Accelerated Healing of Infected Diabetic Wounds by a Dual-Layered Adhesive Film Cored with Microsphere-Loaded Hydrogel Composite Dressing

Abstract:Diabetic wounds, a prevalent chronic disease, are associatedwitholder age. The hyperglycemic microenvironment in diabetic wounds significantlyreduces the immune system, inducing bacterial invasion. The couplingof tissue repair and antibacterial treatment is critical for infecteddiabetic ulcer regeneration. In this study, a dual-layered sodiumalginate/carboxymethyl chitosan (SA/CMCS) adhesive film cored withan SA-bFGF microsphere-loaded small intestine submucosa (SIS) hydrogelcomposite dressing with a graphene oxide (GO)-based antisense transformationsystem was developed to promote infected diabetic wound healing andbacterial eradication. Initially, our injectable SIS-based hydrogelcomposite stimulated angiogenesis, collagen deposition, and immunoregulationin diabetic wound repair. The GO-based transformation system subsequentlyinhibited bacterial viability in infected wounds by post-transformationregulation. Meanwhile, the SA/CMCS film provided stable adhesion coveringthe wound area to maintain a moist microenvironment, which promoted in situ tissue repair. Our findings provide a promisingclinical translation strategy for promoting the healing of infecteddiabetic wounds.
