Abstract:Objective To determine the value of an enhanced recovery after surgery (ERAS) protocol for prostate cancer patients undergoing laparoscopic radical prostatectomy (LRP). Methods We conducted a retrospective cohort study using clinical data for 288 patients who underwent LRP in our hospital from June 2010 to December 2016. A total of 124 patients underwent ERAS (ERAS group) and the remaining 164 patients were allocated to the control group. ERAS comprised prehabilitation exercise, carbohydrate fluid loading, targeted intraoperative fluid resuscitation and keeping the body warm, avoiding drain use, early mobilization, and early postoperative drinking and eating. Results The times from LRP to first water intake, first ambulation, first anal exhaust, first defecation, pelvic drainage-tube removal, and length of hospital stay (LOS) were all significantly shorter, and hospitalization costs and the incidence of postoperative complications were significantly lower in the ERAS group compared with the control group. No deaths or reoperations occurred in either group, and there were no readmissions in the ERAS group, within 90 days after surgery. Conclusion ERAS protocols may effectively accelerate patient rehabilitation and reduce LOS and hospitalization costs in patients undergoing LRP.

Abstract:Objective: To evaluate the feasibility and safety of applying enhanced recovery after surgery (ERAS) protocol in patients undergoing laparoscopic distal pancreatectomy. Methods: Data of 36 patients undergoing laparoscopic distal pancreatectomy from May 2016 to May 2017 in the First Affiliated Hospital, Zhejiang University School of Medicine were reviewed. The patients were divided into ERAS group (n=12) and control group (n=24). The patients in ERAS group received a series of enhanced recovery procedures, including multimodal analgesia, early off-bed activity and early oral food-taking, etc. Operation time, intraoperative blood loss, time to first flatus, postoperative complications, and length of postoperative hospital stay were evaluated. Results: There were no statistically significant differences in operation time and intraoperative blood loss between ERAS group and control group (all P>0.05). The time to first flatus and length of postoperative hospital stay were significantly shortened in ERAS group (all P<0.05). The ERAS group had lower incidence of postoperative complications (41.7% vs. 66.7%), and the complications in ERAS group tended to be milder, but the differences failed to show statistical significance (all P>0.05). Conclusion: The ERAS protocol for laparoscopic distal pancreatectomy can significantly promote gastrointestinal function recovery and shorten postoperative hospital stay, and may reduce the incidence of postoperative complications.

Abstract:Background: The enhanced recovery after surgery (ERAS) program promotes postoperative recovery. The study aimed to evaluate the effects of the ERAS protocol in hepatectomy. Methods: The primary outcomes included time to first flatus and length of hospital stay (LOS). Secondary outcomes included time to first passage of stool, time to off-bed activity, pain score, abdominal distension score, postoperative liver function and complications. Outcomes were evaluated between the ERAS group (n=69) and the control group (n=232). A subanalysis of probiotics use in the ERAS group was also performed. Results: In patients undergoing laparoscopic and open hepatectomy, time to first flatus after surgery was significantly shorter, as well as LOS, in the ERAS group. Lower incidence of post-operative complications was noted in ERAS group. While sub-analysis of probiotics group showed no significance in the primary outcomes. Conclusions: Implementation of the ERAS protocol for hepatectomy could significantly promote postoperative recovery. Short-term application of probiotics during the perioperative period did not improve the outcomes in the ERAS group.

Abstract:Objective To explore the perioperative outcomes and safety of enhanced recovery after surgery (ERAS) in laparoscopic radical cystectomy (LRC).Methods We retrospectively evaluated outcome of 10 LRC patients on ERAS protocol from May 2017 to October 2017,and 39 LRC patients on conventional recovery after surgery(CRAS) protocol from July 2015 to November 2016.There were (60.9 ±11.4) years and (63.7 ± 12.1) years in ERAS group and CRAS group respectively(P =0.514);(25.5 ±2.7) kg/m2 and (24.4 ± 3.6) kg/m2 with body mass index (P =0.375).Both of the median of charlson comorbidity index (P =0.931) and American Society of Anesthesiologists score (P =0.254) were 2 There was no statistical significance between the two groups for type of urinary diversion and preoperative laboratory studies (P ＞ 0.05).Patients' perioperative outcomes,early (30-day) complications and postoperative readmission rate were compared.Results The ERAS group had less intraoperative crystalloid infusion [(950.0 ± 474.3) ml vs.(1 797.4 ± 448.1) ml,P ＜ 0.001],faster removed gastric tube (0 d vs.4 d,P ＜0.001),and shorter passing flatus time [(1.6 ± 0.8) d vs.(2.9 ± 1.4) d,P =0.006] than the CRAS group;however,no difference was found in terms of intraoperative colliod infusion [(1 110.0 ± 331.5)ml vs.(1 117.9 ± 397.9) ml,P =0.954].No patients from either group required conversion to open surgery.There was no significant difference between the two groups for operative time (P =0.311),estimated blood loss (P =0.073),drain days (P =0.681),postoperative hospital stay (P =0.509),overall blood transfusion (P =1.000),intensive care unit stay (P =1.000) and tumor characteristics (pathological stage,histology,nodes removed,positive nodes,lymph node-positive patients,positive surgical margins).The 30-day postoperative complications were documented in 5 (50％)and 23 (59％)patients in groups ERAS and CRAS (P =0.878),respectively.And the most common complication were minor complications (Clavien-Dindo grade 1 and 2) in both groups (100.0％ vs.86.9％,P =0.729).The 30-day readmission rate was 20.0％ (2 patients) in ERAS group and 10.3％ (4 patients) in CRAS group with no statistical significance(P =0.588).Conclusions Our ERAS protocol expedited bowel function recovery after RC and urinary diversion without increasing in 30-day complications compared with CRAS.The key of implement ERAS pathway is to explore and develop their own protocol conformed to their medical treatment enviroment.
Enhanced Recovery After Surgery Protocol Optimizes Results and Cost of Laparoscopic Radical Nephrectomy

Abstract:Purpose To assess the impact of enhanced recovery after surgery (ERAS) protocols in laparoscopic radical nephrectomy (LRN). Methods The clinical data of 89 patients underwent LRN in Zhongnan Hospital of Wuhan University from February 2019 to September 2021 were collected (40 in the ERAS group and 49 in the pre-ERAS group). The clinical characteristics, prognosis, and length of hospital stay (LOS) were compared between the two groups using t test, Mann-Whitney test, and chi-square test. Results Total LOS and postoperative LOS were significantly shorter in ERAS group than in pre-ERAS group [15.0 (13.5-19.5) vs . 12.0 (10.0-14.0), P < 0.001; 8.0 (7.0-10.0) vs . 7.0 (5.0-8.8), P = 0.001]. Compared with the pre-ERAS group, the hospitalization expenses of the ERAS group were also lower (P = 0.023). In addition, the incidence of postoperative complications in the ERAS group also decreased (P = 0.054). Conclusions ERAS protocol in LRN could help accelerate the recovery of patients and is worthy of clinical promotion.

Abstract:Purpose Enhanced recovery after surgery (ERAS) has played an important role in recovery management for radical cystectomy with ileal urinary diversion (RC-IUD). This study is to evaluate ERAS compared with the conventional recovery after surgery (CRAS) for RC-IUD. Methods From October 2014 and July 2016, bladder cancer patients scheduled for curative treatment from 25 centers of Chinese Bladder Cancer Consortium were randomly assigned to either ERAS or CRAS group. Primary endpoint was the 30-day complication rate. Secondary endpoints included recovery of fluid and regular diet, flatus, bowel movement, ambulation, and length of stay (LOS) postoperatively. Follow-up period was 30-day postoperatively. Results There were 144 ERAS and 145 CRAS patients. Postoperative complications occurred in 25.7 and 30.3% of the ERAS and CRAS patients with 55 complications in each group, respectively ( p = 0.40). There was no significant difference between groups in major complications ( p = 0.82), or type of complications ( p = 0.99). The ERAS group had faster recovery of bowel movements (median 88 versus 100 h, p = 0.01), fluid diet tolerance (68 versus 96 h, p < 0.001), regular diet tolerance (125 versus 168 h, p = 0.004), and ambulation (64 versus 72 h, p = 0.047) than the CRAS group, but similar time to flatus and LOS. Conclusions ERAS did not increase 30-day complications compared with CRAS after RC. ERAS may be better than CRAS in terms of bowel movement, tolerance of fluid and regular diet, and ambulation.

Abstract:Objectives: The Enhanced Recovery After Surgery (ERAS) pathway has reduced morbidity and length-of-stay in hospital in orthopedics, colorectal and pelvic surgery. However, its application in pancreatic surgery remains controversial. We initiated an ERAS programme in a high-volume pancreas centre in China to evaluate its safety and efficiency. Methods: Based on the ERAS guidelines and the updated evidences, a unique institutional ERAS programme was developed in combination with our current practice. The ERAS protocol was applied in 343 consecutive patients undergoing pancreatic surgery. Patients (n=88) who received conventional recovery strategies were included as the control group. Achievement of management goals and short-term postoperative complications were analysed. Results: No adverse effect related to ERAS items occurred. Compliance with perioperative items ranged from 43 to 100%. The proportions of patients achieving key targets were: 60% for NGT removal; 94% for urethral catheter removal; 98% for resumption of water on POD1; 80% for resumption of oral fluids on POD2; 58% for tolerating diet on POD4; 84-90% for meeting mobility targets, and 50% for drain removal. The ERAS group had an earlier recovery of oral feeding (p<0.01), mobilisation (p<0.01), NGT removal (p<0.05), and urethral catheter removal (p<0.01). There was no difference between ERAS and control group in mortality, overall morbidity and major complication rates. The mean postoperative hospital stay was 19.6 days in the conventional group and 16.2 days in the ERAS group (p<0.05). Meanwhile, the ERAS pathway significantly reduced hospitalisation costs. Conclusion: ERAS programme can be safely implemented in pancreatic surgery with an earlier postoperative recovery and a shorter LOS than conventional recovery strategies. Monitoring of programme running and appropriate interventions can improve executive ability of the ERAS system during its initiation phase. Disclosure of interest: None declared.

Abstract:Marrow from 5-fluorouracil- or cyclophosphamide-treated mice, injected into lethally irradiated recipients, gives an increasing number of spleen colonies between days 7 and 14. It has been suggested that the later-forming colonies result from the more primitive spleen colony-forming units (CFU-S), which first seed into the marrow, only later to be recirculated and form colonies in the spleen. Strontium 89 (89Sr), a bone-seeking radionuclide, was injected into recipient mice to block such putative recirculation. A dose of 89Sr, which killed at least 99.8% of CFU-S in, or entering, the bone cavities, was incapable of preventing the increase in spleen colony numbers. Similarly, the splenic environment, modified by the presence of spleen colonies and able to provide a better bed for trapping CFU-S from the peripheral circulation, yielded the same number of further CFU-S, whether or not the animal had received 89Sr. Thus, it was concluded that the 12-day CFU-S does not seed initially into the marrow spaces. Direct observation of the quality of CFU-S initially seeding into the bone marrow and spleen showed, by retransplantation into secondary irradiated mice, that a similar spectrum of CFU-S types had seeded both organs.

Abstract:Objective To compare the enhanced recovery program after surgery (ERAS) with conventional perioperative management in patients undergoing radical resection for colorectal cancer.Methods The ERAS protocol included a combination of evidence-based and consensus methodology.A total of 597 consecutive patients undergoing elective colorectal resection were randomized to either the ERAS(n=299) or the control group(n=298).Outcomes related to nutrition and metabolism index,stress index,and recovery index were measured and recorded.Results Demographics and operative parameters were similar between the two groups(P＞0.05),The nutritional status of patients in the ERAS group was improved after surgery compared with that of the control group.On postoperative day (POD) 1,the HOMA-IR in the ERAS group was significantly lower than that in the control group (P＜0.01).The cortisol level in the control group was elevated on both POD 1 (P＜0.01) and POD 5 (P＜0.01)compared to the preoperative level.However,the cortisol level was not increased until POD 5(P＜0.01) in the ERAS group.The levels of TNF-α,IL-1β,IL-6,and IFN-γ were reduced in the ERAS group,indicating less postoperative stress responses compared with the control group.In addition,ERAS group was associated with accelerated recovery of gastrointestinal function.The postoperative length of stay [(5.7±1.6) d vs.(6.6±2.4) d,P＜0.01] and expense[(15 998±2655) RMB vs.(17 763±3059) RMB,P＜0.01] were reduced in the ERAS group.Twenty-eight patients (9.4％) in the control group and 29 (9.7％) in the ERAS group developed complications,while the difference was not statistically significant (P＞0.05).Conclusion ERAS protocol alleviates surgical stress response and accelerates postoperative recovery without compromising patient safety.

Abstract:Objective To explore the validity and security of enhanced recovery after surgery (ERAS) in kidney transplantation recipients during perioperative managemant.Methods The clinical data of 474 kidney transplantation recipients who received kidney transplantation in Kidney Disease Center,the First Affiliated Hospital,School of Medicine,Zhejiang University during July 2016 to August 2017 were retrospectively analysed,among which 315 recipients who got traditional perioperative managemant during July 2016 to April 2017 were divided into traditional group,and 159 recipients who got ERAS during perioperative managemant were diveded into ERAS group.The main content of ERAS included bowel preparation removal,preoperative carbohydrate loading,early-stage feeding,early mobilization,multimodal approach to opioid-sparing pain control and so on.Indexes like mean post operation hospitalization duration,serum creatinine,gastrointestinal reaction,delayed graft dysfunction (DGF),acute reaction during 90 days after surgery,incidence rate of new-onset diabetes mellitus after transplantation and unscheduled readmission rate during 90 days after surgery were compared between the 2 groups.Measurement data like age,weight and time of dialysis preoperative between the 2 groups were compared by Student's t-test,and enumeration data like incidence rate of DGF and acute reaction during 90 days after surgery between the 2 groups were compared by chi-square test.Results No statistical difference was found in gender,age,height,weight,time of dialysis preoperative,type of donor kidney and HLA mismatch number between the 2 groups (P all ＞ 0.05).The post operation hospitalization duration of living donor renal transplantation recipients and cadaveric donor renal transplantation recipients in ERAS group were (7.7 ± 2.0) d and (12.8 ± 8.3) d,respectively,which were shorter than traditional group [(9.0 ±3.8) d and (16.7 ± 12.6) d],and the difference was statistically significant (t =-2.594 and-2.692,P all ＜ 0.05).No statistical difference was found in serum creatinine of 7,30 and 90 days after transplantation between the 2 groups (P all ＞ 0.05).The incidence rate of diarrhea of ERAS group and traditional group were 5.7％ (9/159) and 20.7％ (65/315),respectively,which had statistical difference (x2 =18.092,P ＜ 0.05).No statistical difference was found in DGF,acute reaction during 90 days after transplantation,new-onset diabetes mellitus after transplantation,incidence rate of other complications and unscheduled readmission rate during 90 days after surgery between the 2 groups (P all ＞ 0.05).Conclusions ERAS protocol can decreased post operation hospitalization duration and incidence rate of diarrhea without influence on other complications,which was safe and effective.

Abstract:Sarcoma of the Nose in an Infant.-HAROLD EDWARDS, M.S. This case was shown to the Section twelve months ago' as a case of sarcoma cured by treatment with radium, goat serum, and Coley's fluid. The child was then nearly twelve months old, and had been free from any sign of growth for eight months. Two weeks after the case was shown, a lump appeared in the submaxillary region, and the subsequent history is as follows:24.10.32.-Enlarged submaxillary gland removed. Section showed chronic inflammation only. 9.11.32.-Lump appeared in the cheek. Excised from inside the mouth. Contained a good deal of necrotic material. Section showed spindle-celled sarcoma. 24.11.32.-Recurrence in cheek. Treated by implantation of radon seeds. Continued with goat serum and Coley's fluid. 1.1.33.--All growth apparently disappeared. Much scarring at site of operation. 2.2.33.-Recurrence at original site at root of nose. Treated by implantation of radon seeds. 18.5.33.-Readmitted with ulcer on cheek, with obvious extension of growth downwards and outwards towards the angle of the jaw. Radon seeds again inserted, and goat serum recommenced. Present condition.-The growth is now extending rapidly, and there is cedema of the eyelids. There are several hard but discrete glands in the neck. There is no pain, and the weight is being maintained. No further treatment is contemplated beyond goat serum.

Abstract:Objective: This study was performed to evaluate the application of enhanced recovery after surgery (ERAS) in patients undergoing radical cystectomy (RC). Methods: The clinical data of 192 patients who underwent RC were collected in this retrospective cohort study. Among them, 91 patients who underwent ERAS were allocated to the ERAS group, and the remaining 101 patients who underwent traditional postoperative care procedures were allocated to the non-ERAS group. Perioperative indexes in the two groups were compared. The ERAS components included rehabilitation exercise, carbohydrate fluid loading, cessation of nasogastric tubes, omission of oral bowel preparation, regional local anesthesia, body-warming procedures, reduced drainage use, and early postoperative drinking and eating. Results: The times from RC to first water intake, first ambulation, first anal exhaust, first defecation, and pelvic drainage tube removal were significantly shorter and the hospitalization costs were significantly lower in the ERAS than non-ERAS group. The intraoperative blood loss volume, blood transfusion rate, readmission rate, and incidence of postoperative complications were also significantly lower in the ERAS than non-ERAS group. Conclusion: ERAS may effectively accelerate patient rehabilitation and reduce the length of stay, incidence of postoperative complications, readmission rates, and hospitalization costs for patients undergoing RC.

Abstract:Da Vinci robot-assisted pancreaticoduodenectomy offers advantages, including minimal invasiveness, precise, and safe procedures. This study aimed to investigate the clinical effectiveness of implementing enhanced recovery after surgery (ERAS) concepts in Da Vinci robot-assisted pancreaticoduodenectomy. A retrospective analysis was conducted on clinical data from 62 patients who underwent Da Vinci robot-assisted pancreaticoduodenectomy between January 2018 and December 2022. Among these patients, 30 were managed with ERAS principles, while 32 were managed using traditional perioperative management protocols. Surgical time, intraoperative blood loss, postoperative oral intake time, time to return of bowel function, time to ambulation, visual analog scale (VAS) pain scores, fluid replacement volume, length of hospital stay, total hospital expenses, complications, and patient satisfaction were recorded and compared between the two groups. Postoperative follow-up included assessment of postoperative functional scores, reoperation rates, SF-36 quality of life scores, and survival rates. The average follow-up time was 35.6 months (range: 12–56 months). There were no statistically significant differences in general characteristics, including age, surgical time, intraoperative blood loss, and preoperative medical history between the two groups (P > 0.05). Compared to the control group, the intervention group had an earlier postoperative oral intake time, faster return of bowel function, rapid ambulation, and shorter hospital stays (P < 0.05). The intervention group also had lower postoperative VAS scores, lower fluid replacement volume, lower total hospital expenses, and a lower rate of complications (P < 0.05). Patient satisfaction was higher in the intervention group (P 0.05). Implementing ERAS principles in Da Vinci robot-assisted pancreaticoduodenectomy substantially expedited postoperative recovery, lowered pain scores, and diminished complications. However, there were no notable differences in long-term outcomes between the two groups.

Abstract:Background: The enhanced recovery after surgery (ERAS) program is focused on improving surgical outcomes and enhancing the patient experience before, during and after surgery. We performed this study to evaluate the effect of ERAS in perioperative period of laparoscopic nephron sparing surgery (LNSS). Methods: A retrospective analysis of 287 consecutive patients with localized renal cell carcinoma (RCC) who underwent LNSS from December 2015 to June 2017 was conducted. Our study design included two cohorts. Patients underwent conventional care in one group (n=136), and ERAS protocol in the other group (n=151). The data of recovery of gastrointestinal function, catheter and drainage tube removal time, length of stay (LOS), hospitalization expenses and incidence of postoperative complications were compared between the two groups. Results: Compared with conventional group, cases in ERAS group presented with shorter time of recovery of gastrointestinal function (P<0.05) and LOS (P<0.05), shorter catheter and drainage tube removal time (P<0.05), lower hospitalization expenses and lower perioperative complications (P<0.05). Conclusions: Program of ERAS used in perioperative period of LNSS can apparently accelerate the time of postoperative recovery, reduce complications, shorten time stay in hospital and cut down the total cost, finally improve patients' satisfaction.
Enhanced Recovery after Surgery (eras) Program Attenuates Stress and Accelerates Recovery in Patients after Radical Resection for Colorectal Cancer: A Prospective Randomized Controlled Trial

Abstract:BACKGROUND:The aim of this trial was to compare the Enhanced Recovery After Surgery (ERAS) program with conventional perioperative management in patients who underwent radical resection for colorectal cancer.METHODS:A combination of evidence-based and consensus methodology was used to develop the ERAS protocol. Five hundred ninety-seven consecutive patients who underwent elective colorectal resection were randomized to either the ERAS (n = 299) or the control group (n = 298). Outcomes relating to nutrition and metabolism index, stress index, and recovery index were measured and recorded.RESULTS:Demographic and operative data were similar between the two groups. Patients in the ERAS group showed improved nutritional status when compared with those of the control group. On postoperative day (POD) 1, the HOMA-IR (insulin resistance index) of the ERAS group was lower than that of the control group (p < 0.001). The cortisol level of the control group was elevated on both POD 1 (p = 0.007) and POD 5 (p = 0.002) compared to the preoperative level. However, the cortisol level of the ERAS group was not increased until POD 5 (p = 0.001). Reduced levels of TNF-α, IL-1β, IL-6, and IFN-γ in the ERAS group indicated less postoperative stress responses. In addition, ERAS was associated with accelerated recovery of gastrointestinal function. The postoperative length of stay (p < 0.001) and expense (p < 0.001) for the ERAS group were reduced in comparison to the controls. Twenty-eight cases in the control group and twenty-nine in the ERAS group suffered complications, which was not significantly different.CONCLUSION:The ERAS protocol attenuates the surgical stress response and accelerates postoperative recovery without compromising patient safety.

Abstract:Background: The enhanced recovery after surgery (ERAS) method has been widely used in surgery and anesthesia worldwide and has been applied to a wide range of surgical specialties, including colorectal surgery, gynecology, liver surgery, breast surgery, urology, and spinal surgery. An increasing number of studies have demonstrated its safety and efficacy in various fields. The safety and effectiveness of ERAS for percutaneous nephrolithotomy (PCNL) remain controversial. This study aimed to review the safety and effectiveness of ERAS for PCNL. Methods: The Chinese National Knowledge Infrastructure (CNKI), Wan Fang, Chinese Biomedical Literature Service System (SinoMed), Chinese Science and Technology Journal Full Text Database (VIP), Cochrane Library, PubMed, Web of Science, and Embase databases were searched for eligible studies published until September 19, 2022. Outcome measures included postoperative hospital stay, total hospital stay, incidence of postoperative complications, stone-free rate (SFR), operative time, postoperative indwelling nephrostomy tube time, catheter encumbrance time, and nursing satisfaction. All analyses were performed using random effects or fixed effects models. Clinical heterogeneity was treated with subgroup, sensitivity, or descriptive analyses only when clinical heterogeneity was not excluded. Publication bias was assessed using funnel plots. Twenty-five studies (1,545 observational patients and 1,562 controls) were included. Results: The ERAS group had a shorter postoperative hospital stay [WMD=−2.59, 95% CI=(−3.04, −2.14), P <0.001], total hospital stay [WMD=−2.59, 95% CI=(−3.04, −2.14), P <0.001], and lower complication rate [RR=0.36, 95% CI=(0.29, 0.43), P <0.001] than the control group. The ERAS group had a shorter surgery time [WMD=−3.57, 95% CI=(−5.88, −1.26), P =0.003], postoperative indwelling nephrostomy tube time [WMD=−1.94, 95% CI=(−2.69, −1.19), P <0.001], catheter encumbrance time [WMD=−2.65, 95% CI=(−4.83, −0.46), P =0.02], and higher satisfaction [RR=1.15, 95% CI=(1.05, 1.25), P =0.001] than the control group. The difference in the stone-free rate between the two groups was not statistically significant [RR=1.03, 95% CI=(0.97, 1.09), P =0.38], but the stone-free rate of the observation group (ERAS group) in each study was higher than that of the control group. Conclusion: ERAS not only ensures the safety of PCNL but also promotes postoperative rehabilitation of patients (shorter surgery time, postoperative indwelling nephrostomy tube time, postoperative hospital time, and lower complication rate). At the same time, differences in the stone-free rate were not statistically significant, but the stone-free rate of ERAS in each study was higher than that of the usual care for PCNL patients.

Prospective Implementation of Enhanced Recovery After Surgery Protocols to Radical Cystectomy

Abstract:Background: Multimodal enhanced recovery after surgery (ERAS) regimens have improved outcomes from colorectal surgery. Objective: We report the application of ERAS to patients undergoing radical cystectomy (RC). Design, setting, and participants: Prospective collection of outcomes from consecutive patients undergoing RC at a single institution. Intervention: Twenty-six components including prehabilitation exercise, same day admission, carbohydrate fluid loading, targeted intraoperative fluid resuscitation, regional local anaesthesia, cessation of nasogastric tubes, omitting oral bowel preparation, avoiding drain use, early mobilisation, chewing gum use, and audit. Outcome measurements and statistical analysis: Primary outcomes were length of stay and readmission rate. Secondary outcomes included intraoperative blood loss, transfusion rates, survival, and histopathological findings. Results and limitations: Four hundred and fifty-three consecutive patients underwent RC, including 393 (87%) with ERAS. Length of stay was shorter with ERAS (median [interquartile range]: 8 [6-13] d) than without (18 [13-25], p<0.001). Patients with ERAS had lower blood loss (ERAS: 600 [383-969] ml vs 1050 [900-1575] ml for non-ERAS, p<0.001), lower transfusion rates (ERAS: 8.1% vs 25%, chi-square test, p<0.001), and fewer readmissions (ERAS: 15% vs 25%, chi-square test, p=0.04) than those without. Histopathological parameters (eg, tumour stage, node count, and margin state) and survival outcomes did not differ with ERAS use (all p>0.1). Multivariable analysis revealed ERAS use was (p=0.002) independently associated with length of stay. Conclusions: The use of ERAS pathways was associated with lower intraoperative blood loss and faster discharge for patients undergoing RC. These changes did not increase readmission rates or alter oncological outcomes. Patient summary: Recovery after major bladder surgery can be improved by using enhanced recovery pathways. Patients managed by these pathways have shorter length of stays, lower blood loss, and lower transfusion rates. Their adoption should be encouraged.

Abstract:Introduction: The experience of implementing enhanced recovery after surgery (ERAS) programs in pancreatic surgery is limited. The aim of this study was to evaluate the feasibility of ERAS program in pancreatic surgery in an academic medical center of China.Methods: Between May 2014 and August 2015, 124 patients managed with an ERAS program following pancreatic surgery (ERAS group), were compared to a historical cohort of 63 patients, treated with traditional perioperative care between August 2013 and April 2014 (no-ERAS group). Postoperative hospital stay (POPH), unplanned reoperation, unplanned readmissions, mortality and complications were compared between the two groups.Results: Mean POPH of all patients was significantly reduced (p = 0.007) from 17.1 days (no-ERAS group) to 11.7 days (ERAS group). Especially, mean POPH was reduced significantly in ERAS group of patient with no (7.0 vs. 8.7, p = 0.020) or grade I II (10.6 vs. 14.4, p = 0.001) complications. There was no difference of complication grades and types between two groups, as well as the rate of mortality, unplanned reoperation and readmission.Conclusion: The ERAS program is safe and feasible for patients undergoing pancreatic surgery, and it can decrease the postoperative hospital stay. Abstract:Enhanced recovery after surgery (ERAS) has been rapidly developing by combining several techniques with evidence-based adjustments, including preoperative education, preoperative carbohydrate loading, epidural or regional anesthesia, early initiation of enteral nutrition, ambulation and multi-modal pain management. The core part of ERAS is to reduce and reverse surgical stress and therefore greatly improve clinical outcome. Under the guidance of ERAS, perioperative management of robotic gastric cancer operation should follow the basic principles of ERAS and clinical pathway to maximize the advantages of the robotic surgery. ERAS protocol is safe and feasible for patients undergoing robotic radical gastrectomy and it can reduce surgical stress, shorten hospital stay, improve quality of life and does not increase complications, whose mechanism may be associated with the reduction of inflammation and insulin resistance, the decrease of resting energy exposure, and the protection of mitochondria function. It is worth emphasizing that it is very important to fully understand the changes of pathophysiology during perioperative period, to strictly implement the ERAS pathway based on optimized evidence-based medicine, to cooperate closely with the multidisciplinary team, to observe and manage the postoperative complications dynamically by systemic classification. The improvement of ERAS program on the outcome of patients should be summarized regularly and the new interventional strategies should be evaluated further according to the international standard.
