Previous versions: February 1, 2022: Ahead of Print A 25-year-old man presented with a heart murmur for 25 years. ECG showed sinus rhythm, abnormal right atrium, intraventricular block, left ventricular hypertrophy, and ST-T wave change (Figure [A]). Transthoracic echocardiography showed a double-outlet right ventricle {S, D, D} with subaortic ventricular septal defect (Figure [B] and [C], Video S1), pulmonary artery (PA) aneurysm (PAA), and patent ductus arteriosus (Figure [D]), coarctation of aortic. Transthoracic Doppler echocardiography showed severe pulmonary arterial hypertension with a diastolic PA pressure of 68 mm Hg. Right heart catheterization confirmed PA hypertension with a mean PA pressure of 76 mm Hg. Computed tomography angiography demonstrated double-outlet right ventricle with subaortic ventricular septal defect (Figure [E] and [F]), PAA with dissection of the main PA (Figure [G]), and simultaneously the left main stem was compressed with ostial stenosis (Figure [H]), patent ductus arteriosus (Figure [I]), coarctation of aortic with dysplasia (Figure [J]). Subsequently, a complicated surgical operation was performed, which was included an anatomic correction of double-outlet right ventricle, pulmonary valve leaflet angioplasty, pulmonary angioplasty, and ligation of patent ductus arteriosus. During the operation, it was observed that double-outlet right ventricle with subaortic ventricular septal defect with a diameter of about 3.5 cm, PAA with dissection, ostial stenosis of left main stem, patent ductus arteriosus was about 1.5 cm, and coarctation of aortic with dysplasia. The postoperative course was uncomplicated and without any special clinical complications. The postoperative pathological examination showed a tear of PA wall tissue (Figure [K]). and a large area of cystic necrosis of PA media (Figure [L]).Download figureDownload PowerPointFigure. AAO indicates ascending aorta; AV, aortic valve; LA, left atrium; PV, pulmonary valve; and RV, right ventricular.PAA defined as a pulmonary arterial dilatation to a diameter of at least 1.5× the normal dimension, is less frequently seen, and its clinical significance also remains uncertain.1 Pulmonary artery dissection (PAD) is considered the most life-threatening complication of PAA, and patients can progress from being asymptomatic to sudden death.2 Primary pulmonary hypertension or secondary to congenital heart disease are considered the major cause of PAD. Vascular inflammatory disease, aortopulmonary fistulas, connective tissue disease, and catheter-induced vessel wall injury are also considered to be potential risk factors.3 A complicated congenital heart disease associated with simultaneous PAA and PAD is very rare, and its clinical symptoms are varied. To the best of our knowledge, it may be the first report of the rare association of double-outlet right ventricle with a giant PAA and PAD. In our case, a preoperative echocardiogram failed to detect PAD. Early diagnosis of PAD is very important as it progresses to become chronic, which is often mistaken for a pulmonary valve. Multimodality imaging examination should be performed promptly for early diagnosis and timely treatment.4 PAD is a rare complication of PAA. A grade 3/6 systolic murmur was heard over her precordium. On the chest X-ray, an enlarged cardiac silhouette was seen (Figure 1A). Her electrocardiograph on admission showed myocardial ischemia. Transthoracic echocardiogram revealed a huge aneurysm involving the whole segment of left anterior descending coronary artery (LAD), 2.0 cm in maximum diameter (Figure 1B). Also, a dilated left main coronary artery was noted. Color Doppler imaging demonstrated blood flow from the coronary aneurysm to right ventricle suggestive of a coronary fistula (Figure 1C and 1D, Movie I in the Data Supplement). Coronary angiography confirmed the same findings and showed a normal right coronary system (Figure 1E, Movie II in the Data Supplement). During cardiac operation, we surprisingly found that LAD did not course in epicardium but hide in the myocardium, which was called mural coronary artery (MCA; Figure 1F). Subsequent to introducing a probe from the proximal left coronary artery to the LAD aneurysm and its fistula, we opened the LAD aneurysm (1.5–2.0 cm in diameter). It communicated with the right ventricle chamber via a 1.5 cm-wide-neck situated near the apex (Figure 1G). Both the aneurysm and the fistulous communication were surgically stitched with pericardial patch (Figure 1H) because directly closing via the myocardium revealed fragile tissue where sutures did not hold. Soon after the closure, the aneurysm collapsed and disappeared. The patient tolerated the procedure well and was discharged in 9 days. Treatment with aspirin was started for half a month. Repeated echocardiography 1 month later revealed no residual shunt and exclusion of the aneurysm without regional wall abnormality (Figure 1I). At 1-year of follow-up,the patient was doing well and had no complications.Coronary artery aneurysm is a rare anomaly,accounting only for 0.2% to 0.4% of all congenital heart diseases.1 It is mainly caused by Kawasaki disease and congenital abnormalities in children. This patient had no previous history of Kawasaki’s disease. We then suspected that the most likely cause was congenital malformation. Coronary artery aneurysm can be a potential cause of myocardial infarction because of fistula formation and thrombus formation within the aneurysm.2 Thus,surgical therapy was recommended for this child. Normal coronary artery usually courses epicardially, whereas MCA is defined as the intramural segment with overlying cardiac muscle fibers which is viewed as an inborn coronary abnormality. Although coronary angiography is the gold standard for the diagnosis of MCA, angiographic findings were not apparent in this patient. Some other advanced imaging techniques, such as intracoronary Doppler, can benefit the diagnosis of MCA.3 In this case, aneurysm involving MCA is a novel finding and the delayed detection of MCA in operation makes the procedure more difficult. Because of the fragile myocardium, it cannot be directly ligated or excised which is commonly indicated for coronary artery aneurysm of normal coronary artery. Fortunately,the heart blood supply was not affected by stitching the aneurysm and fistula with pericardial patch in this case. Because of the paucity of data,there is no evidence-based management for such a rare aneurysm. However, this particular case proves that stitching the unusual aneurysm with pericardial patch can be a good choice, and it highlights the potential need to consider MCA in patients with coronary aneurysms before operation.
