Enhanced recovery after surgery in EMS lithotripsy for percutaneous nephrolithotomy: a retrospective cohort study

Abstract:Abstract Background The enhanced recovery after surgery (ERAS) protocols for many urological procedures are well developed, but application of an ERAS protocol in PCNL is still limited. The objective of this study was to investigate postoperative recovery following PCNL combined with EMS Trilogy lithotripsy using our institutional ERAS protocols. Materials and Methods From January 2018 to July 2021, a perioperative ERAS protocol was applied in 104 patients who underwent PCNL combined with EMS Trilogy. A matched cohort of 104 patients for whom a traditional protocol was used between 2012 and 2018 was selected, and the stone-free rate, incidence of complications, time from surgery to first flatus, postoperative bed rest time, postoperative hospitalization and expenses were compared between the two groups. Results The mean time from surgery to first flatus was 14.8 hours in the ERAS cohort and 26.4 hours in the control group (P < 0.001). Patients in the ERAS group also had a significantly shorter postoperative bed rest time (23.8 vs. 47.2 hours, P < 0.001) and postoperative hospitalization (4.49 vs. 6.64 d, P < 0.001). The average hospitalization cost for the ERAS group was 8.5% lower than that for the traditional group (19749 vs. 21584 CNY, P < 0.001). Conclusions Patients in the ERAS group had earlier anal exhaust and out-of-bed times, fewer hospitalization days, lower hospitalization costs, and no increased risks of perioperative complications or residual stones.
