The Significance of Dermoscopic Vascular Features in the Early Diagnosis of Pigmented Purpuric Dermatosis

Abstract:Objective To explore the role of dermoscopy in the early diagnosis of pigmented purpuric dermatosis（PPD） by collecting dermoscopic images and investigating its features.Methods A retrospective analysis was performed using data from 50 patients who were diagnosed with pigmented purpuric dermatosis from the department of dermatology, the Second Affiliated Hospital, Xi′an Jiaotong University. The morphology of vascular structures, background colors, and non-vascular features were evaluated.Results The vascular features of PPD under dermoscopy were red dots, red globules, red patches, annular and comma-like vessels, and linear vessels, among which the most common vascular features were red dots（48/50, 96%） and red globules（46/50, 92%）. We found that the backgrounds were copper-brown, red, and light brown pigmentations, and the most common color was copper-brown background（37/50, 74%）.Brown dots, brown lines reticular, brown patches, brown circles, and white scales were seen in non-vascular structures, which the brown dots was the most common（37/50, 74%）.Conclusion The characteristic manifestations of PPD under dermoscopy are of great significance for the diagnosis and differential diagnosis of this disease.Dermoscopy can reduce the need for histopathological examination and is expected to be an important non-invasive diagnostic tool to facilitate the early diagnosis of PPD.
