Abstract:Behçet’s disease involving the heart is extremely rare. Sometimes it is probable to misdiagnose as infective endocarditis when protracted fever and chill occurs. We report a case of protracted pyrexia of unknown origin after surgical excision of a cardiac tumour. Clinically and pathologically a diagnosis of infective endocarditis was probable but antibiotics had no effect. After case review the diagnosis of Behçet’s disease was established and the patient was treated with glucocorticoids which resulted in resolution.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Background</h3><p>The association between <i>Borrelia burgdorferi</i> and primary cutaneous lymphoma is still unclear. This systematic review and meta‐analysis aims to define the association of <i>Borrelia burgdorferi</i> with primary cutaneous lymphoma and its different entities. </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>Electronic databases were searched for all studies that assessed the presence of <i>Borrelia burgdorferi</i> DNA in specimens of primary cutaneous lymphoma. The association between <i>Borrelia</i> and primary cutaneous lymphomas was assessed with an odds ratio (significant p < 0.05); cutaneous specimens with no lymphoproliferative disorders were used as controls. A secondary analysis was performed to assess the prevalence of <i>Borrelia</i> infection in different lymphoma entities. </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>Ten studies with 506 primary cutaneous lymphomas and 201 controls were included. The prevalence of <i>Borrelia</i> DNA positivity was highly heterogeneous among studies from different regions. <i>Borrelia</i> DNA positivity was significantly associated with primary cutaneous lymphomas (odds ratio = 10.88; p < 0.00001). The prevalence of <i>Borrelia</i> DNA positivity was similar among different entities (marginal zone: 7.3 %; follicular: 8.1 %; diffuse large B‐cell: 7.5 %; mycosis fungoides: 8 %). </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusions</h3><p><i>Borrelia burgdorferi</i> is significantly associated with primary cutaneous lymphomas, with no differences among the several lymphoma entities (both B‐cell and T‐cell), but with strong geographical differences. Molecular testing for <i>Borrelia</i> would be justified in patients with primary cutaneous lymphoma from endemic areas. </p></section>

Abstract:Abstract Background Splenic marginal zone lymphoma (SMZL) is a rare non-Hodgkin lymphoma, and much little is known about its clinical characteristics and management strategies. Here we present a case of SMZL and review relevant literature to provide a better recognition of this disease entity. Case presentation A 49-year-old Chinese female was admitted to our hospital with complaints of abdominal distension and acid reflux. Physical examinations and imaging investigations suggested the presence of splenomegaly. Laboratory workups revealed mildly reduced white blood cell count otherwise was not remarkable. The patient underwent splenectomy. Histological examination combined with immunohistochemical analysis of the resected spleen confirmed the diagnosis of SMZL. The patient recovered uneventfully during follow-ups. Conclusions Due to the rarity and unspecific clinical features, SMZL is extremely challenging to be diagnosed preoperatively. Patients with SMZL are generally associated with favorable prognosis. Combining the staging characteristics of non-Hodgkin’s lymphoma and splenic primary lymphoma may assist in clinical staging management of SMZL.

Abstract:Pyoderma gangrenosum (PG) is a rare and recurrent ulcerating, non-infectious, inflammatory dermatosis, with occasional concomitant extracutaneous manifestations. The pathogenesis and aetiology of PG are unknown. Moreover, early diagnosis is challenging because there are several visceral manifestations that may occur prior to the skin findings, such that misdiagnosis of PG as an infection is common. Here, we present a case of PG in which pulmonary and spleen lesions preceded the cutaneous manifestations. The correct diagnosis was made 6 months after multiple nodules were detected in the lung and spleen, based on the development of skin wound ulcers. To the best of our knowledge, this is the first report of PG in which pulmonary and splenic involvement preceded the appearance of skin lesions, without systemic disease. The patient was followed up for 5 years, during which time complete clinical and radiographic resolution was confirmed. This case demonstrates the challenges in the diagnosis of PG and the importance of using multiple diagnostic methods to determine the cause of unexplained clinical manifestations.

Abstract:Splenic diffuse red pulp small B-cell lymphoma, as a provisional entity, is an uncommon lymphoma with only two series and five individual cases reported. The peripheral blood, bone marrow and spleen are involved in almost cases at diagnosis. It is usually diagnosed by spleen histopathologic examinations. We report one case of an Asian female patient who presented with both typical and specific clinical features, which helps us further understand the disease.

Abstract:Objective To study the clinical characteristics,diagnosis and treatment of primary pulmonary lymphoma.Methods A retrospective review of primary pulmonary lymphoma cases at a single institution from 2006 to 2008 was performed,and relevant literature was reviewed.Results Primary pulmonary lymphoma is a rare disease.The diagnosis was difficult because of the lack of specific characteristics.The most common symptoms were cough and fever.X-ray feature included solitary or multiple nodules and consolidation.Definite diagnosis was made by pathologic and immunohistchemical examinations.The recommended first-line therapy is chemotherapy.Conclusion Appropriate invasive biopsy is necessary for early diagnosis of primary pulmonary lymphoma.
[Primary Peripheral T-cell Lymphoma of the Penis: a Case Report and Review of the Literature].

Abstract:OBJECTIVE:To report a case of primary peripheral T-cell lymphoma of the penis.METHODS:We analyzed the clinicopathological characteristics of the case of primary peripheral T-cell lymphoma using histological, cytochemical and immunohistochemical methods and by review of the literature.RESULTS:The patient was a 65 years old man and presented with a diffuse enlargement of the penis as the initial sign, followed by erosive ulcer in the caput penis and inguinal lymphadenectasis. The tumor was pathohistologically manifested as an epidermal ulcer, with tumorous necrosis around the capillary, infiltrative growth and atypical changes of the neoplastic cells and proliferation of capillaries. Immunohistochemically, the tumor cells were positive for CD43 and CD3, but negative for CD20, CD79a, CD34, CD30, CD56 and CD34. Clinically it responded to the chemotherapy designed for peripheral T-cell lymphoma.CONCLUSION:Primary peripheral T-cell lymphoma of the penis is an extremely rare malignant tumor, the diagnosis of which relies on histopathological examination, immunohistochemical staining and differentiation between squamous cell carcinoma and other types of lymphoma.
Disseminated cryptococcosis with recurrent multiple abscesses in an immunocompetent patient: a case report and literature review

Abstract:Background: Cryptococcus neoformans is frequently present as an opportunistic pathogen mainly affecting immunocompromised populations. Disseminated C. neoformans infection in immunocompetent population is rare and usually involves lung and central nerve system. Cryptococcus from biologic samples can easily grow on routine fungal and bacterial culture media. Besides, cryptococcal latex agglutination test has been established as a reliable diagnostic tool with overall sensitivities of 93-100%. Case presentation: We report a rare disseminated cryptococcosis case which presented with chronic recurrent multiple abscess in an immunocompetent male involving skin, lung, spine and iliac fossa without evidence of central nerve system involving. The results of serum cryptococcal latex agglutination tests and standard microbial cultures were negative. The patient underwent empirical anti-bacterial and anti-tuberculosis therapy which turned out to be effectless. Finally, bedside inoculation of the pus was carried out and revealed Cryptococcus neoformans, which was confirmed by polymerase chain reaction. After the administration of anti-fungal drugs including liposomal amphotericin B, the patient recovered from fever and paraplegia. Conclusions: This case reveals an uncommon pattern of disseminated C. neoformans infection in immunocompetent population presented with chronic multiple abscess and without central nerve system involving. Negative routine microbial cultures may not necessarily rule out cryptococcosis, especially in early stage. Besides, cryptococcal latex agglutination test does have a chance of false negative, which might be related with "capsule-deficiency". Moreover, this phenomenon could be related with low-grade virulence and relative long illness duration.
Primary Cutaneous Marginal Zone B-cell Lymphoma: a Clinical, Histopathological and Phenotypic Study of 15 Cases

Abstract:Objective:To analyse the clinical histopathological and immunophenotypic characteristics of primary cutaneous marginal zone B-cell lymphoma(PCMZL)in Chinese patients.Methods:Fifteen PCMZL patients were collected to evaluate the clinical histological features.The expression of CD20,CD79a,CD21,CD5,CD10,BCL2 and BCL10 were examined by immunohistological methods.Results:Within these 15 patients,6 were male and 9 were female(mean age 61.2 years,me- dian age 58 years).Eleven patients presented with lesions on the head and face,3 patients on the back,and one patient in disseminated pattern.All cases presented with erythematous nodular lesions.Histologically,diffuse infiltration of lymphoid cells was seen in the dermis.In 10 of 15 cases,the infiltrated lymphoid cells formed reactive follicles or nodules.Neoplastic cells were CD20(+),CD79(+),BCL2(+),BCL10(+),CD5(-) and CD10(-).In 5 cases,the expression of CD43 was observed. CD21 staining could display the follicular patern.Conclusions:PCMZL usually involves the head and face,and more often affect the aged women.The combination of morphological features and immunostaining for BCL2 and BCL10 was useful for the diagnosis and differentiation.

Abstract:To explore the clinical manifestation of lymphomatoid granulomatosis (LG) and the early diagnosis. Retrospective analyses of the clinical features of LG and pathology features by open lung biopsies were conducted. The patient was a 42-year-old female with irregular fever, and her chest X-ray and computed tomography showed nodules with cavity and pleural effusion. Open lung biopsy proved LG. LG is seldom seen in clinic. Open lung biopsy is very important for pathology diagnosis. Early diagnosis and treatment are the key to improving survival in these patients. The therapeutic effect is good.

Abstract:Cutaneous tuberculosis is showing a resurgence. It continues to be one of the most elusive and difficult diseases to diagnose in dermatological practice. We sought to describe the clinical manifestations, laboratory features, and antituberculous therapy in cases of cutaneous tuberculosis. Twenty cases of cutaneous tuberculosis were observed in this study. Of the 20 cases, seven were in women (35%) and 13 were in men (65%). There were eight (40%) cases of lupus vulgaris, four (20%) cases of orificial tuberculosis, four (20%) cases of tuberculosis verrucosa cutis, two (10%) cases of scrofuloderma, and two (10%) cases of primary inoculation tuberculosis. In total, 20 strains of acid-fast bacilli were isolated by culture in Löwenstein-Jensen medium at 37°C. Mycobacterial species identification by polymerase chain reaction (PCR) assay showed that all strains were Mycobacterium tuberculosis. In vitro susceptibility testing by use of absolute concentration method showed that the 20 strains were sensitive to rifampin, ethambutol, and isoniazid; however, two strains were resistant to streptomycin. All of the patients responded favourably to antituberculous treatment. Cutaneous tuberculosis is a skin disease that should still be thought much of in mainland China, and in this study lupus vulgaris is the most common clinical presentation.

Abstract:Background: Mycosis fungoides is rarely associated to B-cell malignancies, and the few reported cases are mainly internal lymphomas involving secondarily the skin (ie, chronic lymphocytic leukemia). Objectives: The aim of our study is to describe the clinical and histopathological features of 4 patients presenting with 2 concurrent primary cutaneous lymphomas and review the pertinent literature. Methods: We identified 4 cases of concurrent primary cutaneous lymphomas in our institutions. An extracutaneous lymphoma was ruled out on the basis of a complete work out. We performed a PubMed search to identify reported cases of primary cutaneous composite or concurrent lymphomas. Results: Eleven cases of primary cutaneous concurrent lymphomas have been described in the literature. Counting all together (our cases and the cases previously described in the literature), mycosis fungoides was the most frequent primary cutaneous T-cell lymphoma (TCL) (13/15), followed by 1 case of peripheral TCL-NOS and 1 case of subcutaneous panniculitis-like TCL. Regarding the associated primary cutaneous B-cell lymphomas, 8/15 cases consisted of low-grade B-cell lymphomas [that is, 5 marginal zone lymphoma (in the most recent classification reclassified as marginal zone lymphoproliferative disorder, MZLD, 2 follicular-center B-cell lymphoma (primary cutaneous follicle-center lymphoma) and 1 low-grade NOS B-cell lymphoma]; 4/15 were associated to Epstein-Barr virus; 1 case consisted of a methotrexate-associated lymphoproliferative disease, and 2 cases consisted of primary cutaneous diffuse large B-cell lymphoma-leg type. Conclusions: Primary cutaneous concurrent lymphomas are exceptional. Clinicopathological correlation and a complete workout to reach the correct diagnosis may guide the appropriate treatment in each case.
Clinical Characteristics and Prognosis of Penicilliosis among Human Immunodeficiency Virus-Infected Patients in Eastern China

Abstract:AbstractTalaromyces marneffei infection is increasingly observed in people living with human immunodeficiency virus (HIV) in eastern China, a nonendemic area. This study aimed to draw the clinician's attention to this disease by presenting the clinical characteristics and prognosis of penicilliosis among HIV-infected patients from this region. We retrospectively analyzed HIV-infected patients with culture-proven T. marneffei infection admitted during January 1, 2014-December 31, 2015, at the Shanghai Public Health Clinical Center. A total of 48 patients with confirmed HIV infection and penicilliosis were enrolled, which accounted for a mean of 3.2% (95% confidence interval: 2.4-4.2%) of yearly HIV infection admissions among patients from eastern China. Symptoms included fever, cough, and gastrointestinal complaints, whereas the most common sign was skin lesions. Anemia occurred in 87.5% (42/48) of the patients. The overall mortality rate was 16.7%. Low CD4 T-cell count and hemoglobin level were correlated with mortality. Based on these results, we concluded that penicilliosis should be considered in HIV-infected patients from eastern China who present with fever, cutaneous lesions, and anemia. The clinical characters and the prognosis of patients with penicilliosis are similar to those in endemic areas. More attention should be paid to penicilliosis patients with low CD4 T-cell count and/or low hemoglobin level.

Abstract:A 50-year-old man with general fatigue and weight loss underwent FDG PET/CT scan to detect possible occult malignancy. The images revealed abnormal FDG activity in the tonsils, right lung, and in the cervical, axillary, and inguinal lymph nodes. Lymphoma was initially suspected. However, pathologic examination after cervical lymph node biopsy revealed only reactive hyperplasia. Further examinations including serologic tests led to a diagnosis of secondary syphilis. A follow-up PET/CT scan after a 6-month benzylpenicillin therapy showed significant interval improvement.

Abstract:Background: Pulmonary mucormycosis is most common in patients with hematologic malignancies and transplant recipients. This article describes a case of mucormycosis in the lungs secondary to a hematologic disorder with suspected lung cancer. Methods: Rhizopus (Rhizopus microspores) was detected by blood NGS and bronchoalveolar lavage fluid NGS, and pulmonary mucormycosis was confirmed. Results: Secondary to hematologic disease, pulmonary pneumonia, mycosis, and symptoms improved after comprehensive treatment. Conclusions: Clinical data and radiologic knowledge are combined to diagnose invasive pulmonary mycoses; early empirical medicine is very important.

Abstract:A 50-year-old male had a cutaneous ulcer on the left arm for 2 years. Histopathological examination showed granulomatous inflammation, oval spore and asteroid body observed in PAS staining. Fungal culture was positive, and the isolate was identified as Sporothrix globosa. A case of fixed cutaneous sporotrichosis was diagnosed. Cutaneous lesion partly improved after three months treatment with terbinafine combined with itraconazole. Afterwards, focal cutaneous lesion were not healed after a year and a half of discontinuous treatment with terbinafine combined with potassium iodide.
