An effect factor analysis of the elbow function recovery after the fracture surgery

Abstract:Objective To investigate the effecting factors of elbow function recovery after the fracture surgery. Methods From July 2013 to January 2015, 107 cases of elbow fracture were treated. There were 50 males and 57 females, aged 13 - 85 years old ( average: 43 years ). According to Association for the study of Internal Fixation ( AO ) fracture classiifcation, there were 17 cases of type A, 42 cases of type B and 48 cases of type C. According to soft tissue injury classiifcation, there were 15 cases of type C0, 27 cases of type CI, 38 cases of type CII, and 27 cases of type CIII. According to articular cartilage injury classiifcation, there were 49 severe cases, 40 mild cases and 18 cases without any injuries. The time from the injury to operation ranged from 8 hours to 26 days with the average of 6 days. The distal humerus fracture occurred in 66 cases, and olecranon fracture in 41 cases. All patients underwent open reduction and internal ifxation, 88 cases with plate and screw, 19 cases with tension band. After the operation, the hospital for special surgery ( HSS ) elbow function score was used to evaluate the efifcacy. Ten related factors of postoperative elbow function recovery were analyzed by statistics including age, sex, preoperative AO fracture classiifcation, soft tissue injury classiifcation, articular cartilage injury, the time from the injury to surgery, treatment, quality of the reduction of the articular surface, start point of the postoperative rehabilitation ( early: postoperative < 30 days; delayed: postoperative≥30 days ), and postoperative complications.Results Wounds healed by the ifrst intention in 107 cases. The mean follow-up was ( 6.19 ± 0.33 ) months ( range: 5.57 - 7.68 months ). Clinical union of the fracture was achieved within 3 - 6 months after the operation. Extensor device adhesions and the scope of activities <80° occurred in 4 cases, heterotopic ossiifcation in 2 cases, nerve injury in 4 cases, and ifxation loosening in 1 cases. According to HSS elbow function score, the results were excellent in 47 cases, good in 31 cases, fair in 14 cases and poor in 15 cases with an excellent and good rate 72.90%. A single factor analysis showed that 3 factors ( age, articular cartilage injury, start point of the rehabilitation ) were signiifcantly correlated with the elbow function recovery (P < 0.05 ). A logistic regression analysis showed that age, start point of the rehabilitation were major factors in the elbow joint function recovery.Conclusions Age, articular cartilage injury, and start point of the rehabilitation can affect the elbow joint function recovery signiifcantly. Adjustment of the patient’s preoperative physical condition, rehabilitation education, symptomatic psychological counseling, appropriate strong internal ifxation, early postoperative functional rehabilitation can lessen postoperative complications and maximize the restoration of the elbow joint function.
