Abstract:Objective: To perform a systematic review between all-cause mortality and body mass index ( BMI ) in Asian adults .Method: Relevant prospective studies that reported the relative risks ( RRs) of all-cause mortality for community-based adults in Asia were identified by a literature search .PubMed and CNKI electronic databases were searched from inception through September 30 , 2014 , with language restrictions of English and Chinese .Data were extracted by 1 reviewer and then reviewed by 3 independent reviewers .The overall effect of varied levels of BMI and all-cause mortality were then pooled and analyzed .Potential sources of heterogeneity were detected by stratification analyses and sensitivity analyses .Publication bias was detected by funnel plot, Egger's test and Begg's test.Results:Seventeen studies met the inclusion criteria;these studies included 1 769 369 individuals with 104 888 deaths .Random-effects summary all-cause mortality RRs was calculated .With the use of a BMI ( in kg/m2 ) of 18.5~22.9 as the reference, the summary RRs were 1.39(95%CI:1.31~1.47) for BMI less than 18.5 kg/m2 , 0.88 (95%CI:0.85~0.92) for BMI of 23.0~29.9 kg/m2 and 1.14 (95%CI:1.05~1.23) for BMI more than 30.0 kg/m2 .The RRs tended to be higher when weight and height were self-reported rather than measured .The RRs were higher when papers were published before the year 2005 rather than after the year 2006 .Also, the RRs were higher when the quality scores were higher .Potential sources of heterogeneity were gender , the method of obtaining weight and height , geography , publication year and quality scores .There was no publication bias ( P>0.05) in this meta-analysis .Conclusion:There was an increased risk of all-cause mortality for those both at the lower and higher level of BMI in Asian adults .

Abstract:The effect of body mass index (BMI) on esophageal and gastric carcinogenesis might be heterogeneous, depending on subtype or subsite. However, findings from prospective evaluations of BMI associated with these cancers among Asian populations have been inconsistent and limited, especially for esophageal adenocarcinoma and gastric cardia cancer. We performed a pooled analysis of 10 population-based cohort studies to examine this association in 394,247 Japanese individuals. We used Cox proportional hazards regression to estimate study-specific hazard ratios (HRs) and 95% confidence intervals (CIs), then pooled these estimates to calculate summary HRs with a random effects model. During 5,750,107 person-years of follow-up, 1569 esophageal cancer (1038 squamous cell carcinoma and 86 adenocarcinoma) and 11,095 gastric (728 cardia and 5620 noncardia) cancer incident cases were identified. An inverse association was observed between BMI and esophageal squamous cell carcinoma (HR per 5-kg/m2 increase 0.57, 95% CI 0.50-0.65), whereas a positive association was seen in gastric cardia cancer (HR 1.15, 95% CI 1.00-1.32). A nonsignificant and significant positive association for overweight or obese (BMI ≥25 kg/m2 ) relative to BMI <25 kg/m2 was observed with esophageal adenocarcinoma (HR 1.32, 95% CI 0.80-2.17) and gastric cardia cancer (HR 1.24, 95% CI 1.05-1.46), respectively. No clear association with BMI was found for gastric noncardia cancer. This prospective study-the largest in an Asian country-provides a comprehensive quantitative estimate of the association of BMI with upper gastrointestinal cancer and confirms the subtype- or subsite-specific carcinogenic impact of BMI in a Japanese population.

Abstract:Although a number of previous studies have noted either positive or no association for body mass index (BMI) and gastric cancer risk, little evidence exists in the Chinese population. We prospectively examined the associations of BMI with risk of gastric cancer in the Linxian General Population Trial cohort, with 29 584 healthy adults enrolled in 1985 and followed through to the end of 2014. Body weight and height were measured during physical examination at baseline and BMI was calculated as weight in kilograms divided by height in meters squared. Body mass index from 138 subjects was missing, and a total of 29 446 participants were included in the final analysis. Cox proportional hazards models were used to estimate hazard ratios and 95% confidence intervals. During 30 years of follow-up, we confirmed 1716 newly diagnosed gastric cardia adenocarcinoma (GCA) cases and 626 new gastric non-cardia adenocarcinoma (GNCA) cases. Overall, compared to the lowest quartile (BMI <20.32 kg/m2 ), subjects in the fourth quartile (BMI ≥23.31 kg/m2 ) subjects had lower risk of developing GNCA (hazard ratio, 0.65; 95% confidence interval, 0.51-0.83). Age- and sex-specific analyses showed that this protective effect was only observed in men and older (52 + years) persons. No associations were observed for BMI with GCA incidence. Higher BMI was associated with decreased risk of GNCA in this population, particularly in men and older persons. Future studies are needed to confirm these findings. Quantitative Association Between Body Mass Index and the Risk of Cancer: A Global Meta-analysis of Prospective Cohort Studies.

Abstract:Numerous studies have suggested that excess body weight is associated with increased cancer risk. To examine this putative association, we performed a systematic review and quantitative meta-analysis of cohort studies reporting body mass index (BMI) and the risk of 23 cancer types. PubMed, Embase, and Web of Science were searched for cohort studies, yielding 325 articles with 1,525,052 cases. Strong positive associations were observed between BMI and endometrial cancer (RR: 1.48), esophageal adenocarcinoma (RR: 1.45), and kidney cancer (RR: 1.20); weaker associations (RR < 1.20) were also found for several other cancer types. Interestingly, we found significant inverse associations between BMI and oral cavity (RR: 0.93), lung (RR: 0.91), premenopausal breast (RR: 0.95), and localized prostate (RR: 0.97) cancers. A male-specific association was found for colorectal cancer (p = 0.023), and a female-specific association was found for cancer in brain (p = 0.025) or kidney (p = 0.035). With respect to geography, the strongest positive association was found for total cancer in North America (p = 0.038). This comprehensive meta-analysis provides epidemiological evidence supporting the association between BMI and cancer risk. These findings can be used to drive public policies and to help guide personalized medicine in order to better manage body weight, thereby reducing the risk of developing obesity-related cancer.

Abstract:Objective Obesity is related to various diseases and the risk of cancer. However, most studies overemphasize the risk of obesity, while ignoring the impact of underweight. In this study, we carried out a metaanalysis to assess the impact of a low preoperative body mass index on the postoperative complication and overall survival rates of gastric cancer patients. Methods We identified and selected relevant studies through Pub Med, Embase, the Cochrane Library databases, the web of science, China National Knowledge Infrastructure and Wanfang electronic databases up to February 2019. The relative risk and 95% confidence interval were used to assess the association between the BMI and outcomes of gastric cancer patients. Results Seventeen publications were included in the final meta-analysis. There was a significant increase in the risk of postoperative complications（RR: 1.30, 95% CI: 1.07-1.57, P ＜ 0.05; I~2 =61.1%） in the patients with a low body mass index. Conclusion Further research, including larger-scale prospective studies, is needed to confirm this finding. For underweight patients with poor nutritional status, preoperative nutritional support and BMI adjustment may be helpful to improve the postoperative outcomes.
Body Mass Index and the Risk of Gastric Cancer in Males: a Prospective Cohort Study

Abstract:Objective: To investigate the association between BMI and gastric cancer risk in Chinese males. Methods: Data on body weight, body height and incidence of gastric cancer were collected on a biennial basis in males in Kailuan Cohort during 2006-2015. In addition, electronic databases of hospitals affiliated to Kailuan Group, insurance system of Kailuan Group and medical insurance system of Tangshan were used for supplementary information. Males with normal body weight (18.5 kg/m(2)≤BMI<24.0 kg/m(2)) were used as controls. Cox proportional hazards regression model was used to evaluate the association between baseline BMI and the risk of gastric cancer in males through the calculations of hazard ratio and 95% confidence interval. Results: A total of 109 600 males were included and 272 new gastric cancer cases were identified in Kailuan male cohort study, with a follow-up of 860 399.79 person-years during 2006-2015. The median follow-up period was 8.8 years. When compared with normal weight, the hazard ratios (HR) of underweight (BMI≤18.5 kg/m(2)) for gastric cancer risk were 2.11 (95%CI: 1.23-3.62) after adjusting for potential confounding factors (age, education level, smoking status, alcohol drinking status, dust exposure, salty food intake, tea drinking status). However, overweight or obesity showed no significant association with gastric cancer risk. The stratified analyses based on age, education level, status on smoking, alcohol drinking, tea drinking and dust exposure indicated that underweight showed significant association with gastric cancer risk in those with older age, those with high education level, non-smokers, non-alcohol drinkers, non-tea drinkers and those with dust exposure. Conclusion: Underweight might increase the risk of gastric cancer in males in China, and this positive association might be associated with age, education level, status on smoking, alcohol-drinking, tea-drink, and dust exposure.

Abstract:Introduction: Breast cancer is the most common cancer in women worldwide. The association between body mass index (BMI) and breast cancer risk has been paid more attention in the past few years, but the findings are still controversial. To obtain a more reliable conclusion, we performed a dose-response meta-analysis on 12 prospective cohort studies comprising 22,728,674 participants. Methods: Linear and nonlinear trend analyses were conducted to explore the dose-response relationship between BMI and breast cancer risk. The summary relative risk (SRR) and 95% confidence intervals (CIs) were used to evaluate the cancer risk. Results: The overall results showed a weak positive association between a 5-unit increase in BMI and breast cancer risk, indicating that a 5 kg/m(2) increase in BMI corresponded to a 2% increase in breast cancer risk (SRR: 1.02, 95% CI: 1.01-1.04, p<0.001). Notably, further subgroup meta-analysis found that higher BMI could be a protective factor of breast cancer risk for premenopausal women (SRR: 0.98, 95% CI: 0.96-0.99, p<0.001). In addition, the dose-response result demonstrated that there was a linear association between BMI and breast cancer risk (P-nonlinearity = 0.754). Conclusion: In summary, this dose-response meta-analysis of prospective cohort studies showed that every 5 kg/m(2) increase in BMI corresponded to a 2% increase in breast cancer risk in women. However, higher BMI could be a protective factor in breast cancer risk for premenopausal women. Further studies are necessary to verify these findings and elucidate the pathogenic mechanisms.

Abstract:Increasing evidence indicates that obesity is a risk factor for various tumors. We aimed to clarify the evidence for an association between body mass index (BMI) and cancer risk based on existing systematic reviews and meta-analyses. Eighteen studies were included in this umbrella review after searching PubMed, Embase and Web of science. The results revealed that underweight was inversely associated with the incidence of brain tumors and positively related to the risk of esophageal and lung cancer. Overweight enhances the incidence of brain tumors, kidney cancer, endometrial cancer, ovarian cancer, multiple myeloma, bladder cancer and liver cancer. Obesity was related to the increased incidence of brain tumors, cervical cancer, kidney cancer, endometrial cancer, esophageal cancer, gastric cancer, ovarian cancer, multiple myeloma, gallbladder cancer, bladder cancer, colorectal cancer, liver cancer, thyroid cancer and Hodgkin’s lymphoma. Moreover, dose-response analysis was conducted by 10 studies, and the results demonstrated that each 5 Kg/m2 increase in BMI was associated with a 1.01- to 1.13-fold increased risk of general brain tumors, multiple myeloma, bladder cancer, pancreatic cancer, breast cancer, and non-Hodgkin’s lymphoma. Every 1 Kg/m2 increase in BMI was linked to 6% and 4% increases in the risk of kidney cancer and gallbladder cancer, respectively.

Abstract:Background: This study was designed to evaluate the effects of body mass index (BMI) and weight change on the risk of developing cancer overall and cancer at different sites.Methods: We searched PubMed and other databases up to July 2023 using the keywords related to 'risk', 'cancer', 'weight', 'overweight', and 'obesity'. We identified eligible studies, and the inclusion criteria encompassed cohort studies in English that focused on cancer diagnosis and included BMI or weight change as an exposure factor. Multiple authors performed data extraction and quality assessment, and statistical analyses were carried out using RevMan and R software. We used random- or fixed-effects models to calculate the pooled relative risk (RR) or hazard ratio along with 95% confidence intervals (CIs). We used the Newcastle-Ottawa Scale to assess study quality.Results: Analysis included 66 cohort studies. Compared to underweight or normal weight, overweight or obesity was associated with an increased risk of endometrial cancer, kidney cancer, and liver cancer but a decreased risk of prostate cancer and lung cancer. Being underweight was associated with an increased risk of gastric cancer and lung cancer but not that of postmenopausal breast cancer or female reproductive cancer. In addition, weight loss of more than five kg was protective against overall cancer risk.Conclusions: Overweight and obesity increase the risk of most cancers, and weight loss of >5 kg reduces overall cancer risk. These findings provide insights for cancer prevention and help to elucidate the mechanisms underlying cancer development.Registration: Reviewregistry1786.

Prognostic Nomograms for Patients with Metastatic Gastric Cancer: A Large Population-Based Cohort Study and External Validation

Abstract:Purpose This study aimed to identify the prognostic variables in patients with metastatic gastric cancer (MGC) and to establish a nomogram based on the number of organs for metastasis (NOM) to predict the survival of MGC patients. Methods Data of 3,909 patients (training cohort) were collected from the Surveillance, Epidemiology, and End Results Program (SEER) database and that of 312 patients (validation cohort) were collected from two Chinese hospitals. Univariate and multivariate Cox regression analyses were performed to identify the significant prognostic variables. A predictive nomogram was built based on SEER cohort and validated in the Chinese cohort. The discrimination and calibration abilities were assessed by concordance index (C-index), calibration plot, and area under the (time-dependent receiver operating characteristic) curve (AUC). Results Race, surgery, chemotherapy, radiotherapy, tumor grade, number of organs affected by metastasis (NOM) were identified by univariate and multivariate Cox regression models. A predictive nomogram was constructed based on the SEER cohort. Excellent C-index, calibration plots, and AUC were obtained using the nomogram in the Chinese cohort. An online version was built based on the nomogram. Conclusion Our nomogram provides a reliable tool for predicting the prognosis of MGC patients.

Abstract:Objective To evaluate the relationship between body mass index(BMI) and malignant lymphoma by means of Meta-analysis.Methods Such databases as Web of Science,PubMed,EBbase,CNKI,Wanfang,VIP and CBM were searched from the date of their establishment to April 2011 to collect the case control studies on the relationship between BMI and malignant lymphoma.Two researchers independently selected studies,extracted data and assessed the quality according to the inclusive and exclusive criteria,and then conducted Meta-analyses by using RevMan5.0 software for heterogeneity test and pooled OR calculation.Results Seven case control studies involving 8416 malignant lymphoma patients and 14760 other patients were included.The quality of all studies scored 4,indicating reliable quality.Meta-analyses of the low BMI,overweight and obesity population were OR=0.8,95%CI 0.79 to 0.95,P=0.003;OR=1.04,95%CI 0.98 to 1.11,P=0.16;and OR=1.22 95%CI 1.04 to 1.43,P=0.01,respectively.The stratified Meta-analysis on histological subtypes showed that obesity was associated with a significantly increased risk of diffuse large B cell lymphoma(OR=1.33 95%CI 1.18 to 1.50,P<0.000 01),but was not associated with the follicular lymphoma or small lymphocytic lymphoma/chronic lymphocytic leukemia.Conclusion These findings demonstrate that low BMI is associated with the decrease of malignant lymphoma,and obesity is an increasing risk of malignant lymphoma,especially,the diffuse large B cell lymphoma.

Abstract:AIM: Obesity has been proved as one of the main risk factors for gastric cardia adenocarcinoma (GCA) in the West. The objective of our research was to evaluate the relationship between obesity and the risk of GCA in people from North China.METHODS: A total of 300 patients who had been diagnosed as GCA and had accepted surgical operation at Beijing Cancer Hospital from 1995 to 2002 were enrolled. Data were collected from pathology materials and hospital records. Two hundred and fifty-eight healthy people who had accepted health examination at the same hospital during the same period were enrolled as controls. Height, weight and gender of them at the time of examination were also collected. Obesity was estimated by body mass index (BMI), computed as weight in kilograms per square surface area (Kg/m(2)). The degree of obesity was determined by using BMI less than or equal to 18.5, 24-27.9 and greater than or equal to28 (Kg/m(2)) as the cut-off points for underweight/normal, overweight and obesity, respectively. Associations with obesity were estimated by odds ratios (ORs) and 95% confidence intervals (CIs). All ORs were adjusted for age and sex.RESULTS: The mean level of BMI was significantly lower in the patient group than that in the control group. The ORs for obesity in age groups 30-59 and 60-79 were 1.15 (95% CI=0.37-3.65) and 0.16 (95% CI=0.05-0.44) for males and 0.78 (95% CI=0.26-2.36) and 0.28 (95% CI=0.04-2.05) for-females, respectively. The ORs for underweight were 2.42 (95% CI=0.56-10.53) and 4.68 (95% CI=1.13-19.40) for males in age subgroups 30-59 and 60-79 and 40.7 (95% CI=9.32-177.92) for females older than 60 yrs. BMI was significantly associated with GCA (P<0.01). Underweight people were at high risk for GCA.CONCLUSION: BMI is an independent risk factor for GCA. Underweight is positively associated with GCA.
