takayasu Arteritis

Abstract:(1) Background: Takayasu arteritis (TA) is a chronic inflammatory large-vessel vasculitis. Ultra-wide-field imaging allows describing the retinal lesions in these patients and correlating them with vascular supra-aortic stenosis. (2) Methods: In total, 54 eyes of 27 patients diagnosed with TA were included, and a complete ophthalmological examination was performed, including UWF color fundus photography (UWF-CFP), fluorescein angiography (UWF-FA), and computed tomography angiography measuring supra-aortic stenosis. Eleven patients underwent Doppler ultrasound imaging assessing the blood flow velocity (BFV) in the central retinal artery (CRA). (3) Results: Microaneurysms were detected in 18.5% of eyes on fundus examination, in 24.4% of eyes on UWF-CFP, and in 94.4% of eyes on UWF-FA. The number of microaneurysms significantly correlated with the presence of an ipsilateral supra-aortic stenosis (p = 0.026), the presence of hypertension (p = 0.0011), and the duration of the disease (p = 0.007). The number of microaneurysms per eye negatively correlated with the BFV in the CRA (r = −0.61; p = 0.003). (4) Conclusions: UWF-FA improved the assessment of TA-associated retinal findings. The significant correlation between the number of microaneurysms and the BFV in the CRA gives new insight to our understanding of Takayasu retinopathy. The total number of microaneurysms could be used as an interesting prognostic factor for TA.
