Abstract:Abstract Mounting evidence indicates a potential beneficial effect of vigorous-intensity physical activity on hepatocellular carcinoma (HCC). However, the association between moderate-intensity physical activity, such as brisk walking, and the risk of HCC remains largely unknown. Two prospective cohorts of 77,535 women from the Nurses' Health Study and 44,540 men from the Health Professionals Follow-up Study were included. Weekly time spent on physical activities were updated biennially. The Cox proportional hazard regression model was used to calculate multivariable hazard ratios (HR) and 95% confidence intervals (95% CI). After an average 23-year follow-up, we identified 138 incident HCC cases. A higher amount of total physical activity was not significantly associated with a reduced risk of HCC (top tertile vs. bottom tertile; HR = 0.78; 95% CI, 0.51–1.18; Ptrend = 0.33). For the same comparison, there was an inverse association between moderate-intensity activity and HCC risk (HR = 0.60; 95% CI, 0.38–0.94; Ptrend = 0.04), whereas no statistically significant association with vigorous-intensity activity (HR = 0.88; 95% CI, 0.56–1.37; Ptrend = 0.74). Engaging in brisk walking was significantly associated with a lower risk of HCC (over 1 hour/week vs. non-brisk walking; HR = 0.50; 95% CI, 0.31–0.78; Ptrend = 0.006). The association between brisk walking and HCC risk was generally present across all subgroups, including age, body mass index, type 2 diabetes mellitus, smoking status, aspirin use, and alcohol consumption (all Pinteraction ≥ 0.05). In conclusion, moderate-intensity activity, especially brisk walking, was associated with reduced risk of HCC among U.S. men and women. If confirmed, brisk walking might serve a feasible way for HCC prevention.

Abstract:Abstract Introduction: Vitamin D has generated great interest as a potential preventative agent of chronic disease. For example, laboratory studies suggest that vitamin D has anti-proliferative, anti-inflammation, pro-differentiation, and pro-apoptotic properties. Epidemiologic studies have linked high vitamin D levels with some cancers, yet few studies have examined possible associations with liver cancer or chronic liver disease. Therefore, we evaluated the association of serum 25-hydroxyvitamin D (25(OH)D) with incident liver cancer and chronic liver disease mortality in a case-control study nested within the prospective Alpha-Tocopherol and Beta-Carotene Cancer Prevention (ATBC) Study, a cohort of Finnish male smokers. Methods: The ATBC Study was a randomized primary prevention trial of α-tocopherol and β-carotene supplementation where 29,133 Finnish male smokers, aged 50-69, were recruited from 1985 to 1988 and randomized to daily supplementation of α-tocopherol (50 mg), β-carotene (20 mg), both, or placebo. Supplementation ended in April 1993 (median 6.1 years); however, participants have been followed since that time. At baseline, participants underwent a physical examination by registered nurses to measure height and weight, and to collect an overnight fasting blood sample. Participants also completed a baseline questionnaire concerning their tobacco smoking, general risk factors, diet and their medical history. Serum 25(OH)D levels was measured for 202 incident liver cancer cases (ICD-9=155 and ICD-10=C22), 225 chronic liver disease deaths (CD-9=571 and ICD-10=K70, K73, or K74) occurring between baseline and December 31, 2009, and 427 liver cancer-free controls matched with cases by age and date of blood draw. Odds ratios (ORs) and 95% confidence intervals (CIs) were estimated using unconditional logistic regression models, adjusting for age, date of blood draw, BMI, history of diabetes, number of cigarettes smoked per day, daily intakes of alcohol and coffee, serum α-tocopherol, β-carotene, and cholesterol. Results: High serum 25(OH)D levels were inversely associated with both incident liver cancer and chronic liver disease in models adjusted for age and date of blood draw. After multivariate adjustment, associations remained similar for incident liver cancer (season-specific quartile 4 (Q4) vs. quartile 1 (Q1) OR=0.53, 95% CI=0.31-0.89, P-trend=0.007) but were attenuated somewhat for chronic liver disease mortality (Q4 vs. Q1 OR=0.71, 95% CI=0.41-1.25, P-trend=0.23). Associations were similar in analyses using clinical vitamin D cutpoints and season-standardized quartiles, and in analyses stratified by season of blood collection and intervention group. Similar associations were also observed among cases occurring more than fifteen years after baseline and in analyses performed after excluding cases seropositive for hepatitis B and C viruses. Conclusions: Men with higher serum 25(OH)D levels had a lower risk of incident liver cancer. A similar pattern was observed for chronic liver disease mortality, although associations for this outcome were not statistically significant. However, the association between moderate physical activity, such as brisk walking, and the risk of HCC remains largely unknown. Weekly time spent on brisk walking and other physical activity were updated biennially. The Cox proportional hazard regression model was used to calculate multivariable hazard ratios (HRs) and 95% confidence intervals (95%CIs). After an average 23.1-year follow-up, a total of 138 incident HCC cases were identified. In the multivariable-adjusted model, engaging in brisk walking was significantly associated with a lower risk of HCC (over 1 hour/week vs. non-brisk walking; HR: 0.44, 95%CI: 0.27-0.70; Ptrend<0.001). Further adjustment for body mass index (BMI) and type 2 diabetes (T2D) yielded similar results (HR: 0.51; 95%CI: 0.31-0.83; Ptrend=0.006). The association between brisk walking and HCC risk was generally consistent across all subgroups, including age, BMI, T2D, smoking status, aspirin use, and alcohol consumption (all P>0.05 for interaction). In conclusion, brisk walking was associated with a significantly lower risk of developing HCC among US men and women. If confirmed, brisk walking might serve as a feasible way for some individuals for HCC primary prevention. Abstract:Abstract Models predicting liver cancer are only available for the high risk population with hepatitis carriers but not for the general population. we developed risk prediction models for liver cancer using data collected from a large health screening program in Taiwan with long term follow up. The cohort included 444,023 subjects at baseline. During a median follow up time of 7.4 years, 1,668 subjects developed liver cancer. Epidemiologic data, medical history and routine blood panel including serum transaminases data were collected with optional additional testing for HBV or HCV status. Liver cancer cases were ascertained by computerized record linkage with both the National Cancer Registry and the National Death Certification profiles in Taiwan. Stepwise Cox regression analysis was performed to identify significant predictors in the multivariate models. Individualized risk of developing liver cancer in 10 years was calculated from baseline probability and relative risk profile estimated from the Cox regression model. Models were developed separately to provide risk prediction for subjects who chose to have HCV tested (130,533 subjects at baseline and 416 liver cancer cases) and who chose otherwise (313,490 subjects and 1,252 liver cancer cases). Since the results for both sub-cohorts are comparable and we only reported results for the sub-cohort with HCV tested. In the sub-cohort with HCV tested, the model with only questionnaire data identified significant main effects for male gender, older age, prior history of diabetes, pack year of smoking, alcohol use, and physical inactivity. This epidemiologic model had an AUC of 0.798 (95% CI=0.772-0.815) for 10-year risk prediction. With only data from measures of Transaminases, the model achieved an AUC of 0.927 (95% CI=0.911-0.938), a significant increase as compared to the model with only epidemiologic variables. The addition of HBV improved the AUC to 0.936 (95% CI=0.913-0.958). Similar results were obtained for sub-cohort without HCV testing. The addition of HCV status in combination with HBV status, the AUC improve to 0.941 (95% CI=0.918-0.967). Among models using key history and blood panels, the use of serum Transaminases only, available in routine health check-ups, was able to provide most information needed in predicting liver cancer in the general public. Additional testing of HBV and /or HCV further increased the prediction power. Physical Activity Compared to Adiposity and Risk of Liver-Related Mortality: Results from Two Prospective, Nationwide Cohorts.

Abstract:Background & Aims: Obesity in adulthood has been associated with increased risk of liver-related mortality. Whether higher levels of physical activity counteract the excess risk conferred by obesity remains unknown. We simultaneously evaluated the long-term impact of physical activity and adiposity on liver-related mortality, within 2 nationwide populations. Methods: We conducted a prospective cohort study of 77,238 women and 48,026 men, with detailed, validated assessments of weekly physical activity (metabolic equivalent task [MET]hours]), adiposity (body mass index [BMI], waist circumference), and diet, alcohol use and clinical comorbidities, biennially from 1986 through 2012. Using Cox proportional hazards regression models, we calculated multivariable-adjusted hazard ratios (aHRs) and 95% CIs for liver-related mortality, including death from hepatocellular carcinoma (HCC) and other complications of cirrhosis. Results: Over 1,856,226 person-years, we recorded 295 liver-related deaths (108 HCC; 187 cirrhosis). Risk of liver-related mortality increased monotonically with higher BMI during adulthood (ptrend<0.0001) and with weight gain during early adulthood (ptrend<0.0001). The risk of liver-related mortality also declined progressively, with increasing physical activity (ptrend = 0.0003); the aHRs across increasing physical activity quintiles were: 1.0, 0.70 (95% CI 0.51-0.96), 0.59 (95% CI 0.42-0.84), 0.52 (95% CI 0.36-0.74) and 0.46 (95% CI 0.31-0.66). Compared to lean-active adults (BMI <25; >= 18 MET-hours/week), the aHRs for obese-active, lean-sedentary, and obese-sedentary adults were: 1.04 (95% CI 0.73-1.37), 2.08 (95% CI 1.21-3.33) and 3.40 (95% CI 2.06-5.56), respectively. Findings were similar for HCC-specific and cirrhosis-specific mortality. Overall, engaging in average-pace walking for >3 hours/week could have prevented 25% of liver-related deaths (95% CI 0.12-0.38). Conclusions: In 2 prospective, nationwide cohorts, both excess adiposity and reduced physical activity were significant predictors of liver-related mortality. Achieving higher physical activity levels counteracted the excess liver-related risks associated with obesity. Lay summary: This is the first large, prospective cohort study to simultaneously evaluate the impact of obesity and physical activity on the long-term risk of liver-related mortality in 2 nationwide populations of American men and women. The study demonstrated that obesity predicted significantly increased risk of liver-related mortality, while physical activity predicted significantly lower risk of liver-related mortality. Importantly, the excess risk of liver-related mortality observed with obesity was no longer statistically significant among adults who engaged in the equivalent of average-pace walking for 3 hours or more, per week.
