Abstract:Objective To observe the clinical curative effects and side-effects of metronidazole,tinidazole and ornidazole in treating anaerobic or trichomonat vaginitis, and to analyze the susceptibility of the isolated anaerobes and trichomonads to the three drugs. Methods Based on the bacteriological and parasitological laboratory examina- tion results, 290 anaerobic vaginitis patients and 62 trichomonal vaginitis patients were randomly divided into three groups treated with metronidazole, tinidazole and ornidazole, respectively. Doubling plate dilution method was ap- plied to determine the minimal inhibition concentrations(MIC) of the three drugs in inhibiting the anaerobic iso- lates, and the same dilution method was performed to detect the minimal parasiticidal concentrations (MPC) of the three drugs in killing the trichomonal isolates. Results The effective rate (54. 6%) of metronidazole in anaerobic vaginitis was remarkably lower than those of tinidazole (68. 8, P＜0. 05) and ornidazole (74. 2%, P＜0.01), but they showed no significant difference between the curative rates of tinidazole and ornidazole(P＞0. 05). The curative rates of the three drugs in trichomonal vaginitis were 90. 5%, 100% and 100%, respec- tively with no significant differences among the curative rates (P＞0.05). The total side-effect rate of metron- idazole in the treated patients was obviously higher than those of tinidazole (P＜0.01) and ornidazole (P＜0. 01), but those of tinidazole and ornidazole were similar (P＞0. 05). The MIC_(90) values of metronidazole, tinidazole and ornidazole against 266 Gram-negative anaerobic isolates were 4～8, 1～2 and 1～2μg/mL, and 32～64, 8～32 and 8～16μg/mL for 261 Gram-positive anaerobic ones, respectively. The MPC values of met- ronidazole, tinidazole and ornidazole in killing triehomonads were 25, 5 and 5μg/mL, respectively. Conclusion The outcome of comparison with metronidazole, tinidazole and ornidazole for anaerobic vaginitis shows higher cura- tive effects and lower side-effects, but metronidazole is much cheaper. The clinical anaerobic strains have much higher resistance to metronidazole. All three drugs dislay stronger activities against the Gram-negative anaerobes than the Gram-positive ones, however, the gram-negative anaerobes still show higher sensitivity to ornidazole.

Abstract:Trichomoniasis is the most common nonviral sexually transmitted disease in humans, but treatment options are limited. Here, we report a resorufin-based drug sensitivity assay for high-throughput microplate-based screening under hypoxic conditions. A 5203-compound enamine kinase library and several specialized compound series were tested for the inhibition of Trichomonas growth at 10 μM with Z' values of >0.5. Hits were rescreened in serial dilution to establish an IC50 concentration. A series of 7-substituted 7-deazaadenosine analogues emerged as highly potent anti-T. vaginalis agents, with EC50 values in the low double digit nanomolar range. These analogues exhibited excellent selectivity indices. Follow-up medicinal chemistry efforts identified an optimal ribofuranose and C7 substituent. Several nucleosides rapidly cleared cultures of T. vaginalis at a concentrations of just 2 × EC50. Preliminary in vivo evaluation in a murine trichomoniasis model (Tritrichomonas foetus) revealed promising activity upon topical administration, validating purine nucleoside analogues as a new class of antitrichomonal agents.

Abstract:Metronidazole (MTZ) is the most common drug used against Trichomonas vaginalis (T. vaginalis) infections; however, treatment failures and high rates of recurrence of trichomoniasis have been reported, suggesting the presence of resistance in T. vaginalis to MTZ. Therefore, research into new therapeutic options against T. vaginalis infections has become increasingly urgent. This study investigated the trichomonacidal activity of a series of five imidazole carbamate compounds (AGR-1, AGR-2, AGR-3, AGR-4, and AGR-5) through in vitro susceptibility assays to determine the IC50 value of each compound. All five compounds demonstrated potent trichomonacidal activity, with IC50 values in the nanomolar range and AGR-2 being the most potent (IC50 400 nM). To gain insight into molecular events related to AGR-induced cell death in T. vaginalis, we analyzed the expression profiles of some metabolic genes in the trophozoites exposed to AGR compounds and MTZ. It was found that both AGR and MTZ compounds reduced the expression of the glycolytic genes (CK, PFK, TPI, and ENOL) and genes involved in metabolism (G6PD, TKT, TALDO, NADHOX, ACT, and TUB), suggesting that disturbing these key metabolic genes alters the survival of the T. vaginalis parasite and that they probably share a similar mechanism of action. Additionally, the compounds showed low cytotoxicity in the Caco-2 and HT29 cell lines, and the results of the ADMET analysis indicated that these compounds have pharmacokinetic properties similar to those of MTZ. The findings offer significant insights that can serve as a basis for future in vivo studies of the compounds as a potential new treatment against T. vaginalis.

Abstract:Objectives Bacterial vaginosis (BV) and trichomoniasis are the most common causes of vaginitis. Both infections are associated with increased risk of acquisition and transmission of HIV and other sexually transmitted infections as well as adverse reproductive health outcomes. Co-infection is common, with rates ranging from 60% to 80%. We evaluated the efficacy of single-dose oral secnidazole 2 g for the treatment of trichomoniasis in a subgroup of women co-infected with BV and trichomoniasis. Design Post hoc analysis of data from a phase 3 randomised, double-blind, placebo-controlled, delayed-treatment study. Setting 10 centres in the USA. Participants Subgroup of women (aged ≥12 years) with a confirmed diagnosis of Trichomonas vaginalis and co-infection with BV clinically diagnosed using Amsel's criteria. Intervention Single dose of secnidazole 2 g or placebo. Outcome measures The primary efficacy outcome was the microbiological cure (negative culture for T. vaginalis ) at the test of cure (TOC) visit 6–12 days after dosing in the modified intent-to-treat population (mITT). At TOC, participants received the opposite treatment. Results Of the 131 T . vaginalis -infected participants in the mITT, 79 (60.3%) met ≥3 Amsel's criteria for BV at enrolment. Microbiological cure rates for trichomoniasis at TOC among this subgroup of women were 97.7% (42/43) for secnidazole and 0% (0/36) for placebo. Conclusion Single-dose oral secnidazole 2 g was highly efficacious in curing trichomoniasis in women co-infected with BV. Appropriate and effective treatment options for co-infection are essential for reducing transmission and reinfection. Secnidazole is the only single-dose medication approved by the Food and Drug Administration for the treatment of BV in women and trichomoniasis in women and men. Clinical observation of tinidazole combined with schizonepeta in the treatment of Trichomoniasis vaginalis and effects on the serum concentrations of IL-2, IL-4 and IL-10

Abstract:Objective To investigate the clinical effect of tinidazole combined with schizonepeta in the treat-ment of Trichomoniasis vaginalis (TV), and to study the effects on the serum concentrations of IL-2, IL-4 and IL-10. Methods A total of 120 cases of patients with TV, who admitted to Tongchuan People’s hospital and the First Hospital Affiliated to Xi'an Jiaotong University during January 2013 to January 2016, were randomly divided into control and treatment group with a parallel-group design, with 60 patients in each group. Patients in control group were routinely treated with tinidazole for 14 days, while those in the treatment group were additionally treated with schizonepeta for ex-ternal use based on the control group. The required time for alleviating the signs and symptoms were observed, and vagi-nal secretions were collected to be tested by the microscope. The serum concentrations of IL-2, IL-4 and IL-10 before and after treatment were tested by enzyme-linked immunosorbent assay (ELISA), and all the patients were followed up in 3 months to investigate the recurrence rate. Results The total effective rate of treatment group was 90.00%(55/60), which was significantly higher than 73.33%(44/60) in the control group (P<0.05). The serum concentrations of IL-2, IL-4 and IL-10 before and after the treatment had statistically significant difference in each group (P<0.05). Moreover, the difference of the IL-2 and IL-10 before and after the treatment in the treatment group were significantly more than the control group ((6.5±2.7) pg/mL vs (3.6±1.7) pg/mL, (6.8±2.6) pg/mL vs (2.6±0.9) pg/mL), P<0.05. The difference of IL-4 in the treatment group before and after treatment was a little more than the control group, but there was no statisti-cal significance (P>0.05). In addition, the recurrence rate of control group 18.3% (4/60) was significantly higher than that of the treatment group 6.7%(11/60) in the follow up of 3 months, P<0.01. Conclusion It was of great effective-ness to use tinidazole combined with schizonepeta in treatment of TV, which was much better than simply using the antit-richomonal agents on the improvement of symptoms and the recurrence. Furthermore, it may be related to the regulation the local inflammation as well as and the antitrichomonal effect, which could be the major mechanism to improve the cu-rative effect.
Clinical Effect of Metronidazole Vaginal Effervescent Tablet Combined with Flavescentis Sophora Suppository in the Treatment of Trichomonas Vaginitis

Abstract:For the treatment of trichomonas vaginitis, a combination therapy of metronidazole vaginal effervescent tablet combined with Flavescentis Sophora suppository is proposed. The patients diagnosed with trichomonas vaginitis are divided into the conventional treatment group and the joint group. The conventional group is treated with metronidazole vaginal effervescent tablets, and the joint group is treated with metronidazole vaginal effervescent tablets combined with sophora bolt. Both groups are treated for 3 months to observe the therapeutic effect of the two treatment methods. The expressions of IL-2 and IL-13 in serum of patients are detected by the ELISA. The changes in the vaginal pH value and cleanliness at each time point are observed. The patients are followed up for 6 months after treatment in order to observe the recurrence of trichomonas vaginitis in both groups. Experimental results show that the combination of the two tablets can further improve the clinical efficacy and reduce the inflammatory response and effectively improve the PH value and cleanliness of the vagina, and the recurrence rate is lower, which is worthy of clinical application.

Abstract:The microaerophilic parasite Trichomonas vaginalis occurs worldwide and causes inflammation of the urogenital tract, especially in women. With 156 million infections annually, trichomoniasis is the most prevalent non-viral sexually transmitted disease. Trichomoniasis is treated with 5-nitroimidazoles, especially metronidazole, which are prodrugs that have to be reduced at their nitro group to be activated. Resistance rates to metronidazole have remained comparably low, but they can be higher in certain areas leading to an increase of refractory cases. Metronidazole resistance in T . vaginalis can develop in vivo in clinical isolates, or it can be induced in the laboratory. Both types of resistance share certain characteristics but differ with regard to the dependence of ambient oxygen to become manifest. Although several candidate factors for metronidazole resistance have been described in the past, e.g. pyruvate:ferredoxin oxidoreductase and ferredoxin or thioredoxin reductase, open questions regarding their role in resistance have remained.In order to address this question, we performed a proteomic study with metronidazole-sensitive and –resistant laboratory strains, as well as with clinical strains, in order to identify factors causative for resistance. The list of proteins consistently associated with resistance was surprisingly short. Resistant laboratory and clinical strains only shared the downregulation of flavin reductase 1 (FR1), an enzyme previously identified to be involved in resistance. Originally, FR1 was believed to be an oxygen scavenging enzyme, but here we identified it as a ferric iron reductase which produces ferrous iron. Based on this finding and on further experimental evidence as presented herein, we propose a novel mechanism of metronidazole activation which is based on ferrous iron binding to proteins, thereby rendering them susceptible to complex formation with metronidazole. Upon resolution of iron-protein-metronidazole complexes, metronidazole radicals are formed which quickly react with thiols or proteins in the direct vicinity, leading to breaks in the peptide backbone.

[Comparison of Conventional Methods with Molecular Methods in the Diagnosis of Trichomonas vaginalis and Investigation of Metronidazole Resistance]

Abstract:Trichomoniasis is a sexually transmitted parasitic infection caused by Trichomonas vaginalis. In the diagnosis of trichomoniasis, direct microscopy (DM) is preferred, which is a cheap and fast method, although it has low sensitivity. Culture methods, which are accepted as the gold standard, can only be applied in certain centers due to the need for experienced personnel and the ability to get results within 2-7 days, despite their high sensitivity. In this study, it was aimed to compare conventional microscopic and culture methods used in the routine diagnosis of T.vaginalis with polymerase chain reaction (PCR) method and to investigate ntr4 and/or ntr6 gene polymorphism in the nitroreductase gene region, which are thought to be associated with metronidazole resistance in T.vaginalis strains isolated from clinical specimens. Vaginal swab specimens were collected from the posterior fornix of the vagina with two sterile ecuvion sticks during the gynecological examinations of 200 patients who applied to the Balıkesir University Health Practice and Research Hospital, Obstetrics and Gynecology Polyclinic between March 2019 and August 2021. The first swab sample was used for direct microscopic examination, Giemsa staining and conventional PCR analysis, while the second swab specimen was taken into trypticase-yeast-extract-maltose (TYM) medium for T.vaginalis culture and followed for eight days at 37 °C. All specimens were screened for the presence of T.vaginalis using primers specific to the β-tubulin (btub1) gene region and clinical isolates grown in TYM medium were examined for metronidazole resistance using primers specific for the nitroreductase gene region by using conventional PCR. Drug resistance test was also performed for the isolates in which polymorphism associated with metronidazole resistance was detected. Eight (4%) of 200 patient specimens were found positive by both culture/staining and PCR methods. The mean age of the patients included in the study was 39.9, while the mean age of the patients with positive T.vaginalis was 41.8. The most common clinical findings in the patients were foul-smelling vaginal discharge (36%), groin pain (21%), vaginal itching (19%), and burning sensation during urination (18%). In three out of eight T.vaginalis strains isolated from clinical samples, the presence of polymorphism in the ntr6 gene, which is thought to be associated with metronidazole resistance, was demonstrated by PCR. It was observed that three isolates with ntr6 gene polymorphism were phenotypically resistant to metronidazole (MLK= 390 µM). In this study, the fact that three of eight clinical isolates that were resistant to metronidazole by the broth microdilution method and as well as showing ntr6 gene polymorphism supported the thesis that there might be a close relationship between metronidazole resistance and ntr6 gene polymorphism. As a result, the use of culture and molecular methods in the diagnosis of T.vaginalis, in addition to the microscopy method, may contribute to a more accurate laboratory diagnosis of the agent, to detect metronidazole resistance molecularly and phenotypically, to determine metronidazole resistance rates in our country and to update treatment protocols within the framework of these data.

Abstract:OBJECTIVE:To evaluate the efficacy and adverse effects of 5-nitroimidazole derivatives in treatment of bacterial vaginosis (BV). METHODS:278 BV patients were randomly divided into 8 groups to be treated with (1) oral metronidazole sustained release tablet 750 mg/day for 7 days, (2) oral metronidazole sustained release tablet 750 mg/day for 7 days and vaginal tinidazole 250 mg for 7 days, (3) oral tinidazole 1 g/day for 3 days (2 g for the first dose), (4) oral tinidazole 1 g/day for 3 days (2 g for the first dose), (5) oral ornidazole 2 x 500 mg/day for 3 days and vaginal tinidazole 250 mg for 7 days, (6) oral ornidazole 2 x 500 mg/day for 3 days, (7) oral secnidazole 2 g in a single dose and vaginal tinidazole 250 mg for 7 days, and (8) oral secnidazole 2 g in a single dose. RESULTS:The clinical cure rates of the oral administration groups were 56.76% - 62.50%, all significantly lower than those of the oral/vaginal combination groups (80.00% - 86.11%, all P < 0.05). There was nit significantly difference in efficacy level among the only oral treatment groups and the oral/vaginal combination groups (all P > 0.05). CONCLUSION:Combination of oral and vaginal administration of 5-nitroimidazole derivatives is more effective in treatment of BV than oral administration only.
Anti-trichomonad activities of different compounds from foods, marine products, and medicinal plants: a review

Abstract:Human trichomoniasis, caused by the pathogenic parasitic protozoan Trichomonas vaginalis, is the most common non-viral sexually transmitted disease that contributes to reproductive morbidity in affected women and possibly to prostate cancer in men. Tritrichomonas foetus strains cause the disease trichomoniasis in farm animals (cattle, bulls, pigs) and diarrhea in domestic animals (cats and dogs). Because some T. vaginalis strains have become resistant to the widely used drug metronidazole, there is a need to develop alternative treatments, based on safe natural products that have the potential to replace and/or enhance the activity of lower doses of metronidazole. To help meet this need, this overview collates and interprets worldwide reported studies on the efficacy of structurally different classes of food, marine, and medicinal plant extracts and some of their bioactive pure compounds against T. vaginalis and T. foetus in vitro and in infected mice and women. Active food extracts include potato peels and their glycoalkaloids α-chaconine and α-solanine, caffeic and chlorogenic acids, and quercetin; the tomato glycoalkaloid α-tomatine; theaflavin-rich black tea extracts and bioactive theaflavins; plant essential oils and their compounds (+)-α-bisabolol and eugenol; the grape skin compound resveratrol; the kidney bean lectin, marine extracts from algae, seaweeds, and fungi and compounds that are derived from fungi; medicinal extracts and about 30 isolated pure compounds. Also covered are the inactivation of drug-resistant T. vaginalis and T. foetus strains by sensitized light; anti-trichomonad effects in mice and women; beneficial effects of probiotics in women; and mechanisms that govern cell death. The summarized findings will hopefully stimulate additional research, including molecular-mechanism-guided inactivations and human clinical studies, that will help ameliorate adverse effects of pathogenic protozoa.
The Clinical Effects of Metronidazole Vaginal Effervescent Tablets Combined with Kushen Suppository in the Treatment of Trichomonas Vaginitis

Abstract:Objective. The aim of this study was to explore the clinical effects of metronidazole vaginal effervescent tablets combined with Kushen suppository in the treatment of Trichomonas vaginitis. Methods. Ninety patients with trichomoniasis admitted to our hospital from January 2019 to January 2020 were prospectively analyzed and randomly divided into a control group (n = 45) treated with metronidazole vaginal effervescent tablets and an experimental group (n = 45) treated with Kushen suppository on top of the control group using the random number table method. The clinical effects, inflammatory factors, and microcirculation indexes were compared. We assessed patient's vaginal health by Vaginal Health Score Scale (VHS) before and after treatment and estimated their quality of life according to Generic Quality of Life Inventory-74 (GQOLI-74). A follow-up visit was conducted to compare patient's recurrence 3 months after treatment. Results. Distinctly higher total clinical effective of the experimental group compared with that of the control group was obtained ( P < 0.05 ); the serum level of inflammatory factors of the experimental group was dramatically lower than that of the control group after treatment ( P < 0.001 ); the experimental group experienced a favorable microcirculation index in comparison with the control group ( P < 0.001 ); superior VHS ( P < 0.001 ) and GQOLI-74 ( P < 0.05 ) scores of the experimental group after treatment compared to those of the control group were observed; the recurrence rate of the experimental group 3 months after treatment was significantly decreased in comparison with that of the control group ( P < 0.05 ). Conclusion. Metronidazole vaginal effervescent tablets combined with Kushen suppository can effectively improve the clinical symptoms of patients with trichomonas vaginitis, abate patient's inflammatory reaction, and raise their quality of life, which is worthy of promotion.

Abstract:Trichomonas vaginalis is the causative agent of trichomoniasis, the most prevalent non-viral sexually transmitted infection worldwide. Metronidazole (MTZ) is the mainstay of anti-trichomonal chemotherapy; however, drug resistance has become an increasingly worrying issue. Additionally, the molecular events of MTZ-induced cell death in T. vaginalis remain elusive. To gain insight into the differential expression of genes related to MTZ resistance and cell death, we conducted RNA-sequencing of three paired MTZ-resistant (MTZ-R) and MTZ-sensitive (MTZ-S) T. vaginalis strains treated with or without MTZ. Comparative transcriptomes analysis identified that several putative drug-resistant genes were exclusively upregulated in different MTZ-R strains, such as ATP-binding cassette (ABC) transporters and multidrug resistance pumps. Additionally, several shared upregulated genes among all the MTZ-R transcriptomes were not previously identified in T. vaginalis, such as 5′-nucleotidase surE and Na+-driven multidrug efflux pump, which are a potential stress response protein and a multidrug and toxic compound extrusion (MATE)-like protein, respectively. Functional enrichment analysis revealed that purine and pyrimidine metabolisms were suppressed in MTZ-S parasites upon drug treatment, whereas the endoplasmic reticulum-associated degradation (ERAD) pathway, proteasome, and ubiquitin-mediated proteolysis were strikingly activated, highlighting the novel pathways responsible for drug-induced stress. Our work presents the most detailed analysis of the transcriptional changes and the regulatory networks associated with MTZ resistance and MTZ-induced signaling, providing insights into MTZ resistance and cell death mechanisms in trichomonads.

Double-blind double-simulate randomized controlled clinical trial on secnidazole tablet in the treatment of trichomonal vaginitis

Abstract:Objective To evaluate the efficacy and safety of secnidazole tablet in the treatment of trichomonal vaginitis. Methods A double-blind double-simulate randomized controlled clinical trail was conducted. Sixty four cases in trial group were given secnidazole tablet and 58 cases in control group were given tinidazole tablet at draught. Vulva pruritus, symptoms of urethritis, leucorrhea, signs as well as pathogens were observed respectively 3~7 day after the end of treatment and 3 to 7 day after menorrhea.Results After treatment, the cure rates were 48.44% in trial group, and 50.00% in control group; the efficacy rates were 93.75 % and 98.28%, the eradicated rates of pathogen were 92.19 % and 98.28 % respectively. Three to 7 days after menorrhea, the cure rates were 57.81% in test group, and 58.62% in control group, the efficacy rates were 92.19 % and 89.66%, the eradicated rates of pathogen were 92.19% and 89.66%, respectively. There was no significant statistic difference between these two groups in efficacy and safety (P>0.05). Conclusion Secnidazole tablet is an effective and safe preparation in the treatment of trichomonal vaginitis.

Abstract:Infection by Trichomonas vaginalis has a high incidence/prevalence worldwide. It has been associated with a predisposition to cervical neoplasia or prostate cancer and an increased risk of acquisition of human papillomavirus (HPV) and human immunodeficiency virus (HIV). Besides, resistance to the drugs used for trichomoniasis treatment has increased in the last 30 years. Herein, thirteen phenylthiazolylbenzene sulfonamides were synthesized and evaluated for their in vitro activity against Trichomonas vaginalis. Among them, four derivatives showed higher anti-trichomonal activity than metronidazole (IC50 = 0.93 μM), while their cytotoxicity levels were not significant. These compounds were subject to molecular docking studies using Trichomonas vaginalis ferredoxin as target. The results revealed that the orientation of the nitro group of the active derivatives is toward [2Fe-2S], the cluster responsible for high reactive oxygen species generation. Finally, it was evident that the presence of a nitro group in the structure of the synthesized phenylthiazolylbenzene sulfonamides is essential for their trichomonicidal activity. Resumen. A nivel mundial la infección por Trichomonas vaginalis tiene una alta incidencia/prevalencia y se ha asociado con una predisposición a padecer neoplasia cervical o cáncer de próstata, así como a generar un mayor riesgo de adquirir el virus del papiloma humano (VPH) y el virus de la inmunodeficiencia humana (VIH). Además, en los últimos 30 años, la resistencia a los fármacos utilizados para el tratamiento de la tricomoniasis ha aumentado. En el presente trabajo, trece sulfonamidas de feniltiazolilbenceno fueron sintetizadas y evaluadas in vitro contra Trichomonas vaginalis. Cuatro de ellas exhibieron una actividad anti-tricomonas mayor que el metronidazol (CI50 = 0.93 μM), a la vez que citotoxicidad no significativa. Por tal motivo, estos compuestos fueron sometidos a estudios de acoplamiento molecular utilizando como diana a la ferredoxina de T. vaginalis. Los resultados revelaron que la orientación del grupo nitro de los derivados activos está dirigida hacia el grupo [2Fe-2S], responsable de la generación de especies de oxígeno altamente reactivas. Finalmente, se evidenció que la presencia de al menos un grupo nitro en la estructura de las sulfonamidas de feniltiazolilbenceno sintetizadas es esencial para su actividad tricomonicida.

Abstract:Objective:To investigate the effect of fluconazole capsule combined with Zhibaidihuangwan on the serum IL-10,TNF-αt levels and clinical effect of patients with trichomonas vaginitis.Methods:78 cases of trichomonas vaginitis patients were collected in our hospital and randomly divided into the experimental group and control group,with 39 cases in each group.Patients in the control group were treated with fluconazole,and patients in the experimental group were treated with fluconazole AND Zhibaidihuangwan.Then the changes of serum levels of interleukin-10 (IL-10) and tumor necrosis factor alpha (TNF-o) and the vaginal health score as well as the clinical efficacy were observed and compared before and after treatment.Results:Compared with before treatment,the serum TNF-α,IL-10 levels in both groups of patients after treatment were decreased,the vagina health score were significantly increased(P＜0.05);compared with the control group,the serum IL-10,TNF-o levels were obviously lower of patients in the experimental group,the vagina health score and total efficiency were significantly higher of patients in the experimental group(P＜0.05).Conclusion:Fluconazole capsule combined with Zhibaidihuangwan could effectively enhance the clinical curative effect of trichomonad vaginitis patients,which might be related to the reduction of serum IL-10 and TNF-α levels.

Abstract:Trichomoniasis, a prevalent sexually transmitted infection (STI) caused by the protozoan Trichomonas vaginalis , has gained increased significance globally. Its relevance has grown in recent years due to its association with a heightened risk of acquiring and transmitting the human immunodeficiency virus (HIV) and other STIs. In addition, many publications have revealed a potential link between trichomoniasis and certain cancers. Metronidazole (MTZ), a nitroimidazole compound developed over 50 years ago, remains the first-choice drug for treatment. However, reports of genotoxicity and side effects underscore the necessity for new compounds to address this pressing global health concern. In this study, we synthesized ten pyrazole-nitroimidazoles 1(a-j) and 4-nitro-1-(hydroxyethyl)-1 H -imidazole 2 , an analog of metronidazole (MTZ), and assessed their trichomonacidal and cytotoxic effects. All compounds 1(a-j) and 2 exhibited IC 50 values ≤ 20 μM and ≤ 41 μM, after 24 h and 48 h, respectively. Compounds 1d (IC 50 5.3 μM), 1e (IC 50 4.8 μM), and 1i (IC 50 5.2 μM) exhibited potencies equivalent to MTZ (IC 50 4.9 μM), the reference drug, after 24 h. Notably, compound 1i showed high anti-trichomonas activity after 24 h (IC 50 5.2 μM) and 48 h (IC 50 2.1 μM). Additionally, all compounds demonstrated either non-cytotoxic to HeLa cells (CC 50 > 100 μM) or low cytotoxicity (CC 50 between 69 and 100 μM). These findings suggest that pyrazole-nitroimidazole derivatives represent a promising heterocyclic system, serving as a potential lead for further optimization in trichomoniasis chemotherapy.

Abstract:Trichomonas vaginalis is a protist parasite of the urogenital tract, responsible for human trichomoniasis, an infection sexually transmitted that affects approximately 156 million people worldwide. This pathology is more evident in females and can cause miscarriages, premature births, and infertility. The disease can also lead to a greater predisposition to HIV infection and cervical and prostate cancer. Metronidazole (MTZ) is a drug that treats human trichomoniasis. The data from studies involving human subjects are limited regarding MTZ use during pregnancy. In addition to the toxicity of the treatment, some isolates have become resistant to MTZ. Therefore, searching for new compounds active for treating trichomoniasis becomes necessary. In the present study, we report results obtained using new phospholipid analogs. Two cardanol-based compounds designated LDT117 and LDT134 were active against T. vaginalis with an IC50 of 4.58 and 10.24 μM, respectively. These compounds were not toxic to epithelial cells in culture. Scanning electron microscopy observations revealed a rounding of the cells, a shortening of the flagella, and protrusions on the surface of drug-treated cells. Transmission electron microscopy of treated cells revealed alterations in the plasma membrane with formations of blebs, protrusions, depressions, and vacuoles with myelin figures and vacuolization in the cytoplasm after incubation. Furthermore, after treatments with the compounds LDT117 and LDT134, the parasites presented a positive reaction for TUNEL, indicating death by a mechanism like apoptosis. Given the results obtained, further in vivo studies using animal experimental models are necessary to validate that these compounds are effective for treating human trichomoniasis.

Abstract:Trichomoniasis is the most common non-viral sexually transmitted infection (STI) in the world caused by Trichomonas vaginalis . Failures in the treatment with the 5-nitroimidazole class including parasite resistance to metronidazole elicit new alternatives. Marine natural products are sources of several relevant molecules, presenting a variety of metabolites with numerous biological activities. In this work, we evaluated the anti- T. vaginalis activity of fungi associated with marine invertebrates by mass spectrometry-based metabolomics approaches. After screening of six marine fungi, extract from Penicillium citrinum FMPV 15 has shown to be 100% active against T. vaginalis , and the gel permeation column on Sephadex LH-20® yielded twelve organic fractions which five showed to be active. Metabolomics and statistical analyses were performed with all the samples (extract and fractions), and several compounds were suggested to be related to the activity. These components include citrinin, dicitrinin C, citreoisocoumarin, dihydrocitrinone, decarboxycitrinin, penicitrinone C, and others. The minimum inhibitory concentration (MIC) value of anti- T. vaginalis activity of citrinin was 200 μM. The marine fungi metabolites show potential as new alternatives to overcome drug resistance in T. vaginalis infections.

This case underscores the intricacies involved in managing recurrent Trichomonas vaginalis infections and the importance of a comprehensive evaluation.
