Abstract:Continuous bleeding, bacterial infection, oxidative stress, and vascular damage within a diabetic wound microenvironment necessitate the development of multifunctional dressings that match the complex physio-logical process of skin healing. Herein, copper-tannic acid (CuTA) nanosheets were first synthesized by chelating copper ions with tannic acid (TA). These nanosheets were subsequently integrated into an injectable self-healing hydrogel comprising methacrylic anhydride-modified gelatin (GelMA), cationic guar gum (CG), and borax. Through free-radical polymerization as well as hydrogen and borate ester bonds formation, we fabricated an acidic and highly reactive oxygen species (ROS)-responsive composite hydrogel named GGB-CT, which exhibited remarkable hemostasis and adhesion. With responsive decomposition of GGB-CT, the released CuTA efficiently scavenged excess ROS as well as effectively killed bacteria by inhibiting arginine synthesis, blocking the tricarboxylic acid cycle and inducing cuproptosis-like death in the tested microbes. In the later stages of wound healing, the composite hydrogel substantially promoted macrophage polarization and angiogenesis, thus accelerating the re-epithelialization of the wound area. Overall, this study proposes an innovative hydrogel for treating infected diabetic wounds and may inspire new thinking of high-performance hydrogels for biomedical applications.

Abstract:Diabetic wounds tend to develop into nonhealing wounds associated with the complex inflammatory microenvironment of uncontrollable bacterial infection, reactive oxygen species (ROS) accumulation, and chronic hypoxia. Damaged blood vessels hinder metabolic circulation, aggravating hypoxia, and ROS accumulation and further exacerbating the diabetic wound microenvironment. However, existing treatments with a single functionality have difficulty healing complicated diabetic wounds. Therefore, developing an integrative strategy to improve the hostility of the diabetic wound microenvironment is urgently needed. Herein, multifunctional genipin (GP)-crosslinked chitosan (CS)-based hydrogels decorated with the biomimetic metal-organic framework (MOF)-nanozymes and the natural antibacterial agent chlorogenic acid (CGA), which is named MOF/CGA@GP-CS (MCGC), are prepared. With catalase (CAT)-like activity, these dual-metal MOF-nanozymes are promising bioreactors for simultaneously alleviating ROS accumulation and hypoxia by converting elevated endogenous H2O2 into dissolved oxygen in diabetic wounds. In addition, the other component of natural polyphenolic CGA acts as a mild antibacterial agent, efficiently inhibiting wound infection and avoiding antibiotic resistance. Impressively, the MCGC hydrogels accelerate infected diabetic wound healing by eliminating oxidative stress, increasing oxygenation, and reversing bacterial infection in vivo. In this work, an effective strategy based on multifunctional hydrogel wound dressings is successfully developed and applied in diabetic wound management.
DISCREPANCY BETWEEN BIOAVAILABILITY AND HYPOTENSIVE EFFECT OF ORAL AND SUBLINGUAL NIFEDIPINE

Abstract:Nifedipine, 10-mg capsules, were given orally and sublingually to six healthy volunteers according to a randomized crossover design. Nifedipine plasma levels, blood pressure, and heart rate were determined at several times after medication. Cmax was higher (134 ± 17 vs. 93 ± 2 ngml-1, mean ± SD, P < 0.01) and occurred earlier (0.5 vs. 1 h) with oral than with sublingual nifedipine. However, there was no significant difference in AUC (268 ± 56 vs. 288 ± 35 ng h ml-1) nor in f1/2 (1.8 ± 0.2 vs. 1.9 ± 0.3 h), indicating that sublingual administration decreased the rate but not the extent of nifedipine absorption. Notwithstanding the difference in Cmax, both routes yielded a similar reduction in diastolic blood pressure of 13 ± 1 mm Hg. Heart rate increase, which reflects the activation of homeostatic mechanisms, was greater with oral than with sublingual nifedipine, that is, 18 ± 1 vs. 13 ± 1 beats min1, P < 0.01. It is concluded that slower absorption after sublingual administration increases nifedipine hypotensive efficiency by producing less counteracting homeostatic responses than the more rapidly absorbed oral nifedipine.
A Nanozyme‐immobilized Hydrogel with Endogenous ROS‐scavenging and Oxygen Generation Abilities for Significantly Promoting Oxidative Diabetic Wound Healing

Abstract:Non‐healing wound is a common complication of diabetic patients associated with high morbidity and mortality. Engineered therapeutic hydrogels have enviable advantages in tissue regeneration, however, they are suboptimal for the healing of diabetic wounds characterized by reactive oxygen species (ROS) accumulation and chronic hypoxia. Here, we present a unique biological metabolism‐inspired hydrogel for ameliorating this hostile diabetic microenvironment. Consisting of natural polymers (hydrazide modified hyaluronic acid and aldehyde modified hyaluronic acid) and a metal‐organic frameworks derived catalase‐mimic nanozyme (ε‐polylysine coated mesoporous manganese cobalt oxide), the engineered nanozyme‐reinforced hydrogels can not only capture the endogenous elevated ROS in diabetic wounds, but also synergistically produce oxygen through the ROS‐driven oxygen production ability. These fascinating properties of hydrogels protected skin cells (e.g., keratinocytes, fibroblasts, and vascular endothelial cells) from ROS and hypoxia‐mediated death and proliferation inhibition. Diabetic wounds treated with the nanozyme‐reinforced hydrogels highlighted the potential of inducing the macrophages polarization from pro‐inflammatory phenotype (M1) to anti‐inflammatory subtype (M2). The hydrogel dressings demonstrated a prominently accelerated healing rate as featured by alleviating the excessive inflammatory, inducing efficiently proliferation, re‐epithelialization, collagen deposition, and neovascularization. This work provides an effective strategy based on nanozyme‐reinforced hydrogel as a ROS‐driven oxygenerator for enhancing diabetic wound healing. This article is protected by copyright. Enhanced diabetic wound healing with injectable hydrogel containing self-assembling nanozymes

Abstract:To build a smart system in response to the variable microenvironment in infected diabetic wounds, a multifunctional wound dressing was constructed by co-incorporating glucose oxidase (GOx) and a pH-responsive self-assembly Cu2-xSe-BSA nanozyme into a dual-dynamic bond cross-linked hydrogel (OBG). This composite hydrogel (OBG@CG) can adhere to the wound site and respond to the acidic inflammatory environment, initiating the GOx-catalyzed generation of H2O2 and the self-assembly activated peroxidase-like property of Cu2-xSe-BSA nanozymes, resulting in significant hydroxyl radical production to attack the biofilm during the acute infection period and alleviate the high-glucose microenvironment for better wound healing. During the wound recovery phase, Cu2-xSe-BSA aggregates disassembled owing to the elevated pH, terminating catalytic reactive oxygen species generation. Simultaneously, Cu2+ released from the Cu2-xSe-BSA not only promotes the production of mature collagen but also enhances the migration and proliferation of endothelial cells. RNA-seq analysis demonstrated that OBG@CG exerted its antibacterial property by damaging the integrity of the biofilm by inducing radicals and interfering with the energy supply, along with destroying the defense system by disturbing thiol metabolism and reducing transporter activities. This work proposes an innovative glucose consumption strategy for infected diabetic wound management, which may inspire new ideas in the exploration of smart wound dressing.

Abstract:Chronic diabetic wounds persistently face the threat of evolving into diabetic foot ulcers owing to severe hypoxia, high levels of reactive oxygen species (ROS), and a complex inflammatory microenvironment. To concurrently surmount these obstacles, we developed an all-round therapeutic strategy based on nanozymes that simultaneously scavenge ROS, generate O2 and regulate the immune system. First, we designed a dynamic covalent bond hybrid of a metal-organic coordination polymer as a synthesis template, obtaining high-density platinum nanoparticle assemblies (PNAs). This compact assembly of platinum nanoparticles not only effectively simulates antioxidant enzymes (CAT, POD) but also, under ultrasound (US), enhances electron polarization through the surface plasmon resonance effect, endowing it with the ability to induce GSH generation by effectively replicating the enzyme function of glutathione reductase (GR). PNAs, by mimicking the activity of CAT and POD, effectively catalyze hydrogen peroxide, alleviate hypoxia, and effectively generate GSH under ultrasound, further enhancing ROS scavenging. Notably, PNAs can regulate macrophage responses in the inflammatory microenvironment, circumventing the use of any additives. It was confirmed that PNAs can enhance cell proliferation and migration, promote neoangiogenesis in vitro, and accelerate the healing of infected diabetic wounds in vivo. We believe that an all-round therapeutic method based on PNA nanozymes could be a promising strategy for sustained diabetic wound healing.

Self-healing and shape-adaptive nanocomposite hydrogels with anti-inflammatory, antioxidant, antibacterial activities and hemostasis for real-time visual regeneration of diabetic wounds

Abstract:Diabetic severe wounds are difficult to heal and regenerate, and are extremely challenging to manage in clinical practice with common wound dressings. There is a need to develop multifunctional dressings that meet the requirements for efficient treatment of diabetic wounds. Therefore, the nanocomposite hydrogel with oxidized chondroitin sulfate (OCS), carboxymethyl chitosan (CMC), and phenol red-modified ε-poly- l -lysine (EPL-PR) as the backbones, encapsulated with chondroitin sulfate-modified selenium nanoparticles () and infinite coordination polymer nanomedicine (ICPs) composed of procyanidins, berberine hydrochloride and Fe(III), was designed and fabricated. The prepared nanocomposite hydrogel system allows for rapid in situ gelation and complete coverage of irregular wounds. In vivo and in vitro investigations demonstrate that the nanocomposite hydrogel shows excellent tissue adhesion, antibacterial activities, biocompatibility and free radical scavenging properties. The rapid hemostasis of the nanocomposite hydrogel is confirmed through hemorrhage models of liver and caudal amputation. All the advantages enable the nanocomposite hydrogel to effectively induce skin tissue remodeling, promote vascular and hair follicle regeneration, remove free radicals from wounds, and accelerate wound healing. The nanocomposite hydrogel can also successfully monitor the pH range of 5–10, thus enabling real-time visual monitoring of the wound healing phase, facilitating wound management in clinical settings. This multifunctional wound dressing opens up ideas for the treatment and management of diabetic full-thickness wounds.

Abstract:Easy infection and severe inflammation make diabetic wounds difficult to heal. A nanoenzyme‐reinforced hydrogel is fabricated for facilitating infected diabetic wound repair. The hydrogel with antiglycative, antioxidative, and antibacterial activities comprehensively modulates the hostile wound microenvironment. In vivo studies verify that the prepared hydrogel promotes multiple stages, including hemostatics, inflammation, proliferation, and tissue remodeling, thereby accelerating diabetic wound healing. Diabetic wound healing still faces a dilemma because of the hostile hyperglycemic, oxidative, and easily‐infected wound microenvironment. In addition, advanced glycation end products (AGEs) further impede wound repair by altering the immunological balance. Herein, ceria nanorods with distinctive antiglycative and excellent antioxidative capacities are innovatively introduced into a self‐healing and erasable hydrogel, which could reshape the wound microenvironment by expediting hemostasis, inhibiting infection, reducing AGEs, and continuously depleting reactive oxygen species. The remitted oxidative stress and glycosylation synergistically regulate inflammatory responses, and promote revascularization and extracellular matrix deposition, resulting in accelerated diabetic wound repair. This study provides a highly efficient strategy for constructing nanoenzyme‐reinforced antiglycative hydrogel that regulates every wound healing stage for diabetic wound management.

Multifunctional Bioactive Nanozyme Systems for Enhanced Diabetic Wound Healing

Abstract:The protracted transition from inflammation to proliferation in diabetic wound healing poses significant challenges, exacerbated by persistent inflammatory responses and inadequate vascularization. To address these issues, a novel nanozymatic therapeutic approach utilizing asymmetrically structured MnO₂-Au-mSiO₂@aFGF Janus nanoparticles is engineered. Nanozymes featuring a mSiO₂ head and MnO₂ extensions, into which acidic fibroblast growth factor (aFGF) is encapsulated, resulting in MnO₂-Au-mSiO₂@aFGF Janus nanoparticles (mSAM@aFGF), are synthesized. This nanozyme system effectively emulates enzymatic activities of catalase (CAT) and superoxide dismutase (SOD), catalyzing degradation of reactive oxygen species (ROS) and generating oxygen. In addition, controlled release of aFGF fosters tissue regeneration and vascularization. In vitro studies demonstrate that mSAM@aFGF significantly alleviates oxidative stress in cells, and enhances cell proliferation, migration, and angiogenesis. An injectable hydrogel based on photocrosslinked hyaluronic acid (HAMA), incorporating the nanozymatic ROS-scavenging and growth factor-releasing system, is developed. The HAMA-mSAM@aFGF hydrogel exhibits multifaceted benefits in a diabetic wound model, including injectability, wound adhesion, hemostasis, anti-inflammatory effects, macrophage polarization from M1 to M2 phenotype, and promotion of vascularization. These attributes underscore the potential of this system to facilitate transition from chronic inflammation to the proliferative phase of wound repair, offering a promising therapeutic strategy for diabetic wound management.
A glucose responsive multifunctional hydrogel with antibacterial property and real-time monitoring for diabetic wound

Abstract:The healing of complex diabetic wounds with hyperglycemic microenvironment and bacterial infection was considered an important clinical issue. In this study, glucose oxidase (GOx) and gold nanoclusters (AuNCs) were encapsulated in quaternary carboxymethyl chitosan (QCMCS)/sodium alginate oxide (OSA) hydrogels, and were immersed in tannic acid (TA) solution to achieve antioxidant, antibacterial, pro-angiogenesis, pro-collagen deposition and real-time monitoring functions. In vitro studies showed that TA-QCMCS/OSA@GOx@AuNCs hydrogels had inhibition rates of 98.99% and 99.99% against S. aureus and E. coli, respectively, and the survival rate of mouse fibroblasts (L929) was over 95%. In vivo studies showed that TA-QCMCS/OSA@GOx@AuNCs hydrogels was 97.28% effective in healing diabetic wounds. In addition, image signals from TA-QCMCS/OSA@GOx@AuNCs hydrogels can be collected in real-time to accurately obtain glucose concentration values of diabetic wounds and reflect the healing status of diabetic wounds in a timely manner. The results showed that TA-QCMCS/OSA@GOx@AuNCs hydrogels provides a novel idea for real-time monitoring of diabetic wounds treatment.

Abstract:The regeneration and repair of diabetic wounds, especially those integrated with bacterial infection, has always been a difficulty and challenge in current treatment. Herein, a effective strategy is reported for constructing glucose-responsive functional hydrogels using nanocomposites as nodes. In fact, tannin acid (TA) modification ceria nanocomposites (CNPs) and zinc metal-organic frame (ZIF-8) were employed as nodes. Subsequently, crosslinking with 3-acrylamidophenylboronic acid and obtained functional nanocomposite-hydrogels (TA@CN Gel, TA@ZMG Gel) by radical-mediated polymerization.Compared with the physically simple mixed hydrogel system, the mechanical properties of the TA@CN Gel, TA@ZMG Gel are significantly enhanced due to the intervention of nanocomposites nodes. In addition, this kind of nanocomposite hydrogel can realize the programmed loading of drugs and release drugs in response to glucose /PH, so as to coordinate and promote its application in the regeneration and repair of diabetic wounds and its infectious wounds. Specifically, TA@CN Gel can remove reactive oxygen species and generate oxygen through its various enzymatic activities. At the same time, it can effectively promote neovascularization, thus promoting the regeneration and repair of diabetic wounds. Furtherly, glucose oxidase-loaded TA@ZMG Gel exhibits glucose response and pH-regulating functions,triggering programmed metformin(Met) release by degrading the metal organic framework (MOF) backbone. And exhibited additional synergistic effects of antibacterial, hair regeneration and systemic blood glucose regulation, which makes it suitable for the repair of more complex infectious diabetes wounds.Overall, this novel nanocomposite-mediated hydrogel holds great potential as a biomaterial for diabetic chronic wound healing, opening up new avenues for further biomedical applications.

A polyphenol-modified chitosan hybrid hydrogel with enhanced antimicrobial and antioxidant activities for rapid healing of diabetic wounds

Abstract:High oxidative stress injury and bacterial infection are the main challenges that impair wound healing in diabetic patients. Therefore, a hydrogel with enhanced antimicrobial and antioxidant properties was developed for rapid healing of diabetic wounds. In this study, chitosan methacrylate-gallic acid (CSMA-GA) polymer with antioxidant activity, antimicrobial activity, and ultraviolet (UV)-triggered gelling properties was developed as a hydrogel precursor. Meanwhile, amphiphilic Pluronic F127 molecules were used to load hydrophobic chlorhexidine drug molecules to obtain F127/chlorhexidine nanoparticle (NP) with strong antibacterial activity. Subsequently, F127/chlorhexidine NPs were encapsulated in CSMA-GA hydrogel to further enhance its antibacterial activity. The hybrid hydrogel platform (CSMA-GA/F127/chlorhexidine (CMGFC)) exhibited high antibacterial efficiency (> 99.9%) and strong reactive oxygen species (ROS) scavenging ability (> 80.0%), which effectively protected cells from external oxidative stress (upregulated superoxide dismutase (SOD) and glutathione/oxidized glutathione disulfide (GSH/GSSG) levels and downregulated malondialdehyde (MDA) levels). Moreover, in vivo results proved that the CMGFC hydrogel significantly reduced inflammatory responses (downregulated interleukin-6 (IL-6) and upregulated interleukin-10 (IL-10) levels), promoted angiogenesis (upregulated vascular endothelial growth factor (VEGF) and platelet endothelial cell adhesion molecule-1 (CD 31) levels), and wound healing (enhanced collagen deposition and tissue remodelling). Overall, the CMGFC hydrogel with enhanced antimicrobial and antioxidant properties demonstrated significant potential to enhance diabetic wound healing.

Multifunctional carboxymethyl chitosan/oxidized carboxymethyl cellulose hydrogel loaded with ginsenoside Rg1 and polydopamine nanoparticles for infected diabetic wound healing

Abstract:Besides bacterial infection, diabetic wounds are often accompanied by local inflammatory response, oxidative stress imbalances, and vascular formation disorders, which are the main reasons for long-term non-healing of diabetic wounds. In order to solve this problem, Ch-OCMC-PDA NPs-Rg1 self-healing hydrogel was constructed by Schiff base reaction. With the addition of PDA NPs and Rg1, Ch-OCMC-PDA NPs-Rg1 hydrogel showed excellent physical properties, like compressive strength of 142 kPa, swelling ratio of 148.91 %, and Rg1 carried in the hydrogel could achieve a slow release of 90.59 % within 48 h. What's more, PDA NPs endowed it with highly efficient photothermal antibacterial properties. In addition to excellent biocompatibility, Ch-OCMC-PDA NPs-Rg1 hydrogel could effectively clear intracellular reactive oxygen species, promote macrophages M2 transformation, and facilitate human umbilical vein endothelial cells migration and tube formation. In vivo experiments exhibited that Ch-OCMC-PDA NPs-Rg1 hydrogel could reduce wound inflammation, stimulate early angiogenesis, promote collagen deposition, and shorten the healing process of diabetic infected wounds, and the wound healing rate was significantly increased compared with other groups, reaching 98.41 ± 0.31 %. In summary, the multi-functional dynamic Ch-OCMC-PDA NPs-Rg1 hydrogel provides a new possibility for the treatment of diabetic infection wounds.

Enzyme-mediated multifunctional self-healing lysozyme hydrogel for synergistic treatment of chronic diabetic wounds

Abstract:Self-healing hydrogels have attracted significant attention in chronic diabetic wound healing due to their potential to minimize the risk of secondary infections caused by joint movement or dressing rupture. Herein, a multifunctional self-healing hydrogel mediated utilizing an enzyme-triggered cascade reaction based on dynamic imine bonds was designed. The hydrogel employs three enzymes: lysozyme (LYZ), glucose oxidase (GOx), and catalase (CAT), as building blocks. GOx catalyzes the conversion of glucose and 1-thio-β-d-glucose (β-GlcSH) into hydrogen peroxide (H2O2), gluconic acid (GA), and hydrogen sulfide (H2S). Subsequently, CAT eliminates H2O2, protecting the imine bonds from oxidative damage. The acidic environment created by GA decreases the pH and regulates the crosslinking density of imine bonds, enhancing the self-healing capability and porosity of the hydrogel. This feature enables the sustained release of the drug rosuvastatin calcium (RCa) to promote endothelial cell migration and vascular regeneration. Combined with the antioxidative and anti-inflammatory effects of released H2S gas and the antibacterial properties of lysozyme, this hydrogel exhibits promising therapeutic efficacy for the synergistic treatment of chronic diabetic wounds.
Enhanced ·OH‐Scavenging Activity of Cu‐CeOx Nanozyme via Resurrecting Macrophage Nrf2 Transcriptional Activity Facilitates Diabetic Wound Healing

Abstract:Diabetic wounds are a prevalent and devastating complication of diabetes, which may impede their healing and regeneration. In diabetic wounds, excess reactive oxygen species (ROS) activate the nuclear factor kappa‐B pathway, leading to transcriptional silencing of nuclear factor erythroid 2‐related factor 2 (Nrf2), resulting in a vicious cycle of oxidative stress and inflammation. Conventional nanozymes have limitations in preventing the continuous production of ROS, including the most oxidizing reactive hydroxyl radical (·OH), although they can remove preexisting ROS. Herein, a novel antioxidant nanoplatform addresses this challenge by incorporating JSH‐23 into the mesoporous of cupric‐doped cerium oxide nanozymes. Additionally, for rapid wound adaptability and durable tissue adhesion, a nanozyme hydrogel spray consisting of oxidized sodium alginate and methacrylate gelatin was constructed, named OG@CCJs. This platform resurrected Nrf2 transcriptional activity of macrophages in vitro, curbing the production of ROS at its source, particularly ·OH, while enabling the nanozymes to scavenge previously generated ROS. OG@CCJs significantly alleviated oxidative stress in diabetic wounds in vivo, promoting wound healing. Overall, the proposed nanozyme‐hydrogel spray with enhanced ·OH‐scavenging activity uses a "two‐track" antioxidant strategy to rebuild the antioxidant defense barrier of macrophages. Diabetes immunity-modulated multifunctional hydrogel with cascade enzyme catalytic activity for bacterial wound treatment

Abstract:Diabetes immunity-modulated wound treatment in response to the varied microenvironments at different stages remains an urgent challenge. Herein, glucose oxidase (GOx) and quasi-amorphous Fe2O3 are co-incorporated into Zn-MOF nanoparticle (F-GZ) for cascade enzyme catalytic activities, where not only the high blood glucose in the wound is consumed via the GOx catalysis, but also the effective anti-bacteria is achieved via the degradedly released Zn2+ synergistically with the catalytically produced ·OH during the bacterial infection period with the low pH microenvironment. Simultaneously, the reactive oxygen species scavenging and hypoxia relief is realized via catalyzing H2O2 to produce O2 at the relatively elevated pH environment during the wound recovery period. Subsequently, a multifunctional hydrogel with injectable, self-healing and hemostasis abilities, as well as uniformed F-GZ loading is prepared via the copolymerization reaction. This hydrogel behaves as F-GZ but reduces the toxic effects, which thus accelerates the diabetic wound healing. More importantly, this hydrogel is found to modulate the diabetes immunity possibly mediated via the released Zn2+, which thus contributes to the recovered pancreatic islet functions with improved glucose tolerance and increased insulin secretion for enhanced diabetic wound treatments. This work initiates a new strategy for simultaneous diabetic wound management and also suggests a potential way for diabetic immunity modulation.

Abstract:Chronic wounds caused by hyperglycaemia, hypoxia and bacterial infections are common complications in diabetic patients and chronic wound repair is extremely challenging in clinical practice. A series of hydrogels QPMP with good antioxidant and antimicrobial functions were prepared based on quaternized chitosan (QCS), oxidized pullulan polysaccharide (OP), dopamine-coated polypyrrole (PPY@PDA), and Chlorella vulgaris . The Schiff base cross-linking between the quaternized chitosan (QCS) and oxidized pullulan polysaccharide (OP) constitutes the basic skeleton of the hydrogel, and imparts a certain antimicrobial ability to the hydrogel. Chlorella vulgaris continuously produced oxygen under light conditions to relieve wound hypoxia and promote wound healing. The incorporation of PPY@PDA gave the hydrogel near-infrared (NIR) irradiation-assisted bactericidal activity and antioxidant activity, and as a conductive hydrogel, the hydrogel can be used to sense wound exudate and temperature changes, which can help to achieve the integration of diagnosis and treatment of wound healing. Most importantly, in a chronic wound model, the QPMP hydrogel was more effective in controlling the level of wound inflammation and promoting collagen deposition, angiogenesis, and early wound closure compared to the HeraDerm dressing. Therefore, this conductive oxygen-producing hydrogel is extremely beneficial for chronic wound healing in diabetes.

Abstract:The treatment of diabetic wounds remains a challenge due to the interaction between hyperglycemia, immune imbalance and bacterial infection. Herein, we developed a Glu and GSH double-unlocked immunomodulatory nanocascade reactor to accelerate diabetic wounds healing which dynamically manages the wound microenvironment by controlling blood glucose levels, restoring immune regulation and eliminating bacteria. On the one hand, ultra-small gold nanoparticles with GOx activity can catalyse glucose at the diabetic wound site, which can be further enhanced by photothermal effects, thus destroying bacterial biofilm structures, reversing wounds pH, and reducing bacterial reproduction. On the other hand, the sustained controlled release of NO at specific sites was achieved by depleting over-expressed GSH in the wounds to restore normal immune regulation, which not only promotes M2 macrophage polarisation and expressions of anti-inflammatory factors such as IL-10 and TGF-β1, but also produces VEGF and CD31 via the PI3K/AKT signalling pathway. Especially, the chemical resonance energy transfer (CRET) strategy was first applied to diabetic wounds treatment, achieving chemical photodynamic therapy (PDT) while reducing the secondary tissue damage caused by catalytically-generated H2O2. In , the nanocascade reactor provides a new reference for bacterialy-infected diabetic wounds treatment mode by reducing blood glucose and restoring immune regulation in parallel.

Abstract:Impaired diabetic wound healing represents a devastating and rapidly growing clinical problem associated with high morbidity, mortality, and recurrence rates. Engineering therapeutic angiogenesis in the wounded tissue is critical for successful wound healing. However, stimulating functional angiogenesis of the diabetic wound remains a great challenge, due to the oxidative damage and denaturation of bio-macromolecule-based angiogenic agents in the oxidative diabetic wound microenvironment. Here, we present a unique "seed-and-soil" strategy that circumvents the limitation by simultaneously reshaping the oxidative wound microenvironment into a proregenerative one (the "soil") and providing proangiogenic miRNA cues (the "seed") using an miRNA-impregnated, redox-modulatory ceria nanozyme-reinforced self-protecting hydrogel (PCN-miR/Col). The PCN-miR/Col not only reshapes the hostile oxidative wound microenvironment, but also ensures the structural integrity of the encapsulated proangiogenic miRNA in the oxidative microenvironment. Diabetic wounds treated with the PCN-miR/Col demonstrate a remarkably accelerated wound closure and enhanced quality of the healed wound as featured by highly ordered alignment of collagen fiber, skin appendage morphogenesis, functional new blood vessel growth, and oxygen saturation.

Abstract:Management of diabetic wound has long been a clinical challenge due to pathological microenvironment of excessive inflammation, persistent hyperglycemia, and biofilm infection caused by overdue reactive oxygen species (ROS) production and defective blood vessels. Herein, a multifunctional hydrogel with ROS scavenging and photothermal antibacterial activity based on oxidized dextran (Odex), gallic acid-grafted gelatin (GAG) and Ferric ion, named OGF, was developed for treatment of infected wound in a diabetic mouse. This hydrogel was double-crosslinked by the dynamically Schiff-base bonds formed between aldehyde groups in Odex and amino groups in GAG and the metal coordination bonds formed between Ferric ion and polyphenol groups or carboxyl groups in GAG, which endowed the resulted OGF hydrogel with well injectable, self-healing and adhesive properties. Due to the high-efficiency photothermal effect of Ferric ion/polyphenol chelate, this hydrogel killed Staphylococcus aureus and Escherichia coli rapidly and completely within 3.5 min under near-infrared light radiation. Furthermore, this composed hydrogel presented good antioxidation, hemostasis and biocompatibility. It also remarkably accelerated the complete re‐epithelialization of Staphylococcus aureus ‐infected wound in diabetic mice within 18 days by eliminating infection, mitigating oxidative stress and inflammation, and facilitating angiogenesis. Therefore, the proposed multifunctional hydrogel exerts a great potential for translation in the clinical management of diabetic wounds. Statement of Significance High reactive oxygen species (ROS) levels and vascular defects in diabetic wounds can lead to excessive inflammation, persistent hyperglycemia, biofilm infection and other pathological microenvironments, which can further develop to the chronic wounds. In this study, we designed a multifunctional hydrogel with ROS-scavenging ability and photothermal antibacterial activity for the treatment of infected diabetic wound. As expected, this multifunctional hydrogel dressing highly accelerated the complete re‐epithelialization of Staphylococcus aureus ‐infected wound in diabetic mouse by eliminating infection, mitigating oxidative stress and inflammation, as well as facilitating angiogenesis. This work provides a promising therapeutic strategy for infected diabetic wound by inhibition of oxidative stress and biofilm infection.
